General hospital Dr. H. Soewondo Kendal is a local government hospital. Improving medical support services, especially the quality of medical record archiving services, is essential to maintain the quality of hospitals in the context of good hospital standards. This study aims to predict the need for archive shelves between 2015-2019. The type of research used is descriptive with accidental sampling technique. Patient medical records were collected using a systematic sampling technique. The observation results showed that the average thickness of medical record documents was 0.85 cm based on measurements on 30 samples of medical record documents. The hospital has 48 shelves with 5 sub-shelves on each shelf. The length of the available archives is 525 cm and by 2024 a total of 37 shelfs will be needed. Modeling results based on linear regression equations have decreased medical record documents since 2015-2019 y = - 782.9x +30636. However, the linear regression validation for the cumulative medical record documents for 2015-2019 shows an increase based on the regression equation y = 28439 x + 1112.2, therefore it is necessary to immediately save the medical record document in an active state. The conclusion delivered by Dr. H. Soewondo Kendal does not need an additional archive shelf, but it does require inactive document storage.

The quality of medical records in hospitals also determines the quality of service, completeness of writing Medical Records documents correctly and correctly is very important. The purpose of this study was to analyze the level of compliance of hospital human resources in writing the status of the inpatient Medical Record Dr. Sayidiman Magetan Regional Hospital and the factors that influence it. The design of this study was an observational quantitative study with a cross section approach with the focus of the research directed to be analyzing the level of compliance of hospital human resources in writing the status of the inpatient Medical Record Dr Sayidiman Magetan Regional Hospital and the factors that influenced it with a sample of 192 respondents taken with the Simple Random Sampling technique. The findings found that most of the respondents have high motivation that is 144 respondents (75%). Most of the respondents care to write in the medical record that is 160 respondents (83.3%). Most of the respondents have a high appreciation of 136 respondents (70.8%). Most of the respondents did not comply doing medical record writing of 107 respondents (55.7%). Based on the Linear Regression analysis the motivation variable on compliance p-value 0.015 <0.05, the variable concern for compliance p-value 0.025 <0.05 then H0 is rejected so there is the influence of motivation and concern for compliance with medical record writing by health professionals in Regional General Hospital Dr. Sayidiman Magetan. Linear regression variable rewards for compliance shows that the p-value of 0.665> 0.05 then H0 is accepted so it is concluded that there is no effect of rewards on compliance with writing medical records by health professionals at the Dr Sayidiman Magetan Regional General Hospital. It is expected that respondents can comply to fill out medical records so that the delivery of care to passion can be well integrated

Introduction: The medical record documentation of patient treatment Provides the which in turn, must be maintained Clearly, concisely, comprehensively and accurately. Medical record and its filling criteria must be based on the regulation of the Minister of Health of The Republic of Indonesia No. 269/Menkes/Per/III / 2008 regarding to the medical record. The research was Aimed to unveil the completeness of both criteria and filling on medical records at the General Hospital’s Dental Polyclinic of Cianjur District. Methods: Survey-based descriptive method was applied within the research. Its Data was acquired through the examination on medical records and interviews. Random sampling was conducted to run the sampling technique. 89 pieces of outpatient’s medical records were embodied as samples. Results: Based on the research results, it is discovered that 6 out of 12 criteria (50%) are not listed within the medical record. Thus, the filling on medical records of 100% is found incomplete. Conclusion: Medical records Dental Clinic Regional General Hospital Cianjur according to standards Permenkes No. 269/2008 not inlude on complete criteria according to standards Permenkes No. 269/2008.

Background: Quality of hospital services can be seen from the bed usage. Statistical analysis of efficiency bed usage can be mesured based on inpatient medical records. To determine the efficiency requires four parameters namely bed occupancy rate (BOR), average length of stay (ALoS), turnover interval (TI), and bed turnover (BTR). parameters can be presented using Graphic Barber Johnson. This study aims to determine the efficiency of bed usage at Semen Padang Hospital in 2017.Methods: This research was conducted at Semen Padang Hospital, West Sumatera, Indonesia from January to December 2017. The study used a descriptive method with a qualitative approach. The data was collected from medical records department. The population is all abstraction data of in-patient medical record in 2017, 9796 medical record used total sampling technique. Data analysis was performed by calculating the values of ALoS, BOR, BTR, and TI. Data will be presented based on graphic Barber Johnson. Excel 2010 and graphic Barber Johnson method were applied for data analysis.Results: Number of daily inpatient censuses in 2017 are 31227 and number of service days are 31362. Number of beds 144. Statistical analysis results obtained total BOR 60%, BTR 67 times, TI 2 days and ALoS 3 days. The highest value of bed occupancy rate is 66% on August.Conclusions: Based on statistical, value of bed occupancy rate (60%) and turnover interval (2 days) are efficient at Semen Padang Hospital in 2017. Average length of stay (3 days) and bed turnover rate (67 times) are not efficient.

