Possibilities of osteopathic correction of somatic dysfunctions in children with chronic adenoiditis

Introduction. Chronic adenoiditis is one of the most frequent ENT diseases. It is comparatively resistant, and in advanced cases, the pathological process is hardly reversible. Despite the big number of publications on chronic adenoiditis, in a high percentage of cases specialists still encounter frequent recurrence of the disease. That is why issues of standard treatment, especially conservative, the search for adequate and effective methods of medical rehabilitation of patients with chronic inflammatory diseases, including in frequently ill children, are timely and relevant. Osteopathic methods of correction can potentially improve blood circulation and the innervation of adenoids, which will increase the clinical effectiveness of conservative treatment, reduce medication period and achieve a longer remission. However, studies on the influence of osteopathic correction on the course of chronic adenoiditis have not been found in the literature available to us. That is why it was necessary to conduct this study. The goal of research — to justify the possibility of using osteopathic correction in the complex treatment of frequently ill children with chronic adenoiditis.Materials and methods. From January 2018 to March 2019, a prospective controlled randomized study was performed at the MEDSI Clinic (Moscow). Thirty patients with chronic adenoiditis at the age of 4 to 10 years were examined. Patients were divided by the method of simple randomization using a random number generator into two equal groups of 15 people: the main group and the control group. Children of the main group received osteopathic correction along with drug and irrigation therapy for 2 months. In total, 3 osteopathic sessions were conducted with a frequency of 1 time in 14 days. Children in the control group received only pharmacological and irrigation therapy. All patients before the start of treatment and 2 months after the start of treatment were examined by an otorhinolaryngologist with an endoscopic examination, and by an osteopath with the assessment of the main somatic dysfunctions. Otorhinolaryngological examination included history taking and collecting of complaints with fi lling out a special questionnaire, anterior and posterior rhinoscopy, as well as an endoscopic examination of the nasopharynx with the assessment of the revealed changes in points.Results. After the treatment, the number of complaints and the main symptoms of the disease, the number of changes in the nasopharynx and oropharynx, revealed by the results of endoscopic examination, decreased statistically significantly in patients of both groups. The study showed that regional biomechanical disorders are common for children with chronic adenoiditis. Dysfunctions of head region, neck region (visceral component), thoracic region (structural component) were the most frequent. Against the background of the treatment, patients of the main group presented a decrease in the number of regional somatic dysfunctions. Statistically significant differences were obtained in the frequency of occurrence of somatic dysfunctions: head region, neck region (visceral and structural components), thoracic region (visceral and structural components) (p<0,05). In patients of the control group, a statistically significant decrease in the number of somatic dysfunctions was detected only in the head region (p<0,05). A follow-up assessment of the results of the treatment showed that the inclusion of osteopathic correction in the complex therapy of children with chronic adenoiditis contributes to a longer remission (p<0,05).Conclusion. Based on the study, it can be concluded that the direct results of osteopathic correction in the complex treatment of frequently ill children with chronic adenoiditis are comparable with the results of isolated pharmacological and irrigation therapy (reduction in the number of complaints and positive changes in the results of endoscopic examination of the nasopharynx). However, the inclusion of osteopathic correction in the complex therapy of such children contributes to a longer remission, which is of great importance for this group of patients. The results of the research suggest a further study of the possibilities of osteopathic correction of somatic dysfunctions in frequently ill children with chronic adenoiditis.

