National study on the prevalence of iodine deficiency disorders among schoolchildren 8-12 years of age in Bahrain

A cross-sectional survey of primary-school children in Bahrain was conducted to estimate the prevalence of goitre and iodine deficiency according to age, sex and area of residence. During January-May 1999, 1600 children were randomly chosen from all government schools. Children were examined for goitre and of those, 50% were randomly selected for urinary iodine level assessment. Only 26 children [1.7%] had goitre. Although median urinary iodine was above 100 microg/L, 121 of 749 children [16.2%] had low urinary iodine levels. Although iodine deficiency does not pose a significant public health problem in Bahrain, education about the nutritional value of iodized salts in the prevention of this disorder could increase public awareness.

Objectives: To investigate the present occurrence of stunting and explore the role of iodine deficiency disorders (IDDs) as a predictor of stunting among primary school children in the Aseer Region. Methods: In a cross-sectional investigation on school children in the Aseer region, thyroid enlargement was evaluated clinically. Urine was collected to evaluate iodine content. Results: The present study involved 3046 school-age pupils. The study disclosed a total goiter rate of 24.0% (95% CI: 22.5–25.5%). The median urinary iodine content (UIC) was 17.0 µg/L. A prevalence of stunting (height for age z score of less than −2) of 7.8% (95% CI: 6.9–8.8%) was found. In a logistic regression model, pupils having clinical goiter (aOR = 1.739; 95% CI: 1.222–2.475) and students having UIC of less than 17 µg/L (aOR = 1.934; 95% CI: 1.457–2.571) were considerably related with stunting. In the receiver operating characteristic (ROC) curve, urinary iodine content to forecast stunting was good (AUC = 0.611, 95% CI: 0.594–0.629). The curve recognized the optimum cutoff point of urinary iodine content to be ≤19.0 µg/L. The sensitivity was 59.66% (95% CI: 53.1–66.0) and the specificity was 57.62% (95% CI: 55.8–59.5). Conclusion: The present study showed that stunting among school-aged children presents a mild public health problem. On the other hand, a severe iodine deficiency situation was revealed among school children in the Aseer region. Continuous monitoring of iodine status among school children is therefore necessary. Concerted interventions that blend nutrition-sensitive with nutrition-specific approaches are expected to influence decreasing stunting significantly.

ObjectivesTo identify the prevalence, methods, associations and reported reasons for self-harm among in-school and street-connected adolescents in Ghana.DesignA cross-sectional survey. We applied multi-level regression models and model-based cluster analysis to the data.SettingThree contexts in the Greater Accra region were used: second cycle schools, facilities of charity organisations and street census enumeration areas (sleeping places of street-connected adolescents, street corners, quiet spots of restaurants, markets, train and bus stations, and lorry and car parks).ParticipantsA regionally representative sample of 2107 (1723 in-school and 384 street-connected) adolescents aged 13–21 years.Outcome measuresParticipants responded to a structured self-report anonymous questionnaire describing their experience of self-harm and eliciting demographic information and social and personal adversities.ResultsThe lifetime prevalence of self-harm was 20.2% (95% CI 19.0% to 22.0%), 12-month prevalence was 16.6% (95% CI 15.0% to 18.0%) and 1-month prevalence was 3.1% (95% CI 2.0% to 4.0%). Self-injury alone accounted for 54.5% episodes and self-poisoning alone for 16.2% episodes, with more than one method used in 26% of episodes. Self-cutting (38.7%) was the most common form of self-injury, whereas alcohol (39.2%) and medications (27.7%) were the most commonly reported means of self-poisoning. The factors associated with self-harm were interpersonal: conflict with parents (adjusted OR (aOR)=1.87, 95% CI 1.24 to 2.81), physical abuse victimisation (aOR=1.69, 95% CI 1.16 to 2.47), difficulty in making and keeping friends (aOR=1.24, 95% CI 0.85 to 1.80), sexual abuse victimisation (aOR=1.21, 95% CI 0.78 to 1.87) and conflict between parents (aOR=1.07, 95% CI 0.73 to 1.56).ConclusionsSelf-harm is a significant public health problem among in-school and street-connected adolescents in the Greater Accra region of Ghana. Its origins are very largely in social and familial adversity, and therefore prevention and treatment measures need to be focused in these areas.

