Increasing the number of elderly people needs to be followed by programs that can maintain the quality of life of the elderly. The ratio of elderly dependence on the productive population is increasing. For that, we need a program so that physical and social changes in the elderly do not reduce the independence of the elderly. Physical changes in the form of cognitive disorders namely decreased brain function so that activities related to the ability of attention, concentration, calculation, decision making, reasoning, and abstract thinking. Social changes in the form of job loss, loneliness, risk of disease, can be a trigger for depressive disorders in the elderly. The purpose of this activity is for all parties to have a role in the early detection of cognitive disorders and depression as well as the elderly who have symptoms can be treated early

Abstract Background: Several studies demonstrated the role of depressive mood and cognitive impairment in the background of elevated mortality and decreased Quality of Life (QoL) of the elderly. Methods: In the framework of the ICT4Life project self-administered questionnaires and clinical screening tools were used to assess QoL, depressive symptoms and cognitive functions of 60 elderly over the age of 65.Results: Males found to be depressed and cognitively declined more frequently; and had higher scores on the depression and lower on the QoL scales. Depressed elderly had lower cognitive levels and their QoL was significantly poorer than that of the non-depressed subjects. Depressive disorders were detected in a quarter of the elderly, and the majority of them did not receive adequate antidepressant medication. Conclusions: Close correlation between depression and cognitive impairment was confirmed, as well as the key role of depression in the background of QoL decline. Results also highlighted the problems of recognition and adequate treatment of depression and cognitive decline in elderly, which can be further complicated by the common symptoms of depressive pseudo-dementia. Early recognition of depressive symptoms is important not only to treat the underlying mood disorder, but also to improve QoL of the elderly.

Nutrition is an important factor for health, physical and cognitive function, vitality, quality of life and longevity. Elderly people oft en have low food intake, induced by multifactorial causes, as a result, malnutrition is an independent risk factor for increased morbidity and mortality, regardless of the underlying pathologies (chronic organ failure, cognitive impairment, cancer, cardiovascular disease, type 2 diabetes etc.) including overweight or obese subjects. The prevalence of malnutrition varies from 4% to 10% in the elderly population from the community, from 15% to 38% for institutionalized elderly people and from 30% to 70% for hospitalized ones. The purpose of this study was to assess the nutritional status of institutionalized elderly people and establish correlations between the determinants of malnutrition and fragility syndrome.Th e epidemiological study is part of the project 20.80009.8007.25 “Fragility: diagnosis and prophylaxis in relation to medicopsychosocial problems of the vulnerable elderly”, included 50 patients institutionalized in the Republican Asylum for the Disabled and Retired in Chisinau. All persons were examined by the complex geriatric evaluation, approved by the order of the Ministry of Health no. 619 of 07.09.2010. According to the results, both malnutrition and the risk of malnutrition were high in institutionalized elderly people.Among the determinants of malnutrition, the highest association had the number of concomitant diseases, decreased functionality of the elderly, the presence of depression syndrome and cognitive disorders. At the same time, the malnutrition syndrome correlated with the fragility criteria but also with the degree of severity of frailty of the institutionalized elderly people.

Introduction:Two main demographic phenomena have substantially changed the condition of elderly: the growth of the urban population and the increase in longevity. Objective:The aim of the present review is to investigate how the elderly living in the cities perceive the sense of insecurity compared to those who reside in rural areas, and their Quality of Life (QoL).Method:Studies published from January 2011 to August 2017 were identified on Google and PubMed combining the following terms: “elderly urban/rural QoL” or “old age urban/rural QoL”.Results:We found 18 different papers published. However, there was only one study on how the elderly perceive the violence in the city. Studies on quality of life were not univocal. Studies on depressive disorders in old age were most homogeneous showing a condition worsening in the cities. A study on the perception of violence in US showed in residents of cities and neighborhoods with the entertainment arena and casinos an increase of criminality perception. In contrast, the crime decreased in both above-mentioned neighborhoods. Conclusion:The condition of elderly in the cities is changed considerably in the recent years. It is estimated that this trend will increase in the coming years. We do not know how older people are experiencing these changes and how they perceive the persistence of violence in the cities. Future researches must satisfy this need by addressing the issue with appropriate methodological tools. This is a public health priority.

AbstractAssistive Ambient Living (AAL) in ageing refers to any device used to support ageing related psychological and physical changes aimed at improving seniors’ quality of life and reducing caregivers’ burdens. The diffusion of these devices opens the ethical issues related to their use in the human personal space. This is particularly relevant when AAL technologies are devoted to the ageing population that exhibits special bio-psycho-social aspects and needs. In spite of this, relatively little research has focused on ethical issues that emerge from AAL technologies. The present article addresses ethical issues emerging when AAL technologies are implemented for assisting the elderly population and is aimed at raising awareness of these aspects among healthcare providers. The overall conclusion encourages a person-oriented approach when designing healthcare facilities. This process must be fulfilled in compliance with the general principles of ethics and individual nature of the person devoted to. This perspective will develop new research paradigms, paving the way for fulfilling essential ethical principles in the development of future generations of personalized AAL devices to support ageing people living independently at their home.

