Objective: To investigate the levels of a deoxynivalenol (DON) biomarker in the urine of subjects living in two China provinces with different geographic locations and dietary patterns, and estimate their dietary DON exposures and health risks. Methods: First morning urine samples were collected on three consecutive days from 599 healthy subjects—301 from Henan province and 298 from Sichuan province—to analyze the total DON concentrations (tDON) after β-glucuronidase hydrolysis using a high-performance liquid chromatography tandem mass spectrometry-based method. The consumption of cereal foods in the previous 24 h before each urine collection was recorded using a duplicate diet method. DON exposure levels were estimated based on the urinary tDON concentrations. Results: Total DON were detected in 100% and 92% of the urine samples from Henan and Sichuan, respectively. Mean urinary tDON concentrations were 52.83 ng/mL in Henan subjects and 12.99 ng/mL in Sichuan subjects, respectively. The tDON levels were significantly higher in the urine of Henan subjects than that of the Sichuan subjects (p < 0.001). Urinary tDON levels were significantly different among age groups in both areas (Henan: p < 0.001; Sichuan: p = 0.026) and were highest in adolescents aged 13–17 years, followed by children aged 7–12 years. Based on the DON biomarker and exposure conversion reported by the European Food Safety Authority (EFSA), the mean estimated dietary intakes of DON were 1.82 μg/kg bw/day in Henan subjects and 0.45 μg/kg bw/day in Sichuan subjects. A total of 56% of Henan subjects and 12% of Sichuan subjects were estimated to exceed the PMTDI of 1 μg/kg bw/day. Consistent with urinary tDON levels, the highest estimated dietary DON intakes were also in children and adolescents aged 7–17 years. For all kinds of wheat-based foods except dumplings, the consumptions were significantly higher in Henan than those in Sichuan. The mean consumption of steamed buns was 8.4-fold higher in Henan (70.67 g/d) than that in Sichuan (8.45 g/d). The mean consumption of noodles in Henan (273.91 g/d) was 3.6-fold higher than that in Sichuan (75.87 g/d). Conclusions: The levels of urinary DON biomarker and the estimated dietary DON intakes in Henan province were high and concerning, especially for children and adolescents. The overall exposure level of Sichuan inhabitants was low.

Several studies have shown the presence of aldehydes (i.e., formaldehyde, acrolein) in mainstream emissions of some e-cigarettes. For this reason, concerns have been raised regarding potential toxicity. The purpose of this research was to measure levels of carbonyls in exhaled breath of e-cigarette users during “vaping” sessions and estimate the respiratory tract (RT) uptake of specific aldehydes, including formaldehyde and acetaldehyde. We measured concentrations of 12 carbonyls in e-cigarette aerosols produced directly by e-cigarettes and in the exhaled breath of 12 participants (19 sessions). Carbonyls were sampled on 2,4-dinitrophenylhydrazine (DNPH) cartridges and analyzed with high performance liquid chromatography (HPLC) coupled with a UV/Vis photodiode detector. We found that in most cases, levels of aldehydes and methyl ethyl ketone (MEK) were significantly higher (2–125 times) in exhaled e-cigarette breaths than in pre-exposed breath. Exposure levels for the most abundant individual carbonyls in e-cigarette emissions—formaldehyde, acetaldehyde, acrolein—were between the limit of quantification (LOQ) and 24.4 μg·puff−1. The mean retention of formaldehyde in the respiratory tract was 99.7 ± 0.9% for all participants, while acetaldehyde retention was 91.6 ± 9.9%. Within the limitation of a small number of participants, our results showed that there is an increase in breath carbonyls during e-cigarette use.

Studies on the vitamin E status of Korean children are lacking. Dietary intakes and plasma concentrations of tocopherols of healthy 2- to 6-year-olds living in Kwangju, Republic of Korea, were determined and their vitamin E status evaluated. Subjects included 66 girls and 65 boys, with 21–32 subjects per age group. Subjects’ intakes were recorded by a trained interviewer for foods eaten at preschool/kindergarten; otherwise, three consecutive 24-hour food recalls were obtained from parents. Plasma tocopherol concentrations were determined using high-performance liquid chromatography. Intakes of energy and total monounsaturated and polyunsaturated fats were significantly different (p < 0.05) among age groups, not gender, with 2- and 3-year-olds having lower intakes. Alpha- and γ-tocopherol intakes were similar by age and gender. Sixty-seven percent consumed less than the Korean Adequate Intakes for vitamin E, and 77% had α-tocopherol intakes less than USA/Canadian Estimated Average Requirements. Plasma α-tocopherol concentrations, but not γ-tocopherol, were significantly higher (p < 0.05) for 2-year-olds. Two-thirds of subjects had plasma α-tocopherol concentrations less than 12 μmol/L, which is indicative of vitamin E inadequacy in adults; guidelines for children do not exist. Many of the Korean children had inadequate intakes and likely inadequate plasma concentrations of vitamin E.

