On epidemiology of congenital ichthyosis in Moscow

Ichthyosis is an extensive group of hereditary skin diseases characterized by keratinization disorders of the type of hyperkeratosis, which leads to the formation of scales on the skin resembling fish scales. Of all hereditary diseases of keratinization, ichthyosis is the most common disease. In many countries, there are various support groups for patients with ichthyosis, in particular, the All-Russian Charity Foundation for Helping Children with Ichthyosis, the Children- Butterflies Foundation, the Public Organization Support for People with Ichthyosis (Russia), the Foundation for Ichthyosis and Related Skin Types (USA), etc.Objective. To assess the dynamics of the incidence of congenital ichthyosis in the city of Moscow among the population of all age groups for 2015–2020, according to the data of the Moscow Scientific and Practical Centre for Dermatovenerology and Cosmetology (Russia).Materials and methods. Using federal statistical observation form No. 12 ‘Information on the number of diseases registered in patients living in the service area of a medical organization’, approved by the order of Rosstat No. 679 dated November 22, 2019, we analyzed the incidence of congenital ichthyosis in Moscow for 2015–2020.Results. Analysis of data on the incidence of congenital ichthyosis in the context of age groups of the population of the city of Moscow for 2015–2020 showed a trend towards an increase in the incidence of congenital ichthyosis. The increase in the incidence of ichthyosis was detected in almost all age groups of the city's population and was most pronounced in 2018 and 2019 in comparison with the previous time interval.Conclusions. The revealed increase in the incidence of congenital ichthyosis in Moscow may be due to both the true number of cases and the improvement in the quality and availability of primary specialized health care. An important trend is towards wider implementation in medical practice methods, which make it possible to identify a specific genetic defect, to carry out prenatal diagnostics during pregnancy, an intravital histological (pathological-anatomical) study of a skin biopsy, if necessary, differential diagnosis and verification of the diagnosis, as well as genetic counseling of parents, including to assess the birth of children with ichthyosis during pregnancy.

Aim. To study organization of the outpatient care in Yakutsk, incl. that on the base of Medical Center of Yakutsk in the period from 2012 to 2018. Materials and Methods. For this study, statistical data were used on staffing, attendance, morbidity of the children’s and adult population of the city of Yakutsk by ICD-10 classes (per 100 thousand population) for 2012-2018. Statistical data for the work were obtained from an official source – Yakutsk Republican Medical Information and Analytical Center and medical organizations providing primary health care. The work used the data of official statistical reports (forms 7, 12, 17, 30, 36), demographic parameters of the population of the city of Yakutsk. Statistical data processing was carried out using Statistica10 software (Stat Soft Inc., USA) and Microsoft Excel. SWOT analysis of the organization of outpatient care for the population of the city of Yakutsk was carried out. Results. The health status of the population of Yakutsk was characterized by a high level of morbidity in all age groups. Organization of medical care for the population of Yakutsk was characterized by expansion of the network of medical organizations and strengthening of their material and technical base. Introduction of new management mechanisms into the activities of Medical Center in Yakutsk in 2012-2018 allowed to achieve high results of work among the attached population. Conclusions. Development of quality standards for medical and diagnostic services has reduced the number of acts of insurance companies with defects in the provision of medical care and penalties. Introduction of an «effective contract» made it possible to increase the previous level of salaries of medical workers, to additionally attract new workers, and also to increase the effi-ciency of the organization. Formation in a medical organization of a multilevel performance audit system based on a process approach made it possible to identify the strengths and weaknesses of the organization and develop measures to improve the availability and quality of medical care.

The COVID-19 pandemic is a global challenge, and the key to tackling it is vaccinating a specified percentage of the population to acquire herd immunity. The observed problems with the efficiency of the vaccination campaigns in numerous countries around the world, as well as the approach used at the initial stage of the National Immunization Program in Poland, prompted us to analyse the possibility of using GIS technology to optimize the distribution of vaccines to vaccination sites so as to minimize the period needed to vaccinate individual population groups. The research work was carried out on the example of Warsaw, the capital of Poland and the city with the largest population in the country. The analyses were carried out for the 60–70 and 50–60 age groups, in various approaches and for vaccines of different companies (Moderna, BioNTech, AstraZeneca), used to vaccinate people in Poland. The proposed approach to optimize vaccine distribution uses Thiessen’s tessellation to obtain information on the number of people in a given population group living in the area of each vaccination site, and then to estimate the time needed to vaccinate that group. Compared to the originally used vaccination scenario with limited availability of vaccines, the proposed approach allows practitioners to design fast and efficient distribution scenarios. With the developed methodology, we demonstrated ways to achieve uniform vaccination coverage throughout the city. We anticipate that the proposed approach can be easily automated and broadly applied to various urban settings.

