Total 60 mid stream urine samples were collected from suspected urinary tract infection patients of 1 to 12 years age from indoor and outdoor of BSMMU. A total 28 (46.66%) cultures yielded significant growth of single organism & 32 (53.33%) yielded no growth. E. coli was isolated in 53.57% cases, followed by 17.86% Klebsiella spp, 10.71% Enterococcus spp, 7.14% Enterobacter spp, 7.14% Pseudomonas aeruginosa and 3.5% Proteus mirabilis. E. coli was less sensitive to first line drugs including Amoxycillin, Cotrimoxazole, Cephradin, Nalidexic acid ranging (20-27%), but moderately sensitive to Ciprofloxacin (60%), Gentamicin (60%), Ceftriaxone (60%) and highly sensitive to Ceftazidime (80%) and Imipenem (100%). Klebsiella & Enterobacter spp were 100% sensitive to Imipenem and 70% to Amikacin. Enterococci spp shows good sensitivity to Nitrofurantoin (67%), and Imipenem (100%). Pseudomonas spp was highly sensitive to Imipenem and Netilmicin (100%).Bangladesh J Med Microbiol 2010; 03 (02): 18-22

In the structure of diseases of the urinary system in children, microbial-inflammatory lesions of the kidneys and urinary tract are 70–80 %. The prevalence of infections of urinary system in children in the Russian Federation is on average 18–22 per 1000 children’s population. The aim of the study was to analyze the spectrum of infectious agents of urinary infection in children and to determine the dynamics of bacterial resistance. We analyzed 330 cases of urinary system infection in children aged from 2 months up to 18 years in two time peri- ods – 2002–2004 and 2014–2016 (n = 114 and n = 216, respectively). Patients of both periods were divided into three groups: the first one – cystitis, the second one – pyelonephritis and the third group – a combination of pyelonephritis and cystitis. An obligatory criterion for including the patient in the study was the presence of a pathogmonic combina- tion of leukocyturia and bacteriuria, 10 5 KOE/ml, for the infection of the urinary system. The main causative agent of urinary infection in children remains E. coli. An increase is marked in the specific gravity of E. coli, Enterococcus spp. and Klebsiella spp., Pr. vulgaris. The study revealed the growth of E. coli resistance to cephalosporins of II, III, IV generations; decreased resistance of Enterococcus spp. to ampicillin and gentamicin. In the study of the sensitivity of Klebsiella spp. an increase was detected in resistance to cephalosporins of the III generation. We identified the main pathogens of infections of the urinary system and determined bacterial resistance.

A study of antimicrobial sensitivity of Escherichia coli (E. coli) isolated from clinical sourcesof different diagnostic centers of Dhaka, Bangladesh was carried out to facilitate the choice of drugin the management of E. coli induced symptoms. Very low sensitivity of E. coli towards ampicillin(4%), aztreonam (4%), cloxacillin (5%), nalidixic acid (5%), ciprofloxacin (7.5%), ceftriaxone(12.5%), doxycycline (12.5%), ceftazidime (16.25%), co-trimoxazole (20%), chloramphenicol(22.51%), tetracycline (25%), and netilmicin (35%) was observed. Higher sensitivity pattern wasobserved for gentamicin (56%) and only imipenem (95%) showed sensitivity pattern possiblysusceptible enough to consider for the management of E. coli induced cases in the area under study.The low sensitivity to different antimicrobial could be attributed to their prevailing usage and abusein the area under study.Key words: E. coli; Antimicrobial sensitivity; Clinical isolatesDOI: 10.3329/jbas.v34i1.5497Journal of Bangladesh Academy of Sciences, Vol.34, No.1, 99-101, 2010