The occurrence, concentrations and variables associated with tetracycline, polyether ionophore and anthelmintic residues in the livers of chickens sold in the informal market in South Africa were determined. An ultra-high-performance liquid chromatography–tandem mass spectrometry (UHPLC-MS/MS) was used to simultaneously analyze for four tetracyclines, five polyether ionophores and six anthelmintic residues. The study determined the presence  of residues  in liver samples at both the limit of quantifications (LoQ) and concentrations over the maximum residue limit (MRLs), i.e. non-compliant. Doxycycline (tetracycline group) was detected in 24.5% (24/98) of chicken livers and 15.3% (15/98) were non-compliant. The mean±SD concentration of 919.04±1081.30 (LoQ) and 1410.57±108.89 ppb (MRL) were obtained. Maduramycin was detected in 27.6% (27/98) of chicken livers and 19.4% (19/98) were non-compliant. The mean±SD for LoQ was 117.96±84.56 and MRL was 153.21±76.29 ppb. The concentrations of residues of doxycycline and maduramycin in chicken livers varied significantly across townships. Lasalocid was quantified in 31.6% (31/98) of the samples, of which 5.1% (5/98) contained concentrations above the MRL. The  mean±SD concentrations of Lasalocid was 62.90±170.84 for samples in which  Lasalocid was quantified and 310.16±356.68 ppb for non-compliant samples. The frequencies of chicken livers that contained detectable concentrations of the three anthelmintic residues were 3.1% (3/98), 1.0% (1/48) and 2.0% (2/98) for praziquantel, closantel and rafoxanide, respectively. The presence of three classes of veterinary drugs residues in chicken liver poses food safety implications to consumers and indicates a need for enhanced regulatory enforcement in controlling these drugs in South Africa.

Sugar intake has been associated with increased prevalence of childhood overweight/obesity; however, results remain controversial. The aim of this study was to examine the probability of overweight/obesity with higher sugar intakes, accounting for other dietary intakes. Data from 1165 children and adolescents aged ≥2–18 years (66.8% males) enrolled in the Hellenic National Nutrition and Health Survey (HNNHS) were used; specifically, 781 children aged 2–11 years and 384 adolescents 12–18 years. Total and added sugar intake were assessed using two 24 h recalls (24 hR). Foods were categorized into specific food groups to evaluate the main foods contributing to intakes. A significant proportion of children (18.7%) and adolescents (24.5%) exceeded the recommended cut-off of 10% of total energy intake from added sugars. Sweets (29.8%) and processed/refined grains and cereals (19.1%) were the main sources of added sugars in both age groups, while in adolescents, the third main contributor was sugar-sweetened beverages (20.6%). Being overweight or obese was 2.57 (p = 0.002) and 1.77 (p = 0.047) times more likely for intakes ≥10% of total energy from added sugars compared to less <10%, when accounting for food groups and macronutrient intakes, respectively. The predicted probability of becoming obese was also significant with higher total and added-sugar consumption. We conclude that high consumption of added sugars increased the probability for overweight/obesity among youth, irrespectively of other dietary or macronutrient intakes.

