Background: Brucellosis is associated with people living in close proximity to their animals, where conditions for disease onset and spread exist. An epidemic of brucellosis in Bosnia and Herzegovina (B&H) has persisted since 2004. Zenica-Doboj Canton is one of the most affected areas. Objective: To investigate the epidemiological characteristics of human brucellosis from the year 2008 to2018. Methods: Data collected from paper-based patients/cases reported to the Epidemiology Department were analyzed. Results: After 2008, the annual number of patients diagnosed with brucellosis was decreasing, except in 2017 and 2018 with 20 and 35 cases, respectively. Within the 2008-2018 period, a total of 263 human brucellosis cases were recorded, decreasing from 102 (incidence of 44.7/100,000) cases in 2008 to three cases in 2012, but increased to 35 cases in 2018. Males were predominant, with a total of 205 (77.9%) cases. The mean age of the affected patients was 39.2 years; but the most affected age group was the 25-49 years age group with 117 (44.5%) cases. Most cases (151 cases, 66%) were reported during the period of March-July, and 242 (92%) cases were from the rural areas. Conclusion: With the implementation of the small ruminant vaccination program in 2009, the number of infected humans had declined, while brucellosis still remains.

AbstractBackgroundBrucellosis is an infectious zoonosis, which greatly afflicts human health and animal productivity.ObjectivesTo describe the trends and epidemiological characteristics of human brucellosis in Nahavand county, Hamadan Province, western Iran over 8 years (2010–2017).MethodsIn this registry-based longitudinal study, we analyzed all reported cases of human brucellosis, including 1,130 patients from 2010 to 2017. A checklist including demographic and clinical characteristics of patients with reported cases of brucellosis had been used to gather data. We calculated annual percent changes (APCs) and average annual percent changes (AAPCs) using Joinpoint software to determine the trend of brucellosis using a segmented regression model.ResultsAmong the patients, 60.9% were male and 87.3% lived in rural areas. The mean age of the patients was 35.9 ± 18.34 years. Compared with male patients, female patients were more affected when they were elderly (15.6% men vs. 24.2% women for those aged ≥55 years) (P = 0.001). Of patients with brucellosis, 65.2% had consumed unpasteurized dairy products and 82.3% had a history of contact with animals. The incidence of human brucellosis in Nahavand county increased between 2010 and 2014, then decreased in 2015, and thereafter remained steady. Per 100,000 population, the AAPC pertaining to the incidence was 17.4 in the male population (95% CI: 4.4, 31.9), 13.8 in the female population (95% CI: 0.2, 29.3), and 16.1 in rural dwellers (95% CI: 2.2, 31.8) indicating an increasing trend from 2010 to 2017 (P < 0.05).ConclusionThe incidence of brucellosis in the western part of Iran is high and remains a challenging health problem. In the present study, age, job, sex, and seasonal changes are important risk factors for human brucellosis.

Background: Most of the countries including India have witnessed two or more waves of Covid 19 pandemic. The present study was conducted to compare the differences in clinico-demographic characteristics and outcomes of Covid 19 patients admitted in first and second wave of Covid 19 pandemic in a tertiary care hospital at Jalgaon, Maharashtra. Methods: A retrospective observational study was conducted at a tertiary care Dedicated Covid hospital for Covid 19 at Jalgaon, Maharashtra. All microbiologically proven corona positive patients were included in the study. The demographic records and clinical history was extracted from the case history sheets of the patients from first as well as second wave using standardized data collection form. Clinical outcome of the patients, i.e., development of complications, death or discharge was also recorded for each enrolled subject. Results: 3845 patients of Covid-19 admitted in the hospital during the first wave of epidemic and 2956 patients during second wave of the epidemic were included in the study. The mean age of patients admitted in the second wave was significantly lower as compared to first [48.77(15.31) years vs 50.23 (14.33) years, P<0.005]. There is increase in proportion of patients in the age group of < 15 years in second wave as compared to first wave (74/2956, 2.5% vs 52/3845, 1.3%). The number of patients requiring admission in ICU at the time of admission increased by 13% in second wave as compared to first wave. [827/2956 (28%) vs 577/3845(15%), P<0.0001]. More than half of the patients who got admitted for Covid 19 in first as well as second wave were having one or more comorbidities.But the proportion of the patients with previous co-morbities was significantly higher in second wave (1684/2956, 57% vs 1960/ 3845, 51%, P= 0.0004). The mortality was also higher in second wave (533/2956, 18.03% vs 541/3845, 14%, P=0.0004). Conclusions: The demographic, clinical characteristics and outcome of Covid 19 patients was different in first and second wave of pandemic with involvement of younger patients, increased rates of admission to ICU and more mortality in the second wave as compared to first wave of the pandemic.

