The OBJECTIVE of the study was to assess the current trends in the surgery of acute upper limb ischemia (AULI). MATERIAL AND METHODS. We performed the comparative analysis of clinical data and treatment results of 261 patients with AULI urgently treated between 2008 and 2018 years (main group) and 162 patients with AULI treated between 1975 and 1985 years (control group). RESULTS. For the past 30 years, there were the growing rate of AULI. In 92.7 % of cases, the main cause of AULI was embolism. The main disease leading to embolism became atrial fibrillation in 80.8 % of cases. For the past decades, there was a significant improvement of treatment results in patients with AULI: complete extremity blood flow supply was achieved in 95 % in main group vs 90.5 % in control group (p<0.05); postoperative mortality rate significantly decreased to 3.5 vs 8.8 % respectively (p<0.05). CONCLUSION. AULI continued to remain the actuality in modern vascular surgery.

Aim. Improving outcomes of diagnosis and treatment of patients with liver echinococcosis and its complications. Materials and methods. A comparative analysis of the results of surgical treatment of liver echinococcosis and its complications with traditional laparotomy access surgery (control group) and minimally invasive interventions (main group) was performed.Results. The study included 300 patients (170 in the control and 130 in the main group). In the main group, 37 (28.4%) cases performed open echinococcectomy from various mini-accesses, and 27 (20.7%) performed twostage operations using minimally invasive technology. Laparoscopic echinococcectomy was performed in 23 (17.7%) patients, laparoscopic pericystectomy 12 (9.2%) and laparoscopic liver resection in 10 (7.7%) patients. The frequency of postoperative complications in the main group was 17.7%, in the control 51.8%, postoperative mortality decreased from 2.3% to 0.8%.Conclusion. Minimally invasive technologies in the surgical treatment of liver echinococcosis show the better immediate results compared to traditional open surgical methods.

Abstract Background and Aims Balloon angioplasty (BA) without the use of stents has unsatisfactory results, which may cast doubt on its expediency. At the same time, BA is a very expensive treatment method. We conducted a comprehensive comparative analysis of the native arteriovenous fistula (AVF) patency rates in hemodialysis patients with central venous stenosis (CVS) after endovascular BA and «open» palliative surgery. Method A retrospective study included 80 patients with confirmed central vein stenosis: subclavian, brachiocephalic veins, vena cava inferior, or multiple lesions. The main group included 39 patients who underwent percutaneous balloon angioplasty. The control group included 41 patients who did not have balloon angioplasty for various reasons. In this patients we performed only «open» palliative interventions: thrombectomy, proximalization of arteriovenous anastomosis, AVF blood flow reduction. Results Functional primary patency (the time interval between the start of AVF using and the first intervention) did not differ: groups were comparable in time of stenosis manifestation (fig. 1A). Primary patency (the time interval between the first and second interventions) after BA was statistically significantly better than in the main group (fig. 1B), but difference was minimal: median survival in the study group of 8 months [95% CI 6; 10] vs. - 6 months [95% CI 4.9; 7.1]. There was the strong negative correlation between the primary patency and functional primary patency in the main group (r = –0.627 [95%CI –0.787; –0.388], p &lt;0.0001) but not in the control group (r = 0.049 [95%CI –0.262; –0.351], p = 0.7599). Thus, the later manifestation of CVS related with lower effectiveness of BA. The functional secondary patency (total duration of AVF use) in the main group was significantly better: median survival was 47 months [95% CI 40.9; 53.1] vs. 34 months [95% CI 29.8; 38.2] as well as secondary patency (the time interval between the first intervention and the complete cessation of AVF use): median survival was 16 months [95% CI 12.5; 19.5] vs. 7 months [95% CI 4.9; 9.1] (fig. 1 C and D). The occlusion-free period from the moment of starting the AVF use (functional primary assisted patency – fig. 1E) was higher in the main group, but difference was minimal: median survival was 39 months [95% CI 36.5; 41.5] vs. 32 months [95% CI 27.5; 36.5], as well as occlusion-free period from the moment of the first surgical intervention (primary assisted patency – fig. 1F) median survival was 9 months [95% CI 7; 11], in the control group - 7 months [95% CI 5.6; 8.4]. The need for open interventions was lower in the main group: 0.374 [95% CI 0.24; 0.556] and 2.451 [95% CI 1.1963; 3.023] per 10 patient-months, incidence rate ratio (IRR)= 0.153 [95% CI 0.095; 0.237], р&lt;0.0001; as well as overall need for interventions: 1.511 [95% CI 1.225; 1.843] and 2.451 [95% CI 1.963; 3.023] per 10 patient-months, IRR 0.617 [95% CI 0.461; 0.825] p=0.0011. The value of AVF volume blood flow had a strong negative correlation with the primary patency in both groups (r = –0.529, p =0.0027; r = –0.419, p =0.0101). Conclusion 1. Central vein stenosis is inevitably leads to loss of vascular access on the ipsilateral side. 2. Balloon angioplasty allows to extend the period of AVF use but it is not a radical treatment method of CVS. 3. The results of balloon angioplasty are significantly affected by the length of the period from the start of AVF use to the CVS manifestation. 4. Multiple repeated BA are apparently justified in patients for whom the possibility of creating a new vascular access is doubtful. 4. The AVF volume blood flow is an important factor determining the severity of CVS clinical manifestations and the need for repeated surgical interventions.

