In recent decades, the use of psychosocial and psychiatric care systems has increased worldwide. A recent article proposed the concept of psychiatrization as an explanatory framework, describing multiple processes responsible for the spread of psychiatric concepts and forms of treatment. This article aims to explore the potentials of the Open Dialogue (OD) approach for engaging in less psychiatrizing forms of psychosocial support. While OD may not be an all-encompassing solution to de-psychiatrization, this paper refers to previous research showing that OD has the potential to 1) limit the use of neuroleptics, 2), reduce the incidences of mental health problems and 3) decrease the use of psychiatric services. It substantiates these potentials to de-psychiatrize psychosocial support by exploring the OD’s internal logic, its use of language, its processes of meaning-making, its notion of professionalism, its promotion of dialogue and how OD is set up structurally. The conclusion touches upon the dangers of co-optation, formalization and universalization of the OD approach and stresses the need for more societal, layperson competencies in dealing with psychosocial crises.

Fiji inherited a British colonial healthcare system. In 2010 the long dormant mental health law was replaced by the Mental Health Decree (MHD), which set up divisional mental health units for the purpose of managing mental health problems outside of the old asylum. The Ministry of Health recruited an overseas consultant to help improve training. Under the MHD, stress management wards, stress management clinics and stress management day centres have been set up, to decentralise and deinstitutionalise psychiatric care. These are on the whole doing reasonably well and have good client acceptance.

Providing refugees with psychosocial support is particularly important considering the high level of mental health problems prevalent in this population. A psychosocial walk-in clinic operating within a state reception and registration center in Germany has been supporting mentally burdened refugees since 2016. This study focused on patients’ perspectives on their mental health burden, the psychosocial walk-in clinic, and future help seeking. We conducted interviews with n = 22 refugees attending the walk-in clinic from March to May 2019. Qualitative analysis focused on the following four topics: (1) mental burden from the patients’ perspective, (2) access to the psychosocial walk-in clinic, (3) perception of counseling sessions, and (4) perception of follow-up treatment. The results show that the majority of interviewees were burdened by psychological and somatic complaints, mostly attributed to past experiences and post-migratory stress. Therapeutic counseling and psychiatric medication were found to be particularly helpful. Most of the participants felt motivated to seek further psychosocial support. Key barriers to seeking psychosocial help included shame, fear of stigma, and lack of information. Overall, the psychosocial walk-in clinic is a highly valued support service for newly arrived refugees with mental health issues.

Abstract Background Increasing the capacity of higher education institutes to address students' vulnerabilities has been deemed necessary due to the financial crisis. Multilevel interventions have thus been initiated at the Hellenic Mediterranean University of Crete, as part of a project co-financed by Greece and the European Union (European Social Fund-ESF) through the Operational Programme <<Human Resources Development, Education and Lifelong Learning 2014- 2020 >> (MIS 5045937). The project aims to offer support to students with low family income or a certified disability, monitor their bio-psychosocial needs, increase their accessibility to health and psychosocial care, and improve their academic outcomes. Methods As part of the project, a primary health care unit has been set up to offer medical and nursing care (medical unit) as well as psychosocial support (counseling centre) to students in need. An observatory has also been set up to monitor their health and psychosocial needs and their academic progress. Results A total of 228 eligible students used the services in the second half of 2019 (counseling centre 37; medical unit 191). Out of the 37 individuals who used the counseling centre, 30 (81.0%) were female and 16 (43.2%) were at the first 2 years of their studies. A total of 36 (97.3%) requested psychological support, 6 (16.2%) warranted social welfare services and 1 (2.7%) support for learning disabilities. As for the 191 individuals who used the medical services, 101 (52.8%) were male and 88 (46.0%) were at first 2 years of their studies. Most of them had a health examination to receive a health certificate (74.8%) followed by those who received emergency care (e.g. respiratory infection, allergic reaction, injury, etc), chronic disease management and medicine subscription, as well as vaccination. Conclusions Complex bio-psychosocial needs have been identified, recorded and analyzed to explain the academic progress of socially vulnerable students. Key messages The students’ journey through medical and counseling services is being mapped to offer important information for educational policy. Assessment and monitoring of students’ complex needs are important to achieve quality in higher education.