Introduction. Headache is one of the most common outpatient complaints from patients. According to various authors, annually 90 % of adult women and 83 % of adult men suffer from headaches; its frequency reaches 200 cases per 1000 population and does not tend to decrease. Herewith the tension headache (TH) is the most common variant of headaches in the general population. Modern methods of tension headache treating suggest the long-term use of antidepressants and muscle relaxants. And it may not give a lasting effect in some cases. Also, due to the small number of TH diagnoses by doctors among the population, people have to save themselves from a headache. As a result, the majority of the population suffering from TH takes analgesics uncontrollably. It fi nally leads to another severe type of headache — abusal headache. All these dictate the necessary of searching new methods of treatment, primarily non-medication, one of which could be osteopathy.The goal of research — to evaluate the clinical efficacy of the global osteopathic treatment of patients with tension headache.Materials and methods. A prospective, controlled, randomized trial was performed from 01.2018 to 03.2019 on the basis of the «Clinic of Regenerative Medicine» (Shakhty town) and V. I. Lenin MBUZ GBSMP. There were of 36 patients with a verifi ed diagnosis of chronic tension headache aged 20 to 40 years under supervision. Patients depending on the used treatment methodology were divided by the simple randomization method (using the envelopes method) into two groups: the main (20 people) and control (16 people). Patients of the main group received traditional therapy including medication, massage of the back and cervical-collar zone, physical exercises, and physiotherapy treatment. And in addition to these treatment methods patients of the main group received global osteopathic treatment, 3 procedures with an interval of 5–7 days. Patients in the control group received only standart therapy according to the same described above scheme. All the patients, regardless of the group, underwent an osteopathic examination before and after the course of therapy, with the formation of an osteopathic conclusion, and the severity of pain and asthenia was assessed.Results. The decreasing of regional and local somatic dysfunctions quantity in patients of the main group was observed during the treatment. Statistically signifi cant differences were obtained in the occurrence frequencies of somatic dysfunctions: region of the head, region of the neck, region of the dura mater; vertebral-motor segments СVII–DIII, abdominal diaphragm, I rib (p<0,05). A statistically signifi cant decrease in the pain severity and asthenic conditions (p<0,05) was also recorded after the treatment in the main group. There were no signifi cant changes of these indicators in the control patients group.Conclusion. Based on the study, it can be concluded that osteopathic correction in the complex treatment of patients with chronic tension headache can increase the clinical effectiveness of traditional treatment regimens. The obtained results propose a further deeper study of the osteopathic correction possibilities in the treatment of this patients group.

Introduction. Glaucoma is one of the most significant eye diseases. It is often diagnosed, not always amenable to therapy, and can lead to a complete loss of visual functions. In recent years, the method of osteopathic correction has become widespread as one of the effective methods of treatment and rehabilitation of patients with pathologies of various body systems. In the pathogenesis of glaucoma, it is customary to distinguish a dystrophic concept, which considers primary open-angle glaucoma as a result of dystrophic changes in the connective tissue, as well as in the endothelial lining of the trabeculae and Schlemm′s canal, especially destructive changes in mitochondria and the alteration of their functional activity. A vascular concept is also distinguished. According to this concept, the central link in the pathogenesis of glaucoma is circulatory disorder in the ciliary vessels, ocular artery, and major vessels of the head and neck, it can be assumed that osteopathic correction in the treatment of patients with open-angle glaucoma will be pathogenetically substantiated and will have a positive effect on intraocular pressure and trophicity of the optic nerve. The goal of research — to study the influence of in osteopathic correction on the nature of unoperated glaucoma (stage IIA) and to substantiate the possibility of using osteopathic correction in the complex treatment of patients with this pathology.Materials and methods. A prospective controlled randomized study was conducted at 52 city polyclinics, branch 3, Moscow, from January 2018 to January 2019. 40 patients (70 eyes) aged 50 to 75 years with primary open-angle glaucoma IIA stage were examined. At this stage of the disease, patients most often seek medical care and the issue of conservative management is primarily considered. All patients were divided into two groups of 20 people: the main group and the control group. The treatment in the main group included hypotensive drug therapy and osteopathic correction. Patients of the control group received only drug therapy. All patients underwent ophthalmic (visometry, tonometry, perimetry) and osteopathic examination twice: before the treatment and after 3 months.Results. For patients with primary open-angle IIA non-operated glaucoma, regional (most often regions of the head, neck, dura mater) and local (abdominal diaphragm, iliac bones, hip and knee joints) somatic dysfunctions were the most typical. In the main group a statistically significant decrease in the frequency and severity of dysfunctions at all levels was stated. Also, in patients receiving osteopathic correction, a significant decrease in the level of intraocular pressure and perimetric indices was noted. In patients of the control group, no reliable changes in these indicators were obtained.Conclusion. The results obtained indicate that osteopathic correction is clinically effective in the complex treatment of patients with primary open-angle II A glaucoma.

