Gastrointestinal (GI) symptoms represent an important and often poorly appreciated reason of morbidity in diabetes mellitus. Diabetes can affect nearly all parts of the GI tract; however, data on the prevalence of ‘diabetic gastroenteropathy’ are inconsistent. The significance of disturbed GI motility in diabetes across the patient spectrum and pathophysiological basis also remain inadequately defined. Fluctuating glucose levels, altered drug pharmacokinetics, variable absorption of nutrients, and impaired quality of life are important consequences of GI dysfunction. Diabetic gastroparesis is the best characterised manifestation of GI motility disorder in diabetes. Since there is a poor correlation between subjective GI symptoms and objective motility findings, a diagnosis of delayed emptying in diabetes requires a proper measurement of gastric emptying. There are fewer studies on intestinal motility in diabetes than those on the stomach. Several established modalities exist for the assessment of gastroenteropathy but the lack of standardisation, exposure to radiation, advanced data interpretation, and high cost limit their widespread use. While existing therapeutic choices for the management of diabetic gastroenteropathy are suboptimal, many potential novel agents are in progress. Both endocrinology and gastroenterology specialties working together will facilitate screening and treating patients with diabetes and GI dysmotility.

SummarySocial networking services (SNS)-based online communities are good examples of improving quality of care by incorporating information technology into medicine. Therefore, we created an SNS-based community care webpage for communication among patients with diabetes mellitus (DM). We aimed to identify what diabetic patients wanted to know and were interested in by analyzing their posts and classified content in which users were interested.As opposed to the existing physician-focused health information websites, we built a patient-focused experience exchange website, “I love insulin (http://www.iloveinsulin.co.kr).” The DM communication webpage was divided into “My Web-Chart,” “My community-free board,” and “Life & Health.” The contents analysis targeted users’ postings, and replies were classified by theme from May 2012 to June 2013. The data included number of questions asked, answers, and question-to-answer (QA) ratio in each category.A total of 264 patients registered on the “I Love Insulin” website. The most frequent topic of posts classified as questions were about diabetes itself (23%), diet (22%), and glucose levels (19%). Conversely, most answers and information provided by users were about daily life with no relationship to diabetes mellitus (54%). While there were many questions about diet, there were very few answers (2%). Whereas there was much provision of knowledge about general DM, sharing diet information was rare. The ratios of answers to questions on diet (ratio=0.059, 1/17), glucose level (ratio=0.067, 1/15), insulin regulation (ratio=0.222, 2/9) and webpage (ratio=0.167, 1/6) were significantly low compared to DM itself (all p < 0.001).Patients in Korea with DM tend to have insufficient knowledge about diet and insulin regulation; continuously providing diet and insulin regulation information are desirable. Diabetes mellitus is one of the communicable diseases that have become a public health problem, not only in Indonesia but also the world. Currently morbidity of diabetes mellitus is increasing every year, where in 2006 there were 14 million people in 2011 and ranks fourth with 773 cases. DM is also a cause of disease mortality by 5.8%. And Samarinda own particular Islamic Hospital years 2009, there were 449 patients with DM, and 2011 an increase in the 1931 patients with diabetes mellitus. To increase patients’ knowledge about diabetes and diabetic patients be directing attitudes that support or positive attitude towards keeping blood glucose levels to remain normal. Methode to use pra eksperiment with one group pretest posttest. individual conseling withAudiovisual media. BACKGROUND Multiple laboratory tests are used to diagnose and manage patients with diabetes mellitus. The quality of the scientific evidence supporting the use of these tests varies substantially. APPROACH An expert committee compiled evidence-based recommendations for the use of laboratory testing for patients with diabetes. A new system was developed to grade the overall quality of the evidence and the strength of the recommendations. Draft guidelines were posted on the Internet and presented at the 2007 Arnold O. Beckman Conference. The document was modified in response to oral and written comments, and a revised draft was posted in 2010 and again modified in response to written comments. The National Academy of Clinical Biochemistry and the Evidence Based Laboratory Medicine Committee of the AACC jointly reviewed the guidelines, which were accepted after revisions by the Professional Practice Committee and subsequently approved by the Executive Committee of the American Diabetes Association. CONTENT In addition to long-standing criteria based on measurement of plasma glucose, diabetes can be diagnosed by demonstrating increased blood hemoglobin A1c (Hb A1c) concentrations. Monitoring of glycemic control is performed by self-monitoring of plasma or blood glucose with meters and by laboratory analysis of Hb A1c. The potential roles of noninvasive glucose monitoring, genetic testing, and measurement of autoantibodies, urine albumin, insulin, proinsulin, C-peptide, and other analytes are addressed. SUMMARY The guidelines provide specific recommendations that are based on published data or derived from expert consensus. Several analytes have minimal clinical value at present, and their measurement is not recommended.