Treatment results for 109 patients (mean age 56 ± 1.7 years) with monocondylar impression tibial plateau fractures (ITPF) are presented. Patients from the control group (n=63) were operated on during the period from 2008 to 1010, patients from the main group (n=46) - from 2011 to 2013. Patients from the main group were treated with regard for injury localization relative to plateau center according to proposed operational classification of impression fractures (by CT data) and algorithm to choose the osteosynthesis technique depending on the anatomic and morphologic peculiarities of the intra-articular injury. In patients from the main group the evaluation by Rasmussen scale 36 months after intervention showed excellent results in 15 (38.4%), good - in 22 (56.4%), satisfactory - in 5 (12.8%) of patients, no poor results were recorded, and in patients from the control group - in 6 (11.5%), 8 (15.3%), 36 (69.3%) and 3 (5,8%) patients, respectively. Three (7.6%) complications (secondary displacement of fragments (2), knee contracture (1)) were observed in the main group, and 11 (20.9%) in the control group (20.9%) - local infectious inflammatory process (4), secondary displacement of plateau fragments (6), condylar sag (1). According to the literature, postoperative complications and associated purulent-inflammatory processes occur in 35-60 % of patients, which increases the rates of postoperative mortality to 25 %. The main purpose of our work was to evaluate the effectiveness of the application of the polymeric film «biodep-nano» in the prevention of secondary infection of wounds in patients with hemodialysis. Methods. The results of treatment of 36 patients divided into two groups were analyzed: the main group (18 patients) with applied traditional gauze dressings and the experimental group (18 patients) with additionally applied biodep-nano polymer films (Pat. No. 110594 Ukraine, MPK 2016.01, biodegradable polymer film «biodep-nano» is produced as «Hygienic and prophylactic. Biodegradable polymer film «Biodep-nano» of 50 g according to TU U 20.4-2950221612-001: 2017». Statistical data processing was performed using Student's t-test, 5 % (p≤0.05). Results. The results obtained showed that the temperature of the study area in the main group came to normal on the 3rd day (2.8±0.2 days), and in the group where traditional methods were used – on the 5-7th (5.9±0.4) day, (p<0.05). Reduction of tissue edema by an average of 3.8±1.2 days, and in the control group by an average of 6.1±1.7 days, (p<0.05). With regard to hyperemia, it gradually decreased and in the main group was up to 3.9±2.1 days on an average, and in the control group by 6.0±1.9 days (p<0.05). In the main group, the appearance of purulent discharge was observed in 1 patient, in the control group in 4 cases. The statistical calculations showed a significant statistical difference in the reduction of inflammatory process in both groups. Conclusions. The use of new methods and means of preventing secondary infections after surgery in patients with hemodialysis will enable to carry out dialysis of patients, shorten their stay in the clinic and save lives.

Introduction. Acute limb ischemia due to embolism in patients with atrial fibrillation remains poorly studied. Objective – to study the clinical significance and role of atrial fibrillation (AF) in the development of embolism to the bifurcation of the aorta and the main arteries of the limbs. Materials and methods. Treatment results of 1816 patients with acute ischemia of the extremities due to embolism treated at a specialized vascular surgical department for the past 30 years were analyzed. 1611 (88.7 %) of them had AF. The distribution into studied groups was according to the period time factor. Group 1 (n = 744) consisted of patients admitted in the period from 1991 to 2000; in group 2 (n = 568) – admitted in the period from 2001 to 2010, in group 3 (n = 299) – from 2011 to 2020. Methods included clinical examination, electrocardiography, Doppler-ultrasound, echocardiography. Results. The role of rheumatic heart disease as a cause of AF has decreased over the past decades by almost 10 times (from 19.5 % in group 1 to 2.0 % in group 3). Currently, the main background diseases for the development of AF are arterial hypertension and various forms of coronary artery disease. Embolism in patients with AF may develop in the arteries of all vascular areas of the systemic circulation, but in the practice of a vascular surgeon more often in the main arteries of minor caliber – the brachial (24.5 %) and popliteal (13.0 %). Multiple embolisms to various vascular areas were found in 2.8 % of patients. Urgent surgical revascularization of the limb by open embolectomy was performed in 1481 (91.9 %) patients that allowed 1348 (83.7 %) to be discharged with limb-sparing. Conclusion. In patients with acute limb ischemia of embologenic ethiology, comorbid AF has 88.7 % of them. Urgent embolectomy allows 83.7 % of patients to be discharged without limb amputation. Hospital mortality in the period 1991–2000 was 15.6 %, the last decade has been reduced to 7.4 %.