Urinary tract infection (UTI) is increasingly rising in Bangladesh due to the development of resistance of causative pathogens against commonly prescribed antibiotics. Present study attempted to examine the prevalence of such uropathogens and their antibiogram profiles. Among 462 urine samples collected from patients with UTI, 100 were found to be culture positive. Escherichia coli (70%) was predominating, while Klebsiella spp. (11%) and Enterococcus spp. (6%) were also prevalent. About 96% uropathogens were sensitive against imipenem, and 75% against amikacin. The resistance rate of E. coli and Klebsiella spp. against ampicillin was 98.5 and 100%, respectively, and to cefotaxime, 84.3 and 72.8%, respectively. More than 80% resistance against these antibiotics was scored for other isolates. The frequency of drug resistance was found to be comparatively elevated in E. coli, Pseudomonas spp. and Proteus spp. Overall, the present investigation emphasized the need for routine screening of antibiotic resistance to promote effective medication against UTI. Mortality in broilers (6.56%) and growing ducks (11.0%) caused by Escherichia coli was recorded in the experimental flocks study during the period from May to August 2003. E. coli organisms isolated from broiler birds affected with characteristic lesions of omphalitis and yolk sac infection, fibrinous pericarditis and peri-hepatitis , hemorrhagic enteritis, and accumulation of excessive pericardial and peritoneal fluid, whereas from ducks with lesions of hemorrhagic enteritis and extensive epicardial hemorrhages. Each of the 21 isolates collected from broilers and 11 isolates from ducks was characterized by cultural and biochemical studies, of which 8 isolates from broilers and 5 isolates from ducks were tested for antibiotic sensitivity with 9 different antibiotics. The antibiotic sensitivity pattern showed that the isolates were highly sensitive to ciprofloxacin but an increasing trend of resistance was recorded in broilers (7 / 9) than duck (4 / 9) isolates. It may be concluded from the results of this study that the high rate of E. coli infection in broilers and ducks along with the high resistance of isolates to antibiotics constitute a threat to the poultry industry in Bangladesh. Background: Antimicrobial resistance is a serious problem worldwide and differs from region to region. This study was planned to determine the incidence and sensitivity pattern of Klebsiella pneumoniae (K. pneumoniae), Escherichia coli (E. coli) and Pseudomonas aeruginosa (P. aeruginosa) in our region and discuss the general issues related to antimicrobial resistance.Methods: Prospective study was carried out between March to October 2015. Samples of urine, blood, pus, CSF and miscellaneous samples (fluids, swabs, sputum and stool) were collected from indoor and outdoor patients for isolation and antimicrobial susceptibility of K. pneumoniae, E. coli and P. aeruginosa in the Department of Microbiology G.R. Medical College, Gwalior (MP).Results: Out of the 5000 samples analyzed 1684 showed growth. K. pneumoniae (38.50%), E. coli (33.29%) and P. aeruginosa (28.19%) constituited a total of 805 isolates. Both E.coli and K. pneumoniae showed highest sensitivity for doxycycline (75%; 67% resp.) and second highest for levofloxacin (70%; 64% resp.), whereas, P. aeruginosa showed highest 57% sensitivity for amikacin followed by 48% for levofloxacin. β-lactam antibiotics and aminoglycosides showed high mean resistance (K.pneumoniae-83%, E.coli-79%, P. aeruginosa-86.4%) and (K. pneumoniae-75%, E. coli-61%, P. aeruginosa-70%) resp.Conclusions: The data indicates high resistance among the gram-negative bacteria for β-lactam and aminoglycoside antibiotics. Increasing resistance to doxycycline and flouroquinolones for K. pneumoniae and E. coli and multidrug resistance to P. aeruginosa is a cause of concern in this region. Thus, there is a need to stop misuse of antibiotics with immediate effect and to implement a strong antimicrobial stewardship program.

Background: Urinary tract infections are commonly seen in febrile children, high incidence of Vesico Ureteral Reflux (VUR) is frequently seen in paediatric patient which is of concern as it may lead to renal scarring. Despite presence of established clinical guidelines there is disparity amongst physician in the diagnosis and treatment of UTI, some physician prescribes taking into consideration the symptoms, some prescribe on the basis of smell and colour and some rely on urine culture and sensitivity report. There is dearth of studies in many tertiary health care centers regarding antimicrobial use. Authors conducted this study to evaluate antimicrobial sensitivity pattern, efficacy and cost effectiveness of antimicrobials used for UTI in children.Methods: Patients of urinary tract infection <13 years of age were included in the study. Symptoms of patient i.e. pain in abdomen, haematuria, increased frequency of urination and degree of fever were recorded. Efficacy was measured through calculating degree of defervescence per hour. Cost effective model was prepared by calculating cost effective ratio i.e. dividing cost of antimicrobial required to bring down the fever and degree through which fever came down.Results: E. coli was the most common pathogen isolated from urine positive culture (69.07%). Nitrofurantoin has shown highest sensitivity to all uropathogens (72.73%). Cotrimoxazole, ciprofloxacin and nitrofurantoin were found to be most cost effective.Conclusions: In this current study authors found E coli is the most common uropathogen isolated. Ceftriaxone, cefixime, cotrimoxazole and amikacin were found to be most efficacious. Cotrimoxazole, ciprofloxacin and nitrofurantoin were found to be most cost effective.

