Nurul SDIT Istiqlal Klaten an Islamic-based elementary school that has a high awareness of the importance of the monitoring and evaluation of student learning in order to know the level of success of the learning process is running. In order to maximize the process it created an information system of monitoring and evaluation of student learning web-based and SMS-gateway. This information system can display a list of values of each student in each subject and grade of teaching a teacher with grouping based on the criteria of the KKM. Moreover, it can display a graph of the percentage of the value of grouping results. Values that have been added can be disseminated to parents / guardians of the students. This information system will display a graph the value of each student for all four test scores. System design using the Context Diagram, Hierarchy Process Input Output, Data Flow Diagrams, input design, output design, and database design. The result of this research is the information system of monitoring and evaluation of student learning which has an input in the form of student data, teacher data subject, the data homeroom, the data subject, class data, the data teacher, materials, values and information. While the results of the output of a graph of the results of grouping value, sms information listed value value with KKM and the average grade and disseminated the text contains some information. Based on the calculation of the questionnaire, obtained persentase 84,00% so that it can be concluded that the information system is feasible to implemention in SDIT Nurul Istiqlal Klaten. Keywords: Monitoring, PHP, SMS-Gateway

INTISARI Unit Penelitian dan Pengabdian Pada Masyarakat merupaka salah satu unit pada Universitas Respati Yogyakarta yang memiliki tanggung jawab dalam pengelolaan kegiatan kegiatan penelitian dan kegiatan pengabdian kepada masyarakat yang dilakukan oleh dosen. Salah satu tahapan dalam kegiatan adalah monitoring dan evaluasi pelaksanaan kegiatan penelitian dan pengabdian kepada masyarakat dinilai oleh seorang reviewer. Pelaksanaan monitoring dan evaluasi dilakukan pada saat proses pelaksanaan kegiatan dan di akhir kegiatan. Saat ini proses masih dilakukan secara manual oleh reviewer dan sulitnya melakukan monitoring dan evaluasi karena beberapa hal seperti waktu penilaian terbatas, banyaknya penelitian dan jumlah reviewer yang terbatas.Penelitian yang dilakukan bertujuan mengembangkan sistem monitoring dan evaluasi kegiatan peneitian dan pengabdian kepada masyarakat. Metode pengembangan dilakukan dengan tahapan analisa sistem, perancangan sistem, coding dan implementasi sistem. Pengembangan perangkat lunak nantinya berbasis web dengan perangkat pengembangan menggunakan bahasa pemrograman PHP dan MySQL sebagai DBMS.Hasil pengembangan sistem informasi penelitian dan pengabdian masyarakat diharapkan dapat membantu Unit Penelitian dan Pengabdian Pada Masyarakat dalam proses pengolahan data monitoring dan evaluasi kegiatan peneitian dan pengabdian kepada masyarakat yang dilakukan dosen. Kata Kunci : Sistem Informasi, Monitoring, Evaluasi, ReviewerABSTRACTResearch Unit and Community Service is one unit at Universitas Respati Yogyakarta which has responsibility in management of research activities and community service activities conducted by lecturers. One of the stages in the activity is monitoring and evaluation of the implementation of research activities and dedication to the community assessed by a reviewer. Implementation of monitoring and evaluation is done during the process of implementation of activity and at the end of activity. Currently the process is still done manually by the reviewer and the difficulty of monitoring and evaluation due to several things such as limited assessment time, the number of research and the number of reviewers are limited. The research undertaken aims to develop a system of monitoring and evaluation of research activities and community service. Development method is done by stages of system analysis, system design, coding and system implementation. Software development will be web based with development tools using PHP and MySQL programming language as DBMS.The result of development of information system of research and community service is expected to assist Unit Research and Community Service in the process of monitoring data and evaluation of research activities and dedication to the community conducted by lecturers.Keywords: Information System, Monitoring, Evaluation, Reviewer

