Sea fishing is one of the sectors with the highest risk of illness and work-related accidents. The purpose of the study is to evaluate the exposure of fishing workers to three major risks: biomechanical overload, noise, and whole-body vibrations. We used common methods and measurement tools in the field: observational video analysis, questionnaires, and direct measurement. Noise and vibrations levels were measured aboard five boats belonging to the main fishing communities of Southern Italy. The random sample consisted of 310 workers, of whom 108 agreed to complete a questionnaire to collect data on the perception of occupational risk and self-perception of health conditions. We found that fishermen had a high prevalence of osteoarticular pathologies (42%) and that the biomechanical overload risk is mainly related to handling manual loads. Furthermore, the results indicate that the levels of weekly noise exposure exceed the exposure limit value of 87 decibel A (dBA) for fishing workers, and that the most noisiest area is the engine room. Exposure levels to whole-body vibrations were below <0.5 m/s2. Knowledge on occupational hazards and health effects in the fisheries sector should be used to develop ship technology, raise awareness of the correct use of personal protective equipment, and improve health surveillance of these workers.

Studies have been carried on the effect of rocking on a baby and concluded that baby sleeps easier while being rocked. In Malaysia, as in many Southeast Asian Countries, it is common to put babies to sleep in a baby hammock. the vertical rocking motion generated by baby hammock has exposed babies to whole-body vibration (WBV). It has been shown by ISO2631 (1997) that WBV may lead the discomfort and adverse effect on health. Standards have been set by ISO 2631 (1997) concerning the WBV for people in a recumbent position and consider weighted vibrations of more than 2 m/s2 to be extremely uncomfortable. However, standards concerning the allowable amount vibrations a baby in a baby hammock can safety endure are currently lacking. WBV analysis of the baby hammock with the weight ranged from 3kg to 14kg is conducted. For each measurement, four conditions are considered: manual rocking, auto rocking with low, medium and high speed. In this study, average root-mean-square values for the acceleration were found to be at a maximum of 2.46 m/s2, and to be above the extremely uncomfortable level. This study develops a baseline exposure time for the baby hammock before it reaches the safety values of exposure action value (EAV) and exposure limit value (ELV) set by ISO 2631(1997).

Vibration is experienced when a body is subjected to either internal or external forces which cause oscillation, with most operators of industrial equipment often exposed to high dosage, higher than the stipulated values. In this research, Digital Real-Time Frequency Analyzer (RSA 5106A) was used, while the results obtained were evaluated and compared with the health guidelines of the ISO 2631-1 : 1997 and ISO 2631-5 : 2004 standards, as described in the Health Guidance Caution Zone for a daily exposure action value (EAV) of 0.47 m/s2 and a daily exposure limit value (ELV) of 0.93 m/s. High acceleration was mostly seen on the z-axis in all the results obtained, whereas many were not within the HGCZ (Arms <0.47, and >0.93 m/s2). Comparing (VDV <8.5 m/s1.75 and >17 m/s1.75) with the ISO standard, the accelerations on all x- and y-axes were slightly within the HGCZ, with just a little below 0.47 m/s2 limit. The results obtained clearly showed that urgent action is needed virtually on all the equipment in both the Secondary Manufacturing Department (SMD) and Primary Manufacturing Department (PMD) to minimize vibration exposure on the technical operators.

Vibrations that arouse human health concerns are classified into two main categories: (1) hand-arm vibrations (HAV) and (2) whole-body vibrations (WBV). Hand-transmitted vibration from a power or impact tool affects the upper extremities of the body. WBV affects the entire body and is transmitted from a vibrating seat, bed or floor to a person who is in a sitting, Iying or standing position. The EU Directive April 5, 2002 provides exposure limit and action values for both HAV and WBV. These values have taken into consideration recent advances in knowledge and the political judgment of the Member States so are at variance with the present Health and Safety Executive (HSE) values and the ACGIH TLVs. This paper reviews the development of international standards for vibration and the requirements of the EU Directive.

