Background/Aim. Chronic venous ulcer (CVU), a disease of high incidence, is one of the most serious chronic venous insufficiency complications. It has been estimated that there are 1%-2% of adults with CVU deriving a high social significance. The aim of this study was to, using the clinical experience, determine the influence of subcutaneous paratibial fasciotomy (SPF) on the course and the treatment outcome of CVU. Methods. From February 2006 to September 2009 SPF was applied in a group of 43 patients treated for CVU along with other standard methods of treatment, and its influence on the course of ulcus cruris was followed up regarding the control group of another 43 patients treated with standard methods with no paratibial fasciotomy. Results. In the group of patients treated with SPF there was a significantly better clinical course of ulcus cruris closing as compared with the group of patients in which this method was not applied. In the group with paratibial fasaciotomy there was no Thiersch skin transplant rejection recorded nor ulcus recurrence within a 6-month after-surgery period, while in the control group there was Thiersch skeen transplant rejection in 11 patients, and ulcus recurrence in 9 patients within the same period. Conclusion. SPF is a useful method with a favorable influence on better clinical course of ulcus cruris closing, reducing recurrence rate and improving local microcirculation in the affected region. Operation act itself is safe, requires no specific equipment nor special training of the team of surgeons, thus being applicable to the majority of patients with ulcus cruris indicated for surgery.

AbstractAcromegaly is known to be associated with high incidence of malignancies probably due to the mitogenic effects of IGF-1. Differentiated thyroid cancer (DTC) is reported to be one of the most frequent malignancies associated with acromegaly. But there is no data about the clinical course of DTC in acromegalic patients. In this study, we evaluated the course of DTC in 14 acromegalic patients retrospectively. Fourteen papillary thyroid cancer patients without acromegaly, who were matched with the acromegalic patient group for age, gender and properties of thyroid cancer, were investigated as the control group. We identified no change in the course and treatment responses of DTC in association with the acromegaly activity, gender, age and disease duration, and all patients were found to be in remission for DTC at the time of investigation. Retrospective analysis of this cohort suggests that the activity of acromegaly may not affect the treatment responses and prognosis of coexisting DTC.

The purpose of the work was to study the quantitative and qualitative composition of microbiota of the colon in patients with psoriasis and eczema infectious and to analyze the impact of its violations on the severity of the clinical course of the diseases. There were examined in a complex way 34 patients with psoriasis and eczema in factious illness. The age of the subjectsrangedfrom 23 to 70 years, amongthem: women – 15 (44,1 %), men – 19 (55,9 %), the period of the disease ranged from 2 to 42 years. The control group consisted of 20 practically healthy persons. In order to diagnose concomitant dysbiotic changes in the large intestine, microbiological examination of feces was performed on the patients (according to the method of Epstein – Litvak and Vilshanskaya). A high incidence of various intestinal pathologies (dyspepsia, gastroesophageal reflux disease, irritable bowel syndrome, functional disorders by the hepatobiliary system, gallstone disease etc.) was detected. In the microbiological study of feces, concomitant dysbiosis of the colon was detected in 16 (72,7 %) patients. Disturbance of the microbiota of the colon of varying severity is more often diagnosed in patients with more severe clinical course of psoriasis and eczema of infectious, as well as with a longer duration of the disease. The obtained data suggest that the intestinal microbiota, in particular colon dysbiosis, should be considered an important pathogenetic factor that negatively influences the nature and severity of the psoriasis and infectious eczema and requires timely diagnosis and appropriate medical correction.

Objective. To improve the results of treatment of the ulcer-necrotic affections of the foot soft tissues in patients, suffering an ischemic form of diabetic foot syndrome (DFS). Маterials and methods. The investigated group have consisted of 48 patients, to whom necrectomy was performed, because a tendency for the wounds healing under the influence of standard methods of treatment through two weeks was absent. Results. After application of the tactics elaborated a complete healing of the wounds was registered in 89.6%, and a partial one - in 10.4% patients. Of 38 patients of a control group, to whom a standard treatment was conducted, the wounds have healed completely in 11 (28.9%), and partially - in 12 (31.6%) patients. Conclusion. Plastic coverage of the foot wounds after necrectomy and early performed revascularization operation for the ischemic form of DFS is indicated to patients, in whom the wounds are lacking a tendency towards healing under the influence of standard methods of treatment through two weeks.