The incidence and prevalence of diabetes mellitus and its complications are increasing. Like other complications, most of the diabetes patients have gastrointestinal (GI) symptoms but in majority of cases GI complications are under diagnosed and not treated properly, resulting in impairment of the quality of daily life. GI system including liver and pancreas are involved in diabetes mellitus. These GI complications of diabetes mellitus need proper diagnosis and treatment to get a quality of life and clinician needs clinical suspicion to identify and proper knowledge to treat.

ABSTRAK Diabetes Melitus merupakan penyakit yang paling menonjol yang disebabkan oleh gagalnya pengaturan gula darah. Lidah buaya berkhasiat untuk menurunkan kadar gula dalam darah bagi penderita diabetes dan dapat mengontrol tekanan darah. Tujuan penelitian untuk mengetahui pengaruh jus lidah buaya (AloeBarbadensis Miller) terhadap penurunan glukosa darah puasa GDP) dan 2 Jam PP (Post Prendial) pada penderita DM (Diabetes Melitus). Desain penelitian merupakan Quasi Eksperimental dengan pendekatan one group pretest – postest design. Sampel terbagi menjadi 14 responden. Data dianalisis dengan paired t-test. Hasil menunjukan rata-rata penurunan glukosa darah puasa pada intervensi (28,42 gr/dl) dan glukosa darah 2 jam pp pada intervensi (40,57 gr/dl). Untuk analisis bivariat terdapat perbedaan antara glukosa puasa dan 2 jam pp dengan glukosa darah puasa GDP) dan 2 Jam PP (Post Prendial) pada penderita DM (Diabetes Melitus). Kesimpulan didapatkanlidah buaya dapat menurunkan kadar glukosa darah.Berdasarkan hasil penelitian jus lidah buaya dapat menjadi salah satu alternatif keperawatan non farmakologi dalam penyakit diabetes melitus. Kata Kunci : Lidah Buaya ; Diabetes Mellitus THE EFFECT OF VEGETABLE VOCATIONAL JUICE ON FAST BLOOD GLUCOSE LEVELS AND 2 HOURS OF PP (Post Prandial) IN DIABETES MELLITUS  ABSTRACT Diabetes mellitus is the most prominent disease caused by the failure of blood sugar regulation. Aloe vera is efficacious can to  reduce blood sugar levels for diabetics and can control blood pressure. The purpose of this study was to determine the effect of Aloe Barbadensis Miller on the reduction of fasting blood glucose GDP and 2 hours of PP (post prendial) in patients with diabetes mellitus. The research design is Experimental Quasi with one group pretest - postest design approach. The sample is divided into 14 respondents. Data were analyzed by paired t-test. The results showed an average decrease in fasting blood glucose at intervention (28.42 gr / dl) and 2 hours pp blood glucose at intervention (40.57 gr / dl). For bivariate analysis there was a difference between fasting glucose and 2 hours pp with fasting blood glucose GDP) and 2 hours PP (Post Prendial) in patients with diabetes mellitus. The conclusion is that aloe vera can reduce blood glucose levels. Based on the results of research on aloe vera juice can be an alternative non-pharmacological nursing in diabetes mellitus. Keywords: Aloe Vera ; Diabetes Mellitus

Aim: The aim of the study was to evaluate the QOL of patients with Diabetes Mellitus. Study Design: This was a prospective, observational study. Duration of Study: The study was conducted from August 2019 to January 2021 in Yashoda Hospital, Hyderabad. Methodology: Patients of either sex with ≥1year history of diabetes willing to give the consent were included in the study. Patients of either sex with <1year history of DM, Pregnant/lactating women and patients not willing to give the consent were excluded from the study. Data on Blood glucose levels (FBS, PPBS) and HbA1C was also obtained and assessed. QOLID questionnaire was administered to the patients and assessed which consisted of a set of 34 items representing 8 domains such as Role limitation due to the physical health, Physical endurance, General health, Treatment satisfaction, Symptom botherness, Financial worries, Mental health, and Diet satisfaction). Results: A total of 200 patients were analysed in the study,108(54%) were males and 92(46%) were females. The average age of the patients was 58.5 years with majority being 51-70years (73.5%) of age. Patients with higher age and females had poor QOL compared to others. The correlation between various categorical variables with that of scores of QOL in various domains was assessed, Age of the patients influenced QOL score in various domains like RLPH (p value-0.038), PE (p value-0.0183), and SB (p value-0.0002), Gender has influenced QOL score in domains like RLPH (p value-0.0008), PE (p value-0.0106), TS (p value-0.0005) and Educational Qualification has influenced QOL score in RLPH (p value-0.0008), GH (p value-<0.0001), TS (p value-<0.0001), E/MH (p value-<0.0001). Conclusion: The results concluded that overall QOL was noticeably low in Diabetic patients especially in Women and elderly thus indicating that Diabetes management is not restricted to treatment but also requires attention on QOL of patients.
