The article is devoted to the research of the problem of formation of sence-of-life orientations of a mature personality: the theoretical analysis of the concept of personal maturity, the essence of sence-of-life orientations in the life of an adult personality is presented; the relevance and motivation of research in the direction of humanistic-existential paradigm are substantiated; processes of understanding the life experience of the individual, his role in the formation of a mature personality, the activation of its internal resource are emphasized. An empirical study and analysis of its results on the formation of sence-of-life orientations of the individual in adulthood are presented; the principles of psychological influences in the process of psychological support are presented: social determinism, freedom of choice, personal congruence, responsibility, individual approach, empathy, invaluable attitude to the client. In the course of the research hypotheses were put: the deepening crisis of the existence of a mature personality is mediated by personal experience of living out difficult life situations and overcoming them. The main purpose of crisis counseling as a special help of the individual, is to promote an in-depth understanding that only person is able to make choices, to take responsibility for their life. In the course of research and implementation of psychological influence, the methodological principles of humanistic and existential orientation, which determined professional interaction, were reflected. The essence of psychological care, which we see in the integration of the experience of psychotherapeutic meetings in the real life of the client, is determined. In the perspective of research, the formation of a stable resistance of the individual to stress, adaptation to the situation of frustration and, in particular, the search for new meanings of existence, improving the quality of life.

Patients with urothelial cell carcinoma (UCC) often have comorbidities, which cause trouble for the completion of oncological treatment, and little is known about their quality of life (QoL). The aim of the present study was to obtain and describe patient-reported outcomes (PRO) and QoL data from UCC patients in the treatment for locally advanced muscle-invasive or metastatic UCC. A total of 79 patients with UCC completed four questionnaires (EORTC QLQ-C30, QLQ-BLM30, HADS, and select PRO-CTCAE™ questions) once weekly during their treatment. From those, 26 patients (33%) underwent neoadjuvant treatment for local disease while 53 patients (67%) were treated for metastatic disease. Of all patients, 54% did not complete the planned treatment due to progression, nephrotoxicity, death, or intolerable symptoms during treatment. The five most prevalent PRO-CTCAE grade ≥ 2 symptoms were frequent urination (37%), fatigue (35%), pain (31%), dry mouth (23%), and swelling of the arms or legs (23%). The baseline mean overall QoL was 61 (±SD 24) for all patients (neoadjuvant (73, ±SD 19) and metastatic (54, ±SD 24)) and remained stable over the course of treatment for both groups. A stable overall QoL was observed for the patients in this study. More than half of the patients did not, however, complete the planned treatment. Further supportive care is warranted for bladder cancer patients.

This retrospective observational study summarizes the experiences of 820 patients treated with a new Curcuma extract (Flexofytol®, 4-6 capsules per day), for more than 6 months for various forms of painful osteoarthritis. These experiences were reported by 110 Belgian general practitioners via a questionnaire that included quality-of-life parameters for assessing patient experience. Data were submitted to an independent statistician for analysis. Within the first 6 weeks, Flexofytol® improved patient pain, articular mobility, and quality of life. Excellent tolerance was reported, and more than half of these patients were able to discontinue analgaesic and anti-inflammatory drugs. Patient satisfaction was confirmed by their decision to maintain Flexofytol® therapy for more than 6 months. These data must be confirmed with randomized controlled studies. We currently conclude that Flexofytol® which is based on a new preparation of curcumin, is as a potential neutraceutical for the care of patients complaining of joint problems, with excellent tolerance and rapid benefits for articular mobility, pain, and quality of life.