The method of operation of a pilot car seat rental scheme is described. It is unique in that there are sufficient seats to accommodate every newborn infant in the city. Both infant and child seats are available. The pattern of use since the establishment of the scheme in December 1981 is reported. Currently 60% to 70% of parents rent infant seats and 35% to 40% rent child seats. Yearly roadside observations have shown a steady increase in the number of restrained children. In 1981 no infants traveled in approved restraints. In 1984 66% did so. The 1984 results for other age groups were: 6 to 18 months of age, 88%; 18 months to 2½ years of age, 82%; 2½ to 3½ years of age, 66%; 3½ to 4½ years of age, 62%. At this time no legislation applied to children less than 8 years of age. The success of this pilot scheme suggests it should be expanded on a national scale.

Abstract. Effects of a new aerosol particle formation (NPF) and particle diameter growth process as a single source of atmospheric particle number concentrations were evaluated and quantified on the basis of experimental data sets obtained from particle number size distribution measurements in the city centre and near-city background of Budapest for 5 years. Nucleation strength factors for a nucleation day (NSFNUC) and for a general day (NSFGEN) were derived separately for seasons and full years. The former characteristic represents the concentration increment of ultrafine (UF) particles specifically on nucleation days with respect to accumulation-mode (regional background) concentrations (particles with equivalent diameters of 100–1000 nm; N100−1000) due solely to the nucleation process. The latter factor expresses the contribution of nucleation to particle numbers on general days; thus, it represents a longer time interval such as season or year. The nucleation source had the largest effect on the concentrations around noon and early afternoon, as expected. During this time interval, it became the major source of particles in the near-city background. Nucleation increased the daily mean concentrations on nucleation days by mean factors of 2.3 and 1.58 in the near-city background and city centre, respectively. Its effect was largest in winter, which was explained by the substantially lower N100−1000 levels on nucleation days than those on non-nucleation days. On an annual timescale, 37 % of the UF particles were generated by nucleation in the near-city background, while NPF produced 13 % of UF particles in the city centre. The differences among the annual mean values, and among the corresponding seasonal mean values, were likely caused by the variability in controlling factors from year to year. The values obtained represent the lower limits of the contributions. The shares determined imply that NPF is a non-negligible or substantial source of particles in near-city background environments and even in city centres, where the vehicular road emissions usually prevail. Atmospheric residence time of nucleation-mode particles was assessed by a decay curve analysis, and a mean of 02:30 was obtained. The present study suggests that the health-related consequences of the atmospheric NPF and growth process in cities should also be considered in addition to its urban climate implications.

ABSTRACT Our internal clock system is predominantly dopaminergic, but memory is predominantly cholinergic. Here, we examined the common sensibility encapsulated in the statement: “time goes faster as we get older”. Objective To measure a 2 min time interval, counted mentally in subjects of different age groups. Method 233 healthy subjects (129 women) were divided into three age groups: G1, 15-29 years; G2, 30-49 years; and G3, 50-89 years. Subjects were asked to close their eyes and mentally count the passing of 120 s. Results The elapsed times were: G1, mean = 114.9 ± 35 s; G2, mean = 96.0 ± 34.3 s; G3, mean = 86.6 ± 34.9 s. The ANOVA-Bonferroni multiple comparison test showed that G3 and G1 results were significantly different (P < 0.001). Conclusion Mental calculations of 120 s were shortened by an average of 24.6% (28.3 s) in individuals over age 50 years compared to individuals under age 30 years.

