Background: Information about causes or related factors involving voice change was studied mainly in central tertiary level hospitals previously, not in hospital in peripheral districts in Bangladesh Objective: To find out and evaluate the etiological factors of hoarseness of voice irrespective of age and sex. Setting: Pabna medical college, a 250 bedded hospital in Bangladesh Methods: This was a cross sectional, non- randomized and longitudinal study conducted from 1st august 2011 to July 2017. All the patients with history of hoarseness underwent clinical examination, routine as well as special investigation to find the diagnosis. The final results were analyzed by simple manual analysis with frequency and percentage using SPSS program in 2017. Results: There were total 200 patients included in the study. Among them the age groups of 21 – 30 years and 31 – 40 years were mainly suffer from hoarseness. Similarly, among 200 patients 144 (72%) were males whereas 56 (28%) were females with male to female ratio of 2.5:1. The most common cause as per the distribution was acute laryngitis with frequency of 34% followed by acid peptic laryngitis, 25.5%, neoplasms of larynx 12%, whereas tuberculosis of larynx, intubation granuloma, trauma was very few. Conclusion: There was etiological variation in hoarseness ranging from simple laryngitis to malignancies. So it is important not to ignore the hoarseness and precise history, examination and investigations should be done  Bangladesh J of Otorhinolaryngology; October 2018; 24(2): 125-130

Objective: To evaluate the aetiological factors of hoarseness. Methods: This is a prospective, non- randomized and longitudinal study conducted from 1st august 2011 to 1st august 2012 in department of otorhinolaryngology of Kathmandu university Hospital, Dhulikhel, Nepal. All the patients with history of hoarseness underwent clinical examination, routine as well as special investigation to find the diagnosis. The final results were analyzed by simple manual analysis with frequency and percentage using Microsoft Excel software 2007. Results: There were total 280 patients included in the study. Among them the age groups of 21 – 30 years and 31 – 40 years were mainly suffer from hoarseness. Similarly, among 280 patients 200 (71.45%) were males whereas 80 (28.6%) were females with male to female ratio of 2.5:1.. The most common cause as per the distribution was acid peptic laryngitis with frequency of 37.8% whereas tuberculosis of larynx, papillary carcinoma of thyroid and papilloma of vocal cord accounts for only 0.4% each. Conclusion: There was etiological variation in hoarseness ranging from simple laryngitis to malignancies. So it is important not to ignore the hoarseness and precise history, examination and investigations should be done. Objective: This study was conducted on tracheostomy to find out the different indications and complications occured at the tertiary level hospital in Bangladesh. Study design: Cross-sectional observational study.Place and Duration: This study was conducted at the department of Otolaryngology & Head Neck surgery and department of ICU, Sir Salimullah Medical College Mitford Hospital, Dhaka & DMCH over one year from January 2014 to December 2014.Methodology: 55 tracheostomy operation were performed in both genders and all age groups either in emergency situations or within an elective environment. All the patients were registered with a definite protocol by filling a pre designed proforma equipped with demographic data, details of present illness mirroring the indication for the procedure, relevant physical examination & investigations, details of the surgical procedure of tracheostomy and postoperative follow up with note of any attendant complications described as immediate, early and late postoperative.Results: Mean age of this study was 40.09 years and male female ratio iwas 3.2: 1. 30(54.54%) patients underwent emergency tracheostomy and 25(45.46%) underwent tracheostomy electively. The commonest indications were laryngeal & hypopharyngeal malignancies which affected 28(50.9%) patients. The overall rate of complications was 25.45%. These were 4(28.57%) immediate post operative, 8(57.14%) early post operative and 2(3.63%) late post operative. Out of overall 14 complications, 11(36.66%) complications occurred in 40 patients who underwent tracheostomy in emergency and 3(12%) complications occurred in 25 patients who underwent tracheostomy electively.Conclusion: Upper airway obstruction due to laryngeal and hypopharyngal carcinoma still remains the most common indication for tracheostomy in our centre but prolonged intubation as an indication for tracheostomy is evolving based on recent improvement in the facilities available at our ICU.J Dhaka Medical College, Vol. 25, No.1, April, 2016, Page 67-71