Effective and safe treatment of patients with neuropathic pain (NP), despite the emergence of new clinical trials, remains a challenge for clinicians. A review of epidemiological data from around the world suggests the prevalence of NP at the level of 6.9–10% in the entire population, more often in women and in the elderly [1, 2]. Chronic NP is associated with a reduced quality of life of patients. In this group of patients, anxiety, and depressive disorders as well as sleep disorders are more common [3]. Hence, effective management of patients with NP is an important problem that requires further research. The aim of the article is to present the current knowledge about pregabalin and gabapentin, their mechanisms of action, effectiveness in patients with NP, the potential risk of side effects and the risk of inappropriate use, as well as to indicate their place in the standards of neuropathic pain treatment in the light of current clinical trials.

Arterial hypertension in the metabolic syndrome in the elderly contributes to the formation of cognitive disorders and anxiety-depressive disorders. It is shown that the severity of these deviations of the psychological continuum significantly decreases after 1 year, under the influence of an antihypertensive fixed combination of amlodipine, mesinopril and rosuvastatin in the chronotherapy mode (evening reception), than in the traditional application (morning reception in the equivalent dosage of 5/10/10 mg per day in both cases. The dynamics of cognitive impairment in chronotherapeutical approach in patients 6074 flights from arterial hypertension with metabolic syndrome is characterized by a significant increase in the average score on a scale MMSE, f source 17.8 0.3 to 23.5 0.4 points (р 0.001), against 16.9 0.3 to 20.4 0.4 points (р 0.001) in the morning taking the drug. Situational anxiety decreased from 40.8 2.2 to 30.6 1.8 points (р 0.05) and from 40.0 2.5 to 32.1 2.0 points (р 0.05), personal anxiety from 48.8 2.0 to 25.4 1.9 (р 0.001) and from 44.9 1.9 to 30.7 1.7 (р 0.01) points in the evening and morning the reception, respectively. Depressive disorders decreased slightly more significantly in chronotherapy (14.1% vs. 7.7%) than in the traditional scheme, but despite this, both cases with (groups) corresponded to depressive spectrum disorders. The results obtained indicate that chronotherapy is more effective than the traditional use of a fixed combination of amlodipine, lisinopril and rosuvastatin in arterial hypertension with metabolic syndrome.

The prevalence of malnutrition in people over 60 is 10–40%. Inadequate nutrition worsens the quality of life and the functional status of patients in elderly and senile age. The development of eating disorders may be due to various factors. Diagnosis of malnutrition is aimed at identifying it and finding the causes. Treatment should be comprehensive, including adequate nutritional support and correction of factors leading to the development of malnutrition. It must be remembered that some eating disorders can cause the development or progression of various geriatric syndromes (falls, urinary incontinence, cognitive disorders, depression, etc.)

The problem of preserving the quality of life in the postoperative period, one of the criteria of which is the cognitive function, retains its relevance. In patients with advanced and senile age, after undergoing surgery, cognitive disorders are swept aside, which, according to various authors, reaches 64 %. Postoperative cognitive dysfunction was studied in 168 elderly and elderly patients operated on for complications of colon cancer. Patients were divided into two groups. Patients of the control group were operated on operations conventionally, the main one — laparoscopically, using endovideosurgical technologies. Cognitive status was assessed before the operation, as well as on the first, third and seventh days of the postoperative period using the MMSE test (abbr. English Mini Mental State Examination). The incidence of postoperative cognitive dysfunction in both groups was traced. In the development of postoperative cognitive disorders, along with such factors as general anesthesia, age, aggravated neurological history, depressive disorders, operative trauma plays an important role. The use of endoscopic technologies allows reliably reducing the risk of postoperative cognitive impairment, which in turn has a beneficial effect on the postoperative period.

The elderly not only face the problem of physical changes but also undergo emotional, psychological and social changes. Some individuals cope with these changes effectively but others will experience extreme frustration and mental agony. The family members should understand the psychosocial changes and stresses experienced by the elderly and fosters necessary care and concern to them. The biological age and chronological age are closely related but will not correlate. As the age advances, the health problems tend to increase with age and these problems also depend on economic condition and family and social support system. The main objective of this investigation is to study the plight of the elderly women in the institutional settings with the following broad objectives: To make a study of personal profile of the respondents; To understand the living arrangements of the elderly women in the institutions; To assess the extent of family relationship as well as the psychological condition of the respondents; To know and understand the social status of the elderly women in the institutions; and to study the coping patterns of elderly women. Total 80 respondents were interviewed from four care homes in the Udupi District through simple random sampling method. The study reveals that majority of the respondents are from rural area and economically poor status. They suffer from depression since they lack psychological and emotional support. They express their emotions by getting short tempered and irritable. Some try to cope up with indulging themselves more with religious activities like prayer and meditation. The healthy interaction in an institution will help in getting integration and healthy bond with other inmates. The study concludes by saying the most important aspect in every human being life is to have a financial plan for future at the middle age itself.
