Objectives: To determine the mean serum electrolyte level in neonates with perinatal birth asphyxia with moderate hypoxic ischemic encephalopathy. Study Design: Descriptive Cross Sectional study. Setting: Pediatric Ward, Emergency and OPD, Allied Hospital, Faisalabad. Period: December 2017 to May 2018. Material & Methods: In our study, out of 190 cases were included, Results: mean g. age was calculated as 39.44+1.51 weeks, 49.47% (n=94) were male and 50.53% (n=96) were females. Mean APGAR score at 1 minute was calculated as 4.59+0.49 whereas 5.41+0.67 at 5 minutes. Mean weight of the neonates was calculated as 2771.58+256.83 grams. Frequency of delivered through history of fetal distress was recorded in 43.68% (n=83), mean serum electrolytes shows that serum sodium 137.71+1.69 mg/dl, serum potassium 3.92+0.18 mg/dl and serum calcium 8.45+0.51 mg/dl. Conclusion: We concluded that mean serum electrolyte level in neonates with perinatal birth asphyxia with moderate hypoxic ischemic encephalopathy were in normal range, however, in absence of control group and data analysis in qualitative way, we are unable to record the frequency of hypocalcemia, hyponatremia and hyperkalemia, which may be done in coming trials.

<b><i>Introduction:</i></b> There is a paucity of studies examining temporal trends in the incidence and mortality of moderate-to-severe hypoxic-ischemic encephalopathy (HIE) during the last decade of therapeutic hypothermia (TH). <b><i>Methods:</i></b> Multicenter cross-sectional study of all infants ≥35 weeks gestational age diagnosed with moderate-to-severe HIE within 6 h of birth in an extensive region of Spain between 2011 and 2019, in order to detect trend changes over time in the (1) annual incidence, (2) severity of neurological and systemic organ involvement, and (3) neonatal death from HIE. <b><i>Results:</i></b> Annual incidence rate of moderate-to-severe HIE was 0.84 (95% confidence interval [CI] 0.7–0.97) per 1,000 births, without trend changes over time (<i>p</i> = 0.8), although the proportion of severe HIE infants showed an average annual decline of 0.86 points (95% CI 0.75–0.98). There were 102 (70%) infants diagnosed with moderate HIE and 44 (30%) with severe HIE. TH was offered to 139/146 (95%) infants. Infants with clinical and/or electrical seizures showed a decreasing trend from 56 to 28% (<i>p</i> = 0.006). Mortality showed a nonstatistically significant decline (<i>p</i> = 0.4), and the severity of systemic damage showed no changes (<i>p</i> = 0.3). Obstetric characteristics remained unchanged, while higher perinatal pH values (<i>p</i> = 0.03) and Apgar scores (<i>p</i> = 0.05), and less need for resuscitation (<i>p</i> = 0.07), were found over time. <b><i>Conclusion:</i></b> The annual incidence of moderate-to-severe HIE has stabilized at around 1 per 1,000 births, with a temporal trend toward a decrease in severe HIE infants and a slight decline of mortality. No association was found between temporal trends and changes in perinatal/obstetric characteristics over time.

Background: To study indications, intraoperative and postoperative complications and fetomaternal outcome in cesarean sections done at full dilatation.Methods: This is a prospective cross-sectional study which was conducted in the department of obstetrics and gynecology, RNT medical college, Udaipur from November 2018 to April 2019. 100 cases of caesarean sections at full dilatation which were performed during this period were analyzed for indications and maternal and fetal morbidity.Results: Among these 100 cesarean sections, majority of cases were in the age group of 21-30 years (46%), booked and  Primigravida(81%).Most common indications were cephalo-pelvic disproportion (27%) and fetal distress (21%). Most commonly baby was delivered either by vertex (44%) or by Patwardhan (31%). Intraoperative complications were higher in terms of hematuria in 41%, Atonic PPH in 35%, uterine incision extension in 28% of cases. In one case bladder injury was noticed. Increased incidence of post-operative febrile illness and wound infection were noted. 44% baby’s required nursery admission, most commonly due to birth asphyxia (16%) and RDS (11%).Conclusions: Cesarean section in the 2nd stage of labor is associated with significantly increased maternal morbidity, Neonatal morbidity and mortality. So proper monitoring during labor and involvement of skilled obstetrician in decision making and delivery is crucial to minimize fetomaternal complications.

