The transition of the Russian Federation to new regulations in the field of labor protection in 2021 requires preliminary and periodic medical examinations of about 30–40 million office employees, which creates a number of organizational and economic problems for employers. The basis for conducting medical examinations of office personnel is the presence at the workplace of the electromagnetic field of the broadband frequency spectrum (5 Hz — 2 kHz, 2 kHz — 400 kHz), which is considered as a prerequisite for the appearance of occupational diseases associated with the influence of the electromagnetic field, leading to a disorder of the autonomic (autonomous) nervous system and with visually strenuous work, leading to the appearance of progressive myopia. The order of the Ministry of Health of the Russian Federation No. 29n of 28.01.2021 does not contain the exceptions characteristic of the latest editions of the order of the Ministry of Health and Social Development of the Russian Federation No. 302n concerning the intensity of the impact of the factor «when exceeding the maximum permissible level» or «work on reading, entering information, working in the dialog mode in the amount of at least 50 % of the working time», which means that the employer must conduct medical examinations of office employees working with a PC. The number of necessary laboratory and functional tests decreased by 1.8 times. Paradoxical is the transfer from the order of the Ministry of Health and Social Development (No. 302n) to the new order of the Ministry of Health of the Russian Federation (No. 29n) of the connection of electromagnetic fields exclusively with ophthalmological problems, ignoring no less significant problems of damage to the nervous system. The inclusion of a dermatovenerologist in the commission is completely unjustified, since the impact of the electromagnetic field of the broadband frequency spectrum does not cause significant problems of the dermatovenerological profile. Taking into account the requirements of the order of the Ministry of Health and Social Development No. 417n, it was proposed to replace a dermatovenerologist with a psychotherapist in the composition of the commission for conducting medical examinations of office employees with a psychotherapist with the inclusion in the table «Frequency and volume of mandatory preliminary and periodic medical examinations of employees» of the Appendix of the order of the Ministry of Health No. 29n of laboratory and functional studies of a psychotherapeutic profile.

Article is devoted to the analysis of the Order of the Russian Ministry of Health of 19.03.2020 № 198n (an edition of 29.04.2020) “About a temporary order of the organization of work of the medical organizations for implementation of measures for prevention and reduction of risk of spread of a new koronavirusny infection of COVID19” regarding formation of the temporary staff list of the division of the medical organization providing medical care to citizens with a koronavirusny infection of COVID-19 and also the requirement with the staff of this division. The criteria recommended by the Ministry of Health of the Russian Federation for determining the number of rates of medical workers in these divisions, requirements for medical workers of the division have been considered, a practical example of calculation of rates has been given.

Purpose. To evaluate the clinical and functional results of the technique of transcleral fixation of the artificial iridochrustalic diaphragm during its decentration in a patient with aniridia and aphakia. Material and methods. Under observation was a 32-year-old patient K. with a diagnosis on the right eye: iridochrustalic diaphragm decentralization, posttraumatic aniridia, posttraumatic aphakia, who underwent a method of transcleral fixation of a displaced artificial iridochrustalic diaphragm developed at the Academician S. N. Fedorov Eye Microsurgery of the Moscow Ministry of Health of the Russian Federation. Results. Patient K. is subjectively satisfied with the visual functions obtained in the right eye. Conclusion. Thus, this clinical case demonstrates a very successful implementation of the fixation of the iridochrustalic diaphragm with its displacement to obtain satisfactory visual functions in the patient. This research analyses school physical evaluation forms obtained from one of Omsk public schools’ roll books as well as the physical development assessment of 820 students of this school. The findings uncovered a violation of the requirements of SanPiN, the provisions of the Russian Federation Ministry of Health orders regarding the maintenance of physical evaluation forms, the definition of medical groups for students in physical education, and conflicting information about the children’s health, which may cause severe clinical conditions in physical education classes. The noncompliance of the regulatory framework for RPN (in terms of the Health List) and the Russian Federation Ministry of Health (in terms of physical education regulation for children with accommodations) was also uncovered.

In 2019 The Ministry of Health of the Russian Federation together with experts of the National Research Center of Therapy and Preventive Medicine of the Ministry of Health of the Russian Federation has developed programs to strengthen the health of employees, which include behavioral medicine practices aimed at changing lifestyle and correcting risk factors. Scientific evidence shows the effectiveness of such programs, and new components of programs aimed at improving the well-being of employees can increase the effectiveness of preventive measures. In this article, we will review the domestic and international experience of using comprehensive health and well-being promotion programs to improve the health of employees in Russia.

