The article analyzes the autopsy reports of cancer patients who died in the Clinical Emergency Medicine Clinical Hospital in Smolensk in order to study the complications? Related to oncologycal disease in these patients. The complications of oncological diseases that doctors have to deal with in emergency hospitals and their specific manifestations depending on the location of the tumor are also analyzed.

AbstractPelvic masses are commonly diagnosed following clinical examination. These may be located in the genital organs or may interest other pelvic and abdominal organs. The pathology of the pelvic masses varies with age. The postmenopausal period is most commonly associated with malignant pathology while reproductive age is associated with benign pathology. The purpose of this paper is to present the experience of the Obstetrics and Gynaecology Clinic of the Sibiu County Clinical Emergency Hospital in laparoscopic myomectomy. We evaluated the patients admitted to the Obstetrics and Gynaecology Clinic of the Emergency County Clinical Hospital in Sibiu who underwent a laparoscopic myomectomy between 1.01.2015 - 06.06.2019. During this time, 14 laparoscopic myomectomies were performed in our clinic. Patients ranged in age from 29 to 57 years.

Background Approximately one million cancer patients live in Ukraine. These persons are at a high risk of manifestation of psychiatric disorders, especially depressive spectrum disorders and anxiety disorders, which may be treated with antidepressants if indications are present. Besides, oncological patients suffer from a number of miscellaneous somatic and psychosomatic conditions, that may also be totally or partly alleviated by using antidepressants. Thus, proper safe and evidence-based usage of these drugs is required. Unfortunately, there’s a lack of Ukrainian scientific literature highlighting this issue. This review is published to fulfill the aforementioned gap. Objectives To survey the usage of antidepressants in the cohort of cancer patients, to evaluate a modern state of evidence base concerning this issue, and to study indications, contraindications, and other peculiarities in respect of prescribing these drugs to oncological patients. Methods We reviewed and analyzed literature (meta-analyses, RTCs, observational studies, case studies) published in the last 10 years by searching PubMed and Google Scholar. We used the following keywords: “antidepressants”, “oncology”, “cancer”, “cancer symptoms”, “depression” and “drug interaction”. For our purposes, we also used NCCN, ASCO, ESMO, IPOS guidelines in case they were available and concerned our subject. Results The review focuses attention on the usage of antidepressants in oncological patients. Best-practice recommendations for choosing a drug are given, information on a drug-to-drug interaction with chemotherapy and other common in oncology drugs, issues with regard to comorbid somatic problems that define prescribing, and off-label indications (pain, hot flushes, pruritus, etc.) are discussed. Conclusion Despite the paucity of high-quality researches, a huge amount of evidences support the statement, that antidepressants significantly improve cancer patients’ quality of life by ceasing both depressive/anxious symptoms and typical cancer-related symptoms. To provide best professional help with these drugs, implementation of guidelines recommendations as well as adherence to experience collected from prescribing antidepressants to non-cancer population, which supports the effectiveness of this class of drugs for the treatment of depression, anxiety and pain are essential.

The study objective is to analyze surgical outcomes in patients with hemorrhagic stroke treated by endoscopic aspiration in five regional vascular centers in Russia and to evaluate capabilities and benefits of this surgical procedure. Material and methods. The study included 296 patients with hemorrhagic stroke (with various locations of the hemorrhage) that underwent endoscopic aspiration between 2006 and 2018 at one of the following institutions: N.V. Sklifosovsky Research Institute for Emergency Medicine (Moscow), City Clinical Emergency Hospital No. 1 (Omsk), Regional Clinical Hospital (Yaroslavl), N.A. Semashko Republic Clinical Hospital (Simferopol), Irkutsk Regional Clinical Hospital. Neurosurgery departments of these healthcare institutions are the parts of regional vascular centers. We used Glasgow Outcome Scale to evaluate outcomes of surgical treatment. Results. Complete recovery was observed in 52 (17.5 %) patients, while 52 (17.5 %) patients developed moderate disability and 131 patients (44.5 %) developed severe disability. Four (1.5 %) patients developed a vegetative state. Fifty-seven (19 %) patients died after surgery. Conclusion. The experience of treating hemorrhagic stroke in five neurosurgical centers demonstrates that endoscopic aspiration of hypertensive intracerebral hemorrhages is in no way inferior to microsurgery. It ensures satisfactory results and can be used in patients with various hemorrhages.