The processing data is process of input and output data become another form that very needed which is the special information of livestock processing data. The unit of implementation technical production and health of animal (Prokeswan) Lape sudistrict and Lopok subdistrict as one of a government service that have various type of affairs, like like the processing data of livestock that cover the result of registration of livestock data and a mutation data of livestock. A livestock data collection is activities that carried out by subdistrict officials to know the number of livestock in every village. Research items to design and build information system processing of livestock in information of livestock processing data. The unit of implementation technical production and health of animal (Prokeswan) lape subdistrict and Lopok subdistrict in Web-based. The system was developed using the PHP language programming and using MySql as a database. The method of the developed of this software using waterfall method. The testing of this software through Black Box testing that is testing that do with testing the software in functional terms. The instruments Technic Collecting data in this research using interview and literature review. The final result of research is design and build an information system processing of livestock data Web-based can facilitate officer and employer to processing livestock data, this system also can speed up saving data, processing, collecting and display data to produce the quality information and also processing the data become a report that able to present the complete livestock data of UPT Prokeswan Lape subdistrict and Lopok subdistrict.

Tanah Laut Regional Water Supply Company (PDAM) is engaged in the management and sales of drinking water services with about 7,848 customers which spread throughout Tanah Laut Regency. Complaints of interference carried out by the customer manually to the customer relations department by coming directly to the office of the Tanah Laut PDAM which will then be distributed to the technical and distribution transmission. Based on these problems, a web-based PDAM Tanah Laut PDAM complaint information system is built that can make it easier for customers to make complaints of disturbances and to make it easier for officers to distribute it. This can produce accurate and efficient complaints data. This web-based information system for complaints of disturbance at PDAM Tanah Laut is built with programming languages PHP, HTML, CSS and JavaScript. And designed using Entity Relationship Diagrams and Data Flow Diagrams. The results of blackbox testing show that the functionalities of the system are perform properly.

Sistem Informasi Benih dan Induk merupakan sebuah sistem informasi berbasis web. Tujuan dibuat adalah untuk mempermudah dalam mengolah dan menyimpan data informasi yang cepat dan tepat. Diharapkan aplikasi ini menjadi informasi alternatif yang dapat dipertanggungjawabkan. Informasi yang dihasilkan dari aplikasi ini bisa memnberikan dampak pada keberhasilan dalam peningkatan produksi induk dan benih hasil riset. Melalui sistem ini juga diharapkan dapat mempermudah kegiatan monitoring dan evaluasi (monev). Monitoring merupakan sebuah kegiatan untuk menjaga, mengontrol, dan memastikan pelaksanakan kegiatan budidaya sesuai dengan rencana. Sedangkan evaluasi merupakan kegiatan untuk mengetahui tingkat keberhasilan atau capaian yang telah dilakukan, dalam hal ini keberhasilan penyebaran dan produksi induk dan benih. Kegiatan monitoring dan evaluasi ini merupakan salah satu kontrol dalam melakukan kajian tingkat keberhasilan dari teknologi budidaya pada jenis dan atau varietas tertentu, baik induk maupun benih unggul hasil domistikasi atau pemuliaan yang telah di rilis kemasyarakat.The seed and parent information system is a Web-based information system. Purpose made is to facilitate the processing and storing of information data that is fast and precise. This application is expected to be an alternative information that can be accounted for. The information generated from this application can give an impact on success in increasing the production of parent and seedresearch results. The system is also expected to facilitate monitoring and evaluation (MONEV) activities. Monitoring is an activity to maintain, control, and ensure the implementation of cultivation activities according to the plan. While evaluation is an activity to know the level of success or achievement that has been done, in this case the success of the spread and production of parentand seed. This monitoring and evaluation activity is one of the controls in conducting a review of the success rate of the cultivation technology of certain types and or varieties, both parent and seed superior results of domistication or breeding that has been released Society.

Mohan Grosir Solo is a store engaged in the field of Sales and Sales Cooperation Sales with Credit Sales System. Sales of credit that runs from direct submission to the cashier by submitting identity cards only. Customers do the taking of goods in accordance with the value of the approved filing then the customer will pay in accordance with the applicable provisions. With the credit sales information system is expected small errors up to the large can be resolved, especially the problem of payment arrears and repayment payment. System design that includes: Diagram Context, HIPO, Data Flow Diagrams, Document Flow Diagrams, Database Design, Input Design, Output Design, Implementation of the program by using appropriate data stimulation. The purpose of this research is to create and design multi-level loan sales information system at Mohan Grosir Solo. Then the results obtained from this system are Input of Goods Data, Input of Customer Data, Input of Supplier Data, Input of Data of Department of Goods, Input of Cashier Data, Purchase Transaction, Purchase Return Transaction, Credit Sale Transaction, Sales Return Transaction, Cash Transaction, Customer Data Report, Supplier Data Report, Department of Goods Data Report, Cashier Data Report, Purchase Transaction Report, Purchase Return Report, Credit Sales Transaction Report, Sales Returns Report, and Cash Advance Report. System information created using Microsoft Visual Basic 6 and SQL Server database has been done blackbox testing with a design that is appropriate. Keywords: Information System, Multiuser, Credit Sales.

