Background. Ulcerative colitis is a chronic inflammatory disease of unknown origin, characterized by a clinically recurrent course with periods of bloody diarrhea and pathomorpholo­gical-diffuse inflammatory process in the colon. The problem of ulcerative colitis requires further study of the clinical features of the disease, taking into account the localization, degree of activity of the inflammatory process, changes in the structure of the mucous membrane that will help increase the efficiency of ulcerative colitis diagnosis in childhood. Materials and methods. On the basis of clinical and statistical analysis of 116 case histories of children aged 4–18 years with ulcerative colitis, the features of its clinical forms — total, segmental and distal — were studied during the period of exacerbation of the disease. Four hundred and forty-five biopsy specimens obtained during colonoscopy were histologically examined. After biopsy sampling, specimens were fixed in 10% formalin and were processed according to the generally accepted histological method with section staining using hematoxylin-eosin and according to Van Gieson. Results. Changes in the architecto­nics of the large intestine mucosa, which reduce the resistance of the mucous barrier, as well as impaired blood supply — a factor in the development of hemic hypoxia — are significant for the mechanisms of ulcerative colitis exacerbation.

This article presents frequency of occurrence of clinical forms of the disease depending on the localization and activity of the inflammatory process, their age and gender differences, risk factors and disease triggers, based on a retrospective analysis of 116 cases of children with ulcerative colitis at the age of 4-18 years. Comparative clinical, laboratory and endoscopic characteristics of total, segmental and distal colitis have been also analyzed. It was found that clinical activity of total colitis is characterized by more severe course of the disease, accompanied with systemic and local extraintestinal manifestations (OR = 4,504±0,506, p<0.05), more pronounced changes in hemo- and proteinogram parameters (p<0.05). Endoscopic criteria for differences in the clinical forms of ulcerative colitis are the presence of ulcers (OR = 9,667±0,645, p <0,05), erosions (OR = 3,569±0,429, p<0,05), contact bleeding (OR = 4,364± ,444, p< 0.05), changes in the vascular pattern (OR = 3,748±0,477, p<0.05). Correlation analysis of the relationship between clinical (PUCAI), endoscopic (Rachmilewitz index) and laboratory markers of the inflammatory process (leukocytes, platelets, erythrocyte sedimentation rate, γ-globulins, fecal calprotectin, hemoglobin) has been also performed, the criteria of which might be used to monitor the course of the disease and the effectiveness of therapy.

Ulcerative colitis is a chronic inflammatory disease mainly affects sigmoid colon and rectum. The inflammation process will activate NF-κB and leads to proinflamatory cytokine release such as Interleukin-6 (IL-6). Fig leaves contain a high level of flavonoid which can prevent NF-κB activation, and further inhibits IL-6 secretion. This research aims to see the preventive effect of methanolic extract of fig leaves towards colon histopathological feature and IL-6 serum level on ulcerative colitis induced mice. Balb/C male mice were randomly assigned into 5 groups (n=5). The treatment groups were dextran suphate sodium (DSS) control group (group I), methanolic extract of Fig leaves dose 28 mg/ day control group (group II), and methanolic extract of Fig leaves dose 7 mg/ day (group III), 14 mg/ day (group IV), and 28 mg/ day (group V) for 14 days, which at the 8th till 14th day were given DSS to induce colitis. The results showed that both of colon mucosal damage and IL-6 serum level of group I were significantly different from other groups (p=0,029). In  conclusion, the methanolic extract of Fig leaves can improve colon mucous damage and decrease IL-6 serum level on ulcerative colitis-induced mice. Keywords: ulcerative colitis, fig leaves, colon mucousal damage, interleukin-6

Abstract Background and objective As a chronic inflammatory disease of an unknown origin, the treatment of granulomatous mastitis has always been controversial. According to some researchers, surgical treatment and certain medications, especially steroids, are more effective in treating the disease. This study aimed at evaluating the results of treatment in a group of patients with granulomatous mastitis. Materials and methods This longitudinal cohort study evaluated the treatment outcomes of 87 patients with pathology-confirmed granulomatous mastitis referred to the surgical clinic of Central Hospital in Sari, Iran. Demographic, clinical, and pathological information, treatment methods and results, and the recurrence rate were analyzed. Findings A total of 87 female patients with granulomatous mastitis aged 22–52 years with a mean age of 34 years were evaluated. All patients had palpable masses; the breast masses were painful in 48.3% of patients, and 55.2% of patients suffered from erythema and inflammation, and8% had fistulas and ulcers at the inflammation site. The patients were followed-up for an average duration of 26 months (8–48 months) after treatment and recovery. The overall recurrence rate was 24.1%, and the recurrence rate was 29.4% in patients underwent surgery, 34.8% in patients received high-dose prednisolone, and 17% in those received low-dose prednisolone together with drainage (p < 0.001). Conclusions According to the results, the low-dose prednisolone plus drainage was more effective with a lower recurrence rate than only surgical excision or high-dose prednisolone. In fact, the use of minimally invasive methods such as drainage plus low-dose steroids is a more effective method with fewer side effects than the other two methods.