The objective: was to optimize the treatment of patients with post-stress disorders of menstrual function by conducting pathogenetic complex therapy from the perspective of an interdisciplinary approach. Materials and methods. 54 women aged 19-37 years with psychogenic amenorrhea (PA) – the main group – were examined. The control group included 25 practically healthy patients aged 18 to 35 years. Results. The variants of hormonal disorders in this pathology are determined, the analysis of the results of electroencephalography and the study of individual and reactive anxiety in patients with PA are performed. The presence of three pathogenetic variants of hormonal disorders was established, which became the criterion of the proposed differentiated pathogenetic treatment. Conclusion. Features of the pathogenesis of psychogenic amenorrhea, the complexity of neuroendocrine and hormonal disorders indicate the need for an interdisciplinary approach in the examination, treatment and rehabilitation of this category of patients with the involvement of related specialists – a neurologist and psychotherapist. Aim. To study the possibility of using and estimate the efficiency of a new tablet form of biologically active supplement in patients with vegetovascular dystonia syndrome. Materials and methods. One hundred patients with vegetovascular dystonia syndrome, including 50 patients of the main group and 50 patients of the control group were examined. There is presented the composition of prescription formula of biologically active supplement, based on participation of its components in the correction of disturbed metabolic processes of the nervous system. Results. Additional application of biologically active supplement in combination with drug significantly improves cerebral blood flow, decreases blood viscosity and risk of cellular membrane impairment, normalizes arterial pressure, elevates initiative and attention, reduces headache in patients of the main group with vegetovascular dystonia syndrome. Conclusions. Use of biologically active supplement has a positive effect in complex therapy of patients with vegetovascular dystonia syndrome.

Causes of unsatisfactory outcomes of ingrown nail and mycotical pathology complex treatment were insufficiently studied for approaches to preventing relapses. The need for complex research on surgical nail pathology is primarly determined by a large number of clinical observations of uncomplicated and complicated cases, especially relapses. The mycotic paronychia and the chronic subungual abscess are compressed along the nail edge. Not all surgical procedures that have been successfully treated paronychia you can apply for the correction of ingrown nail. Late compression relapses with monoonychocryptosis are 5–18 %, and with ingrown nail combined with onychomycosis – 30–70 %, which is also confirmed by our previous studies. Fungal infections of the nails (onychomycosis) in combination with ingrowth remain one of the most serious problems of dermatology and dermatological surgery. In domestic literature there is a significant number of works devoted to pathology of the nail plate, however, the surgical aspects of the onychology are assigned a minimal, secondary role.The aim of the study – optimal sequence of surgical treatment, local and system antimycotic therapy, clinical and biochemical parallels after moving away of the incarnated nails at trichophytosis and destructive polyonychomycosis, complicated by the secondary ingrown nail for some patients with the complicated defeat of nails.Materials and Methods. Over a five-year period 436 unguis incarnates diagnosis (among them 325 cases of incarnated multifocal mycotical-assotiated nail pathology – the main group, included sub-selections of patients with diabetes mellitus and metabolic syndrome) in 259 men and 177 women 28–86 years old were performed. Adequate system therapy of patients with comorbid diabetes mellitus and metabolic syndrome was carried out. In 182 patients late relapses of onychocryptosis were confirmed after previous surgeries at other clinics. Conservative treatment was recommended only at early stages of ingrowth. Removal of the affected nails was performed in patients with mycotic lesions (local and systemic fungicide therapies were used). Investigation of the morphogenesis of destructive aspect of the mycotic lesions was carried out. The analysis justifies the feasibility of establishing predictive relationships between clinical variants of chronic purulent necrotic infections and combined comorbidity.Results and Discussion. 