Objective: To identify common clinical features, sociodemographic characteristics and laboratory parameters of diagnosed cases of Subacute Sclerosing Panencephalitis.Design: Cross sectional descriptive type of study.Setting: Department of Paediatrics, Dhaka Medical College Hospital.Study period: January 2006 to December 2008.Subjects: Twenty clinically diagnosed patients of SSPE.Results: The mean age at presentation was 8 years. Male: Female ratio was 19:1. Most of the patients came from lower socio-economic group (70%). Forty five percent had history of primary measles infection and seventy percent were vaccinated against measles. Most common presenting features were fall to ground (95%), cognitive decline (85%), myoclonic seizures (80%), altered speech (70%), gait disturbance (60%), personality changes (55%), dysphagia (50%) and less commonly blindness (20%). EEG showed abnormal findings in 100% of patients who underwent this test. Measles specific IgG antibody in CSF was positive in 90% cases. Neuroimaging findings were abnormal in 43% cases.Conclusion: The diagnosis of SSPE should be considered in children presenting with deteriorating milestones of development especially cognition and behavior, fall to ground along with myoclonic jerks in an endemic country for measles infection. Investigations like CSF and serum antibody to measles virus and characteristic EEG changes may help further in the diagnosis. Aims: Teenage pregnancy has globally recognized as high risk pregnancy. Under grown pelvic bones at delivery pose greater risk of obstructed labour in such young girls resulting in poor feto-maternal outcome. Objective of this study was to determine the frequency of obstructed labor in teenage pregnancy and to know fetomaternal outcome. Methods: This Cross Sectional Observational study was conducted at department of Obstetrics and Gynecology unit I and II, Shaikh Zyed Women Hospital Chandka Medical College, Shaheed Mohtarma Benazir Bhutto Medical University Larkana Sindh Pakistan from 1st January 2010 to 31st December 2010. 468 women admitted with obstructed labour of all age groups, out of these which 257 patients were teenage mothers. Patients selected after fulfilling selection criteria. Obstructed labour in teenage patients wasdiagnosed on the basis of history of prolonged labour and clinical presentation. Demographical characteristics noted. Mode of the delivery and fetomaternal outcome was observed. Statistical analysis was performed using SPSS. 12 version. Results: Total deliveries in both units during study period were 9000. Among them 468(5.2%) patients found to have obstructed labour. Out of these 257(2.85% of total) patients were teenagers.. 82% (210) teengers were non booked and admitted in emergency while only 18 %(47) came through OPD having a single visit. Mean age was 16±2 years and parity was 2±1.5.Mode of delivery was LSCS in 84% (214) ,assisted vaginal delivery in 12% (32) and 4% (11)had spontaneous delivery with episiotomy. Cephalopelvic disproportion remained the commonest reason of obstruction (66%). 1.94% (5) of patients died of septicemia .PPH seen in 41% (105),1.1% (3) had scar dehiscence due to prolonged trial by untrained birth attendant.1.94% (5) patients developed vesicovaginal fistula later on. Perinatal mortality was around 54.6% (142) while 44.75 % (115) babies born alive. Conclusions: Adolescent pregnant women not only face pregnancy related problem but also they are prone to have obstructed labour due to their developing pelvic bones. Obstructed labour is one of the most common and preventable causes of maternal and perinatal deaths and disabilities. Objectives: To know the frequency of obstructive pattern on spirometry in asymptomatic chronic smokers. Study Design: Cross-sectional study. Settings: Department of Pulmonology, Sahiwal Medical College Sahiwal. Period: 1st Nov, 2019 to 1st May 2020. Materials & Methods: A total of 205 chronic smokers aged 30-60 years were included. Patients with history of Ischemic heart disease, previously diagnosed cases of COPD and asthma were excluded. Spirometry was done by using portable spirometer by consultant pulmonologist (at least 2 years of post-fellowship experience) and obstructive pattern (COPD) was noted. Results: Majority males with ages between 41 to 50 years( mean age 47.78 ± 4.90 years)  out of the 205 patients, 137 (66.83%) were male and 68 (33.17%) were females with male to female ratio of 2:1. Mean duration of smoking was 8.81 ± 4.67 years. Mean height was 161.81 ± 9.23 cm. Mean weight was 78.81 ± 5.67 kg. Mean BMI was 26.21 ± 5.39 kg/m2. In this study, frequency of obstructive COPD on spirometry in asymptomatic chronic smokers was found in 29 (14.15%) patients. Conclusion: This study established that frequency of obstructive pattern on spirometry in asymptomatic chronic smokers is high.