ObjectivesPregnancy complications are common contributors to perinatal mortality and morbidity. Still, the cause(s) of gestational hypertensive disorders and diabetes are largely unknown. Some occupational exposures have been inconsistently associated with pregnancy complications, but exposure to whole-body vibrations (WBV) has been largely overlooked even though it has been associated with adverse birth outcomes. Therefore, the aim was to assess whether occupational WBV exposure during pregnancy is associated with pregnancy complications in a nationwide, prospective cohort study.MethodsThe Fetal Air Pollution Exposure cohort was formed by merging multiple Swedish, national registers containing information on occupation during pregnancy and diagnosis codes, and includes all working women who gave birth between 1994 and 2014 (n=1 091 044). WBV exposure was derived from a job-exposure matrix and was divided into categories (0, 0.1–0.2, 0.3–0.4 and ≥0.5 m/s2). ORs with 95% CIs were calculated using logistic regression adjusted for potential confounders.ResultsAmong women working full time (n=646 490), we found increased risks of all pregnancy complications in the highest exposure group (≥0.5 m/s2), compared with the lowest. The adjusted ORs were 1.76 (95% CI 1.41 to 2.20), 1.55 (95% CI 1.26 to 1.91) and 1.62 (95% CI 1.07 to 2.46) for preeclampsia, gestational hypertension and gestational diabetes, respectively, and were similar in all sensitivity analyses. There were no clear associations for part-time workers.ConclusionsThe results suggest that women should not be exposed to WBV at/above the action limit value of 0.5 m/s2 (European directive) continuously through pregnancy. However, these results need further confirmation.

The European Union Physical Agents Directive (vibration) sets vibration exposure limits for an employer. The measured values from different work phases should be time weighted and summed together to represent a daily 8-hour exposure value. The directive has two limit values, the daily exposure action value and the daily exposure limit value. For the employer the greatest concern is to comply with the exposure limit value, but also exceeding the action value will cause implications. To calculate the daily 8-hour value is not a simple task, because there are normally several different work phases that the worker does in a normal day. Also the assessment of measurements of the vibration levels have to be made in real working environments. The work phases need to be calculated correctly to get the representative daily value. Also breaks and other non-work operations need to be included. This will make the complete assessment very difficult without proper knowledge. In this study a large number of measurements from different mobile work machines were made to establish whether the daily values would exceed the limit values. Analyses were made using three examples to show how the employer can use the standard and directive to assess the daily exposure values or times. The results were largely dependent on the work pattern and the dominant levels.

Background: Research shows that there is a high prevalence of suicide among nurses. Despite this, it has been 15 years since the last literature review on the subject was published. Aim: The aim of this article is to review the knowledge currently available on the risk of suicide among nurses and on contributory risk factors. Method: A search was conducted in electronic databases using keywords related to prevalence and risk factors of suicide among nurses. The abstracts were analyzed by reviewers according to selection criteria. Selected articles were submitted to a full-text review and their key elements were summarized. Results: Only nine articles were eligible for inclusion in this review. The results of this literature review highlight both the troubling high prevalence of suicide among nurses as well as the persistent lack of studies that examine this issue. Conclusion: Considering that the effects of several factors related to nurses' work and work settings are associated with high stress, distress, or psychiatric problems, we highlight the relevance of investigating work-related factors associated with nurses' risk of suicide. Several avenues for future studies are discussed as well as possible research methods.