363 cases of destructive purulent-necrotic superficial chronic, combined and combined lesions of the distal phalange of the toes with nail plate ingrowth were studied in patients aged 12–75 years, 236 men and 127 women operated in surgical departments were investigated. All surgical procedures are performed correctly according to local protocols. Nosological forms of lesions are associated with some degree of onychocryptosis, according to the dominant clinical manifestations of ICD 10 were divided into sub-samples – actually onychocryptosis, dermatophytosis and candidal onychomycosis with incarnation of the nail. Other 73 patients with uncomplicated mycosis some conservative treatment were performed correctly according to local protocols. Analysis of subonychial scraping allowed stating the prevalence of red trichophytia, where in 74 % of cases it was associated with mold, in 26 % cases it was associated with yeast fungi; and in 31 % cases – with the bacterial flora; applied 4 "pulses" of itraconazole 400 mg/day. We studied some indicators in the lipid profile, which were significantly higher in both groups of patients, p <0.01 for both groups; noted the positive correlation between the level of total cholesterol and leptin (p <0.01). The concentration of high-density lipoprotein cholesterol in patients of the main group – (5.2±0.1) mmol/L compared with patients in control group – (2.8±0.2) mmol/L. The average content of nitrogen oxide in the study group (metabolic syndrome) was higher than that in healthy patients – (15.1±0.9) mcmol/L, p <0.05. Patients of the main and the comparison groups with type 2 diabetes mellitus with ingrown polyonychomycosisexperienced considerable decreasing HOMA -index of β-cells function and increasing HOMA -index of insulin resistance (8.11±1.1) in the main group and (2.2±1.2) in the control group). The late unsatisfactory results of the complex treatment of destructive onychomycosis associated with incarnation (occurrence of compression relapses) are determined by the technical errors of the operation interventions (inadequate selection of the method and volume of resection, traumatic performance of onychectomy, failure to perform partial matrixectomy), disregard of pathogenetic and morphogenetic factors of destructive onychomycosis, the refusal to perform simultaneous surgical interventions on deeply placed structures in case of combined mycotic-associated lesions, ineffective pre- and intraoperative prophylactic actions to prevent spreading mycotic infection to deeply placed structures.Conclusions. In all cases of mycotic onychocryptosis (secondary ingrown toenail) underwent a comprehensive treatment of comorbid pathology, system therapy of itraconazole to operative treatment (basic onychial defeats sanation) and in a postoperative period was carried out, some patients with combined pathology got 4 seven-day system "pulses" of 400 mg/day itraconazole therapy. Sanation of other nails for prevention of mycotic reinfection was carried out by ciclopirox or amorolfine lacquer. We recommend using more radical and effective three-component surgical methods: nail resection or removal of the nail plate, supplemented by excision of pathologically altered eponycheal tissues and partial marginal matrixectomy in the area of ingrowth. In patients, the low-impact methods of excision of the nail and partial marginal matricectomy by mechanical carving and coagulation with the further dermatophytoma scraping off with the Volkmann spoon were embedded and applied.

Introduction. Many key questions regarding the etiology, pathogenesis, clinical manifestations and treatment of chronic gastritis remain open. So, despite the success of chronic gastritis pharmacotherapy, much attention is paid to non-drug methods of therapy, in particular, osteopathy. However, evidences of the osteopathic methods effectiveness for the chronic gastritis treatment, obtained by objective instrumental methods, are insufficiently presented in the modern literature.The goal of research — to study the results of osteopathic correction inclusion in the complex therapy of patients with chronic gastritis.Materials and methods. The study involved 50 patients with chronic gastritis, divided by simple randomization into a control group (25 people) and a main group (25 people). The participants in the control group received standard eradication therapy according to a three-component scheme. The participants of the main group additionally received osteopathic correction. In both groups, at the beginning and at the end of the study, there were performed fibroesophagogastroduodenoscopy with targeted biopsy to assess the gastric mucosa state, Helicobacter pylori identification, and intragastric pH-metry to assess gastric juice acidity.Results. According to the study results, a statistically significant (p<0,05) decrease in edema and hyperemia of gastric mucosa was found in the control and main groups. There was a statistically significant (p<0,001) decrease in gastric aciditywith osteopathic accompaniment, compared with unaccompanied drug treatment. In both groups, there was a statistically significant (p<0,05) decrease in the incidence of Helicobacter pylori carriage. Conclusion. Based on the obtained results, it can be assumed that an integrated approach using osteopathic correction in the treatment of chronic gastritis may be more effective than the standard course of treatment.

