Background: To compare the frequency of various types of fractures of femur in patients less than and more than 40 years ofage in a tertiary care hospital. Design: Descriptive cross-sectional study. Setting: Emergency department. Period: January 2009 to April 2011.Method: A total of 1756 patients were divided into two groups with respect to age. Then they were further categorized according to the part offemur involved, and compared in both age groups. Results: The study was conducted on a total number of 1756 patients over a period of 2 yearsfrom 2009 to 2011. 51.1% patients were less than 40 years of age and 48.9% patients were more than 40 years of age. In patients more than 40years of age, pertrochanteric fracture was most common (86.40%). In patients less than 40 years of age, mid shaft of femur was most commonregion involved (87.02%). Conclusions: The incidence of proximal femur fractures is high in elderly patients because of several factors likeosteoporosis, visual impairment and cognitive disability. Whereas incidence of fractures of mid-shaft of femur is more common in youngpatients.

Objectives: To determine the frequency of hepatoma in patients with cirrhosis attending tertiary care hospital Karachi. Study Period: Six months July to December 2017. Study Design: Descriptive cross sectional study. Setting: Department of Gastroenterology, Liaquat National postgraduate Medical centre Karachi. Patients and Methods: The clinical history was taken from patients and ultrasound of liver was done to and mass if any and its size was noted. Noninvasive criteria for diagnosing hepatoma were utilized and all information was noted and entered in the proforma and analyzed in statistical software. Results: A total of 213 cases, 15.02% individual hepatitis B and 21.13% subject hepatitis C. Frequency of hepatoma in cirrhotic population detected as 10.33% (22/213). Rate of hepatoma was not significant among different age groups (p=0.202). Rate of hepatoma was also not significant between male and female (p=0.59). Conclusion: Persistent chronic viral hepatitis B and C infection with cirrhosis responsible for acquiring hepatoma.

Background: Under nutrition is a global public health problem. There are numerous methods of assessment of malnutrition among which CIAF is the most recent, relatively robust since it envisages all the parameters for estimation of nutritional status of the children. So, this study was conducted to find the rates of composite index of anthropometric failure in the children attending the Immunoprophylaxis clinic in a tertiary care hospital of Nagpur and to find associations with the socio demographic variables.Methods: This descriptive cross-sectional study was conducted at the Immunoprophylaxis clinic of a tertiary care institute in Nagpur from September 2016 to January 2017. The study subjects were the children attending the OPD of Immunoprophylaxis clinic of a tertiary care hospital. Necessary permissions were taken before the start of the study. Data was collected using pretested and pre-designed questionnaire. Height and weight were measured using standard guidelines and categorized into groups of CIAF as described by Nandy et al.Results: A total of 460 study subjects were enrolled in the study. The mean age group of the study subjects was 2.47±1.56 years. According to the grades of CIAF, 48.51% were having no failure. Age and gender were factors which were associated with failure status by CIAF.Conclusions: We found high rates of anthropometric failure in our study. The age groups less than 3 years had higher odds of being in failure when compared to more than 3 years age. Further, males had higher odds of being into failure when compared to females. Community based studies are recommended.

Objectives: To determine the frequency & severity of liver involvement in dengue infection among adults at tertiary care hospital. Period: Six months (9th May 2016 to 9th Nov 2016). Study Design:  Cross-sectional study. Setting: Department of Gastroenterology and Department of Medicine Liaquat National Hospital, Karachi. Patients and Methods: Results: A total of 343 patients with Dengue fever were selected to conduct this study with mean age of 34.67±9.09 years. Mean duration of the fever was 7.31±1.62 days. Mean duration of hospital stay of the patients came out to be 3.50+0.82 days. Severity of liver enzymes derangement was mild in 80 patients (23%), moderate in 246 (72%) & severe in 17(5%). Mortality was seen in 14% (47 patients) & 100% mortality was seen in dengue fever patients with severely deranged Liver enzymes. Conclusion: Dengue fever patients with deranged liver enzymes had statistically worse outcome thus can lead to early recognition of high risk cases.