Background: Musculoskeletal disorders, an increasing concern among school going children, primarily affect muscles and tendons. They lead to secondary damage to nerves and joints in the neck, upper back, shoulders, arms, and hands, etc. The objective of this study was to determine the association of carrying heavy school bags with musculoskeletal discomfort among primary school children in government schools of Islamabad, Pakistan. Material and Methods: This cross-sectional survey was conducted in different government schools of Islamabad, Pakistan from June 2018 to November 2018. After an informed consent from parents, 377 healthy school going children aged 6-14 years were included in this study. Data was collected through Backpack Questionnaire. Chi square test was used to compare the frequency of musculoskeletal discomfort with demographic characteristics and backpack-related features. P-value less than .05 was considered statistically significant. Results: Mean age of the school children was 9.49 ± 1.53 years with 179 (46.9%) male and 203 (53.1%) female students. Majority of students (89.8%) were found with school bags more than 15% of their body weight. Shoulder pain was the most commonly reported complaint (67.3%). There was a significant association between pain and perceived backpack weight (P=.001), and between pain and self-perceived posture (P=.001). Leaning forward (66.2%) was the most commonly adopted posture followed by leaning sideways (15.4%) and stooping (2.9%), respectively. The association between pain and duration of carrying backpack from home to school was insignificant (P=.055) in contrast to pain and duration of carrying bag from school to home (P=.007), respectively. Conclusions: Musculoskeletal discomfort was found commonly among primary school children with shoulder pain being the most frequent, as maximum students were carrying heavy school bags.

Background: Iodine insufficiency is prevalent throughout the world, and is rated as one of the most significant public health concerns in more than 125 countries. Secondary to the insufficient dietary consumption of iodine, almost two billion individuals are prone to suffer from iodine deficiency disorders worldwide. The present survey was conducted in district Chamba of Himachal Pradesh in order to assess the extent of iodine deficiency disorders, 55 years after the initiation of salt iodisation programme.Methods: A cross sectional survey was conducted in district Chamba of Himachal Pradesh. A close ended pretested questionnaire was used to collect the socio-demographic information of the participants. Iodine content of salt and water was assessed with the use of I-Check and AQUA test kits in the households.Results: A total of 230 households were surveyed for the consumption of iodised salt which showed that 100% of the study population was using packet iodised salt for consumption. Majority of the population (92.6%) were placing their salt container near chulah in their kitchen and 73.5% were adding the salt in the beginning of the cooking followed by 16.1% in the middle of the cooking. Iodine content of salt sample collected revealed that 95.7% contain only 7 ppm iodine and 99.6% of the water sample contains less than 0.1 level iodine in water testing.Conclusions: The results of study indicated that population of district Chamba is using iodised salt but there is still need of strengthening of monitoring system as well as health education regarding the cooking practices in the households.

Sand flies are insect vectors of several diseases including leishmaniases. These vector-borne diseases represent a public health problem in several countries around the world, including Morocco. The objective of this study was to assess simultaneously the knowledge of health professionals and inhabitants on sand flies; a cross-sectional survey was conducted between April and June 2019 among 424 people, 34% of whom were health professionals and 66% of whom were inhabitants of the province of El Hajeb in central Morocco; 46.3% of doctors, 50.7% of nurses, 66.7% of midwives and 69.4% of inhabitants showed a low knowledge of sand flies. Most participants believed that sand flies breed in stagnant and polluted waters. Negative attitudes were found among 72.2% of the inhabitants. Factors associated with a high level of knowledge included continuing education among health professionals and information on vector-borne diseases among residents. The conceptual and cognitive gaps in the knowledge of sand flies reflect the lack of information and training on sand flies. The results of the sand fly knowledge review can be integrated into the national leishmaniases control program and the integrated vector management strategy to raise public awareness on the health risks of sand flies.

Low iodine intake during pregnancy may cause thyroid dysfunction in pregnant women and their newborn. In the present study, iodine status among a nation-wide representative sample of Belgian pregnant women in the first and third trimester of pregnancy was determined, and determinants of iodine status were assessed 1 year after the introduction of bread fortified with iodised salt. The women were selected according to a multistage proportionate-to-size sampling design. Urine samples were collected and a general questionnaire was completed face to face with the study nurse. The median urinary iodine concentration (UIC) among pregnant women (n1311) was 124·1 μg/l and 122·6 μg/g creatinine when corrected for urinary creatinine. The median UIC in the first trimester (118·3 μg/l) was significantly lower than that in the third trimester (131·0 μg/l) but significantly higher than among non-pregnant women (84·8 μg/l). Iodine-containing supplement intake was reported by 60·8 % of the women and 57·4 % of the women took this supplement daily. The risk of iodine deficiency was significantly higher in younger women, in women not taking iodine-containing supplements, with low consumption of milk and dairy drinks and during autumn. Women with a higher BMI had a higher risk of iodine deficiency but the risk was lower in women who reported alcohol consumption. The median UIC during pregnancy indicates iodine deficiency in Belgium and some women are at a higher risk of deficiency. The current low iodine intake in women of childbearing age precludes the correction of iodine deficiency in pregnant women supplemented with multivitamins containing 150 μg iodine as recommended.