Objective — to increase the effectiveness of treatment of psoriasis patients with degenerative changes in the nails, by including in the complex therapy of the topical drug Onychocid® Emtrix, aimed at restoring the integrity of the nail plates. Materials and methods. 38 patients with psoriasis and various forms of onychodystrophy (OD) were examined and treated. The effectiveness of therapy was evaluated based on the results of treatment of patients who were divided into two clinical groups — the main and the comparison ones. Patients in both groups received basic therapy. In addition, patients of the main group were prescribed a combined drug Onychocid® Emtrix, which was applied on the nail plates and under the tips of damaged nails once a day for 3 – 6 months. Results and discussion. Patients with psoriasis are diagnosed with dystrophic changes of nail plates of varying severity. OD of the fingers only was detected in 14 (36.8 %) patients; toes — in 5 (13.2 %); fingers and toes at the same time — in 19 (50.0 %) patients. As a result of complex treatment, in patients of the main group there was an early (already on the 2—4th week of use of the drug Onychocid® Emtrix) improvement of the appearance of deformed nails. After the course of treatment, these patients showed a positive clinical effect, i. e. decrease in thickness, normalization of density, color and structure of nail plates. At the same time, «significant improvement» and «improvement» were noted in 34 (89.5 %), «slight improvement» — in 3 (7.9 %), «no improvement» — in 1 (2.6 %) patients. Conclusions. Treatment of psoriasis patients with dystrophic nail lesions should be complex and focused, including the restoration of the integrity of the nail plates. The inclusion in the complex therapy of patients with psoriatic OD of the combined drug Onychocid® Emtrix increases the effectiveness of treatment, in particular, helps to restore the structure, shape, integrity, thickness and color of the nail plates.

Objective — to establish the Antral efficacy in terms its effects on the intensity of clinical and biochemical syndromes of non‑alcoholic steatohepatitis (NASH) against the background of obesity at comorbidity with chronic obstructive pulmonary disease (COPD). Materials and methods. The examinations involved 65 NASH patients with I degree obesity and COPD 2 — 3 D. The patients were divided into two groups: 32 patients control groups received NASH therapy (Essentiale forte N (Sanofi Avensis/Nutterman and Cie GmbH) 300 mg, 2 caps., 3 times per day) for 30 days and basic COPD therapy. The second, main group consisted of 33 NASH patients with I degree obesity and COPD 2 — 3 D, who along with the basic therapy for COPD received hepatoprotector Antral (Farmak, Ukraine) 200 mg, 3 times daily for 30 days. The mean age of the patients was 56.3 ± 3.21 years. The comparison group consisted of 30 apparently healthy people (AHP). The values of 30 practically healthy individuals were used as reference. Results. According to the scale of subjective assessment of the intensity of asthenia after treatment in patients of the main group, the effectiveness of its elimination was 3.9 times higher than that of patients in the control group (relative risk 3.87; 95 % CI [1.16 — 12.91], p < 0.05). The manifestations of dyspepsia disappeared faster in the patients of the main group. The use of antral 3.7 times more influenced the manifestations of cholestasis (p < 0.05). Body mass index (BMI) > 32 kg/m2 after treatment, which included a hypocaloric diet and Antral, remained in 13 patients in the main group (39.4 %) versus 29 patients in control group (90.6 %) (p < 0.05). During one month after the treatment, markers of cytolysis remained in 9 patients in control group (28.1 %), and in 1 patient of the main group (3.0 %) (OR 9.28; 95 % CI [1.11 — 77, 52], p < 0.05). The increased activity of cholestasis markers after treatment remained only in 5 patients in the main group (15.2 %) in comparison with 22 (68.8 %) controls. In the dynamics of treatment of patients in the main group, significant increase in the protein‑synthesizing function of the liver was revealed (albumin levels increased by 30.56 % (p < 0.05) relative to the indicator in the control group). Conclusions. The use of Antral in the complex therapy of NASH of patients with the concomitant obesity and COPD was more effective than standard therapy in terms of removal of clinical NASH syndromes: asthenic‑vegetative, dyspepsia, abdominal discomfort, cholestasis, hepatomegaly, degree of liver steatosis; biochemical syndromes: cytolysis, cholestasis, mesenchymal inflammation, hepatocellular failure.