This study is designed to assess the mortality in OPP patients with low GCS scores. Study Design: Cross Sectional study. Setting: Jinnah Postgraduate Medical Center. Period: From 1st February 2018 to 31st August 2018. Material & Methods: Included patients with low GCS, both genders and age of 18-60 years diagnosed with OPP. Their GCS was calculated and the outcome was determined in terms of mortality. Results: Out of 70 patients, the mean age was 35.2+16.5 years with the majority (62.9%) <30 years. 62.9% of these were males. Most (57.1%) of them presented after ingestion of <15mL of OP, 60% had the poisoning for >60 minutes, and the majority (74.3%) had taken it orally. When the GCS was computed, 81.40% had that of >5, while the mean came out to be 6.64 ±1.43. The mortality rate here was 17.10%. Conclusion: GCS toll can be a helpful and practical tool in assessing the mortality among the patients of OPP. However, because of the limited literature on the subject further studies are recommended to improve its validity.

Background: Refractive error is one of the most common causes of the visual impairment and second leading cause of treatable blindness. The objective of the study was to determine the mag­nitude of refractive errors in children. Methods: This was a hospital based cross-sectional study conducted on 254 children attending Ophthalmology OPD of Manipal Teaching Hospital, Pokhara. The children whose visual acuity was worse than 6/6 but improved with pinhole were included in this study. Vision test, retinoscopy and subjective refraction was done in all subjects and cycloplegic refraction was done when needed. Statistical analysis was carried out using Epi-info version 7. Results: The commonest type of refractive error was astigmatism (46.06%) followed by myopia (42.31%). Majority of children had low grade of refractive errors (46.85%). Among the children, “with the rule astigmatism” was maximum (27.56%). Majority of children were in the age between 11 to 15 years (77.95%). The refractive error was seen more in female (63.78%). Among the chil­dren of refractive errors, 29.13% had family history, 33.46% had given the history of wearing spec­tacles and 10.24% children had amblyopia. There was statistically significant association between refractive errors and age groups, history of wearing spectacles, amblyopia and grading of refractive errors. However, there was no statistically significant association of refractive error with gender, residence and family history. Conclusions: Astigmatism was the common type of refractive error followed by myopia. This study emphasizes the importance of detection of refractive error in children.

Background: Japanese encephalitis (JE) vaccination in India started in 2006 with SA-14-14-2 live attenuated JE vaccine (JEV) following large outbreaks of JE in some districts of Eastern Uttar Pradesh and Bihar in 2005. Age groups 1-15 yrs are first vaccinated with a single dose of JEV in a campaign mode followed by integration of this vaccine in routine immunization. It is beyond doubt that added to vaccination campaigns, proper awareness on JE can play significant role in controlling the disease.Methods: An observational study with cross sectional design was conducted in Kolkata Medical College and Hospital, Kolkata during JE Vaccination campaign during January, 2018 among 85 respondents, to assess the awareness on JE, among care-givers who brought their children for vaccination at the immunization clinic.Results: It was found that only 37.6% the respondents attending the campaign knew the name of the disease; 17.6% respondents were aware about disease transmission, and 5.9% could state two or more clinical features that might be associated with Japanese Encephalitis. 69.4% had no knowledge of up to what age JE vaccines can be administered; 23.5% said it can be administered till the beneficiaries attain fifteen years of age.Conclusions: IEC activities during JE vaccination campaign was not able raise awareness on JE to the desired level. However beneficiaries were informed about service availability and could be mobilised to come for vaccination.

