Background: The increased frequency and management of antibiotic resistance pattern in urinary tract infection (UTI) is a challenging task for the clinicians. Therefore, the current study was planned to identify the microbial etiology of UTI and the most suitable antibiotics used. The objective of the study was to assess the frequency of antimicrobial sensitivity pattern in patients with urinary tract infection.Material and Methods: This cross-sectional study was carried out at Al Sayed Hospital, Kidney Centre, Rawalpindi over a period of seven months i.e. June 30, 2018 to January 30, 2019. A total of 152 patients were enrolled according to the inclusion and exclusion criteria of the study. Urine culture proceedings were done as per latest recommended guidelines of Clinical and Laboratory Standard Institute (CLSI) for UTI.Results: Regarding overall sensitivity of antibiotics for the pathogens of urine culture, highest sensitivity was observed for carbapenems (83.5%), followed by polymyxin B (72.3%), phosphonic acid derivatives (65.8%), aminoglycoside group (65.7%), extended spectrum penicillin (63.1%), imidazolidinedione (59.9%) and tetracycline (59.9%) groups. The least sensitivity was observed for Oxazolidinone (linezolid) (14.5%), teicoplanin (13.8%), tigecycline (10.5%) and first-generation cephalosporins (1.3%).Conclusions: Carbapenems showed maximum sensitivity for all urine isolates. The second and third options were polymyxin B and phosphonic acid derivatives, respectively.

Abstract Urinary tract infection is the presence and active multiplication of microorganism within the urinary tract. UTIs are group of infections that affects any part of urinary tract. The cross sectional descriptive study was conducted to determine the antibiotic susceptibility pattern of bacteria causing urinary tract infection in diabetic and non-diabetic patient from February 2016 to March 2016. Among the total 601 urine sample, 250 were diabetics and 351 were non diabetics. All samples were investigated by standard laboratory procedures. Out of diabetic patient 111(44.4%) were female and 139(55.6%) were male and among non-diabetic, 234(66.7%) were female and 117(33.3%) were male. The UTI prevalence rate was found to be 78 (13%) was statistically significant (p = 0.02), among the significant growth 6.8% diabetic and 6.2% non-diabetic. Escherichia coli (54) was the most predominant organism (42.5% in diabetic and 57.5% non-diabetic) followed by Staphylococcus aureus (8). Amikacin, Cotrimoxazole and Nitrofurantoin were most sensitive to E. coli isolated in diabetic and non-diabetic patients among the tested antimicrobials. High rate of resistance was observed with Norfloxacin and Nalidixic acid. Gentamicin, Cefotaxime, Cotrimoxazole and Ciprofloxacin were highly sensitive to S. aureus in diabetic patients while Oxacillin and Azithromycin were resistance and in non-diabetic patient highly sensitive antimicrobials were Azithromycin, Gentamicin, Cefotaxime, Cotrimoxazole, Vancomycin and Ofloxacin while Oxacillin was resistance. The antimicrobial susceptibility testing of bacterial isolates should be performed before the treatment of UTI. Key words : Diabetic, Urinary tract infection, Antimicrobial sensitivity.

BACKGROUND Urinary tract infections (UTI) constitute a common cause of morbidity in infants and children. When associated with abnormalities of urinary tract, they may lead to long-term complications including renal scarring, loss of function and hypertension. Most urinary tract infections remain undiagnosed if investigations are not routinely performed to detect them. Prompt detection and treatment of urinary tract infections and any complicating factors are important. The objective of the study is to know the clinical, epidemiological and bacteriological profile (i.e. clinical signs and symptoms, age, sex, family history, associated urinary tract abnormalities, & causative organisms) of urinary tract infections in febrile children with culture positive urinary tract infection. METHODS This descriptive, cross sectional observational study was conducted at outpatient clinics of our “child health clinics” between May 2016 and April 2017 (one year). All children aged 0 to 12 years with culture positive urinary tract infections were included in this study to evaluate the clinical, epidemiological and bacteriological profile. RESULTS A total of 69 children with culture positive urinary tract infections were included in this study. Out of 69 children included in this study, 36 (52.2 %) were females and 33 (47.8 %) were males. Overall female preponderance was seen and the M: F ratio was 0.9:1. But during first year of life in our study group we had more boys (10, 14.49 %) affected with urinary tract infection than girls. 49.3 % of urinary tract infections in the present study belonged to lower socio-economic status. Most common organism causing urinary tract infection in our group was E. coli (56.5 %). Fever (100 %), anorexia or refusal of feeds (52.2 %), dysuria (46.4 %), vomiting (46.4 %) and abdominal pain (39.1 %) were the predominant clinical manifestations observed in our study. CONCLUSIONS Urinary tract infection is a common medical problem in children and it should be considered as a potential cause of fever in children. As febrile children with urinary tract infection usually present with non-specific signs and symptoms, urine culture should be considered as a part of diagnostic evaluation. IntroductionIn developing countries, urinary tract infections (UTIs) are one of the most commonly diagnosed diseases among the patient seeking medical service and being treated with empirical antibiotics which causes resistance.ObjectiveTo find out the prevalence of urinary tract infection and sensitivity pattern of antibiotics among bacterial pathogens isolated in patients attending Kathmandu Medical College Teaching Hospital, Duwakot.MethodologyAll the patients with the clinical suspicion of UTI were sent for urine culture. Prevalence of UTI and urinary pathogens isolated with antimicrobial profile was correlated.ResultA total number of 1735 clinically suspected as UTI cases were sent for urine culture sensitivity. Total culture positive were 239 (13.8%). The majority of isolates were from female patients comprising 180 (75%). The most common pathogenic microorganism isolated was E.coli (79.1%). Second most common organism was Klebsiella (11.7%) followed by Citrobacter (3.34%) and Proteus (2.92%). The isolated microorganism showed maximum number of sensitivity with the antibiotics Ofloxacin and Ciprofloxacin. The isolated microorganism demonstrated resistance with Nalidixic Acid and Co-Trimoxazole. ConclusionUrinary tract infection is a commonly encountered case in general practice. Females are commonly affected than men. The microbiological profile and the antibiotic sensitivity pattern while initiating empirical treatment must be taken into account while planning for the management. Background: Determination of contributing factors for jaundice would result in better programming for prevention. This study was performed to determine the prevalence rate of Urinary Tract Infection (UTI) among neonates with jaundice admitted in Arash Hospital from October 2013 to October 2014. Methods: In this descriptive cross-sectional study, term neonates with non-hemolytic jaundice admitted in Arash Hospital from October 2013 to October 2014 were evaluated for UTI by urine culture and the data were recorded by checklist using medical documents. Results: A total of 436 term infants were enrolled in the study. Mean age at the time of admission was 6.03±3.355 days with age range of 3-18 days including 291(0.66%) males and 145(0.33%) females. Urine culture was positive in 32 neonates (7.3%).The most common germs were Klebsiella in 37.5%, Escherichia coli (E. coli) in 21.9%, Staphylococcus in 9.4%, Pseudomonas in 3.1%, Enterococcus in 15.6%, Enterobacter in 9.4%, and Group B Streptococcus (GBS) in 3.1%. Conclusion: Urine culture should be a part of routine clinical evaluation for all icteric neonates at the age of 5-6 days or more who have no risk factors to exclude the possibility of coincidental UTI.