ABSTRAK Pandemi COVID-19 merupakan bencana non alam yang berdampak tidak hanya pada kesehatan fisik saja tetapi berdampak pula terhadap kesehatan jiwa dan psikososial masyarakat. Dampak kesehatan jiwa dan psikososial ini dapat menurunkan sistem imunitas tubuh manusia khususnya kelompok rentan yaitu kelompok lansia. Semakin seseorang merasa cemas yang berlebihan, maka tubuh mengalami kegagalan dalam menghasilkan antibodi untuk melawan virus.. Klien yang terdiagnosis positif COVID-19 merasa sangat takut dengan bayangan kematian, dan secara sosial diisolasi yang akan menimbulkan stigma sehingga mungkin terjadi ansietas dan depresi yang bila berlanjut dapat menyebabkan keputusasaan yang akan memperburuk kondisi fisik. Pemberdayaan generasi muda dalam hal ini adalah siswa SMAN 1 Sumberpucung dalam mendukung kesehatan jiwa dan psikosial lansia, yaitu lansia yang berada dalam keluarga siswa. Metode yang digunakan di masa pandemic COVID-19 dengan kegiatan atau pelatihan DKJPS (Dukungan Kesehatan Jiwa dan Psikososial) dievaluasi selama 1,5 bulan berupa melakukan upaya promosi kesehatan (peningkatan imunitas fisik dan imunitas jiwa) dan pencegahan masalah kesehatan (pencegahan penularan dan pencegahan masalah kesehatan jiwa) yang pada akhirnya disebut sebagai Adaptasi Kebiasaan Baru (AKBmelalui virtual menggunakan zoom dan platform Whatsapp Group Chat dengan metode ceramah, Tanya jawab, diskusi dan demonstrasi. Hasil Jumlah audience: 50 siswa dan jumlah lansia dan pra lansia: 11 orang Kata Kunci : Kesehatan Jiwa, Psikososial, Generasi Muda, Lansia  ABSTRACT The COVID-19 pandemic is a non-natural disaster that has an impact not only on physical health but also on the mental and psychosocial health of the community. This mental and psychosocial health impact can reduce the immune system of the human body, especially vulnerable groups, namely the older adults. The more a person feels excessively anxious; the body fails to produce antibodies to fight the virus. Clients who are diagnosed positive for COVID-19 feel very afraid of the image of death and are socially isolated which will lead to stigma so that anxiety and depression may occur if it continues. Can cause despair that will worsen the physical condition. The empowerment of the younger generation in this case is the students of SMAN 1 Sumberpucung in supporting the mental and psychosocial health of the older adults, which was the older adults who are in the student's family. The method used during the COVID-19 pandemic with DKJPS (Mental Health and Psychosocial Support) activities or training was evaluated for 4 weeks in the form of health promotion efforts (increasing physical immunity and mental immunity) and prevention of health problems (prevention of transmission and prevention of mental health problems. ) which is finally referred to as New Habit Adaptation (AKB through virtual using zoom and the Whatsapp Group Chat platform with the methods of lecturing, question and answer, discussion and demonstration Results Number of audiences: 50 students and the number of older adults and pre-elderly: 11 people Keywords: Mental Health, Psychosocial, Young Generation, Older Adults

Aims and MethodA prospective descriptive study was set up to evaluate the feasibility, acceptability and activity of an innovative weekly primary care service for patients admitted for acute psychiatric care.ResultsDuring 10 months, 36 clinics were held and 123 appointments were attended. Presenting complaints included a wide range of acute and chronic conditions, affecting all body systems. As well as treating specific complaints, the doctor providing this service undertook considerable health promotion work and gave advice about patient management to junior psychiatrists.Clinical ImplicationsIt appears that there is considerable need for primary care expertise within an acute psychiatric unit, and that a weekly clinic is a feasible model of care.