Summary Background: To evaluate the implementation of the guidelines of the German Society of Phlebology for venous crural ulcer a survey was conducted during the annual meeting of the German Society of Phlebology 2008 in Bochum. Methods: All 719 medical participants got an anonymized questionnaire asking for supply of crural ulcer in their institution. Results: The recurrent 66 questionnaires (9.2%) were filled by colleagues from practice or hospital, mostly surgeons, dermatologists, phlebologists and vascular surgeons. As basic diagnostics vein doppler (56.1%), duplex (75.8%) or measurement of brachial-ankleindex (83.3%) were performed. Compression therapy is used in all institutions. Mainly used wound dressings are polyurethane foam dressings, alginates, hydrocolloids and silver dressings. About 2/3 conduct surgical therapy of ulcers. Conclusion: Supply of ulcus cruris by the participants of the annual meeting of the DGP corresponds mainly, but not in all aspects to the guidelines. Further efforts for a spread of the guidelines are necessary.

Introduction. Back pain is not only a high prevalence, but also a costly health problem. That is why the search for new and the optimization of existing methods of dorsopathies treatment acquire an important clinical and medico-social significance. In recent years, the medical community has increased the interest in non-drug methods of treatment, including osteopathy. Previous studies have substantiated the possibility of effective application of osteopathic correction methods in the treatment of patients with dorsopathies. A specific object of the osteopath′s work is somatic dysfunction (SD). And if earlier SD was perceived from the standpoint of purely biomechanical disorders, then in recent years there have been serious changes in understanding the heterogeneity of this state nature. The biomechanical, rhythmogenic and neurodynamic components are conventionally distinguished in the structure of SD. One of the neurodynamic disorders types is the violation of motor stereotypes, the so-called static-dynamic disorders, revealed through special dynamic tests and manifested in the functional impossibility of building of active movement at various levels. At the same time, the generally accepted biomechanical approaches do not allow to completely eliminate violations of the statodynamic stereotype.The goal of research — the study was to research the effectiveness of the osteopathic approach using in combination with kinesitherapy for correction of the statodynamic stereotype violations in patients with dorsopathy at the cervicothoracic level.Materials and methods. A controlled randomized prospective study was conducted on the basis of a private rehabilitation center «Ezramed-Clinic» in Omsk in the period from February 2019 to December 2019. 52 patients with a diagnosis of dorsopathy at the cervicothoracic level were observed. 12 people were knocked out during the study in accordance with the exclusion criteria. As a result, 40 patients participated in the study. Depending on the applied treatment method, the patients were divided by simple randomization into two groups (main and control), each of which consisted of 20 people. Both groups of patients received osteopathic correction three times with a frequency of receptions 1 time in 7–10 days. The main group of patients additionally independently performed daily for 10 minutes a set of exercises aimed to restore the disturbed motor stereotypes (normal synkinesis). The control group of patients additionally independently performed a set of exercise therapy for the cervical spine every day for 10 minutes. All patients, regardless of the group, underwent an osteopathic examination before and after treatment with the formation of an osteopathic conclusion; the severity of pain syndrome and the volume of active movements in the cervical spine were assessed. The severity of the pain syndrome and the range of active movements were assessed before and immediately after treatment, as well as 3 months after the first session.Results. The use of osteopathic correction in conjunction with kinesitherapy (both special exercises and a complex of exercise therapy) in patients with a diagnosis of dorsopathy at the cervicothoracic level leads to a statistically significant increase in the range of motion in the cervical spine in the sagittal and frontal planes (p<0,05). The combination of osteopathic correction together with exercise therapy in patients of the control group led to a statistically more significant increase (p<0,05) in the range of motion in the cervical spine in the frontal plane after treatment compared with the results of the main group. However, 3 months after treatment, the indicators in patients of both groups did not have statistically significant differences.The combination of osteopathic correction in conjunction with kinesiotherapy in the form of special exercises for the motor stereotype correction in the main group of patients with diagnosed dorsopathy at the cervicothoracic level led to a statistically significant (p<0,05) decrease in the intensity of the pain syndrome at the 2nd session.Conclusion. In order to increase the effectiveness of treatment, osteopathic correction of somatic dysfunctions in patients with dorsopathy at the cervicothoracic level can be supplemented with complex kinesitherapy, both in the form of traditional exercise therapy, and in the form of special exercises for correction of altered motor stereotype.