Mucinous endocervical adenocarcinoma is characterized by increased watery vaginal discharge, but the early diagnosis is sometimes difficult because biopsy specimen might only serve to sample a superficial part of the tumor. The patient presented with complaints of abdominal distention. No vaginal bleeding or watery discharge was observed. Hydrometra was suspected by imaging studies. Rapid reaccumulation of hydrometra was seen despite drainage. Papanicolaou smear of endocervix and endometrium followed by fractional curettage was performed, but failed to confirm the diagnosis. To investigate the unknown origin of hydrometra, an exploratory laparotomy with total hysterectomy and bilateral salpingo-oophorectomy was performed, followed by pelvic lymphadenectomy because biopsy specimens during operation suggested adenocarcinoma of the cervix. The final pathologic study of surgical specimens revealed mucinous adenocarcinoma, which was located on the proximal area of cervix. Adjacent to carcinoma tissue, lobular endocervical glandular hyperplasia (LEGH) was detected. Pyloric gland mucin (HIK1083), MUC6, and MUC5AC were diffusely immunopositive in the cytoplasm of LEGH cells and the immunoreactivity became weaker in adenocarcinoma cells with tumor progression and loss of differentiation. Based on histopathologic features of the present case, there seems to be a possible link between LEGH and conventional mucinous endocervical adenocarcinomas. The physician should keep in mind the possible existence of endocervical adenocarcinoma in a patient showing rapid reaccumulation of hydrometra, when uterine malignancies are clinically suspected and biopsy finding fails to confirm the diagnosis.

Abstract Context.—In some adult patients with chronic intractable diarrhea, the diagnosis remains elusive even after detailed evaluations, and colonic or duodenal biopsy specimens may appear unremarkable on routine hematoxylin-eosin staining. Objectives.—To assess the concentration of mast cells in colonic or duodenal biopsy specimens by immunohistochemical analysis for mast cell tryptase from patients with chronic intractable diarrhea and to evaluate their response to drugs affecting mast cell function. Design.—Mast cells per high-power field were assessed in biopsy specimens from 47 patients with chronic intractable diarrhea, from 50 control subjects, and from 63 patients with other specific diseases that cause chronic diarrhea (inflammatory bowel disease, celiac disease, collagenous colitis, and lymphocytic colitis). Patients with chronic intractable diarrhea who had more than 20 mast cells per high-power field were administered drugs affecting mast cell mediator function and release. Results.—The mean ± SD concentration of mast cells in the 50 control subjects was 13.3 ± 3.5 cells per high-power field; hence, patients with more than 20 mast cells per high-power field were considered to have increased mast cells. Thirty-three (70%) of 47 patients with chronic intractable diarrhea had increased mast cells, and symptoms were controlled by drug therapy in 22 (67%) of the 33 patients. No patient had systemic or cutaneous mastocytosis. No increase in mast cells was seen in patients with other common causes of chronic diarrhea. Conclusions.—In chronic intractable diarrhea, colonic or duodenal biopsy specimens may appear unremarkable on routine hematoxylin-eosin staining, but increased mast cells may be demonstrated by immunohistochemistry for mast cell tryptase, with the novel term mastocytic enterocolitis describing this condition. Similar increases in mast cells are not apparent in control populations or in patients with other specific diseases that cause chronic diarrhea. The cause of the increased mast cells remains to be elucidated.

Atherosclerosis is a chronic inflammatory disease that begins with endothelial dysfunction, it caused fat accumulation and plaque growth in the inner arteries walls. Endothelial dysfunction will activate the Mitogen Activated Protein Kinase (MAPK) pathway involving ERK1, ERK2, JNK1, JNK2, and p38MAPK proteins, as well as the Nuclear Factor Kappa B (NF-kB) pathway involving IKK proteins. Terpinen-4-ol is constituent found in the bangle rhizome. The purpose of this study were to determine the affinity and mechanisms of terpinen-4-ol against ERK1, ERK2, JNK1, JNK2, and p38MAPK proteins as anti-inflammatory in atherosclerosis performed using molecular docking method. The study was conducted exploratively with several steps such as preparation and optimization of terpinen-4-ol structure, preparation of 3D ERK1, ERK2, JNK1, JNK2, and p38MAPK proteins, validation method of molecular docking, and docking terpinen-4-ol in these proteins. The docking result are assessed from the binding energy and hydrogen bonds formed between terpinen-4-ol and proteins. The smaller value of binding energy terpinen-4-ol with target proteins showed the complex that form more stable. The result showed that terpinen-4-ol and has activity in inhibiting the inflammatory process because it is able to disturb ERK1, ERK2, JNK1, JNK2, and p38MAPK proteins with respective bond energy values -5,12; -5,24; -5,08; -5,88; and -4,99 Kcal/mol. The molecular mechanism in inhibiting the activity of ERK1, ERK2, JNK1, JNK2, and p38MAPK proteins is through the formation of hydrogen bonds in these proteins. These results show that terpinen-4-ol have the potential to inhibit inflammatory process and the formation of atherosclerotic plaque can be obstructed. In all, 28 koalas (Phascolarctos cinereus), 26 of which had died exclusively of natural causes, were autopsied and in 21 of these a probable cause of death could be recognized. Different forms of pneumonia head the list of these causes with six cases, including two where the primary lesion was trauma. Hepatitis with suppurative cholangitis was observed in three instances. Cryptococcosis, an infection by the yeast Cryptococcus neoformans, was responsible for three deaths, and two forms of blood dyscrasia, i.e. lymphoblastic leukaemia, and an anaemia of unknown origin accounted for two more deaths. Cystic disease of the ovary was observed in six koalas, and in four cases was complicated by infection and was the main cause of death. Middle ear sepsis, ulcerative colitis, and cardiac failure associated with senility were seen once each. In the remaining seven cases the cause of death was indeterminate, though senility appeared to be the predisposing cause in two.