ABSTRACT A number of factors appear to discourage physicians from seeking help for mental illness. This reluctance may be exacerbated by fears – well-founded or imagined – that by seeking help, physicians may put their medical license in jeopardy. To examine this risk, an analysis of all state medical board (SMB) license applications was followed by a seven-item survey mailed to SMB executive directors, and 70 percent responded. Follow up interviews were conducted with a sample of those not responding and also with a small group of directors whose responses were problematic. Thirteen of the 35 SMBs responding indicated that the diagnosis of mental illness by itself was sufficient for sanctioning physicians. The same states indicated that they treat physicians receiving psychiatric care differently than they do physicians receiving medical care. In follow-up interviews all 13 indicated that without evidence of impairment or misrepresentation any such sanctioning was likely to be temporary. A significant percentage (37 percent) of states sanction or have the ability to sanction physicians on the basis of information revealed on the licensing application about the presence of a psychiatric condition rather than on the basis of impairment. The same percentage state they treat physicians receiving psychiatric care differently than they do those receiving medical care. Physicians’ perceptions of this apparent discrimination is likely to play a role in their reluctance to seek help for mental health-related conditions. Suggestions are made for how SMBs and state physician health programs and state and county medical societies might collaborate in ways that while protecting patients decreases barriers to physicians help seeking.

In recent studies on children and electronic media, children are acknowledged as active users, interpreting TV-texts in various meaningful ways, according to their previously constructed knowledge of narratives and relating the texts to their everyday lives. Still, there is a tendency that toddlers' (ages 1 to 3) viewing is neglected, and seen as mere fascinations of patterns, bright colours and movements without focusing on the social uses or uses in which television narratives come to play an important part in small children's experimenting with building identity and self-image. This article focuses on the meaning-making processes that take place when toddlers watch television and DVD, and the way in which they broaden the reception-situation to different arenas, for instance through play and different uses of merchandise connected to the television programs. Also, it studies the context of children's media use, the way both parents, media and market set up the frames of children's reception.

Objective To update a 2010 meta-review of systematic reviews of effective interventions to support carers of ill, disabled, or older adults. In this article, we report the most promising interventions based on the best available evidence. Methods Rapid meta-review of systematic reviews published from January 2009 to 2016. Results Sixty-one systematic reviews were included (27 high quality, 25 medium quality, and nine low quality). The quality of reviews has improved since the original review, but primary studies remain limited in quality and quantity. Fourteen high quality reviews focused on carers of people with dementia, four on carers of those with cancer, four on carers of people with stroke, three on carers of those at the end of life with various conditions, and two on carers of people with mental health problems. Multicomponent interventions featured prominently, emphasizing psychosocial or psychoeducational content, education and training. Improved outcomes for carers were reported for mental health, burden and stress, and wellbeing or quality of life. Negative effects were reported in reviews of respite care. As with earlier work, we found little robust evidence on the cost-effectiveness of reviewed interventions. Conclusions There is no ‘one size fits all’ intervention to support carers. There is potential for effective support in specific groups of carers, such as shared learning, cognitive reframing, meditation, and computer-delivered psychosocial support for carers of people with dementia. For carers of people with cancer, effective support may include psychosocial interventions, art therapy, and counselling. Carers of people with stroke may also benefit from counselling. More good quality, theory-based, primary research is needed.

This chapter takes, as its starting provocation, Tanya Kevorkian’s suggestion that performances of Bach’s cantata compositions would originally have taken place within a somewhat noisy social environment. Drawing from the wider field of literature on listening and noise in other historical and contemporary contexts, it examines the potential for sonic interactions between different actors to set up patterns of social resonance. It draws out the consequences of taking these sonic and social interactions seriously in understandings of musical performance and experience. Many analyses consider noise primarily as a distraction to attentive listening. This chapter, however, highlights its role in processes of meaning making and in establishing different sets of relationships between performers, congregation, and the divine.

Older adults who experience challenges related to mental health are unlikely to seek professional help. The voices of older adults who have navigated through mental health issues and systems of care to arrive at psychological treatment are less well understood. We conducted individual interviews with 15 adults aged 61 to 86 who sought psychological treatment. Interviews were audio-recorded, transcribed, and analyzed using narrative methods. We identified several main storylines that describe the meaning-making and treatment-seeking journeys of older adults: resistance to being labeled with mental health problems (telling stories of resistance, defining mental health issues in mysterious and uncontrollable terms, and experiencing internal role conflict); muddling through the help-seeking process (manifestations of chaos and system-level barriers); and emotional reactions to psychological treatment (hope, fear, and mistrust). Findings add to the literature base in the area of narrative gerontology, and highlight the complex experiences that older adults face when seeking psychological treatment.