Background and setting Recent reports have indicated the increasing incidence of lung cancer in women and younger age groups. A cross- sectional, retrospective study was undertaken to study the age and sex distribution and histological patterns of lung cancer in the patient population presenting to St.John's Medical college Hospital, Bangalore. Methods The age, sex and the clinical features of patients and gross findings of the specimens were retrieved from the archives of the department.Hematoxylin and Eosin stained paraffin sections were used for assessment of histological patterns. Results and Conclusions 80 patients(13.5%) were histologically diagnosed as having lung cancer in the study done over a period of 6 years(1995-2000).Peak incidence was seen in 5th and 6th decade. 15 patients were <40 years of age with a male-female ratio 1.5:1 while the trend changed to 4.9:1 in >40 years age group. among the classifiable patterns, the common years age group. Among the classifiable patterns, the common histological pattern was squamous cell carcinoma, while 26.5% of lung cancer cases were poorly differentiated and could not be further categorized indicating the need for advanced techniques. In 18.5% of patients who were less than 40 years of age with lack of male preponderance, the genesis was probably multifactorial. Market integration Correlation is a measure of market integration. Correlation Coefficients of Prices, which include trend, seasonal and cylindrical effects are not adequate to precisely measure the degree of market integration. Integration of "pure errors" (pure errors means error series free from autocorrelation effects) of several time series enables us to determine the significance or otherwise integration of markets.

Pathological changes in the prostate gland occur commonly with advancing age including inflammation, atrophy, hyperplasia and carcinoma and a change in volume is also evident. Estimation of volume of prostate may be useful in a variety of clinical settings. A cross-sectional descriptive study was designed to see the changes in volume of the prostate with advancing age and done in the Department of Anatomy, Dhaka Medical College, Dhaka from August 2006 to June 2007. The study was performed on 70 post-mortem human prostates collected from the unclaimed dead bodies that were under examination in the Department of Forensic Medicine, Dhaka Medical College, Dhaka. The samples were divided into three age groups; group A (10-20 years), group B (21-40 years) and group C (41-70 years). Volume of the sample was measured by using the ellipsoid formula. The mean ± SD volume of prostate was 7.68 ± 3.64 cm3 in group A, 10.61 ± 3.99 cm3 in group B and 15.40 ± 6.31 cm3 in group C. Mean difference in volume between group A and group C, group B and group C were statistically significant (p<0.001). Background: Information technology (IT) is a new way of teaching and learning. One of the promising media of information technology is e-Learning, which is used to enhance knowledge and skills among users. A student gains better and deep knowledge through a useful tool. This survey aimed to determine practices among medical students for e-Learning. Methods: This cross-sectional survey was conducted on 184 students amongst the 500 students currently enrolled in medical college. Data was collected using questionnaires and were analyzed through SPSS version 22. Chi-square was used for qualitative values. Results: Majority 90.80% (n=167) students were aware of e-Learning and were statistically high in first year students (p-value: 0.018). The student did not show statistically significant results for content learned through e-Learning with a p-value of 0.063. Different resources were used for e-Learning in which videos had the highest percentage (87.60%) and audios were used as the least resource for e-Learning (29.20%). Daily, 56% of the students use e-Learning for 1 hour or less and only 3% of the students used it for more than 4 hours. Conclusion: Majority of undergraduate medical students were aware of the use of e-Learning and most of them preferred e-Learning for their course work and studies showing a significant increase in understanding and use, compared to studies conducted earlier. Participants found e-Learning useful and effective tool in increasing knowledge and understanding of their subject. Keywords: e-Learning; Practices; Students.