Introduction Chemotherapy-induced nausea and vomiting (CINV) may affect adherence to planned chemotherapy treatments and compromise patients’ quality of life during the therapy. NEPA is an oral fixed combination of netupitant, a highly-selective NK1-RA and palonosetron, a 5HT3-RA, approved for the prevention of acute and delayed CINV. The aim of this study was to evaluate the efficacy and safety of NEPA with dexamethasone for CINV prophylaxis in the challenging setting of carboplatin and gemcitabine combination chemotherapy, after failure of prophylaxis with 5HT3 receptor antagonist. Methods Eligible patients were undergoing carboplatin and gemcitabine combination chemotherapy for metastatic non-small cell lung cancer (NSCLC), ovarian cancer or urothelial cancer and experienced nausea and/or vomiting after the first cycle of chemotherapy, despite an antiemetic prophylaxis with a 5HT3-RA and dexamethasone. Primary efficacy endpoint was complete response (CR: no emesis, no rescue medication) obtained with NEPA, during the overall phase (0–120 h), after the start of chemotherapy. Results During the first cycle of chemotherapy, 15 out of 30 (50%) patients did not properly control CINV with a 5HT3-RA plus dexamethasone used as primary antiemetic prophylaxis and then were switched to NEPA from the subsequent cycle. During NEPA administration, 13 out of 15 patients (86.7%) achieved an overall CR (no emesis, no rescue medication). Antiemetic treatment with NEPA was very well tolerated with only two patients (13.3%) that experienced a grade 1 TEAE. Conclusions Our experience showed that NEPA has proven to be very effective and well tolerated in the prophylaxis of CINV induced by carboplatin-based chemotherapy.

Business managers are faced with complex decisions involving a wide range of issues – technical, social, environmental, and financial – and their interaction. Our education system focuses heavily on presenting structured problems and teaching students to apply a set of tools or methods to solve these problems. Yet the most difficult thing to teach is the defining of the problem itself. Unlike a jigsaw puzzle, in which the overall picture and the individual pieces are clearly defined, real-life business problems require managers to see the ‘big picture’ and define the dilemma. If they cannot see the ‘whole’, they cannot see the ‘holes' where information is lacking. The boundaries of the problem are too often based on what managers know, not on what they need to know. This paper reviews the roots of holistic systems thinking in the work of philosophers, psychologists and physical scientists. It looks at the extent to which the ability to see and develop models of whole systems is a natural skill of some people rather than a learned skill. It reviews efforts to measure the capability in individuals and the psychological influences on holistic thinking. It then discusses alternative ways in which the capability to see the (w)holes can be developed.

The availability of the production certificate gives an advantage to any company that cares about improving the efficiency of processes and quality of work. The production certificate is first of all necessary for the enterprises which plan to take partin tenders for supply of products for railway transport and its infrastructure. The production certification procedure involves checking the compliance of regulatory, technological, metrological support, personnel qualification level, organizational arrangements aimed at ensuring the stability of quality indicators of products or services provided, assessment of technical capabilities of the applicant company to ensure stable production/services that meet the requirements of current regulations in Ukraine (national standards, enterprise standards, etc.). Therefore, the assessment of the technical capabilities of the manufacturer, including PC-type terminals of separate rail fastening of the railway track, is the main and integral part of the production certification process. The information and data provided in this paper are based on the real-life experience of the certification body and relate to the quantitative assessment of the technical capabilities of the enterprise that manufactures PK-type terminals. The statement of the problem of this research is conditioned by the results of the monitoring conducted by the author of the article on the quality of provision of educational services in the field of tax education and tax education to Ukrainian citizens, “the state of which does not satisfy the requirements that it faces in terms of building Ukrainian statehood, cultural and spiritual revival of the Ukrainian people. This is manifested primarily in the inadequacy of education to the needs of the individual, the social needs and worldly achievements of mankind; in the devaluation of the social prestige of education and intellectual activity; in distorting the goals and functions of education; bureaucratization of all links in the educational system. The dynamism inherent in modern civilization, the growth of the social role of the individual, the humanization and democratization of society, the intellectualization of labor, the rapid change of technology and technology all over the world, all require the creation of such conditions under which the people of Ukraine would become a constantly learning nation.”. The fundamental relevance of the investigated issues is, first of all, directly related to the official recognition that the directions of education development conducted during the 29 years of our country were not sufficiently systematic and complex in nature and therefore did not contribute to the formation of a comprehensive state policy in the field of education. Therefore, the relevance of the problem of improving the quality of tax, tax education and education of Ukrainian citizens is extremely important. The study is conditioned by the results of the monitoring conducted by the author of this article on the quality of provision of educational services in the field of tax education and tax education to Ukrainian citizens. Creating an actionable, real-life, small, compact and effective Tax Code of Ukraine will become a major source of tax knowledge and tax culture.