Objective. The study aimed to evaluate the sound pressure levels of selected traffic enforcer sites in the City of Manila. Methods. A Brüel & Kjær Integrating Sound Level Meter type 2225 was used to measure sound pressure levels in dB(A) to estimate personal noise exposure of traffic enforcers designated at Quezon Boulevard near Quiapo Church and Recto – Rizal Avenue on a weekday and a weekend. Graphs were generated while appropriate measures were calculated for the noise exposure levels. The mean exposure levels were compared with the Philippine Occupational Safety and Health standards by computing the corresponding permissible exposure limit for each work shift using the Equal Energy Principle.17 Results. Noise exposure levels at Quezon Boulevard ranged from 75.0 dB(A) to 91.5 dB(A) with mean noise exposure level of 84.3 ± 3.7 dB(A) and 82.5 ± 2.6 dB(A) for the weekday AM and PM shift, respectively. The mean noise exposure level at Quezon Boulevard for the weekend AM shift was 82.4 ± 2.6, whereas 80.4 ± 2.8 for the PM shift. The noise exposure levels at Recto – Rizal Avenue ranged from 81.5 dB(A) to 99.3 dB(A) with mean noise exposure level of 86.7 ± 2.6 dB(A) and 86.0 ± 2.1 dB(A) for the weekday AM and PM shift, respectively. The mean noise exposure level at Recto – Rizal Avenue for the weekend AM shift was 86.7 ± 2.3, whereas 89.0 ± 4.0 for the PM shift. Conclusion. The study showed that traffic enforcers designated at Quezon Boulevard and Recto – Rizal Avenue are exposed to noise levels that do not exceed the Philippine Occupational Safety and Health standards.

Introduction: Occurrence of G induced loss of consciousness (G-LOC) during centrifuge training is a known entity. The Indian Air Force is currently undertaking high G training of its fighter pilots in High Performance Human Centrifuge (HPHC) which has significantly higher operational capabilities. The study aimed to analyse the incapacitation periods and myoclonic jerks associated with G-LOC occurring during HPHC training. Material and Methods: Records of episodes of 161 G-LOC during closed loop Rapid Onset Rates (ROR) runs over a period of 7 years from 2011 to 2017 were analyzed. The video recordings of these G-LOC episodes were assessed in terms of the participant demographics, +Gz onset rates, incapacitation periods, and presence or absence of myoclonic jerks during the G-LOC episodes. Descriptive statistics was applied to analyze the incapacitation periods and the duration of the myoclonic flail movements. Single tailed t-test was used to analyze the difference between the incapacitation periods of the aircrew who suffered myoclonic flail movements and those who did not. One-way ANOVA was carried out to assess the differences in incapacitation periods, if any, between aircrew of different age groups. Significance was set at P < 0.05. Results: Of these 161 episodes of G-LOC, 43.5% were seen in trainee aircrew. The mean Absolute Incapacitation Period (AIP), Relative Incapacitation Period (RIP) and Total Incapacitation Period (TIP) was 6.9 ± 2.3 s, 12.2 ± 4.7 s, and 19.1 ± 5.5 respectively. Age and flying experience did not show any significant effect with any of the incapacitation periods (p>0.05). The TIP correlated better with the RIP than with the AIP (Pearson’s correlation values of 0.9 and 0.52 respectively). Myoclonic flail movements were witnessed in 25.5% of G-LOC episodes with an average duration of 5.3 s and more than 50% occurring at ≥8Gz. The mean duration of RIP was significantly higher (P=0.03) when G-LOC was associated with myoclonic flail movements, whereas, the duration of AIP did not show any significant difference. Conclusion: The shorter incapacitation periods observed in the study compared to that reported in most of the previous studies could be attributed to the faster Gz offset rates of the present HPHC. Episodes of G-LOC having myoclonic movements were found to have higher relative incapacitation periods. Even though these movements were associated with G-LOC occurring at higher Gz levels, the durations were independent of the Gz levels.

Aim: Data on the prevalence of vitamin D deficiency in Lithuania are scarce. The aim was to assess the reserves of vitamin D in different age groups of out-patients, regarding the season of the year. Methods: Data on serum 25-hydroxyvitamin D (25(OH)D) levels from blood tests made in 2012–2014 were obtained from one laboratory, and a retrospective cross-sectional analysis was performed. Results: A total of 9581 subjects were included. The mean age of the participants was 33 ± 23 years. The mean levels of vitamin D were higher in males than in females (p < 0.001). The highest mean 25(OH)D levels were in 0–9-year-old group, the lowest were in the 10–19-year-old group and in the group of participants that were 70 years and older (p < 0.001). The lowest vitamin D status was found in January, February, March, and April. The highest status was found in August and September. Overall, vitamin D deficiency, sufficiency, and hypervitaminosis were detected in 67%, 21%, and 12% of cases, respectively. Most cases with hypervitaminosis were in the group of children up to 2 years of age. Conclusion: Vitamin D status demonstrated clear seasonality. Significant sex-related differences of vitamin D statuses were also determined. Vitamin D deficiency was very prevalent in almost all age groups. Young children (aged up to 2 years) are of special interest for further research involving other types of 25(OH)D assays, such as those based on high-performance liquid chromatography (HPLC), since the real prevalence of “true” vitamin D hypervitaminosis in Lithuania’s children is still to be determined.