The research works was conducted with a view to isolate and identify the Escherichia coli (E. coli) organism from diarrhoeic cases of buffalo reared in selected areas of Bangladesh as well the prevalence and antibiotic sensitivity pattern of the isolated E. coli in the Department of Microbiology and Hygiene, Bangladesh Agricultural University (BAU), Mymensingh-2202 during the period from April 2008 to May 2009. A total of 50 rectal swab samples were collected from 4 different places namely Haluaghat and Boira of Mymensingh, Madupur of Tangail and Kazipur of Sirajgonj districts. The samples were aseptically carried to the laboratory of the Department of Microbiology and Hygiene and subjected to different cultural, morphological and biochemical examinations. Upon cultural, morphological and biochemical examinations 23 (45%) samples were found to be positive for E. coli. The highest prevalence was found in Haluaghat, Mymensingh (53.33%) and the lowest (40.00%) in Boira, Mymensingh and Kazipur, Sirajganj. Antibiogram study revealed that the isolated E. coli was highly sensitive to Enrofloxacin and Ciprofloxacin, moderately sensitive to Cefalexin and Amoxicillin, and resistant to Nalidixic acid and Erythromycin. DOI = 10.3329/bjvm.v8i1.7398 Bangl. Introducción: Las infecciones del tracto urinario (ITU), constituyen una de las infecciones bacterianas más prevalentes, sus agentes etiológicos incluyen Escherichia coli, Klebsiella spp, Proteus spp, Enterococcus spp, Pseudomonas spp y Staphylococcus spp, los cuales presentan prevalencias y perfiles de susceptibilidad antibiótica diferentes entre poblaciones. Objetivo: Determinar la prevalencia de ITU, uropatógenos y el perfil de susceptibilidad antimicrobiana en una Institución Prestadora de Servicios de Salud (IPS) de Medellín, 2011-2012. Métodos: Estudio de prevalencia en 1.959 individuos atendidos en una IPS de tercer nivel. Se calcularon medidas de resumen, proporciones, razones de prevalencia, Chi cuadrado y Fisher. Se cuantificó la modificación del efecto (confusión o interacción) con análisis estratificado y modelos de regresión logística binaria en SPSS 21.0®. Resultados: La prevalencia de ITU fue 31% los principales agentes etiológicos fueron E. coli (69%), Enterococcus spp (11%) y Klebsiella spp (8%). La ITU y la infección por E. coli fueron estadísticamente mayores en mujeres y adultos mayores. La mayor frecuencia de resistencia de E. coli fue para ampicilina (61%), Ácido nalidixico (48%), TrimetropinSulfa (48%) y Ciprofloxacina (42%); mientras que en Klebsiella spp fue Ampicilina (100%), TrimetropinSulfa (23%), Ampicilina- Sulbactam (22%) y Cefalotina (19%). Conclusión: La elevada prevalencia de ITU, la multiplicidad de uropatógenos aislados, la identificación de grupos de mayor riesgo y la diversidad de perfiles de resistencia antibiótica, evidencian la necesidad de desarrollar investigaciones locales que permitan orientar las acciones en salud y vigilancia epidemiológica, acordes con las particularidades de cada población.