Introduction. The combined impact of unfavorable factors of the production environment in miners leads to the development of associated pathology of the bronchopulmonary and cardiovascular systems, the predisposition to which depends on the individual susceptibility of the body. In this regard, it is important to comprehensively study the pathogenetic mechanisms underlying the formation and course of occupational and work-related diseases for a personalized approach to the diagnosis, prevention and therapy of this pathology. The purpose of this study was to study the mechanisms of damage to the cardiovascular system in miners with dust lung pathology on the basis of morphological and genetic studies. Materials and methods. For genetic studies, venous blood drawing was conducted in 190 Kuzbass miners. The main group included 126 miners of the main professions with the previously proven diagnosis "dust lung pathology", the comparison group consisted of 64 workers without a proven diagnosis working in similar sanitary and hygienic conditions. Morphometric studies were carried out using autopsy material obtained during 80 forensic medical examinations of miners in the Kemerovo region. All the miners were divided into 4 groups depending on their underground work experience. The control group was formed from 20 cases of forensic medical examinations of men who died in road accidents and did not have organ pathology according to the results of autopsies. Results. The study of the autopsy material revealed the presence of morphostructural changes in the vascular walls of the miners’ hearts in the form of hypertrophy of the smooth muscle cells of the medial layers, thickening of the endothelial lining, and the development of fibroplastic changes in the perivascular zones. These changes began to form from the first years of work in the underground conditions and progressed with increasing work experience contributing to the "recalibration" of the heart vessels with the formation of the lumen "obstruction". One of the mechanisms of endothelial damage in miners was a change in the expression of the EDN1 gene, which regulates the synthesis of endothelin-1. The risk and resistance genotypes of the development of dust lung pathology for the rs5370 polymorphism of the EDN1 gene were identified. Morphostructural rearrangement of the endothelium in the combination with its pathological activation contributed to the occurrence of endothelial dysfunction in miners. Conclusions. The conducted studies of the parameters of the vascular endothelium indicate its key role in the pathogenesis of bronchopulmonary and cardiovascular pathology in miners of the main professions. Getting into the body of workers, particles of coal-rock dust lead to morphostructural rearrangement of the cells of the endothelial layer and its pathological activation. The contribution of molecular and genetic mechanisms to the development of occupational lung pathology and associated diseases of the circulatory system in miners is revealed. Ethics. The studies were carried out in compliance with the ethical standards of the Bioethical Committee of the Research Institute for Complex Problems of Hygiene and Occupational Diseases, elaborated on the basis of the Helsinki Declaration of the World Medical Association "Ethical Principles for Conducting Human Scientific Medical Research" as amended in 2013 and the "Rules of Clinical Practice in the Russian Federation" approved by the Order of the Ministry of Health of the Russian Federation No. 266 dated 19.06.2003. All workers were informed about their participation in the molecular-genetic study and gave written consent to carry it out. The research of the dead miners was based on the secondary examination of blocks and ready-made histological micro-preparations of the material of the Bureau for Forensic Medical Expert Examination of the cities of Novokuznetsk, Osinniki, and Prokopyevsk. The study of pathomorphological material was carried out in accordance with the Federal Law of 21.11.2011, No. 323-FZ "On the Fundamentals of Health Protection of Citizens in the Russian Federation", in particular, with Article 67 "Carrying out pathological and anatomical autopsies", Federal Law of 12.01.1996, No. 8-FZ "On burial and funeral business" (Article 5, paragraphs 1, 2), as well as on the basis of the Order of the Ministry of Health of April 29, 1994, No. 82 "On the procedure for conducting pathological and anatomical autopsy" (Annex to the Order of the Ministry of Health and Medical Industry of 29.04.1994 No. 82), the Order of the Ministry of Health of Russia of 24.03.2016. No. 179n "On the rules for conducting pathological and anatomical examinations".

The article deals with the establishment, formation and activity of the Supreme representative (legislative) body of the Russian Federation. It is shown that the bicameral Parliament of the Russian state was formed under the influence of international, national political, legal and social development. It is noted that the essence of the bicameral Parliament is a legal reflection of the specifics of the development and state structure of Russia. The influence of reception and succession on the formation of the Federal Assembly of the Russian Federation is considered. The article analyzes the consequences of modern constitutional changes in the sphere of political and legal status of the Russian Parliament and their subsequent impact on the state mechanism of the Russian Federation.
