Background: Symptoms of obsessive-compulsive disorder (OCD) are often underreported by patients and mainly triggered in the patients private domain, making it harder for clinicians to recognize OCD. Virtual reality (VR) can be used to assess OCD symptoms in the clinician's office. We developed a VR game in order to provoke subjective and physiological OCD symptoms. We hypothesize that (1) the VR game provokes more OCD symptoms in patients compared to healthy controls, (2) performing virtual compulsions leads to a reduction in emotional responses in OCD patients and that (3) the severity of VR game provoked symptoms correlates with severity of OCD symptoms.Methods: Participants played the VR game on a laptop while physiological measures were recorded simultaneously. We measured emotional responses, virtual compulsions and physiological arousal in response to our VR game in 26 OCD patients and 26 healthy controls. We determined correlations between emotional responses, virtual compulsions and OCD severity.Results: We found higher levels of VR-provoked anxiety (U = 179.5, p = 0.004) and virtual compulsions in OCD patients compared to healthy controls (p = 0.001). There was a significant reduction in emotional responses after performing virtual compulsions in the OCD patients. The emotional responses and virtual compulsions did not correlate significantly with Y-BOCS scores. A baseline difference between patients and healthy controls was found in heart rate variability (HRV), but no significant change in HRV, heartrate and skin conductance was found during the VR gameConclusions: Our study clearly shows our OCD VR game is capable of provoking more anxiety and virtual compulsions in patients with OCD compared to healthy controls. Providing a direct patient-rated measurement in the clinicians room, the VR game could help in assessing core OCD symptoms and recognizing OCD.Clinical Trial Registry Number: Netherlands Trial Register NTR5935.

Preliminary studies have provided promising results on the feasibility of virtual reality (VR) interventions for Obsessive-Compulsive Disorder. The present study investigated whether VR scenarios that were developed for contamination concerns evoke anxiety, disgust, and the urge to wash in individuals with high (n = 33) and low (n = 33) contamination fear. In addition, the feasibility of VR exposure in inducing disgust was examined through testing the mediator role of disgust in the relationship between contamination anxiety and the urge to wash. Participants were immersed in virtual scenarios with varying degrees of dirtiness and rated their level of anxiety, disgust, and the urge to wash after performing the virtual tasks. Data were collected between September and December 2019. The participants with high contamination fear reported higher contamination-related ratings than those with low contamination fear. The significant main effect of dirtiness indicated that anxiety and disgust levels increased with increasing overall dirtiness of the virtual scenarios in both high and low contamination fear groups. Moreover, disgust elicited by VR mediated the relationship between contamination fear and the urge to wash. The findings demonstrated the feasibility of VR in eliciting emotional responses that are necessary for conducting exposure in individuals with high contamination fear. In conclusion, VR can be used as an alternative exposure tool in the treatment of contamination-based OCD.

Background: The main aim was to examine changes in coping strategies, anxiety and depression in obsessive–compulsive and schizophrenia patients during COVID-19, in addition to controlling the influence of intolerance to uncertainty and experiential avoidance. Method: The first time, the study comprised (15–30 April 2020) 293 patients, 113 of whom were diagnosed with obsessive–compulsive disorder, 61 with schizophrenia and 119 healthy controls, aged 13–77 years (M = 37.89, SD = 12.65). The second time (15–30 November), the study comprised 195 participants (85 obsessive–compulsive patients, 42 schizophrenic patiens and 77 healthy controls participants). The evaluation was carried out through an online survey. Results: The clinical groups worsened over time in cognitive coping, anxiety and depression, while the control group only worsened in depression. Intergroup differences in anxiety, depression and coping strategies were maintained, highlighting the use of some maladaptive strategies (avoidance, spiritual) in clinical groups. Experiential avoidance and tolerance for uncertainty mainly affected coping. Conclusions: The duration of COVID-19 not only produced changes in anxiety and depression in clinical groups but also in coping strategies to face this pandemic and its consequences.

BackgroundObsessive–compulsive disorder (OCD) patients typically overmonitor their own behavior, as shown by symptoms of excessive doubt and checking. Although this is well established for the patients’ relationship with external stimuli in the environment, no study has explored their monitoring of internal body signals, a process known to be affected in anxiety-related syndromes. Here, we explored this issue through a cardiac interoception task that measures sensing of heartbeats. Our aim was to explore key behavioral and electrophysiological aspects of internal-cue monitoring in OCD, while examining their potential distinctiveness in this condition.MethodWe administered a heartbeat detection (HBD) task (with related interoceptive confidence and awareness measures) to three matched groups (OCD patients, panic disorder patients, healthy controls) and recorded ongoing modulations of two task-relevant electrophysiological markers: the heart evoked potential (HEP) and the motor potential (MP).ResultsBehaviorally, OCD patients outperformed controls and panic patients in the HBD task. Moreover, they exhibited greater amplitude modulation of both the HEP and the MP during cardiac interoception. However, they evinced poorer confidence and awareness of their interoceptive skills.ConclusionsConvergent behavioral and electrophysiological data showed that overactive monitoring in OCD extends to the sensing of internal bodily signals. Moreover, this pattern discriminated OCD from panic patients, suggesting a condition-distinctive alteration. Our results highlight the potential of exploring interoceptive processes in the OCD spectrum to better characterize the population's cognitive profile. Finally, these findings may lay new bridges between somatic theories of emotion and cognitive models of OCD.