The aim of the study: assessment of influence of complex treatment of chronic recurrent chlamydial cervical infection with probiotics of various forms of the use on the immune status and the vaginal microbiocenosis in women of reproductive age. Materials and methods. The study surveyed 87 women of age from 18 to 35 years, from which 67 patients of recurrent chlamydial cervicitis, which (according to the protocol) were eligible for inclusion. The control group included 20 gynecologicaly healthy women. All surveyed women were representative by age and data of the obstetric-gynecologic and somatic anamnesis. In accordance with the objectives of the study, all patients with chlamydia infection (CHI) before the appointment of therapy were randomized into two groups matched for age, symptoms and duration of the inflammatory process. The I (main) group included 35 patients who underwent complex therapy. Part of therapy included drugs Sumamed 250 mg (1 capsule) 1 time a day for 5-7 days according to the scheme: 1st day – 1 g (4 capsules) with the 2nd -3rd day – 500 mg per day (2 capsules) and 4th – 7th day – 250 mg per day (1 capsule). Topically applied combination of reparative, anti-inflammatory and antimicrobial drug Depantol in the form of suppositories – appointed standard one vaginal suppository twice a day for 10 days. Later was appointed eubiotic Lacto® 2 capsules 3 times a day during 3 weeks. The II group included 32 patients (comparison group) who underwent treatment involving Sumamed and Depantol, but with the subsequent appointment of local eubiotiks (1 vaginal suppository at night) – 10 days. Results. The results of the survey of women of reproductive age with chronic recurrent chlamydial cervicitis during colposcopic study found a high frequency of cervical pathologies, namely, inflammatory changes – 100%, ectopia prismatic epithelium – 32.8%, the true erosion – 19.4%, that in 5.9% cases was combined with leukoplakia, and dysplasia – 16.4%, which correlated with cytological examination (ASCUS – 68.6%, LSIL – 25.5%, HSIL and 2.9%). Most women with CHI noted deep violations of microbiocenosis of the vagina with a predominance of opportunistic pathogenic bacteria: Peptostreptococcus – in 43.3%, Corynebacterium – in 56.7%, epidermal staph in 65.7% and gardnerellas – in 74.6%, candida – in 46.3% of cases. Patients with CHI were stated a lack of interferon Genesis – reduction of IFN-a 1.4-fold and IFN-g – 1.8 times. Conclusion. The complex therapy of chronic recurrent chlamydial cervicitis in women of reproductive age was conducted by the developed algorithm with the inclusion of oral probiotic Lacto® contributed to the increase of parameters of nonspecific resistance of the organism in 88% of cases, which was reflected in the reduction of titers of specific IgM 1.4-fold, IgG – 1.8 times, raising the level of lysozyme in cervical mucus – 1.5 times and sIgA – 1.2 times, the restoration of cytokine profile and normalization of microbiocenosis of the vagina in 76.4% of women. Background. Osteopathic methods are used in practical medicine, but there is little scientific research in this area, especially in the treatment of somatic diseases. The purpose of the research is to evaluate the effectiveness of osteopathy in the complex therapy of patients with chronic stone-free cholecystitis from the point of view of evidence-based medicine. Methods. The study involved 114 patients who were divided into two groups, the control group and the main group, in which osteopathic procedures were applied. The functions of the gallbladder, bile and blood biochemistry, and cholecystokinin secretion were analyzed. Results. It was found that osteopathic methods significantly increased the effectiveness of standard therapy both in terms of regression of clinical symptoms of the disease and in the functioning of the gallbladder. It is proved that the main group of patients significantly increases the secretion of cholecystokinin, which correlates with an improvement in the biochemistry of bile and its allocation. It was found that the effectiveness of osteopathic treatment methods does not depend on the gender of patients. Conclusion. The results of the study indicate a fairly pronounced therapeutic effect of osteopathic procedures in the complex therapy of patients with chronic stone-free cholecystitis. The increase in the secretion of cholecystokinin is difficult to explain from the point of view of correction of functional blocks, and this problem remains the subject of further research.
