The Sensitive Delusion of Reference is a clinical entity described by Ernst Kretschmer and never integrated into mainstream nosographic systems. It represents the possibility of developing psychosis starting from a personality characterized by sensitivity, scrupulousness, and fear of judgment of others. The presentation of the following clinical case highlights how the overlap between this clinical entity and mood disorders leads to characteristic psychopathology, which has not been sufficiently detailed. In particular, the delusions, which always starts from the idea of reference and the shame in the face of the judgment of others, takes on characteristics of guilt during the depressive phases and persecutory themes during the activation phases. This clinical observation, which obviously needs to be confirmed on a larger scale, encourages a renewed interest in the concept of Kretschmer's Sensitive Delusion of Reference and creates the possibility of intersecting multiple psychopathological levels, for a more complete perspective on the individual case.

Eczema herpeticum is a chronic dermatosis with erosive and ulcerative lesions of the skin in children of a predominantly young age. The clinical case presented in this article shows the severe course of herpesvirus infection combined with atopic dermatitis in a 5-month infant. A rash in the form of vesicles and pustules throughout the body with a predominant localisation on the skin of the face, the scalp, neck, and chest was found in the course of the examination of the child. Influence of infections on the course of allergic processes is an ambiguous and complicated issue. It has been proved that an infection can contribute to the development of allergies and exacerbate the course of already existing allergic inflammation. In recent years, an increasing number of studies have shown the preventive effect of infection on the development of allergic pathology in children, especially during the first years of life.

IntroductionIncest may be defined as sexual relations between close blood relatives. Legally, incest and sexual aggression toward minors are classified as a criminal behaviour. Tunisia is among the countries from which incest cases are rarely reported. Objectives and methodThe aim of this study is to investigate the relationship between the psychotic structure and incest, and to describe the individual, clinical, and criminal traits of the incestuous father through clinical observation.Case reportMr T.G is 46 years old. He is married and has six daughters. His wife appears to be passive, and largely dependent on her husband. Mr T.G has had incestuous relationships, initially, with his two eldest daughters. The acts were followed by the mother's complicit silence and the non-denunciation of the daughters. Two years later, he starts an incestuous behavior with his third daughter. Incest took place in the context of delusion. The patient was convinced that he is responsible of his daughters’ sexuality education. He develops an incoherent theory of purification with a tendency towards morbid rationalism. It is only after four years of insufferable paternal incestuous relationships that the third daughter filed a complaint to the police. A psychiatric expertise concluded that the accused is exempt from criminal responsibility.ConclusionIncest is a multi-faceted phenomenon, which makes its approach, comprehension, and treatment quite complex. For a psychotic patient incest is a means to deny alterity by crushing other. It also allows him to find, in this complete power, control over his annihilation anxiety.Disclosure of interestThe authors have not supplied their declaration of competing interest.

Wired facial lifting is a minimally invasive technique that allows for a quick lifting effect. The aim of this study is to address the effectiveness of spiked Polydioxanone (PDO) threads (cog) for facial rejuvenation of the lower third of the face, through a clinical case report. The methodology was characterized by a brief bibliographic review, through an electronic search in the databases: Pubmed, Lilacs, Cochrane and bibliographic bases published in the USA and Brazil, associated with a clinical case report, making it possible to compare it with data from the literature on complications, safety and efficacy in the short and medium term. It is concluded that the aesthetic treatments seek to soften the signs of time and rescue the elasticity and firmness of the dermis. For this, it is necessary to elucidate the physiological effects, providing opportunities for safe treatment.

BackgroundAssociations have been demonstrated between contextual (area level) factors and a range of physical health outcomes, but their relationship with mental health outcomes is less well understood.AimsTo investigate the relative strength of association between individual and area-level demographic and socioeconomic factors and mood disorder prevalence in the UK.MethodCross-sectional data from 19 687 participants from the European Prospective Investigation into Cancer and Nutrition in Norfolk.ResultsArea deprivation was associated with current (12-month) mood disorders after adjusting for individual-level socio-economic status (OR for top v. bottom quartile of deprivation scores 1.29, 95% C11.1–1.5, P < 0.001). However, this association was small relative to those observed for individual marital and employment status. Significant residual area-level variation in current mood disorders (representing 3.6% of total variation, P=0.04) was largely accounted for by individual-level factors.ConclusionsThe magnitude of the association between socio-economic status and mood disorders is greater at the individual level than at the area level.