Abstract Pyridinoline (PYD) and deoxypyridinoline (DPD), two collagen-based cross-links found in bone, were measured by high-performance liquid chromatography in urine samples from 65 control subjects and 97 patients with either untreated or progressive cancer. Patients with cancer had significantly (P &lt; 0.001) higher urine concentrations of PYD and DPD than did control subjects. Both cross-links were increased in cancer patients with and without clinically detectable bone metastases, although patients with bone and liver involvement had higher mean concentrations. The mean concentrations of both cross-links were also significantly higher in the urine samples of inpatients than in an outpatient ambulatory population. These findings suggest that the measurement of PYD and DPD in urine may be useful in assessing bone metastases and bone resorption in cancer patients.

Because of logistics and cost constraints, monitoring of the compliance to antimalarial chemoprophylaxis by the quantitation of drugs in biological samples is not a simple operation on the field. Indeed, analytical devices are fragile to transport and must be used in a perfectly controlled environment. This is also the case for reagents and supplies, and the waste management is constraining. Thus, samples should be repatriated. They should be frozen after collection and transported with no rupture in the cold chain. This is crucial to generate available and interpretable data but often without any difficulties. Hence, to propose an alternative solution easier to implement, a quantitation method of determining doxycycline in urine has been validated using a volumetric absorptive microsampling (VAMS®) device. As blotting paper, the device is dried after collection and transferred at room temperature, but contrarily to dried spot, the collection volume is perfectly repeatable. Analysis of VAMS® was performed with a high-performance liquid chromatography coupled to a mass spectrometer. The chromatographic separation was achieved on a core-shell C18 column. The mean extraction recovery was 109% (mean RSD, 5.4%, n = 6) for doxycycline and 102% (mean RSD, 7.0%) for the internal standard. No matrix effect has been shown. Within-run as within-day precision and accuracy were, respectively, below 14% and ranged from 96 to 106%. The signal/concentration ratio was studied in the 0.25–50 µg/mL range, and recoveries from back-calculated concentrations were in the 96–105% range (RSD < 11.0%). The RSD on slope was 10%. To achieve the validation, this new quantitation method was applied to real samples. In parallel, samples were analyzed directly after a simple dilution. No statistical difference was observed, confirming that the use of VAMS® is an excellent alternative device to monitor the doxycycline compliance.

ABSTRACTDesbutyl-lumefantrine (DBL) is a metabolite of lumefantrine. Preliminary data fromPlasmodium falciparumfield isolates show greater antimalarial potency than, and synergy with, the parent compound and synergy with artemisinin. In the present study, thein vitroactivity and interactions of DBL were assessed from tritium-labeled hypoxanthine uptake in cultures of the laboratory-adapted strains 3D7 (chloroquine sensitive) and W2mef (chloroquine resistant). The geometric mean 50% inhibitory concentrations (IC50s) for DBL against 3D7 and W2mef were 9.0 nM (95% confidence interval, 5.7 to 14.4 nM) and 9.5 nM (95% confidence interval, 7.5 to 11.9 nM), respectively, and those for lumefantrine were 65.2 nM (95% confidence interval, 42.3 to 100.8 nM) and 55.5 nM (95% confidence interval, 40.6 to 75.7 nM), respectively. An isobolographic analysis of DBL and lumefantrine combinations showed no interaction in either laboratory-adapted strain but mild synergy between DBL and dihydroartemisinin (sums of the fractional inhibitory concentrations of 0.92 [95% confidence interval, 0.87 to 0.98] and 0.94 [95% confidence interval, 0.90 to 0.99] for 3D7 and W2mef, respectively). Using a validated ultra-high-performance liquid chromatography-tandem mass spectrometry assay and 94 day 7 samples from a previously reported intervention trial, the mean plasma DBL was 31.9 nM (range, 1.3 to 123.1 nM). Mean plasma DBL concentrations were lower in children who failed artemether-lumefantrine treatment than in those with an adequate clinical and parasitological response (ACPR) (P= 0.053 versusP> 0.22 for plasma lumefantrine and the plasma lumefantrine-to-DBL ratio, respectively). DBL is more potent than the parent compound and mildly synergistic with dihydroartemisinin. These properties and the relationship between day 7 plasma concentrations and the ACPR suggest that it could be a useful alternative to lumefantrine as a part of artemisinin combination therapy.