Background Urinary tract infection (UTI) is the most commoncause of fever in children. Less frequent disposable diaper changingis thought to be a cause ofUTI in children. While wearing a diapei;the perinea! area becomes damp with urine allowing bacteria tomigrate from the anus to the external urethral orifice.Objective To assess for an association between the frequencyof disposable diaper changing and urinary tract infections inchildren.Methods This cross-sectional study was conducted in thechildren's outpatient clinic of Haji Adam Malik Hospital fromApril to June 2010. Urine culture was performed in children withsuspected UTI who wore disposable diapers every day. Subjects'ages ranged from 2 months to 2 years 6 months, with samplesmatched and collected by consecutive sampling. Diagnosis ofUTIwas based on urine cultures with bacterial count 2:: 105/mL. Eightychildren were followed in this study and divided into two groups:positive (n=40) and negative (n=40) urine cultures. Chi squaretest was use d to analyze the association between the frequency ofdaily disposable diapers changing during a one week period andthe urine culture results.Results The frequency of daily disposable diapers changing in80 subjects was as follows: < 4 times (22.5%), 4-5 times (40%),and 2::6 times (37.5%) . The frequency of daily disposable diaperchanging in children with positive urine culture was as fo llows:< 4 times (1 8 out of 40), 4-5 times (22 out of 40), and 2::6 times(O out of 40); (P < 0.0001). The most common bacterial speciesfound in urine cultures was Escherichia coli.Conclusion Lower frequency of daily disposable diaper changingis significantly associated with higher UTI incidence in children.

The urinary tract is the most common site of nosocomial infections accounting for more than 40% of the total number reported by acute care hospitals and affecting approximately 600,000 patients per year. Catheter Associated Urinary Tract Infection (CAUTI) defines in terms of “bacteriuria” and “urinary tract infection” frequently. Bacteriuria or funguria levels >103 colony- forming units (CFU) have been shown to be highly predictive of CAUTI, given that these levels increase to 105 CFU within 24 to 48 hours. In Indian population, catheterassociated urinary tract infection (CAUTI) is an important cause of morbidity and mortality, affecting all age groups. Biofilm is the predominant mode of growth in aquatic ecosystems and, as such, plays a central role in the pathogenesis of Catheter Associated Urinary Tract Infections (CAUTI). The present review focuses to evaluate the incidence and pattern of microbes in catheter associated urinary tract infection and provides information about the etiology of CAUTI. Most of the studies concluded that gram negative pathogen E.coli showed the highest incidence rate and other pathogens like Klebsiella pneumonia, Pseudomonas aeruginosa, and staphylococcus species also having the incidence rate in the patient having CAUTI. The antibiotic resistance pattern showed the variation in resistance and sensitivity of antibiotics against the pathogens. The present study focuses on the incidence of the microbial growth in patient having catheterization and also elucidates the antibiotic sensitivity pattern. It is necessary to determine the antibiotic resistance and sensitivity status during and after the catheterization.

Objective: Urolithiasis could be caused by urinary tract infection (UTI). UTI in patients with urolithiasis need to be diagnose and manage as soon as possible. This study was aim to determine the diagnostic value of urinalysis examination to diagnose urinary tract infection (UTI) in patients with urolithiasis.Material & method: This study was an observational analytic with cross sectional study design, comparing one diagnostic tool with the gold standard tool to diagnose UTI. Total samples collected were 186 patients at Sardjito General Hospital, Yogyakarta. Urine culture and urinalysis were performed in patients with urinalysis and UTI. The results of urine culture and urinalysis were then compared by 2x2 table.Results: The sensitivity results on leukocyte esterase, eritrocyturia, bacteriuria, and nitrite respectively were 82.7%, 57.14%, 37.59%, and 13.53%. The specificity results on nitrite, bacteriuria, eritrocyturia, and leukocyte esterase respectively were 63.26%, 56.60%, 50.94%, and 33.96%. The highest level of accuracy was leukocyte esterase with 68.81% accuracy.Conclusion: This study showed that leukocyte esterase had good sensitivity with an accuracy of 68.81%. Examination of nitrite had the highest specificity compared to the other variables in urinalysis examination.
