The treatment of metastatic spinal cord compression is complex. The three treatment modalities that are currently applied (in a histologically non-specific manner) are surgery, radiotherapy and the administration of steroids. The development of new spinal instrumentations and surgical approaches considerably changed the extent of therapeutic options in this field. These new surgical techniques have made it possible to resect these tumours totally, with subsequent vertebral reconstruction and spinal stabilization. In this respect, it is important to clearly identify those patients who can benefit from such an extensive surgery. We present our management algorithm to help select patients for surgery and at the same time identifying those for whom primary non-surgical therapy would be indicated. The retrospective review of surgically treated patients in our department in the last four years reveals a meagre application of conventional guidelines for the selection of the appropriate operative approach in the surgical management of these patients. The reasons for this discrepancy are discussed.

For many years, the department’s staff has been such scientists and water engineers with extensive experience as M.A. Volynov, V.S. Verbitsky, S.S. Medvedev, N.V. Lebedev, B.C. Panfilov, T.G. Voynich-Syanozhentsky, V.A. Golubkova, G.V. Lyapin and others. The department solved a wide range of tasks, the main areas of research were the following: – theoretical and applied hydrodynamics and hydraulics, with reference to the open channel flows that affect the state and level of safety of the hydraulic structures; – integrated use and protection of water bodies – water sources and water sources of water resources used in land reclamation; – development of measures and technical solutions for the protection of objects from the negative effects of water; – theoretical substantiation of works to improve the safety level of the GTS (declaration); – development and implementation of digitalization methods for solving design, construction, operation and control of landreclamation facilities. Currently, promising areas of research is the development of a decision-making algorithm in the designation of measures to rationalize the provision of resources to water amelioration. The algorithm is developed on the basis of a detailed study, systematization and processing of data both on safety and on the efficiency of systems and structures, ensuring the delivery of irrigation water of the required quality and in sufficient quantity from a water source to the field.

A significant number of epidemiological studies have shown that hyperuricemia is highly associated with the risk of developing cardiovascular disease, chronic kidney disease, and diabetes. In this connection, increased attention is required to monitor serum uric acid levels in patients, not only from a rheumatological point of view, but also with regard to reducing cardiovascular and renal risks. This article is a review of studies on the association of hyperuricemia with cardiovascular risk and a new consensus for the management of patients with hyperuricemia and high cardiovascular risk, published in december 2019 by a group of experts of the Russian Medical Society for Arterial Hypertension, which, among other things, includes a management algorithm of this category of patients.

Security is a pending challenge in cooperative spectrum sensing (CSS) as it employs a common channel and a controller. Spectrum sensing data falsification (SSDF) attacks are challenging as different types of attackers use them. To address this issue, the sifting and evaluation trust management algorithm (SETM) is proposed. The necessity of computing the trust for all the secondary users (SUs) is eliminated based on the use of the first phase of the algorithm. The second phase is executed to differentiate the random attacker and the genuine SUs. This reduces the computation and overhead costs. Simulations and complexity analyses have been performed to prove the efficiency and appropriateness of the proposed algorithm for combating SSDF attacks.

Assessing productivity, cost, and delays are essential to manage any construction operation, particularly the concrete batch plant (CBP) operation. This paper focuses on assessing the above-mentioned items for the CBP using stochastic mathematical models. It aims at (i) identifying the potential sources of delay in the CBP operation; (ii) assessing their influence on production, efficiency, time, and cost; and (iii) determining each factor share in inflating the CBP concrete unit expense. Stochastic mathematical models were designed to accomplish the aforementioned objectives. Data were collected from five CBP sites in Indiana, USA, to implement and verify the designed models. Results show that delays due to management conditions have the highest probability of occurrence (0.43), expected value of delay percent (62.54% out of total delays), and relative delay percent. The expected value of efficiency for all plants is 86.53%; however, the average total expense is US$15.56/m3 (all currency are in US$). In addition, the expected value of effective expenses (EE) is $18.03/m3, resulting in extra expenses (XE) of $2.47/m3. This research is relevant to both industry practitioners and researchers. It develops models to determine the effect of delays on concrete unit cost. They are also beneficial to the CBP management.Key words: concrete batch plant, delays, management conditions, cost models, cost management, stochastic mathematical models.