OBJECTIVE: To describe the epidemiological trends and characteristics of extrapulmonary tuberculosis (EPTB) in Galicia, Spain, from 2000 to 2019.METHODS: This was a retrospective cohort study based on data from the Galician TB information system.RESULTS: Of the total number of TB cases (n = 15,871), 5,428 (34.2%) had EPTB. The absolute number of cases and incidence of EPTB decreased dramatically (from 480 cases and 17.8 cases/100,000 in 2000, to 172 and 6.4 cases/100,000 in 2019, respectively), with a mean annual decrease of respectively 64% and 4.7% for absolute cases and incidence rates. The risk for EPTB was higher in men than in women (RR 3.86, 95% CI 3.66–4.07). The most frequent age group was 15–44 years (2,234 patients, 41.2%); overall reductions per age group were 82% (0–14 years), 75% (15–44 years), 44% (45–64 years) and 63% (≥65 years), with statistically significant differences. The most frequently locations were the pleura (1,916 cases; 35.3%) and the lymph nodes (1,504; 27.7%).CONCLUSION: The incidence of EPTB in Galicia has decreased significantly in the last 20 years. The epidemiological characteristics have not changed, except for the number of patients with risk factors. This improvement of EPTB epidemiological trends coincides with the implementation of the programme for the prevention and control of TB, which suggests that it has been very effective in the control of the EPTB.

 Background: Acute appendicitis is very common surgical cause of acute abdomen and needs surgical removal either by laparoscopic or open appendicectomy. The aim of this study is to compare frequency of surgical site infection (SSI) in patients undergoing laparoscopic and open appendicectomy. Materials and Methods: The study was prospective study conducted in NMCTH, Biratnagar. Total 200 patients with diagnosis of acute appendicitis admitted through the emergency department of our hospital were included in the study. The patients were randomly allocated in two groups: Laparoscopic appendicectomy group (LA) and Open appendicectomy group (OA). Both groups underwent successful emergency appendicectomy. Wound infections in terms of surgical site infection (SSI) if present were recorded. All age groups and both sexes were included.  Results: Two hundred patients underwent appendicectomy, one hundred Laparoscopic appendicectomy (LA) and another hundred open appendicectomy (OA). The mean age of patients with acute appendicitis was 30.63±16.14 years with minimum of 6 years and maximum of 77 years. The highest number of patients were in age group of 10 to 20 years (29.5%). In LA group SSI noted in 3 patients (3%) whereas in OA group it was found in 12 patients (12%). Conclusion: Laparoscopic appendicectomy is better and offers great advantages in terms of SSI as compared to Open appendicectomy.

Abstract Previous studies have estimated the U.S. incidence of TTP to be 3.7/106/year, based on U.S. death certificates (Am J Hem1995;50:84), and 3.8/106/year, based on claims submitted to a national health insurer (Epidemiol2004;15:208). These methods are indirect and require extrapolation to determine incidence rates. We estimated the incidence directly from the Oklahoma TTP-HUS Registry. The Registry accrues all patients treated with plasma exchange (PE) for a clinical diagnosis of TTP or HUS (therefore excluding only children with typical HUS) in central, western, and southeastern Oklahoma since the Oklahoma Blood Institute (OBI) is the sole provider for PE in this region of 2.3 million people. Our estimates are based on patients who had their initial episode from 1/1/1996, after routine ADAMTS13 measurements began, to 6/30/2004. During these 8.5 years ADAMTS13 activity was measured in 189 (90%) of 209 consecutive patients. Prior to 1996, the number of patients accrued in the Oklahoma TTP-HUS Registry increased each year, similar to other data (JAMA2003; 290:1351); since 1996, there has been no consistent change in the annual number of patients accrued. The incidence of patients with idiopathic TTP-HUS (3.92/106/year) was similar to the previous reports for TTP. The incidence of patients with severe ADAMTS13 deficiency (<5% activity) was 1.37/106/year. For all patients with TTP-HUS the incidence of patients living in an urban area was 1.4-fold the incidence of patients living in rural areas. The rate of urban excess was greater for patients with idiopathic TTP-HUS (1.8-fold) and patients with ADAMTS13 deficiency (3.2-fold) (data not shown). Part of the urban excess may reflect better recognition of TTP-HUS because of familiarity with the Registry. However much of the urban excess can be attributed to the racial disparity that is greatest among patients with severe ADAMTS13 deficiency. Although the number of patients with severe ADAMTS13 deficiency in the Oklahoma TTP-HUS Registry is small, the incidence among the Black population was 8.7-fold the incidence among the non-Black population (95% CI, 4.1–18.5, p<0.0001). The explanation for why Black race is a risk factor for TTP is unknown, but may be related to the reasons why there is a greater prevalence of other vascular and thrombotic diseases among Blacks.