Introduction. In patients with chronic loco-motor system diseases the pain often persists after arthroplasty and does not respond to symptomatic therapy.Purpose of study: to evaluate the efficacy of perioperative use of Meloxicam in patients after hip arthroplasty. Patients and methods. The follow-up covered 120 patients (mean age 64.4±5.23 years) after hip arthroplasty. In the main group (n=60) Meloxicam was given 7 days prior to and 3 weeks after surgery; after intervention narcotic analgesic was used on the request. In control group (n=60) the patients were only on narcotic analgesic on request after operation. Treatment results were assessed by 100-millimeter visual analog scale (VAS) and D’Aubigné-Postel Score.Results. Seven days before the surgery the pain severity in the main and control groups was comparable: 85±2.3 and 84±2.1, respectively. In 2 days after operation the pain relief was more pronounced in the main group — 69±2.1 mm versus 82±3.4 mm in the control group (p0.05). In 3 months those indices made up 10±2.1 and 35±12.6 mm (p0.001), respectively. In the main group the result was assessed as the excellent and good in 22 patients, in the rest of patients as satisfactory by d’Aubigné-Postel Score. In the control group the good result was recorded in 9, satisfactory — in 47 and poor — in 4 cases.Conclusion. Meloxicam may be considered as an effective perioperative analgesic in large joints arthroplasty.

Aim. To study the possibility of using and estimate the efficiency of a new tablet form of biologically active supplement in patients with vegetovascular dystonia syndrome. Materials and methods. One hundred patients with vegetovascular dystonia syndrome, including 50 patients of the main group and 50 patients of the control group were examined. There is presented the composition of prescription formula of biologically active supplement, based on participation of its components in the correction of disturbed metabolic processes of the nervous system. Results. Additional application of biologically active supplement in combination with drug significantly improves cerebral blood flow, decreases blood viscosity and risk of cellular membrane impairment, normalizes arterial pressure, elevates initiative and attention, reduces headache in patients of the main group with vegetovascular dystonia syndrome. Conclusions. Use of biologically active supplement has a positive effect in complex therapy of patients with vegetovascular dystonia syndrome.

Burn disease affects the functioning of almost all functional systems, having a negative effect on them due to the development of endotoxicosis and vascular disorders. At the same time, the greatest attention is paid to the shifts that form in the systemic hemodynamics, primarily in the coronary and pulmonary blood flow, which can cause the development and progression of life-threatening conditions. At the same time, the microcirculatory system is also undergoing changes, but these changes have been studied in much less detail. The aim of this research was to study the state of skin microcirculation in healthy and burned rats. The experiment was carried out on 20 adult male Wistar rats divided into two equal groups. The first group of animals (n=10) was a control group (intact), with its representatives there were not any manipulations, except for a single study of the state of microcirculation. Rats of the second (main) group (n=10) were simulated contact thermal burn using their own patented method. The assessment of microcirculation parameters in the main group of animals was carried out by laser Doppler flowmetry on the device "LAKK-M" on the 1st day (2-3 hours after the injury), on the third and tenth days after the burn period. It was found that thermal injury has a negative impact on the microcirculation system, which is seen in a decrease in the intensity of blood flow through small-diameter vessels. This is achieved by reflex vasospasm, reducing the release of nitrogen oxide by endothelial cells and activating precapillary sphincters, and simultaneously activating the shunting ways of cardiac shunt from the arterial directly to the venous channel, bypassing the capillaries. Thus, in the post-thermal period, a compensatory "robbing phenomenon" is realized in the border zone of the burn, which requires pathogenetic correction.

Objective. To determine the character and optimal volume of standard open surgical interventions in an acute complicated pancreatitis (ACP) with elaboration of the main technical elements of the operations. Маterials and methods. There were operated 96 patients, suffering ACP. The main group consisted of 47 patients, in whom open interventions were performed primarily in 19, and as the second-stage procedure after application of miniinvasive interventional technologies (МIТ) – in 28. The comparison group consisted of 49 patients, in whom standard operations were done only. Results. Application of MIТ have promoted the indications narrowing for performance of primary open operations more than twice. In the main group the arcuate-like subcostal access was applied predominantly - in 26 (55%) observations (χ²=14.287; р=002), while in a control one – a median upper laparotomy – in 37 (76%) observations (χ²=38.43, р < 0.001). The method of closed draining in accordance to procedure of Beger was used predominantly in the main group of patients – in 43% of observations, comparing with a control group - 9% of observations (χ²=12.965; р=0.003). In 23% patients of the main group and in 26% patients of a control one (χ²=0.0013; р=0.05), when the extended purulent-necrotic inflammation process have presented, a staged sanation was applied, using programmed relaparotomies. Some technical elements of the surgical intervention were improved. Conclusion. Application of standard open operations in accordance to elaborated principle is accompanied by improvement of the treatment results in patients, suffering ACP.