At the analysis of clinical efficiency of differential therapy inthe  patients  with  AP  with  BO  the  type  vegetative  nervous system is defined. In the patients with hypersympathicotonic VR  (group  II)  and  asympathycotonic  VR  (III  group).during differential treatment there was noted reduction in duration of dyspnea, pulmonary cyanosis,  moist pulmonary rales . In  the  patients  with  AP  with  BO  alongside  with  changes biochemical, immune and vegetative status there was found wrong  attitude    of  the  family  to  the  child,  and  also the psychological behavioral deviations in the child as increasd anxiety,  aggression;  on  a  background  of  complex  therapywere  performed  psychological  correctional  methods  oftreatment:  musictherapy,  psychogymnastics,  gameexercises. At  catamnestic  observation  within  one  year  of  40  childrenwho  have  received  on  a  background  of  other  methods  of treatment psychocorrection, repeated bronchial obstructionwas  observed  only  at  15  %  observable  children.  In  control group  already  through  3  months  45  %  of  children  have addressed  with  repeated  episodes  BO  at  pneumonia  and

Background: SARS-CoV-2 (Covid-19) pandemic which was firstly identified in Wuhan/China in December 2019.There still exists no precise treatment for this pandemic yet despite many agents are tried in prophylaxis and treatment for Covid-19. In this study, we aimed to investigate the efficacy of 2-aticyto complex in clinical course in these patients. Materials and Methods: 150 patients who applied to the Infection Diseases Polyclinics between dates September 2020-November 2020 having diagnosed with Covid-19 were included in our study. The patients were randomized into 2 groups (75 in each group) of which the first group only had the treatment of National Guideline and the second group had Viruthol® including 2-aticyto complex (at the dose of 27000 mcg/day) in addition to the treatment of the National Guideline. The laboratory parameters, clinical outcomes and the first day on which PCR result turned into negative of the patients were compared. Results: In Viruthol® group, LDH, CRP, D-Dimer and ferritin levels were significantly decreased compared to the control group, while lymphocyte levels were higher. (p=0.02, p=0.001, p=0.01, p=0.02, p=0,001 respectively).  In addition to that, clinical symptoms such as fever, headache, weakness, loss of taste and smell and muscle-joint pain recovered more rapidly in contrast with the control group (p=0.001, p=0.05, p=0.001, p=0.001, p=0.001, respectively). PCR results of the Viruthol® group turned negative in a statistically significant shorter period of time with respect to the control group (p=0.001). Conclusion: Viruthol® containing 2-aticyto complex may be an agent that can be used both symptomatic treatment and improving the clinical situation and recovery of the patients followed up for Covid-19.

The aim is to determine the frequency and dynamics of detection of specific lipopolysaccharide (LPS) O-antigens LPS/O-antigens of causative agents in hospitalized diarrhea patients. Materials and methods. A total of 146 hospitalized diarrhea patients were examined with the use of an immunological method of the reaction of coagglutination (RCA) for the presence of LPS O-antigens of Shigella, Salmonella, Yersinia and Campylobacter in feces as markers of major intestinal infections pathogens. The control group was consisted of 40 blood donors. Results. In acute diarrhea patients there was detected the predominance of Yersinia and Salmonella LPS O-antigens over Shigella and Campylobacter; the high frequency of intestinal bacterial mixt-infection in total (68%), as well as elevated rate incidence of Shigella, Salmonella, Campylobacter, Yersinia antigens in mixt-infections in comparison with those in monoinfections (24%). The total O-antigen "load" in patients with mixt-infection was 3 times higher than in patients with monoinfections; with the increasing of LPS/O-antigen "load" the rate of Salmonella inoculation declines. Conclusion. Under the same severity of the clinical course of the disease, the decline of Salmonella inoculation rate in cases of high antigenic "load" may indicate to the presence of the phenomenon of summation of toxic effects of LPS O-antigens when the concentration of each pathogen in the feces is insufficient (for inoculation) for bacteriological examination.