Background: The declines in peak heart rate (HR) and fitness level with age are related; however, whether this association differs based on gender is not well appreciated. In a large cross-sectional cohort of women and men referred for a clinically indicated exercise treadmill test (ETT), we set out to determine whether the decrease in peak HR by age varied by gender (and fitness) in the Henry Ford Exercise Testing (FIT) project. Methods: We analyzed data on 38,196 apparently-healthy patients aged 18-96 [mean age 51 ± 12 yrs, 25% black, 48% women] who completed an ETT. Those with history of coronary heart disease, congestive heart failure, diabetes on medications, atrial fibrillation or flutter, or taking AV nodal blocking medications were excluded. Being “fit” was defined as achieving ≥ the median MET level for each sex/age-decile group. Peak HR vs age was plotted, and regression lines were used to determine the intercept and slope for each group. Results: Men had higher peak HR than women but with a greater decline over time; the respective intercepts and slopes for peak HR estimates were 202.9 and 0.90 for men and 197.3 and 0.80 for women, (p-interaction = 0.023). Fit people also started out with higher peak HR but approached unfit people at higher age groups; respective intercept and slope by fitness status were 203.0 and 0.87 for fit and 194.7 and 0.83 for unfit (p-interaction <0.001). Separate regression lines were generated for categories of fit men/unfit men, fit women/unfit women ( Figure ). Fit and unfit men had similar declines in peak HR with increasing age (slope=0.92); whereas fit women (slope=0.81) had a slightly greater decline in peak HR with increasing age than unfit women (slope=0.73). However, peak absolute HR for fit people still remains higher than for unfit people even into elderly ages. Conclusion: In this cross-sectional cohort of patients referred for a clinical ETT, we found that the age-related decline in peak HR is influenced by both gender and fitness status.

PurposeThis research aimed to explore the prevalence of musculoskeletal disorders (MSDs) and related factors among subcontracted cleaners in a teaching hospital in Thailand.Design/methodology/approachA cross-sectional study was conducted of 393 subcontracted cleaners in a teaching hospital, from May to June 2020. Face-to-face interviews were carried out using a standard questionnaire, consisting of four parts: (1) participant characteristics, (2) stress test, (3) work characteristics and (4) standardized Nordic questionnaire, Thai version, for MSDs outcome. Multiple logistic regression analyses were performed to determine the association between MSDs and related factors.FindingsThe prevalence of MSDs was 81.9%, involving mostly the lower back (57.7%), followed by the shoulder (52.6%). Factors significantly associated with MSDs were as follows: male gender (OR = 3.06, 95% CI [1.19, 7.87]), severe stress (OR = 2.72, 95% CI [1.13, 6.54]), history of injuries (OR = 4.37, 95% CI [1.27, 15.11]), mopping posture (OR = 2.81, 95% CI [1.43, 5.50]) and task duration (OR = 1.90, 95% CI [1.01, 3.57] for 2–4 h and OR = 3.39, 95% CI [1.17, 9.86] for more than 4 h). Sick leave due to MSDs was associated with history of injuries, Thai nationality and having another part-time job.Originality/valueThe study findings about MSDs in terms of prevalence and related factors contributed to limited pool of the knowledge among subcontracted hospital cleaners in Thailand and middle-income country settings. With growing popularity in outsourcing cleaning services among hospitals in these countries, the study findings could raise a concern and inform policymakers and hospital administrators the importance of the magnitude and risk factors for MSDs necessitating design of preventive strategies.