As infecções bacterianas do trato urinário (ITUs) são causa comum de doença em cães, gatos e humanos. Embora bactérias Gram positivas como Staphylococcus spp., Streptococcus spp. e Enterococcus spp., possam ocasionar ITUs, as bactérias Gram negativas (Escherichia coli, Proteus spp., Klebsiella spp., Pseudomonas spp. e Enterobacter spp.) respondem por 75% dos casos. Este estudo teve como objetivo determinar a frequência de diferentes gêneros de bactérias em ITUs em cães e gatos, bem como a sua sensibilidade aos antimicrobianos utilizados na rotina clínica. Portanto, amostras de urina de 100 cães e gatos com sinais de ITU foram coletadas assepticamente, sofrendo avaliação microbiológica por meio de métodos qualitativos e quantitativos, além de urinálise. Todos os isolados foram submetidos a testes de sensibilidade aos antimicrobianos. ITU foi confirmada em 74% dos animais, não havendo predominância quanto ao sexo. No que diz respeito à idade, 85% dos cães e 87% dos gatos tinham idades superiores a seis anos. Noventa e cinco cepas bacterianas foram isoladas, com maior frequência de Escherichia coli (55% do total) dos sorogrupos O6 e O2. Constatou-se níveis elevados de resistência a antimicrobianos nas cepas isoladas. Para as cepas Gram positivas, tetraciclina (46,1%), enrofloxacina, cotrimazol e estreptomicina (42,3% cada) foram as drogas com os maiores índices de resistência. Para as Gram negativas, amoxacilina e tetraciclina apresentaram percentuais acima de 50%. Multiresistência foi verificada em mais de 50% dos principais gêneros isolados. Considerando-se que as cepas de E. coli apresentam potencial zoonótico e forte participação na disseminação de resistência aos antimicrobianos, ressalta-se a importância do papel do médico veterinário na prevenção e controle das ITUs animais e sua contribuição para a saúde pública.

A prospective study was carried out in 50 burn patients admitted in Burn unit of Dhaka Medical College Hospital over a period of one year from January 2005 to December 2005 to evaluate time-related changes in aerobic bacterial colonization and their sensitivity pattern. Periodic swabs were taken from the burn wound on Day 0, Day 7, Day 14 and Day 21 to see the changing pattern of organisms during hospital stay of patients. In the present study burn injury was highest in the age group 11-20 yrs (34%). Male to female ratio was 1.38:1. The mean percentage of burn was 35.79% of total body surface area (TBSA). Fire was the major cause of burn (38%) followed by electric burn (20%). Among the 200 samples, single organism was isolated in 71% samples and mixed organism in 13.5% and no growth in 14.5%. Among single isolates Pseudomonas aeruginosa was leading (28%) followed by Escherichia coli (17.5%), Staphylococcus aureus (16%), coagulse negative Staphylococcus (4.5%) and Klebsiella (2%). Among mixed growth Pseudomonas aeruginosa was still leading (11%) follow by E. coli (9.5%) Staphylococcus (5.5%), Proteus (1%) and Klebsiella (0.5%).There were time -related changes in bacterial isolation from burn wound during hospital stay of patients. On admission 42% of the isolated organisms were Staphylococcus aureus and only 6% each Pseudomonas aerunginosa and E. coli were isolated. No growth was found in 28%. samples. These findings were gradually changing with time and on day 21 Staphylococcus aureus were only 4% whereas Pseudomonas aeruginosa were 40% and E. coli 28%. Antimicrobial sensitivity test showed that pseudomonas aeruginosa was highly resistant to antimicrobial agents. It was most sensitive to Imipenem (98.72%) followed by Aztreonam (33.44%), Ceftazidime (38.32%) and Gentamicin (19.23%). E. coli was also found most sensitive to Imipenem (98.15%) followed by Gentamicin (38.95%), Chloramphenicol (37.1%), Ciprofloxacin (35.25%) and ceftraixone (29.70%). Staphylococcus aureus was 100% sensitive to Vancomycin followed by Amoxiclav and Oxacillin (53.43% each), Gentamicin (44.70%) and Cloxacillin (39.52%). It is crucial for every burn institution to determine the specific pattern of burn wound microbial colonization, the time-related changes in dominant flora, and the antimicrobial sensitivity profiles. This would enable early treatment of septic episodes with proper empirical systemic antibiotics without waiting for culture results, thus improving overall infection related morbidity and mortality.