Background: Studies in high-income countries have reported that school-aged children who survive neonatal jaundice (NNJ) and hypoxic-ischemic encephalopathy (HIE) develop long-term neurocognitive problems. However, less is known about the patterns of functioning in school-aged survivors of NNJ and HIE in sub-Saharan Africa. This study examined patterns of functioning in school-aged children who survived NNJ and HIE in Kilifi, Kenya. Methods: This is a cross-sectional study that included 107 survivors of NNJ/HIE (64 with NNJ, 43 with HIE), aged 6-12 years, admitted to Kilifi County Hospital on the Kenyan Coast. The Gross Motor Function Classification System (GMFCS), Adapted Communication Profile, Raven’s Coloured Progressive Matrices (RCPM) and an epilepsy screening tool were used to assess gross motor function, communication function, intellectual functioning, and epilepsy, respectively. Results: Most of the survivors of NNJ (95.2%) and HIE (95.3%) had no impairments in gross motor functioning. A small percentage of the children in the NNJ and HIE groups had profound problems in their communication (4.7% and 4.7%); expressive communication function (4.7% and 4.7%); social functions (3.1% and 2.3%); receptive communication (4.7% and 2.3%); and communicative effectiveness (4.7% and 2.3%). Cognitive impairment was reported in 10.9% and 11.9% for NNJ and HIE survivors, respectively. Active epilepsy was detected in 1.6% of survivors of NNJ and 2.3% of survivors of HIE. All children had normal hearing and visual functioning except one participant who presented with mild visual acuity problems. Conclusions: Most school-aged children who survive with NNJ and HIE have normal motor and communication function; however, one in ten are likely to present with lowered intellectual functioning compared to the normative sample.

Introduction: The definition of birth asphyxia is given by the ACOG, AAP includes the existence of 3factors: Metabolic or mixed acidemia (pH<7) which is determined by umbilical cord arterial bloodsamples. The major causes of neonatal seizures are Hypoxic-ischemic encephalopathy (HIE), whichrepresents about 50% of the causes of neonatal seizures. Metabolic abnormalities, infection,intracranial hemorrhage, developmental anomalies, and other like inborn errors of metabolism areother causes of neonatal seizures. Material and Methods: A cross-sectional study. This is ahospital-based prospective study conducted at Nehru Hospital, BRD Medical College, Gorakhpur (UP)in the Neonatal Intensive Care Unit of the pediatric department. This study was conducted from July2018 to July 2019. Result: Prevalence of Hypocalcemia was 7.6%, Prevalence of hypernatremia was1.3%, Out of 315 patients 32 (10.2%) patients expired and 283 (89.8%) was successfullydischarged. Conclusion: No association between Hypocalcemia and Hypomagnesemia Prevalence ofhyponatremia was 6.3% and no association between hyponatremia and Hypomagnesemia. Theprevalence of hypernatremia was 1.3% and there was no association between hypernatremia andHypomagnesemia.

Introduction: Electrolytes are very essential for normal functioning of vital organs including heart. Electrical activity of the heart is regulated by Na, K and Ca. Normal homeostasis of these electrolytes are vital for normal functioning of heart. Electrolyte imbalance is often common after episode of acute coronary syndrome (ACS) and electrolyte imbalance may have important role in altering the prognosis of ACS. Aims & Objectives: To assess the serum electrolytes imbalance (Na, K and Ca) in ACS patients within first 48 hours of onset of symptoms. Materials and Methods: This hospital based cross sectional study conducted on 50 cases of ACS and 50 healthy controls. Serum Na, K and Ca (total) concentration were estimated in healthy controls and newly diagnosed cases of ACS within 48 hours of onset of symptoms. Result: Mean value of serum Na, K and Ca (total) in case and control group were 138±4.23 and 140±1.99 mEq/L (p=0.0018), 3.87±0.43 and 4.16±0.35 mEq/L (p=0.0006), 8.85±0.59 and 9.68±0.44 mg/dL (p<0.0001) respectively. Hyponatremia, hypokalaemia and hypocalcemia were found in 14% (n=7), 12% (n=6) and 18% (n=9) of ACS patients respectively. Conclusion: There were statistically significant lower levels of serum Na, K and Ca (total) found in ACS patients compared to healthy controls.