IntroductionDementia with Lewy bodies (DLB) is one of the most common degenerative dementias. Despite the fact that most individuals with DLB die from complications of the disease, little is known regarding what factors predict impending end of life or are associated with a quality end of life.Methods and analysisThis is a multisite longitudinal cohort study. Participants are being recruited from five academic centres providing subspecialty DLB care and volunteers through the Lewy Body Dementia Association (not receiving specialty care). Dyads must be US residents, include individuals with a clinical diagnosis of DLB and at least moderate-to-severe dementia and include the primary caregiver, who must pass a brief cognitive screen. The first dyad was enrolled 25 February 2021; recruitment is ongoing. Dyads will attend study visits every 6 months through the end of life or 3 years. Study visits will occur in-person or virtually. Measures include demographics, DLB characteristics, caregiver considerations, quality of life and satisfaction with end-of-life experiences. For dyads where the individual with DLB dies, the caregiver will complete a final study visit 3 months after the death to assess grief, recovery and quality of the end-of-life experience. Terminal trend models will be employed to identify significant predictors of approaching end of life (death in the next 6 months). Similar models will assess caregiver factors (eg, grief, satisfaction with end-of-life experience) after the death of the individual with DLB. A qualitative descriptive analysis approach will evaluate interview transcripts regarding end-of-life experiences.Ethics and disseminationThis study was approved by the University of Florida institutional review board (IRB202001438) and is listed on clinicaltrials.gov (NCT04829656). Data sharing follows National Institutes of Health policies. Study results will be disseminated via traditional scientific strategies (conferences, publications) and through collaborating with the Lewy Body Dementia Association, National Institute on Aging and other partnerships.

On the experience of old age in the context of contemporary existential dilemmas. Film Elena by Andrei ZvyagintsevThe article constitutes the attempt of looking at the problem of the old age from the point of view of Vladimir, the main male character of Andrei Zvyagintsev’s film Elena. His retired existence can be treated as the model example of the golden autumn of life. This vision can be associated with the images promoted by today’s media as the period of so called “third age”, i.e. old age full of new opportunities, with health good enough to take advantage of newly marketed products guaranteeing comfortable and worry-free life. The publication examines the quality of the family relationships, built up by the selected film character, on the basis of the interpretation of space in which he is meant to function. The analysis allows the author to come to the conclusion that the old age and life experience does not protect the characters from impulsive behavior, which is the consequence of spiritual crisis of the contemporary society and disintegration of the harmony between the individual and the community. The old age in Zvyagintsev film turns out to be the time when the opportunity of improving interpersonal relationships is wasted.Проблема старости в контексте современных экзистенциальных дилемм. Фильм Елена Андрея Звягинцева Статья представляет собой попытку взглянуть на проблему старости с точки зрения Владимира, главного мужского героя фильма Андрея Звягинцева Елена. Его пенсионное существование можем рассматривать в качестве примера модели золотой осени жизни. Такой подход связан с изображениями сегодняшних СМИ, представляющих старость как „третий возраст”, то есть период полный новых возможностей, в достаточно хорошем здоровье, чтобы воспользоваться новыми вещами, гарантирующими комфортную беззаботную жизнь. Автор рассматривает качество семейных отношений, созданных Владимиром, анализируя аспекты пространства, в котором он функционирует. Интерпретация позволяет прийти к выводу, что старость и жизненный опыт не защищают героев перед импульсивным по поведением, которое является следствием духовного кризиса современного общества и распада гармонии между индивидом и обществом. Старость в фильме Звягинцевa оказывается периодом, когда возможность повышения межличностных отношений не осуществляется.