RiassuntoScopo - Analizzare l'attività del Servizio Psichiatrico di Diagnosi e Cura di Pavia nel periodo compreso tra il 1978 (anno dell'apertura del servizio) ed il 1990. Disegno - Analisi retrospettiva dei dati contenuti in un archivio computerizzato, nel quale, a partire dal 1978, sono state raccolte informazioni relative alle caratteristiche socio-demografiche e nosologiche dei pazienti ricoverati, nonché alia modalità ed all'andamento dei ricoveri. Setting - Il Servizio Psichiatrico di Diagnosi e Cura di Pavia, il cui bacino di utenza è costituito dall'ULSS 77, comprendente la città di Pavia e 61 comuni limitrofi. Principali indici utilizzati - Per ciascun anno considerato sono stati ricavati: indicatori d'uso (primi ricoveri, ricoveri complessivi, giornate di degenza, pazienti ricoverati); presenza media giornaliera e media annuale dei massimi mensili di pazienti ricoverati; indicatori d'intensita d'uso o di flusso (degenza media, indice di occupazione media dei posti-letto, indice di turnover, indice di rotazione dei posti-letto). La produttivita del servizio è stata indagata utilizzando il monogramma di Barber. Risultati - Tra il 1978 ed il 1983 si è verificato un progressivo incremento nel numero dei pazienti ricoverati, dei ricoveri complessivi e delle giornate di degenza; è seguito un periodo stazionario protrattosi fino al 1987; infine, durante gli ultimi tre anni il numero dei ricoveri complessivi e quello dei pazienti ricoverati sono leggermente diminuiti, mentre le giornate di degenza sono rimaste sostanzialmente stabili. Il numero annuale dei pazienti entrati in contatto per la prima volta con il servizio ha segnato due picchi negli anni 1979 e 1983 (N = 250), diminuendo poi progressivamente fino ai valori di 139 nel 1989 e di 152 nel 1990. Le frazione percentuali dei primi ricoveri rispetto ai ricoveri complessivi e rispetto ai pazienti ricoverati si sono ridotte nel corso del periodo considerato: nel 1990 esse erano pari a 30% e 50% rispettivamente. Presenza media giornaliera e media annuale dei massimi mensili di pazienti ricoverati sono aumentate tra il 1979 ed il 1983, passando da 12 a 21 pazienti/die, e da 14 a 25 pazienti/die, rispettivamente; negli ultimi otto anni i due parametri si sono stabilizzati su valori di 20-21 e 24-25 pazienti/die rispettivamente. II nomogramma di Barber, costruito sulla base dei valori assunti dagli indicatori d'intensita d'uso nel corso di ciascun anno considerato, ha mostrato che tutti gli anni in esame si sono distribuiti entro la zona di produttivita accettabile, ad eccezione del 1980 e del 1981. Design - Retrospective analysis of data stored in a computerized database, in which, since 1978, information has been collected on sociodemographic and nosological characteristics of patients admitted to the service, as well as on clinical aspects of all admissions. Setting – A General Hospital Psychiatric Inpatient Service in Pavia, whose catchmentarea (ULSS 77) includes the town of Pavia and 61 close municipalities. Main outcome measures -For each yearover the period considered we reported on: the indices of service utilization (first admissions, total admissions, days of stay, number of admitted patients); the mean daily number of inpatients, and the mean of the largest daily numbers of inpatients by month; the indices of intensity of service utilization (mean lenght of stay, index of bed occupation, index of turnover, index of bed rotation). The service productivity was determined according to the Barber's nomogram. Results – Between 1978 and 1983 patients admitted to the service, total admissions, and days of stay gradually increased; a steady period then followed until 1987; over the last three years, total admissions and the number of admitted patients slightly decreased, while daysof stay remained substantially stable. The annual number of patients admitted for the first time to the service marked two peaks in 1979 and 1983 (N = 250), and then gradually decreased to 139 in 1989 and 152 in 1990. The percent ratio of first admissions by total admissions and that of first admissions by the number of admitted patients showed a reduction over the period considered: in 1990 they were 30% and 50% respectively. The mean daily number of inpatients and the mean of the largest daily numbers of inpatients by month increased between 1979 and 1983, from 12 to 21 and from 14 to 25 patients/day respectively; over the last eight years the values remained rather constant, around 20-21 and 24-25 patients/day respectively. The Barber's nomogram, obtained on the basis of the indices of intensity of service utilization over the period considered, showed that each year fell within the area of acceptable productivity, with the exception of 1980 and 1981. Conclusions – After an initial phase of gradual expansion, the service then reached asteadystate, providing an acceptable productivity over most of the period considered.