A history of severe asphyxia is a risk factor for thesebrain problems in infants.Objective To evaluate the prevalence of abnormal EEGs infull term neonates and to assess for an association with severeasphyxia, hypoxic ischemic encephalopathy (HIE), and spontaneousdelivery.Methods This cross-sectional study was conducted at thePediatric Outpatient Department of Sanglah Hospital, Denpasar,from November 2013 to January 2014. Subjects were fullterminfants aged 1 month who were delivered and/or hospitalized atSanglah Hospital. All subjects underwent EEG. The EEGs wereinterpreted by a pediatric neurology consultant, twice, with aweek interval between readings. Clinical data were obtainedfrom medical records. Association between abnormal ECG andsevere asphyxia were analyzed by Chi-square and multivariablelogistic analyses.Results Of 55 subjects, 27 had a history of severe asphyxia and 28were vigorous babies. Forty percent (22/55) of subjects had abnormalEEG findings, 19/22 of these subjects having history of severeasphyxia, 15/22 had history of hypoxic-ischemic encephalopathy(HIE), and 20/22 were delievered vaginally. There were strongcorrelations between the prevalence of abnormal EEG and historyof severe asphyxia, HIE, and spontaneous delivery.Conclusion Prevalence of abnormal EEG among full-term neonatesreferred to neurology/growth development clinic is around40%, with most of them having a history of severe asphyxia. AbnormalEEG is significantly associated to severe asphyxia, HIE, andspontaneous delivery.

Background: Rising rates of cesarean delivery is becoming a concern to developing countries. Though cesarean section is a lifesaving surgical intervention, the rise in rates has resulted in significant health problems of mothers and newborns. Objective: The aim to undertaking this research is to know the prevalence, indications and outcome of cesarean section delivery In Jugal Hospital, Harari regional state, Ethiopia.Methods: Hospital based record review cross-sectional study was conducted in Jugal Hospital, Harar, Ethiopia from September 1, 2015 to September 1, 2019. Trained data collector and the principal investigator collected data employing a pre-tested extraction format. After checking the filled extraction format for completeness, data was entered, cleaned and analyzed using SPSS software Version 20.Results: In five years’ time (2015-2019), there were 6,111 deliveries, of which 1,817 were delivered by cesarean section which gives the overall rate of 29.7%. The most common indications for cesarean section were Cephalo-pelvic Disproportion (CPD) 323 (17.78%) followed by fetal distress 292(16.07%) and the least indication was pre-eclampsia 9(0.5%) and the most cause of prenatal death was birth asphyxia 38% followed by prematurity34%.Conclusion: In this study, there is a steady increase in the rates of cesarean delivery in five years’ time, the overall rate of C-section in Jugal Hospital was 29.7%. This rate is higher than the World Health Organization standard. Policies and efforts aimed at decreasing the unnecessary cesarean sections should be promoted and implemented at each health facility.

Background: Thyroid hormones influence nearly all major metabolic pathways. Their most obvious and well-known action is the increase in basal energy expenditure obtained by acting on protein, carbohydrate and lipid metabolism. The lipid metabolism is more influenced by the thyroid hormone. Methods: A cross-sectional study was conducted on 100 patients with suspicion of thyroid disorders were taken as cases. One hundred patients with normal thyroid profile and no history of other chronic diseases were taken as control group. Results: The serum TC, TG and LDL levels in hypothyroid individuals (both overt and subclinical) were significantly higher than euthyroid subjects but the levels were comparable between hyperthyroid and euthyroid group. Conclusion: Dyslipidemias are associated with thyroid disorders, so biochemical screening for thyroid dysfunction in all dyslipidemic patients. Therefore, patients presenting with dyslipidemia are recommended for investigation to explore thyroid dysfunction. Keywords: Thyroid profile, Total cholesterol, Triglycerides and LDL

Background: Noise pollution or noise is an unwanted sound which is disturbing to human beings.However small or soft the sound, if it is undesirable it is considered as noise. Noise induced hearingloss is a sensorineural hearing loss that is commonly encountered second to presbycusis. Purpose: Toknow the effect of traffic noise exposure on hearing impairment to the employees of the Parking DistrictCompany of the Denpasar city and to improve diagnostic detection on hearing impairment caused bynoise. Method: A cross sectional study was conducted at the Parking District Company office. Thepopulations of this study were the employees of the Parking District Company. Samples of this study were the employees who were exposed to traffic noise and control samples were an employee who was unexposed. Samples were selected by simple random sampling. Results: From 40 parking attendants,27 persons (67.5%) aged above 35 years old. The parking attendants who had been working for ten to fifteen years were 36 persons (90%) and no history using ear protection when working. Seven persons(17.5%) had referred DPOAE upon examination with increase hearing threshold on audiogram result.In this study the parking attendants who had hearing deficit induced by noise were 7 persons (17.5%)and only one person (2.5%) in control group. There was a statistically significant effect of traffic noiseto hearing function deficit (p<0.05). Conclusion: Traffic noise has effect in hearing function deficit onthe parking attendants.ORLI Vol. 40 No. 2 Tahun 2010Key words: NIHL, parking attendant, audiometry, DPOAE.