Background: Goiter is one of the most visible manifestations of iodine deficiency disorders (IDD) that is caused due to overstimulation of thyroid as an adaptation to Iodine deficiency. IDD is among the major public health threats and important micronutrient deficiencies in developing countries.Methods: This cross-sectional study was conducted among 6-12 years school children in district Chamba of Himachal Pradesh from February to June 2019. The sample size of 336 was calculated and a total of 504 children from government schools were enrolled in the study. The assessment of goitre was done clinically by inspection and palpation of thyroid gland.Results: In this study, 504 school aged children of 6-12 yrs from district Chamba with mean age of 9±1.86 years were included. The prevalence of total goitre rate was 16% with mean age of 8.75±1.37 years. The highest prevalence (%) was observed in age of 10 years (28.4%) followed by 8 years and 9 years (25.9 % and 23.5% respectively).Conclusions: This study showed mild goitre prevalence in school aged children of 6-12 years in district Chamba of Himachal Pradesh. There is dire need of periodic survey to assess the magnitude of IDD in future.

OBJECTIVES: Refractive errors and strabismus continue to remain a significant public health problem. Among school children it has an impact on child development. The aim of the study was to determine the prevalence and pattern of refractive error and strabismus among primary school children. METHODOLOGY: It was a cross-sectional study. Stratified random sampling technique was used to select the school children. Data was collected in November-December 2013. Visual impairment was defined as, presenting VA of < 6/12. A total of 2288 children (1746 boys and 542 girls) were screened from 26 randomly selected schools. Stratified sampling technique was used. Data were collected on pre-designed questionnaires and entered and analyzed by SPSS software version 16. RESULTS: The mean age of children was 8.10 + 2.3 years. About 4% children were found visually impaired. The prevalence of refractive error was found to be 5.4%, of which 88.6% was uncorrected refractive error. The prevalence of refractive error was found higher in girls than boys, 6.8% and 5.0% respectively. Among types of refractive error, myopia prevalence was 2.3%, hypermetropia prevalence was 2.0% and astigmatism prevalence was 1.1%. The prevalence of strabismus was 1.3%. Refractive error was found as a main cause of visual impairment in school children. Prevalence of strabismus among this study population is relatively low as compared to the global prevalence of 2% to 5%. CONCLUSION: It is recommended that more study to determine the extent of the problem and possible factors for development of refractive error and strabismus types are needed to mitigate these conditions.

Introduction. Two main strategies are currently recommended for the prevention and control of iodine deficiency in the world: implementation of universal salt iodisation programmes and permanent monitoring of iodine consumption by the population. Although iodine intake and coverage iodised salt have increased in the world population, iodine deficiency disorders (IDDs) may still be a public health problem in a few countries or communities. Objective. To assess the impact of salt iodisation programmes on urinary iodine concentrations and goitre rates in the world population. Methodology. A systematic review based on the PRISMA method. We obtained articles from Scopus, Science Direct, MEDLINE databases, and other sources between March and April 2020, without limitation of dates. “Iodisation” AND “urinary iodine concentrations” AND “goitre” in English, Portuguese, and Spanish without filters and clinical trial, case-control, and cross-sectional studies were included in this review. Results. Of 479 abstracts, twenty-three were eligible. Coverage on iodised salt was in the range of 16 to 98%, and 11 studies had been sufficient, whilst eight studies had adequate iodine concentration in salt and three excess. 81.8% of studies that had an adequate median of UIC had a good impact in their respective salt iodisation programmes. Conclusion. After 18 years of salt iodisation programme implementation in the 13 countries, the majority achieved sustaining elimination of IDD whilst all had adequate median UIC; however, more detailed studies are still needed to confirm that all communities are equally protected of IDD.

Background. Labor related injuries among Palestinian schoolchildren are a significant undocumented public health concern. This study aimed at documenting the prevalence and nature of work related injuries among schoolchildren as well as identifying sociodemographic factors that predict these injuries. Methods. A cross-sectional survey included 15,963 children of whom 6458 (40.8%) completed an optional package related to labor. Students aged 12–18 years self-completed the international WHO collaborative HBSC valid questionnaires between April and May of 2006. Results. Approximately 73.8% of the students who filled the optional package reported working during the last 12 months, of whom 79.1% sustained a work related injury. Work injuries were significantly higher among boys, younger children, and children enrolled in UNRWA schools and living in Gaza Strip P<0.05. Children working ≥3 hours/day were more likely to experience injuries, 1.73 (95% CI, 1.53–1.95), than those working ≤3/day. About half of the children worked in retail trade (51.5%), agriculture (20.0%), and cleaning (11.4%). Injury type was related to the type of work performed. Conclusions. The high prevalence of injuries among working Palestinian schoolchildren confirms its severity as a public health problem. To reduce occupational injuries, policymakers and professionals should develop intervention programs that target the public and health providers.
