Aim. To evaluate the results of surgical treatment of patients with moderate chronic periodontitis with different dental pulp status by determining sRANKL, OPG and FGF-2 in mixed saliva and dynamic densitotomometry data.Materials and methods. Surgical treatment of 24 patients with moderate chronic periodontitis was performed. Three months before the operation, according to the indications, endodontic treatment of the examined teeth was performed in some patients. Depending on the status of the dental pulp, the patients were divided into two groups: I – patients with vital teeth, II – patients with teeth after endodontic treatment. Before and after surgery, all patients underwent clinical examination, enzyme immunoassay of saliva remodeling markers and optical densitotomometry of periodontal bone to compare the results of treatment. Results. The OHI-S index showed a significant decrease in the mean value by 12 months of observation in both groups, which was statistically significant. Higher indices of the PI index by the end of the observation period were detected in the area of depulpated teeth. An equivalent dynamics of a decrease in the value of periodontal attachment according to the CAL index was revealed in both groups after treatment. The saliva showed an equivalent decrease in sRANKL, an increase in OPG by the end of the observation period, statistically insignificant when compared between groups. FGF-2 remained undetected at any of the observation periods in all groups. Densitotomometry data indicated an increase in the density characteristics of periodontal bone by the end of observations with a stable dependence on lower values in the area of depulpated teeth. Conclusions. In the saliva of patients, the processes of periodontal bone resorption were found to be statistically equivalent in intensity, regardless of the status of the dental pulp. The absence of FGF-2 content in saliva until the end of the observation period should be considered as an indicator of periodontal healing of vital and depulpated teeth by the formation of fibrous connective tissue. Dynamic densitotomometry of alveolar bone tissue revealed statistically significantly lower density characteristics in the area of teeth with a condition after endodontic treatment.

Periodontal disease in the adult population is one of the most pressing problems of dentistry around the world. Thanks to the introduction of modern technologies in practice, it was possible to identify the main mechanisms of the development of this disease at the molecular and genetic level. The interaction of periodontal pathogens with immune factors of antimicrobial protection of the body is the basis of periodontal tissue inflammation and further leads to the destruction of the alveolar bone. Epithelial cells of the gum mucosa play a crucial role against periodontal pathogenic bacteria. Factors of innate immunity play a role not only in antimicrobial protection, but they also support the conditions necessary for the healing and regeneration of periodontal tissues. Therefore, various therapeutic approaches that affect the factors of innate immunity are considered as effective and promising. Clinical examination and determination of factors of innate immunity were performed in 115 patients. Healthy individuals made up a group of 30 people. Patients with chronic generalized periodontitis with varying degrees of severity were 85 people. Each patient underwent orthopantomography on the orthopantomograph Orthophosis XG DS/Ceph (SIRONA Dental System GmbH, Germany) in order to assess the state of the bone tissue of the jaws (the degree of destruction of the cortical layer, the degree of resorption of the interalveolar partitions). Targeted intraoral radiographs were used to assess the state of bone tissue and the quality of surgical treatment. Immunological methods of research were carried out in several stages. The findings of this article make the claim that the use of autologous complex immunopeptides in the surgical treatment of periodontitis reduces the time of achieving a therapeutic effect 2 times, resulting in the rapid relief of the symptoms of inflammation and acceleration of reparative processes.

Background. 3D scaffolds plays an important role in developing new approaches in modern dentistry. They are used to establish optimal conditions for cell differentiation, vascularization and remodeling of regenerating bone tissue.Objective. Evaluation of the possibility of creating scaffolds developed on the basis of 3D modeling of periodontal bone defects and containing tooth pulp stem cells.Materials and Methods. The computer tomography data of the maxillar bone tissue defect were analysed. Anatomical prototype — a mold representing defects of the vestibular and palatal fragments of bone tissue was created by 3D printing. This 3D form was filled with fibrin glue and dental pulp stem cells. The fibrin glue was prepared from autologous blood plasma and mixed with dental pulp stem cells.Results. Fibrin glue prepared from an autologous plasma concentrate (fibrinogen 20 g/l) retains its shape for 4 days. On the day 5, the clot retraction became clearly visible and on the day 7, the clot diameter decreased to 50 % of the original size. The proliferation rate of cells, grown both inside the scaffold and in 2D conditions, did not differ. The immunophenotype of cells of both groups corresponded to the immunophenotype of mesenchy mal stromal cells. The mesenchymal immunophenotype is a feature of dental stem cells. Alizarin red staining of cells both grown on adhesive culture plastic and extracted from glue on day 10 after the induction of osteogenic differentiation did not differ.Conclusion. The fibrin glue is a good material for creation a scaffold with suitable mechanical characteristics. The cells enclosed in the fibrin glue maintain their viability, immunophenotype and osteogenic potential. This technology can be used for bone tissue repair in dentistry and maxillofacial surgery.