Abstract Internal patient transfer is the process of transferring patients from one room to another in a hospital while still being oriented towards quality and patient safety. The results of the observations showed that the filling of the internal patient transfer form was not optimal so that there were still incomplete forms due to the large number of patients and the weak coordination between health workers. To determine the effect of the completeness of filling out the internal patient transfer form on the quality of medical records. The research method used is quantitative with a descriptive approach. Data collection techniques used are observation, questionnaires and literature study. The sampling technique was simple random sampling technique so as to obtain a sample of 91 internal patient transfer forms. From the results of observations, 22% of the internal patient transfer forms were found that were not completely filled in, especially in the signature and clear name. The two variables have a strong relationship. The effect of the variable completeness of the internal patient transfer form on the medical record quality variable is 90.1% and the remaining 9.9% is influenced by other factors. Based on these studies, it can be concluded that the lack of accuracy and coordination of nurses, doctors and other officers in filling out internal patient transfer forms so that this affects the quality of medical records in the aspect of accuracy. Hasil observasi menunjukkan bahwa belum optimalnya pengisian formulir transfer pasien internal sehingga masih terdapat formulir yang tidak lengkap disebabkan oleh faktor dari banyaknya pasien dan lemahnya koordinasi antara tenaga kesehatan. Untuk mengetahui pengaruh kelengkapan pengisian formulir transfer pasien internal terhadap mutu rekam medis. Metode penelitian yang digunakan yaitu kuantitatif dengan pendekatan deskriptif. Teknik pengumpulan data yang digunakan adalah observasi, kuesioner dan studi pustaka. Teknik pengambilan sampel adalah teknik simple random sampling sehingga memperoleh sampel sebanyak  91 formulir transfer pasien internal. Dari hasil observasi ditemukannya formulir transfer pasien internal yang belum terisi lengkap sebanyak 22% terutama pada tandatangan dan nama jelas. Kedua variabel memiliki hubungan yang kuat. Pengaruh variabel kelengkapan formulir transfer pasien internal terhadap variabel mutu rekam  medis sebesar 90,1% dan sisanya 9,9% dipengaruhi oleh faktor lain. Berdasarkan penelitian tersebut dapat disimpulkan bahwa kurangnya ketelitian dan koordinasi perawat, dokter dan petugas lain dalam pengisian formulir transfer pasien internal sehingga hal ini mempengaruhi mutu rekam medis pada aspek keakuratan. Newborn asphyxia is a condition where the baby is born can not immediately breathe spontaneously and regularly after birth (Wiknjosastro, 2007). AKB in Kendari City in 2014 as many as 26 people out of 6,228 newborns with the incidence of asphyxia in newborns in Kendari City General Hospital in 2015 as many as 78 people out of 950 newborns and the number of infants experiencing serotinus as many as 40 babies (Medical Record of Kendari City Hospital, 2015). Objective: To determine the relationship between serotinus and the incidence of asphyxia in newborns at the Kendari Regional General Hospital in 2015. Research methods: The type of research used was analytic with the design of Case Control. The sampling technique was purposive sampling, in which all asphyxial infants were taken as a case. The systematic sampling technique was random sampling, where all non-asphyxial infants were numbered, and from 872 non-asphyxial infants divided by the number of controls taken 872: 78 = 11.2, so that the sample for control is a multiple of 11. The results of the study: based on data analysis obtained results, namely from 78 infants with asphyxia there were 10 people (12.8%) babies born with serotinus. The value of X2counts 5,778> X2 table 3,841 so that H0 is rejected and Ha is accepted with OR = 5.588 (p= 0.032). Conclusion: There is a relationship between serotinus and the incidence of asphyxia in newborns in Kendari Regional General Hospital in 2015

Introduction. The incompleteness of medical records is a significant problem that affects the quality of health care services in many hospitals of Ethiopia. Improving the completeness of patient’s records is an important step towards improving the quality of healthcare. Methods. Pre- and postintervention study was conducted to assess improvement of inpatient medical record completeness in Menelik II Referral Hospital from September 2015 to April 2016. Simple random sampling technique was used. Data was collected using data extraction checklist and independent sample t-test was used to compare statistical difference that exists between pre- and postintervention outcomes at confidence interval of 95% and P value less than 0.05 was considered statistically significant. Result. The overall inpatient medical record completeness was found to be 84% after intervention. An enhancement of completeness and reporting of inpatient medical record completeness increased significantly from the baseline 73% to 84% during postintervention evaluation at P value < 0.05. Conclusion and Recommendation. The finding of this project suggests that a simple set of interventions comprising inpatient medical record format and training healthcare provider showed a significant improvement in inpatient medical record completeness. The Quality Officer and Chief Executive Officer of the study hospital are recommended to design and launch intervention programs to improve medical record completeness.