Objective: to conduct a study of the basic epidemiological and clinical data of myasthenia in Leningrad region. The analysis of inpatient and outpatient charts of all patients with myasthenia, who were treated at the neurology department of the Leningrad Regional Clinical Hospital (LOKB) from 2003 to 2012. The estimation of incidence and prevalence of the disease per 100 000 population, the basic gender-age characteristics and clinical features of myasthenia in Leningrad region. According to the findings myasthenia gravis is not a rare disease, the prevalence of myasthenia gravis in the whole area is 4.6 cases, the average incidence was 0.29 cases per 100 000 population. The prevalence of myasthenia gravis in Leningrad region is lower than in other regions of Russia. Among patients women predominate in the age group 45-59 years, and there is a significant number of patients with disease onset in middle and old age.

<p><strong>Background: </strong>Acquired heart diseases (AHD) are heterogeneous diseases including rheumatic heart disease (RHD) and other affecting the heart and blood vessels after birth but usually appear clinically after 5 years of age. AHD considered as major cause of   morbidity and mortality. The present study was designed to document the spectrum, the affected age and clinical characteristics of AHDs in children of Gadarif state, eastern Sudan.</p><p><strong>Methods</strong>: This was a descriptive cross section study conducted in Gadarif pediatrics teaching hospital in period from June to Dec 2019.</p><p><strong>Results: </strong>A total of 46 children from Gadarif pediatrics teaching hospital with confirmed AHD were enrolled in this study. The females were 27 (58.7%), with a male-to-female ratio of 1.4:1. The mean age was 10.6±3.9 years (age’s ≤16 years old); the most common affected age group was 10-14 years accounted 18 (39.1) cases. RHD was the commonest AHD found in 38 (82.6%) patients, the commonest valvular lesion was mitral regurgitation 31 (73.9%).</p><p><strong>Conclusions: </strong>RHD was the most common AHD in the children in the present study, there is need to improve the scope of intervention facilities in the Sudan particularly in rural areas to prevent the growing and spread of these diseases.</p>

Aim. To demonstrate our experiences of thrombolytic therapy in acute ischemic stroke. Subjects and Methods. Patients with ischemic stroke treated with rt-PA, admitted at the Department of Neurology, Tuzla, Bosnia and Herzegovina, in the period between April 1, 2008, and December 31, 2012, were included. Results. Between April 2008 and December 2012, intravenous rt-PA was given to 87 patients with acute ischemic stroke, which represents 3.2% of patients with acute ischemic stroke admitted to our department in that period. Hypertension was the leading stroke risk factor. The mean NIHSS score before thrombolysis was 12 (range 4–21). Large artery arteriosclerosis was the most common stroke etiology. The mean door-to-needle time was 72 minutes and onset-to-needle time 152 minutes. Half of patients (44/87) had a significant improvement within the first 24 hours. Parenchymal hemorrhage occurred in 5 patients (6%) and was fatal in two cases. At 3-month follow-up, 45% of patients (39/87) had good outcome (mRS 0 or 1). Sixteen patients were dead at 3 months, and mean baseline stroke severity was significantly higher in patients who died (NIHSS 16.5 versus 11, P=0.003). Conclusion. The number of patients with acute ischemic stroke treated using rt-PA in the Department of Neurology, Tuzla, is lower than in developed countries. Thrombolytic therapy is safe and leads to favorable outcome in half of the patients.

Background: Rhinosporidiosis has been known for over a hundred years since its first description in Argentina. It is a chronic granulomatous,non contagious infective condition characterized by a production of nasal polyps and other manifestation of nasal hyperplasia, with frequent recurrence after surgery. The objective of this present study was Impact of Socio-economic condition of patients on Rhinosporidiosis.Methods: 34 number of patients were included in this study with symptomatic Rhinosporidiosis.The main outcome of study were age, sex, socioeconomic status, Occupation, bathing habit and living area of patients.Results: The study showed that 61.76 percent of patients were in the age group of 6-20 years. The mean age of 20.62 (SD±13.87) years. Most of the Rhinospridiosis patiets were from lower economic class 24(70.59%) followed by middle class 9(26.47%) and 1(2.94%) patients were upper class of socioeconomic status.Student was the most common occupation of Rhinosporidiosis 17(50%) followed by farmer 7(20.59%). Most of the Rhinosporidiosis patients 31(91.18%) were taking bath in pond where cattle also take bath.Conclusion: Rhinosporidiosis are very common in male and frequent age incidence are 6-20 years.