Food hypersensitivity is a reaction to the food consumed, regardless of the pathogenetic mechanisms that cause the symptoms. It is an actual and controversial problem in pediatric practice. Nowadays there is an active search for the causes of disease progression, a large role is given to the study of genetic and external factors (food, environmental, social). This disease arises many questions due to the similarity of the clinical representation in different kinds of food hypersensitivity and in different pathogenetic mechanisms, which are involved. The purpose of the study is to perform the analysis of factors, which cause appearance of food hypersensitivity in toddlers. Materials and methods. A study of the number of children with food intolerance was conducted using a specially compiled questionnaire. Thus, 4,500 questionnaires were distributed in pre-school and medical establishments to question parents. Results and discussion. Analysis of 3,214 questionnaires was conducted, which enabled to obtain information from parents on anamnesis and living conditions of toddlers. Values of 56 factors were analyzed, calculating correlation coefficients with a formation of food hypersensitivity for each of them. Statistical analysis allowed distinguishing 15 signs among these factors, which significantly correlated with the formation of food hypersensitivity in young children. The investigation enabled not only to detect factors that affect formation of food hypersensitivity in young children, but also to suggest a mathematical model of individual calculation of risk factors for this pathology. Data of conducted mathematical analysis can be used for elaboration of a complex of prophylaxis measures on development of food hypersensitivity in toddlers. Conclusion. The formation of hypersensitivity to cow's milk in children is provoked by the presence of contact reactions in the child, adverse reactions after medication, positive family history (bronchial asthma in relatives, skin diseases in parents (father and / or mother)), smoking in the family, living in the city; at the same time, preventive factors are living in an apartment, in a new building, in a dry apartment. The formation of food hypersensitivity in young children is generally provoked by a positive family history (bronchial asthma, hay fever, urticaria, diseases of the stomach and duodenum in relatives, skin diseases in parents), smoking in the family; frequent consumption of food in a mass catering points; living in the city plays a preventive role

Seasonal affective disorder (SAD) famously follows annual cycles, with incidence elevation in the fall and spring. Should some version of cyclic annual pattern be expected from other psychiatric disorders? Would annual cycles be similar for distinct psychiatric conditions? This study probes these questions using 2 very large datasets describing the health histories of 150 million unique U.S. citizens and the entire Swedish population. We performed 2 types of analysis, using “uncorrected” and “corrected” observations. The former analysis focused on counts of daily patient visits associated with each disease. The latter analysis instead looked at the proportion of disease-specific visits within the total volume of visits for a time interval. In the uncorrected analysis, we found that psychiatric disorders’ annual patterns were remarkably similar across the studied diseases in both countries, with the magnitude of annual variation significantly higher in Sweden than in the United States for psychiatric, but not infectious diseases. In the corrected analysis, only 1 group of patients—11 to 20 years old—reproduced all regularities we observed for psychiatric disorders in the uncorrected analysis; the annual healthcare-seeking visit patterns associated with other age-groups changed drastically. Analogous analyses over infectious diseases were less divergent over these 2 types of computation. Comparing these 2 sets of results in the context of published psychiatric disorder seasonality studies, we tend to believe that our uncorrected results are more likely to capture the real trends, while the corrected results perhaps reflect mostly artifacts determined by dominantly fluctuating, health-seeking visits across a given year. However, the divergent results are ultimately inconclusive; thus, we present both sets of results unredacted, and, in the spirit of full disclosure, leave the verdict to the reader.

Aim. To define characteristics of growth and development of children and adolescents of school age in the city of Samara.Methods. Measurements of anthropometric parameters of 2617 children aged 7-17 years, of which 1397 children studied in general educational institutions of Samara, 1220 schoolchildren - in the regional centers of the Samara region, were performed. The obtained values of anthropometric parameters of physical development of schoolchildren in Samara were compared with the study results of the appropriate age and sex groups of children and adolescents living in the Samara region.Results. Children of the regional capital and district centers come to school with virtually the same anthropometric parameters. Due to the inhomogeneous influence of conditions and nature of education, nutrition, motor activity indicators of physical development in the educative process acquire significant differences. The height of the city of Samara boys aged 7-15 years is lower than height of peers living in the Samara region (p ˂0.01). Body weight in girls to 9 years, and in boys since 7 years of age is less than in their peers who live in the district centers of the Samara region (p ˂0.05). The chest circumference of the Samara schoolchildren is significantly less than that of their peers of the Samara region in the age groups of 7-14 years in boys and 8-14 years in girls. Waist circumference of Samara boys in the age groups of 7-11 and 13 years and in girls aged 10-14 years is significantly less than that of the Samara region children. Hip circumference of the Samara schoolchildren in junior (7-10 years) and middle (11-14 years) school grades is less than in children of Samara region (p ˂0.05).Conclusion. Revealed differences in anthropometric parameters of children of the city of Samara and Samara region demonstrate the need to develop regional standards to evaluate the physical development of children and adolescents of big cities and rural areas.