The quality of a good medical record must meet the indicators of completeness and accuracy of filling, timeliness in safety and provision of medical records that meet legal requirements. The purpose of this study was to analyze the influence of procedures and supervision on the quality medical records for inpatient. This research is survey research with explanatory research design. The research was conducted at Imelda Hospital. The research population was 238 people and the sample was 100 people. The sampling technique is stratified random sampling which divides the population into homogeneous groups (strata) proportionally: 21 doctors, 65 nurses, and 14 medical records officers, from each stratum a random sample is taken. Data analysis using univariate, bivariate with the chi-square test, and multivariate using multiple logistic regression tests. The results showed that the procedure factor had no effect on the quality of medical records at the Imelda Hospital, p-value 0.520> 0.05. The controlling factor influences the quality of medical records at Imelda Hospital, p-value 0.001 <0.05. The hospital implements routine surveillance with evaluations every week, every month, every six months, and every year so that medical records become of high quality, that is, medical records are complete, accurate, returned on time, and meet legal requirements. In the current era of global competition, it requires every hospital as a health service facility to be able to provide quality services in order to foster patient loyalty as service users. The medical record is one of the medical support services which is the basis for assessing the quality of medical services. Completeness of medical record files in RSUD Syekh Yusuf Kab. Gowa in a period of three years has fluctuated, namely 20% in 2017, 66% in 2018, and decreased in 2020 to 17%. This study aims to determine the description of medical record data management at RSUD Syekh Yusuf Kab. Gowa 2019.This study used a qualitative descriptive research method with the selection of informants using a purposive technique, and 6 informants were obtained, including 4 medical record officers, 1 head of the inpatient room, and 1 head of the medical records department.The results of the interview showed that the personnel in the medical records department were deemed insufficient, the flow and SOP were not implemented, the facilities and infrastructure were inadequate. In the implementation of medical records, there are still files that are filled in incompletely which results in delays in making reports.It is hoped that the hospital management will increase the number of personnel so that there is no double burden on officers, provide training for medical record officers, and pay attention to facilities and infrastructure to switch from a conventional system to an electronic-based system.

The purpose of this study was to determine the factors that affect the quality of life of CRF patients in Hemodialysis Unit at Pringsewu District General Hospital. This study was conducted from February to May 2018 with cross sectional study design. The sampling technique using total sampling technique. Measurement of quality of life using KDQOL-SFTM version 1.3. The results found that quality of life scores were quite low in some domains and subscales. The mean of total score was 55.70 ± 21.30 with mean of Physical Health Composite (PHC) = 38.85 ± 9.26 and mean of Mental Health Composite (MHC) = 36.13 ± 7.08. Regarding the targeted area of ESRD, the scale of renal disease burden and occupational status scale resulted in the lowest score. The sleep quality scale score was 56.18 ± 20.72. Only 61 patients responded to questions of sexual activity with a score of 55.53 ± 27.44 on the scale of sexual function. In the 36-item health survey, the mean total score was 45.90 ± 21.95. The lowest score represented the limitations of roles caused by physical and emotional health problems. The result of statistical test showed that the variables significantly related to the quality of life of CRF patients were age, income, duration of hemodialysis and family support. Thus, family support was the variable that had the greatest impact on determining the quality of life of CRF patients. The CRF patients who lacked family support were 4.6 times more likely to lead poorer life compared to CRF patients who received good family support after being controlled by age, income, duration of hemodialysis, gender, working status, and diabetes mellitus variables.

This study's intended objective is to evaluate the effect of the work capability on the welfare of the apparatus and work facilities and its impact on the efficiency of the Mamuju police force in the province of West Sulawesi. This study uses a causal research design that illustrates a causal or causal association in the variables examined and used a quantitative approach. In this report, all the Mamuju Police staff, the West Sulawesi Province, were as many as 95. The sampling technique was a non?probability sampling process of systematic sampling, which implies that the samples were taken according to the order of the counted population members. Data collection was carried out by observation, distribution of questionnaires, interviews, and documentation. This review analyses and processes data with multiple linear regression using SPSS ver. 25. All the hypotheses suggested in this study are acceptable. The variables of the level of welfare and work facilities are critical variables that can influence staff's workability to enhance their performance. The story of welfare is necessary to inspire workers to function more efficiently; the more affluent the team, the more likely they are to increase their efficiency. Work facilities indicate that the types of facilities given by the company have facilitated an improvement in officers' efficiency, that the needs of the apparatus provide an appropriate standard of facilities, not just to facilitate their work, but also to promote satisfaction and ease of work.
