Respiratory muscle weakness is common in neuromuscular disorders and leads to significant respiratory difficulties. Therefore, reliable and easy assessment of respiratory muscle structure and function in neuromuscular disorders is crucial. In the last decade, ultrasound and MRI emerged as promising imaging techniques to assess respiratory muscle structure and function. Respiratory muscle imaging directly measures the respiratory muscles and, in contrast to pulmonary function testing, is independent of patient effort. This makes respiratory muscle imaging suitable to use as tool in clinical respiratory management and as outcome parameter in upcoming drug trials for neuromuscular disorders, particularly in children. In this narrative review, we discuss the latest studies and technological developments in imaging of the respiratory muscles by US and MR, and its clinical application and limitations. We aim to increase understanding of respiratory muscle imaging and facilitate its use as outcome measure in daily practice and clinical trials.

Over the past twenty years, significant technical strides have been made in the area of vascularized composite tissue allotransplantation (VCA). As in solid organ transplantation, the allogeneic immune response remains a significant barrier to long-term VCA survival and function. Strategies to overcome acute and chronic rejection, minimize immunosuppression and prolong VCA survival have important clinical implications. Historically, large animals have provided a valuable model for testing the clinical translatability of immune modulating approaches in transplantation, including tolerance induction, co-stimulation blockade, cellular therapies, and ex vivo perfusion. Recently, significant advancements have been made in these arenas utilizing large animal VCA models. In this comprehensive review, we highlight recent immune strategies undertaken to improve VCA outcomes with a focus on relevant preclinical large animal models.

Abstract This paper builds on previous research to address the question of whether there is practical value for a made-in-Ontario municipal management system standard (MSS) for wastewater and stormwater related activities, in addition to the Drinking Water Quality Management System Standard (DWQMS) that is already statutorily required. This research specifically addressed the questions: is there value in a mandatory or voluntary MSS; are there neutral, positive, or negative effects of having an MSS; and what standard is more adequate? Through a focus group method, this research finds evidence in support of and wide recognition of the practical value an MSS in assisting municipalities in meeting their environmental objectives, addressing property damage risks, providing an additional mechanism of public accountability, and improving alignment with the legal structure. It was also apparent that there is no political appetite in the provincial government to embark on a mandated MSS, so the preferred option at this time appears to be a provincially endorsed, voluntary, sector-specific standard for wastewater and for stormwater, which could constitute a catalyst to boost voluntary uptake of MSS by small to medium municipalities (as is already occurring with large municipalities). This standard could be based on a customized variation of the ISO 14001 and DWQMS.

ABSTRACT Complications and pitfalls are unavoidable sequel of any surgical procedure. Although advances have been made in surgical techniques and fixation devices, these are far from ideal. The aim of trauma surgeon is to restore the patient to pre-injury form and function, but it may not be possible in each and every case. Treatment failure and poor outcome in a case of severe maxillofacial trauma is not uncommon. This is mainly dependent upon the extent of injury, delay in management, improper diagnosis, wrong treatment plan, poor execution of a treatment plan, wrong choice of fixation devices, inexperience of the surgeon and inability to seek help from other specialists, etc. The objective of this article is to discuss and analyze common complications and pitfalls along with the various methods and modalities to prevent and manage it.