The quality assurance system at STMIK DCI Tasikmalaya is now started with the determination of policies, standards, manuals, and form documents. Only in the implementation is not supported by information systems, so the process of socialization of policies, standards, manuals, and is not effective and may result in limited access for the parties in need. In addition, in the implementation of monitoring and evaluation (Monev) the standard documents of education and teaching also became less efficient and can hamper the implementation of internal quality assurance system. To that end, then created a prototype web-based quality assurance information system for STMIK DCI Tasikmalaya and modeled it using UML. With the prototype of quality assurance information system is expected to assist the implementation of quality assurance system for the STI Tasikmalaya DCI to improve the quality of the institution.

ABSTRACT         Mentawai Fast is a company engaged in the field of transportation services. The problem is that there is no web-based ship ticket booking information system, ticket purchases are still manual or a passenger must go to the agent to buy tickets and requires a long time. In this system, it starts with analyzing the current system, which is done by output design, input design, file design. The research method used is SDLC (System Devolopment Life Cyle) with stages a). Planning, b) Analysis, c) Designing Systems and Software, d). Implementation, using design tools, namely UML (Unified Modeling Language). With the design of this ordering information system, it is expected to provide maximum service to customers and minimize errors in data processing. Abstract. West Java Provincial Health Office still faces difficulties in managing information, especially in medical records. Recording and reporting of malnutrition are still done in some stages starting from collecting data from village midwives, puskesmas, Regency/City Health Office then Provincial Health Office and forwarded to the the central office. It is necessary to manage information through service system by utilizing Cloud Computing based on information technology. This research uses The Open Group Architecture Framework (TOGAF) approach in Architecture Development Method (ADM), from Architecture Capability Iteration to  Architecture Development Iteration. Monitoring and Evaluation (M & E) are two integrated activities in the context of controlling a program. The results of this research are planning a medical record information system architecture and monitoring malnutrition based on Cloud Computing with the name of M2Rec (Medical Record and Monitoring) in the form of integrated recommendation and development between current information system and proposed information system architecture.Keywords: togaf adm, medical record and monitoring, cloud computing Abstrak. Sehingga perlu diupayakan pengelolaan informasi melalui sistem pelayanan dengan memanfaatkan teknologi informasi berbasis Cloud Computing. Penelitian ini menggunakan pendekatan framework The Open Group Architecture Framework (TOGAF) Architecture Development Method (ADM), yaitu iterasi ke satu pada Architecture Capability Iteration daniterasi ke dua pada Architecture Development Iteration. The purpose of this research is to know the geographic information system of tourism that is in Dinas Kepemudaan dan Olahraga dan Pariwisata Kebumen Regency and develop it Research methods used by doing observationin Dinas Kepemudaan dan Olahraga dan Pariwisata Kebumen Regency, then proceed with the system development method. the research method used is by the method of SDLC (System Development Life Cycle). By using the Software Notepad ++ to build tourism Geographical Information System web-based. PHP as a programming language, MySQL as the database server and the design of the map using the Google Maps API. The object-oriented approach that is used UML (Unified Modeling Language) can explain the flow of the existing system. Dinas Kepemudaan dan Olahraga dan Pariwisata Kebumen Regency has been doing promotion through mass media such as newspapers and brochures in the delivery of information. But the way is not enough to inform tourism and places of attractions. It is therefore through the design of Geographical information system of tourism was able to resolve the issue. After the results of the study of geographic information systems is expected delivery of tourism information becomes more widespread, and the tourists could be quick and precise in finding information a tourist want to visit.