Objective. To analyze the dynamics of the incidence of helminthiasis among children, who live in the city of Perm. Materials and methods. We have retrospectively analyzed the monthly reports of the Center for Hygiene and Epidemiology of the Perm Region for 20152020. These reports include the dynamics of the incidence of helminthiasis among four age groups of children: from 1 month to 2 years, 36 years, 714 and 1517 years. Results. Enterobiosis, ascariasis and toxocarosis predominate in the structure of the morbidity in the city of Perm. We found that over the past five years, there has been an increase in the incidence of enterobiosis by 16 %, but the increase in the incidence of ascariasis and toxocarosis reduced 18 and 25 %, respectively. The rate of decline remains insufficient and the incidence of enterobiosis exceeds the All-Russian one by 2.8 times, ascariasis by 2.7 times and toxocarosis by 4.3 times in comparison with the incidence of parasitic infestations in the Russian Federation. We found that the peak incidence of each nosology falls on the preschool age from 3 to 6 years. Just at this age period, children enter organized groups for the first time, get acquainted with the world through "taste it" and have low sanitary skills. Conclusions. A routine parasitic screening is effective, since in 2020, more than a half of sick children in Perm were identified during preventive examinations (65.7 %) and only in 34.3 % of cases the diagnosis of parasitic infestation was established on the basis of clinical data.

Data from records on population affected by contagious diseases in the period from 1991 to 1996 are entered in the computer data base and are managed by the Epidemiology corps of the City Public Health Institute. These data are used to serve the operative needs of the epidemiological corps in order to control contagious diseases within the district. They are analysed to obtain an insight into the problem of contagious diseases in general, and each individual disease in particular, while monitoring the distribution of diseases by gender, age and commune. In the period from 1991 to 1996 no person in Belgrade was down with a particularly dangerous contagious disease. The lowest number of the diseased was registered in 1991 (14788) and the highest in 1994 (27368). In 1996, the number of the diseased was higher by 33% than in 1991. Among the most widely spread contagious diseases was chicken pox (its share ranging from 22.5% to 54.6%), contagious diarrhea (from 7.4% to 17.3%), salmonellosis (from 2.9% to 6.1%), angina (from 2.6% to 5.6%). In the period reviewed, the above four diseases were among the ten most frequently incurred contagious diseases. In the course of 1993, measles fell within the group of ten and acquired epidemic proportions. The disease affected 3524 inhabitants of Belgrade (44. 7% children below 14 years of age and 32% young people from 15 to 19 years of age). The epidemic was caused by the discontinuation of regular vaccination of children against measles caused by the shortage of imported vaccination and by population migration. In the same year, the epidemic of rubella also broke out. The epidemic extended to 1994 and 1995. A breakdown of the diseased by gender ranged from 51.0% to 52.9% for males and from 47.1 % to 49.0% for females. Majority of the diseased were in the age groups 5-9 and 0-4 years while the least number of the diseased were among the aged. The youngest population was most frequently affected by chicken pox (from 35.6% to 69.4% of the overall number of those stricken by the disease), contagious diarrhea (from 4.5% to 17.5%), salmonellosis (from 3.1% to 7.4% ), angina (from 3.4% to 9.7%) and scarlet fever (from 3.1% to 7.3%). During the period reviewed, 69 people died from contagious diseases. Mortality from contagious diseases was lowest in 1992 and 1993 (3.1 and 3.4 per 1,000,000). It gradually rose to reach the highest value in 1996 (12.6% per 1,000,000). The largest number of people affected was reported in the communes of Novi Beograd, Zemun and Cukarica.