The methicillin resistant Staphylococcus aureus (MRSA) is affecting the economy worldwide contributing to the increased morbidity and treatment costs. The present cross sectional study was conducted to determine the association of MRSA with kidney and liver patients from Tertiary Care Hospital in Lahore, Pakistan and its antibacterialresistance patterns. The study was conducted for the period of one year; from April 2019 to April 2020. A total of 1984 different samples were obtained and examined for bacterial cultures using standard procedures. From total samples, 51.05% were collected from male and the rest 48.95% were collected from females including different age groups. Samples were inoculated on different culture media for isolation of S. aureus and susceptibility testing. A total of 253/1984 clinical samples exhibited S. aureus characteristics while 39 of 253 S. aureus (15.41%) were identified as MRSA. MRSA strains were found highly resistant to Amikacin (AK), Cefoxitin (FOX),Clindamycin (DA), Fusidic acid (FD), Gentamicin (CN) and Tobramycin (TOB) while highest sensitivity was noted against Vancomycin (100%) and Linezolid (100%). The presence of MRSA and multidrug resistance pose serious therapeutic burden to critically ill patients. A systematic and concerted effort is essential to identify high-risk patients rapidly and to reduce the burden of antimicrobial resistance.

Background: In Nepal pesticide poisoning is a common problem and organophosphorus poisoning (OP) is the most common form of acute poisoning responsible for majority of deaths. However, sufficient data is still lacking from Western region of Nepal. The present study analysed the sociodemographic factors of OP poisoning cases, the association of severity based on admission clinical score (POP scale) and lag time with mortality.Methods: This is a cross-sectional study conducted in patients of organophosphorus poisoning, admitted to our casualty ward during a period of one-year June 2015 to 2016.Results: In this study incidence of OP poisoning was more in females 51 (65.38%) compared to males 27 (34.46%). The commonest age group involved in poisoning was 21-30 years 30 (38.4%). Housewives 32 (41.02%) were the commonest victims of poisoning. Incidence was high during rainy season 34 (43.58%) and mainly during late hours 27 (34.61%) of the day. Majority 24 (30.76%) cases reached hospital within 2 hours of poison intake. Methyl parathion (Metacid) 24 (30.76%) was the commonest OP compound consumed by the victims. Suicide 62 (79.48%) was the main motive of poison intake and financial problem (37.17%) was the main reason behind poison ingestion. In majority of the cases the sign and symptoms were mild (80.76%) in severity. In this study majority of the victims survived (9.30%) with prompt and appropriate treatment.Conclusions: The numbers of OP poisoning cases are increasing every year and poisoning is seen commonly in younger age groups. Therefore, strict legislature on the availability of OP compounds, preventive measures and appropriate health education should be introduced to decrease the incidence.

Background: Intravenous infusions (IV) are a rapid mean of administering drugs. Hospitals are commonplace for their use, but at the same time they have many serious potential complications. This study aims to know the justified use of intravenous infusion among patients of a tertiary care hospital and awareness about its use.Methods: It was a cross-sectional study in which 249 subjects were conveniently studied. Nil per oral, severity of dehydration, vomiting, and diarrhoea was used as confirmatory criteria for using IV Infusions. Questionnaire with verbal consent was used to collect the data. Descriptive statistical analysis was applied to analyse the data and presented as means, frequencies and percentages in the form of tables and figure.Results: Total n249 (100%) subjects, n68 (27.3%) males and n181 (72.7%) females with average age was 24.05±14.21 years participated. Among n116 (46.6%) irrational users, females n76 (30.5%) were more then males n40 (16.1) with significant difference, p= 0.018. However, no significant difference was found among other variables (age groups, profession, education, awareness and rational or irrational use of IV infusion). Majority n249, n204 (81.9%) were not aware of the IV infusion medication error and doctors were most common unjustified prescribers of IV infusion, n105 (90.5%).Conclusions: The study concluded with the more prevalent irrational IV infusion use in our setup. Doctors appeared the dominant prescriber of irrational IV infusion. This statement is astonishing and must not be overlooked. Female gender, adult age, under-grade patients and students are more exposed to irrational IV infusion.