Cancer is the second leading cause of death in Romania and worldwide. Cancer patients are at increasing risk of acquiring bacterial infection with multi-resistant germs, including multidrug-resistant (MDR) strains of Gram-negative bacteria involved in nosocomial infection. Romania is one of the South-Eastern European countries with one of the highest prevalence rates of MDR pathogens. To determine the resistance pattern of bacterial profile and antibiotic resistance pattern in cancer patients admitted at the County Emergency Clinical Hospital Craiova, Romania. A retrospective study of bacterial pathogens was carried out on 90 adult cancer patients admitted from January to December 2018. The analysis of the resistance patterns for the action of the appropriate antibiotics was performed using Vitek 2 Compact system and diffusion method. In this study there were analysed 92 samples from 90 oncological patients (37-86 years). A total of 157 bacterial isolates were obtained, of which 37 strains of Staphylococcus aureus (23.56%), followed by Streptococcus pneumoniae (23- 14.64%), Klebsiella spp. and Escherichia coli (22 - 14,01%). The most common isolates were from respiratory tract (86 isolates - 54.77%). High rates of MDR were found for E. coli (63.63%), MRSA (61,11%) and Klebsiella spp. (54,54%), while one third of the isolated strains of Pseudomonas aeruginosa, Acinetobacter spp. and Proteus spp. were MDR. The findings of this study may be the basis for further more extensive studies highlighting the germs involved in the infectious pathology of cancer patients, in order to determine the antimicrobial resistance and to improve the methods of prophylaxis and treatment. The article describes the research work to determine the methods of psychological protection of nursing staff when working with cancer patients. Results. A training program has been developed, thanks to which nurses working with cancer patients will be able to analyze their experiences, actions and statements, which will help them to correct their behavior and increase their work efficiency. Conclusion. The study made it possible to study, evaluate and implement effective techniques of constructive psychological defense in the context of vocational training, as well as to study emotional stability and assess the level of emotional burnout as an indicator of the psychological health of specialists.

Reactivation of overt or occult HBV infection (HBVr) is a well-known, potentially life-threatening event which can occur during the course of immunosuppressive treatments. Although it has been described mainly in subjects receiving therapy for oncological or hematological diseases, the increasing use of immunosuppressant agents in non-oncological patients observed in recent years has raised concerns about the risk of reactivation in several other settings. However, few data can be found in the literature on the occurrence of HBVr in these populations, and few clear recommendations on its management have been defined. The present paper was written to provide an overview of the risk of HBV reactivation in non-neoplastic patients treated with immunosuppressive drugs, particularly for rheumatological, gastrointestinal, dermatological and neurological diseases, and for COVID-19 patients receiving immunomodulating agents; and to discuss the potential strategies for prevention and treatment of HBVr in these settings.

Aim. The medical care quality remains one of the most pressing issues of the health care system, especially at the stage of reforming of the medical sector. Medical-statistical information comprising reporting forms is the basis for analyzing the status of medical care quality and incorporating management decisions to improve it. Material and Methods. The study was performed by the means of analysis of the annual reports of the Urology Department of Municipal Non-profit Enterprise "Lviv Clinical Emergency Hospital" over a ten-year period: from 2010 to 2019. Statistical analysis included the analysis of average and relative values, as well as the calculation of indicators of the dynamic range: growth rate and absolute increase. Results and Discussion. Analysis of the use of bed stock of the Urology Department of MNELCEH during 2010-2019 showed that the average quantity of inpatient days is 251.12-340.18 days per year. Inpatient turnover was in the range of 30.28-36.55 patients per year. The indicator of surgical activity increased by 5.4 percentage points (from 33.0% in 2010 to 38.4% in 2019). There was also a positive tendency of decrease of the duration of post-surgery stay of patients at the inpatient department of the Urology Department (from 9.25 in 2010 to 8.55 in 2019). During the study period of 2010-2019, the ratio of post-surgical complications remained relatively stable and did not exceed 5.1% (the indicator is in the range of 4.38% -5.09%). The analysis of the total mortality among all treated patients was in the range of 0.06%-0.65%. Conclusion. The indicators analyzed in the annual reports make it possible to identify the main problematic issues in the work of the department. In order to provide professional specialized care, quality and rational use of property and human resources, algorithms should be developed to increase treatment of patients with malignant tumors and urolithiasis at the department, as in recent years we could observe an outflow of patients with these diagnoses to other healthcare establishments. There is a need for individual analysis of the causes of post-surgical complications and mortality. These issues can be resolved by introducing clinical audit into the work of the department

The exposure to a life-threatening disease such as cancer may constitute a traumatic experience that in some cases may lead to the development of posttraumatic stress disorder (PTSD). In recent years, several studies investigated this syndrome in patients with cancer, but few focused on the underlying neurobiology. The aim of this work was to review the current literature of neurobiology of PTSD in oncological diseases, focusing on a comparison with the results of neurobiological studies on PTSD in non-oncological patients and on treatments resulted effective for such disorder. Brain structures having a role in the appearance of PTSD in psycho-oncology, and in particular, in intrusive symptoms, seem to be the same involved in non-oncologic PTSD. These findings may have important implications also at clinical level, suggesting that psychotherapies found to be effective to treat PTSD in different populations may be offered also to patients with cancer-induced posttraumatic symptoms. Further studies are needed to deepen our knowledge about cancer-related PTSD neurobiology and its treatment, aiming at transferring the results into clinical practice.