Abstract Background The purpose of this case report is to emphasize the importance of curing any clinical radiological elements in this historical period, especially in the area of endemic to coronavirus disease 19 (COVID-19) such as Lombardy and to stress the importance of the management of the asymptomatic patient, their crucial role in the spread of contagion. Case presentation We reported the case of incidental diagnosis of interstitial pneumonia by first finding on whole-body MR (WB-MR) in the patient affected by multiple myeloma (MM), with a negative respiratory symptoms at the time and with previous (1 month before) negative chest X-ray. The patient was promptly subjected to chest CT, which confirmed the suspicion of interstitial COVID-19 pneumonia and, in hospitalization, performed nasopharyngeal swabs for real-time polymerase chain reaction (RNA-PCR), with a doubtful outcome. Once the bacterial nature of the alterations was serologically and radiologically excluded, the patient was definitively diagnosed with COVID-19 and appropriately treated in hospitalization. Conclusion The clinical choices must, therefore, to make use of all the diagnostic tools available and full knowledge of the limitation of each of them.

Abstract Background Hepatitis C virus (HCV) infection represents a global health issue with severe implications on morbidity and mortality. This study aimed to evaluate the impact of HCV infection on all-cause, liver-related, and non-liver-related mortality in a population living in an area with a high prevalence of HCV infection before the advent of Direct-Acting Antiviral (DAA) therapies, and to identify factors associated with cause-specific mortality among HCV-infected individuals. Methods We conducted a cohort study on 4492 individuals enrolled between 2003 and 2006 in a population-based seroprevalence survey on viral hepatitis infections in the province of Naples, southern Italy. Study participants provided serum for antibodies to HCV (anti-HCV) and HCV RNA testing. Information on vital status to December 2017 and cause of death were retrieved through record-linkage with the mortality database. Hazard ratios (HRs) for cause-specific mortality and 95% confidence intervals (CIs) were estimated using Fine-Grey regression models. Results Out of 626 deceased people, 20 (3.2%) died from non-natural causes, 56 (8.9%) from liver-related conditions, 550 (87.9%) from non-liver-related causes. Anti-HCV positive people were at higher risk of death from all causes (HR = 1.38, 95% CI: 1.12–1.70) and liver-related causes (HR = 5.90, 95% CI: 3.00–11.59) than anti-HCV negative ones. Individuals with chronic HCV infection reported an elevated risk of death due to liver-related conditions (HR = 6.61, 95% CI: 3.29–13.27) and to any cause (HR = 1.51, 95% CI: 1.18–1.94). The death risk of anti-HCV seropositive people with negative HCV RNA was similar to that of anti-HCV seronegative ones. Among anti-HCV positive people, liver-related mortality was associated with a high FIB-4 index score (HR = 39.96, 95% CI: 4.73–337.54). Conclusions These findings show the detrimental impact of HCV infection on all-cause mortality and, particularly, liver-related mortality. This effect emerged among individuals with chronic infection while those with cleared infection had the same risk of uninfected ones. These results underline the need to identify through screening all people with chronic HCV infection notably in areas with a high prevalence of HCV infection, and promptly provide them with DAAs treatment to achieve progressive HCV elimination and reduce HCV-related mortality.

To date, testing and evaluation of whole-body vibration in ground vehicle systems have not always fully utilized appropriate experimental design methodology, applicable statistical tests, or relevant criteria. A test design and evaluation methodology was developed to eliminate these oversights. This methodology uses inferential statistics, questionnaires, and a comparison of vibration data with representative mission scenarios. The methodology was employed in the evaluation of two alternative tracked ground vehicle designs. The independent variables were track type, terrain, vehicle speed, and crew position. The dependent variables were International Standards Organization (ISO) 2631 whole-body vibration exposure limit times at the lateral, transverse, and vertical axes. Two different multivariate analyses of variance (MANOVAs) performed on the exposure limit data indicated that all main effects, as well as several interactions, were significant (p < .01). A comparison of exposure limits to a representative mission scenario indicated that both track types would exceed ISO 2631 exposure, comfort, and fatigue limits during expected travel over cross-country terrain. Crew questionnaires also indicated crew discomfort when exposed to this type of terrain. The experiment demonstrated that the procedure was useful in helping to determine the extent that vehicle vibration permits the performance of the vehicle mission, within limits dictated by safety, efficiency, and comfort.