The article demonstrates a case of diagnosis of a rare variant of primary skin lymphoma, namely of primary cutaneous CD4+ small/medium-sized pleomorphic T-cell lymphoma with an indolent course. The diagnosis of SMPTCL is based on the combination of the clinical picture, histological and immunomorphological data. A 70-year-old patient presented at the clinic with complaints about tumor-like formations on the skin of the face and back accompanied by moderate itching. The diagnosis was verified by pathomorphological and immunohistochemical studies of the biopsy of the affected skin. The presented clinical case emphasizes the importance of doctors’ clinical oncological alertness and the need for clinical and laboratory examination of the patient using modern histological and immunohistochemical methods for studying the skin biopsy.

PURPOSE: To illustrate the complexities of the evidence-based approach in clinical oncology practice and the implications for guidelines and evaluation of processes of care. PATIENT AND METHODS: A case report is presented in which a limited systematic review of the literature was used to address a specific clinical problem in an individual patient. Experts' opinions were also sought. RESULTS: A reasonable clinical decision was made by a participating patient based on indirect evidence of benefit that would be insufficient to support the same decision as a health policy in some jurisdictions. CONCLUSION: The practice of evidence-based oncology requires clinical judgment about the validity and applicability of research evidence. The factors that influence an evidence-based decision in the clinical context differ from those in the broader policy context, which could lead to legitimate differences in recommendations based on the same information. Used properly, the individual case report can be a powerful tool to illustrate complex clinical decision phenomena.

The lip brake is a fold in the oral mucosa, originating in the upper lip and insertion in the alveolar mucosa. At birth, the labial frenum may be inserted in the palatal papilla. As the individual grows, he can migrate to a more upper region, featuring a normal lip curl. However, when it remains inserted in the papilla, it is a case of hypertrophic lip curl. When pathological, this structure can promote the formation of interincisal diastema, periodontal pocket, gingival retraction, aesthetic problems, difficulties in phonation and brushing. The objective of this article is to report a clinical case of upper lip frenectomy using the double clamping technique or Archer technique, performed at the Oral Minor Surgery clinic of the Dentistry School of the Federal Fluminense University, in a male patient, leucoderma, 23 years old, indicated by his orthodontist after recurrence of interincisal diastema. From the procedure performed, it can be concluded that the surgical intervention and the applied technique were effective in solving the reported clinical case.

Introduction: Stroke is the most common disease of the central nervous system. Whereas craniocerebral trauma is defined as the function of the skull bones, broken soft tissue continuity and / or brain contents, including cranial nerves and cerebrospinal opponents. Aim: The aim of the study was to present the importance of kinesitherapy in the process of improving a 72-year-old patient with craniocerebral trauma after an ischemic stroke of the left hemisphere of the brain. Material and Methods: The method of individual case was used in the work. The study included a patient aged 72 with craniocerebral trauma after an ischemic stroke of the left hemisphere of the brain. The description of the individual case study was based on the subject and subject tests performed before and after rehabilitation. Results: The physical examination used the Lovett test to assess muscle strength, range of joint mobility using a goniometer, measurement of limb circumferences and the Ashworth scale to assess spasticity. The tests and functional scales were carried out twice in the patient, in whom her clinical condition was assessed. Conclusions: Based on the case report, it was found that the kinesitherapy used improved the patient’s physical condition. The pain and lymphoedema she was struggling with decreased. A significant increase in the range of mobility, strength and muscle mass of the limbs affected by hemiparesis was also observed. Individually selected rehabilitation program has improved proprioception, standing and gait re-education. The patient moved with the help of orthopedic equipment in the form of elbows.

The use of Nasogastric (NG) feeding in the provision of artificial nutrition and hydration at the end of life has, for the most part, been regarded as futile by the medical community. This position has been led chiefly by prevailing medical data. In Singapore, however, there has been an increase in its utilization supported primarily by social, religious and cultural factors expressly to prolong life of the terminally ill patient. Here this article will seek to review the ethical and clinical impact of this treatment and provide some understanding for such decisions in the light of the Duty of Palliative Care [DoPC]. Complemented by virtue ethics theory, the DoPC highlights and seeks to realize the individual case specific goals of care that maximize comfort and quality of life of the patient in the face of rapid attenuation of treatment options and the eminence of the final outcome by considering each of these factors individually in order to provide the best outcome for the patient and the family.