Aim. The aim of this study is to systematize the scientific knowledge and reconstructed periodontium reliability degree by means of cell-potentiated surgical treatment of patients with the chronic periodontitis.Materials and methods. All 76 patients included in the study (aged 23 to 68 years, Mo 55.3 years) underwent a surgery for directed regeneration of periodontal tissues which was supplemented by transplantation of vascular stromal cell fraction isolated from the patient's adipose tissue (VSF AT). The evaluation of the results of the main periodontal parameters, processed by means of variation statistics in 690 periodontal defects of different topography, was carried out within 10 years.Results. The use of the VSF AT demonstrated the advantage of the method expressed by the formation of a new timeresistant dentogingival junction with the reconstruction of the alveolar bone with a minimum amount of the marginal recession and the probing depth.Conclusion. The percentage of positive results in the representative statistical sample was 90.3% in the main group, 75.6% in the control group within 10 years.

Relevance: to indentify if periodontal treatment which is presented by photoactivated disinfection (PAD) adjunctively to scaling and root planing (SRP) yield better outcomes than ozone therapy as an adjunct to SRP in periodontitis.Materials and methods: we examined 57 (mean age 49,3 ± 1,02) patients with chronic periodontitis, divided into groups, SRP + PAD, SRP + ozone therapy and SRP alone. Subgingival plaque samples were subjected to DNA extraction and real time PCR amplifcation for detection Porphyromonas gingivalis (Pg), Prevotella intermedia (Pi), Tannerella forsythensis (Tf), Treponema denticola (Td), Aggregatibacter actinomycetemcomitans (Aa). The amount of periodontopathogens and clinical parameters including plaque index, clinical attachment loss, pocket depth, bleeding on probing were measured at baseline, after 40, 90 and 180 days.Results: the results in groups of PAD+SRP and ozone therapy+SRP showed an improvement in all clinical parameters PI, BOP, PD, CAL and the quantity of Pg, Td and Tf compared to the control group during an observation period.Conclusion: the results showed additional benefts from PAD and ozone therapy as an adjunctive treatment to SRP for patients of chronic periodontitis.

Chronic periodontitis is nowadays popularly regarded as Dysbiosis, [1] which causes destruction of tissues rich in collagen like periodontal ligament, alveolar bone and gingival connective tissue. The oral biofilm comprises many periodontal pathogens better regarded as ‘triggers’ in causing chronic periodontitis. Since, not everyone will be affected in the same manner due to periodontal pathogens. Some might not elicit a host response while, the others might have exaggerated response. So, host modulation therapy came into existence to counteract the exaggerated host response. The chemically modified tetracyclines (CMTs) have emerged to inhibit the inflammatory response or to reduce the collagenolytic activity of host. Though a derivative of tetracyclines, it still lacks an antimicrobial action and hence, can be used for periodontitis for longer duration with no adverse effects of gastrointestinal toxicity which parent tetracyclines have.

The subject was a 53-year-old male. An alveolar bone sample was obtained from the site of the lower left first molar, before dental implant placement. Although the details of the trabecular structure were not visible with conventional computed tomography, micro-computed tomography (microCT) three-dimensional images of the alveolar bone biopsy sample showed several plate-like trabeculae extending from the lingual cortical bone. Histological observations of the bone sample revealed trabeculae, cuboidal osteoblasts, osteoclasts and hematopoietic cells existing in the bone tissue at the implantation site. Bone metabolic markers and calcaneal bone density were all within normal ranges, indicating no acceleration of the patient’s bone metabolism.Using microCT, and histological and histomorphometrical techniques, a great deal of valuable information about the bone tissue was obtained from a biopsy sample extracted from the patient’s planned implant site.