Neurophysiological hyperactivation of cortical and subcortical brain areas has been reported in obsessive-compulsive disorder (OCD) using functional imaging techniques and electroencephalography (EEG). Also sleep disturbances and delayed sleep phases have been associated with OCD symptomatology. However, vigilance regulation in OCD during the transition phase from wakefulness to sleep onset remains unclear. Therefore the aim of this study was to analyze EEG-source estimates and EEG vigilance regulation in OCD patients in comparison to healthy controls.A 15 minute resting EEG was recorded in 30 unmedicated OCD patients and 30 healthy, age and gender matched controls. EEG power source estimates of the whole time series were computed by exact Low Resolution Brain Electromagnetic Tomography (eLORETA). Each consecutive one second EEG-segment was classified into one out of seven EEG-vigilance stages (0, A1, A2, A3, B1, B2/3, C) using Vigilance Algorithm Leipzig (VIGALL). The eLORETA analysis (log of F-ratios, p < 0.05, corrected for multiple comparison) revealed significantly increased delta power in the right superior frontal gyrus for OCD patients in comparison to healthy controls. Vigilance analysis yielded significantly increased amounts of high vigilance stage A2 (Mann-Whitney test, p < 0.001, corrected for multiple comparison) for OCD patients.This study repeated findings of altered EEG-power in frontal areas in OCD patients. Alterations of EEG-vigilance regulation were found with increased amounts of high vigilance stage A2. This is in line with a hypothesis of cortical hyperactivation in OCD. The value of EEG-vigilance as a possible biological marker for e.g. treatment response should be focus of further studies.

The purpose of the study was to summarize data from foreign studies about the use of virtual reality (VR) technology in the treatment of patients with various mental disorders.Materials and methods: we selected and analyzed articles from MEDLINE / PubMed databases during the period from 2000 till 2020, as well as relevant references in the bibliography of the analyzed articles. 45 articles were included in further analysis from 575 English-language articles.Results: The use of VR technologies in the treatment of mental disorders appears to be one of the promising directions in psychiatry. The successfulness of these methods in patients with eating disorders, anxiety-phobic disorders, and obsessive-compulsive disorder is supported to be evidentiary information. There is evidence of the effectiveness of such methods in patients with schizophrenia, particularly, one of the most significant results is an increase in social functioning. The absence of side effects data makes it possible to consider t VR therapy as a safe method. It is needed to make further study the areas of application of VR therapy and conduct research to identify possible side effects of this method.Conclusion: The obtained results are important for drawing attention to the prospect of using VR technologies in Russian psychiatry and demonstrate the need for further study of this method.

Objective: To assess executive functions in patients with Body Dysmorphic Disorder (BDD) and Obsessive-Compulsive Disorder (OCD) compared with healthy controls. Methods: Adults diagnosed with BDD (n=26) or OCD (n=29) according to DSM-5, and healthy controls (n=28) underwent validated and computerized neuropsychological tests; spatial working memory (SWM), Intra- extra dimensional set shifting (IED) and Stop signal task (SST), from the Cambridge Neuropsychological Test Automated Battery (CANTAB). Test performance was compared between groups, and correlated to standardized symptom severity of BDD and OCD. Significance level was set to p<0.05. Results: There were no statistically significant between-group differences on key outcome measures in SWM, IED, or SST. There was a weak positive correlation between symptom severity and test errors on SWM and IED in both OCD and BDD groups; increased clinical severity were associated with more errors in these tests. Further, there was a negative correlation between symptom severity and SST in the BDD group. Conclusions: Patients with BDD or OCD did not differ from healthy control subjects in terms of test performance, however there were several statistically significant correlations between symptom severity and performance in those with BDD or OCD. More studies on EF in BDD and OCD are required to elucidate if there are differences in EF between these two disorders.

Background New technologies are set to profoundly change the way we understand and manage psychiatric disorders, including obsessive-compulsive disorder (OCD). Developments in imaging and biomarkers, along with medical informatics, may well allow for better assessments and interventions in the future. Recent advances in the concept of digital phenotype, which involves using computerized measurement tools to capture the characteristics of a given psychiatric disorder, is one paradigmatic example. Objective The impact of new technologies on health professionals’ practice in OCD care remains to be determined. Recent developments could disrupt not just their clinical practices, but also their beliefs, ethics, and representations, even going so far as to question their professional culture. This study aimed to conduct an extensive review of new technologies in OCD. Methods We conducted the review by looking for titles in the PubMed database up to December 2017 that contained the following terms: [Obsessive] AND [Smartphone] OR [phone] OR [Internet] OR [Device] OR [Wearable] OR [Mobile] OR [Machine learning] OR [Artificial] OR [Biofeedback] OR [Neurofeedback] OR [Momentary] OR [Computerized] OR [Heart rate variability] OR [actigraphy] OR [actimetry] OR [digital] OR [virtual reality] OR [Tele] OR [video]. Results We analyzed 364 articles, of which 62 were included. Our review was divided into 3 parts: prediction, assessment (including diagnosis, screening, and monitoring), and intervention. Conclusions The review showed that the place of connected objects, machine learning, and remote monitoring has yet to be defined in OCD. Smartphone assessment apps and the Web Screening Questionnaire demonstrated good sensitivity and adequate specificity for detecting OCD symptoms when compared with a full-length structured clinical interview. The ecological momentary assessment procedure may also represent a worthy addition to the current suite of assessment tools. In the field of intervention, CBT supported by smartphone, internet, or computer may not be more effective than that delivered by a qualified practitioner, but it is easy to use, well accepted by patients, reproducible, and cost-effective. Finally, new technologies are enabling the development of new therapies, including biofeedback and virtual reality, which focus on the learning of coping skills. For them to be used, these tools must be properly explained and tailored to individual physician and patient profiles.
