Indonesian Navy ships (KAL) is one element of the Integrated Fleet Weapon System (SSAT), which plays an important role in carrying out tasks in the defense field. So, we need an optimal KAL readiness to support the passage of the operation to be carried out successfully and smoothly. One of the ways used to prepare KAL is to carry out routine maintenance through the docking process. However, in practice there are accidents  suffered by workers.This research was conducted using the HIRARC method. HIRARC is a method of hazard identification, risk assessment and risk control measures that are used for this method is considered moreappropriate and more accurately where the dangers that arise in the process described docking of any work activity. In this method also provides appropriate control measures for each hazard. Based on the results of the HIRARC analysis, each suction work activity of docking based Risk Ranking will be sorted according to the level of Risk Matrix and will be discussed further against high risks and implemented controls. Then the resulting draft recommendation to minimize workplace accidents in the docking process is by way of elimination, Administration Control and Use of PPE (Personal Protective Equipment).Keywords: HIRARC method, Docking process, KAL.

The Republic of Indonesia warship (KRI) is one of the elements of the Integrated Fleet Weapon System, which has an important function in carrying out existing tasks in the defense sector. So that KRI readiness is needed optimally to support the operation to be carried out successfully. Water Treatment Plant (WTP) is an important infrastructure to ensure human health and the environment. In its development, aspects of environmental safety and health are of concern. This paper case study was conducted at the Water Treatment Plant Company in Semarang, Central Java, Indonesia. Hazard identification and risk assessment is one part of the occupational safety and health program at the risk management stage. The purpose of this study was to identify potential hazards using hazard identification methods and risk assessment methods. Risk assessment is done using criteria of severity and probability of accident. The results obtained from this risk assessment are 22 potential hazards present in the water purification process. Extreme categories that exist in the risk assessment are leakage of chlorine and industrial fires. Chlorine and fire leakage gets the highest value because its impact threatens many things, such as industrial disasters that could endanger human life and the environment. Control measures undertaken to avoid potential hazards are to apply the use of personal protective equipment, but management will also be better managed in accordance with hazard control hazards, occupational safety and health programs such as issuing work permits, emergency response training is required, Very useful in overcoming potential hazards that have been determined.

ABSTRAK Rumah Potong Hewan merupakan suatu bangunan yang memiliki desain dan konstruksi khusus digunakan sebagai tempat pemotongan hewan. Aktivitas kerja di RPH memiliki potensi bahaya yang dapat mengancam keselamatan dan kesehatan kerja. Berdasarkan observasi awal, RPH Kota Pekanbaru ini tidak memiliki laporan bulanan maupun tahunan mengenai data kecelakaan, dan juga belum pernah melakukan identifikasi, dan penilaian terhadap bahaya yang ada. Adapun tujuan penelitian ini yaitu untuk menganalisis keselamatan kesehatan kerja (K3) terhadap risiko bahaya lingkungan fisik di tempat kerja dengan metode “HIRARC” (Hazard Identification, Risk Assessment, and Risk Control). Jenis penelitian ini adalah kualitatif analitik. Lokasi penenlitian dilakukan di rumah potong hewan Kota Pekanbaru, penelitian dilakukan pada bulan Juli-Agustus 2020. Informan dalam penelitian ini sebanyak 6 orang, sebagai informan utama yaitu Kepala UPTD, 2 orang informan pendukung yaitu koordinator lapangan dan dokter hewan, dan informan kunci yaitu pekerja, metode yang digunakan peneliti yaitu wawancara mendalam dan observasi langsung. Berdasarkan hasil penelitian yang dilakukan potensi bahaya tingkatan risiko “Ekstrim” seperti diseruduk sapi pada proses penggiringan sapi menuju killing box, tertimpa katrol dan sapi pada proses penggantungan dan pemindahan sapi menggunakan katrol. Rumah Potong Hewan belum pernah melakukan identifikasi bahaya, penilaian risiko akan tetapi untuk pengendalian risiko sudah dilakukan beberapa upaya di rumah potong hewan, seperti SOP, Shift kerja, dan juga APD. Rekomendasi yang diberikan yaitu diharapkan sebaiknya RPH memiliki fasilitas yang berhubungan dengan kesehatan pekerja. Perlu adanya upaya pengendalian risiko secara engineering control, contohnya, membuat kerangkeng sapi. administrative control, seperti, memberikan sosialisasi, maintenance, housekeeping dan inspeksi, terhadap alat, mesin dan pekerja. ABSTRACTSlaughterhouse is a building having the design and construction specifically used as animal slaughterhouse. Slaughterhouse’s work activities have potential of hazards which can cause harm to workers’ safety and health. Based on pre observation, the slaughterhouse in Pekanbaru does not have any monthly and annual report about the data of accident. This livestock production also has no doing identification and assessment of hazard. The purposes of this study are to analyze occupational health and safety toward risk of danger physical environment at work, this study uses Hazard Identification, Risk Assessment, and Risk Control “HIRARC” method which using likelihood and severity level of each hazard as references to define a risk value. There are 6 informants the maininfromants is a UPTD head, 2 supporting informants are the field coordinator and venetarian, and the key informant is a worker, The methode that use in this study is interview And observation. The study shows that the most extreme are getting hit by a cow in cattle herding to killing box, falling of a cow hoist material from conveyor railing in shackling and hoisting process. Slaughterhouse has never done hazard identification and risk assessment. However, for risk control have done several attempts in slaughterhouse, such as Standard Operating Procedures, work shifts, and Personal Protective Equipment. Recommendation is given that the slaughterhouse should has facilities related to health of workers. It needs an effort to risk control through engineering control such as making cow cages, administrative control, such as giving socialization, maintenance, housekeeping and inspection of tools, machines and workersKeywords : Slaughterhouse, Hazard, Risk, Physical Environtmen, HIRARC.

ABSTRACTRevolution in the industry sector has been rapidly grown to fill up all the needs of the consumer products. One involves  supporting advanced machinery such as “Cutting, Skiving, Stitching, Emboss Logo, Roving, Punch Hole, Juki, BrushingEdge, Hammer Over Lapping and Two Molding”. In the factory production process, there are various types of high-risk activities, especially on line upper. The main of this research is to study the risk assessment on export companies line the upper part of the shoes export company using Job Safety Analysis. This research was conducted observational crosssectional design. Observations made to the hazards and control measures. Interviews were conducted to 12 employees. Variables in this research is production activity, hazard identification, risk assessment, risk control and residual risk. The results of hazard identification has been done, there are 91 known potential hazards, for risk assessment found 7 high risk and low risk 5. Machine classified as high risk on the risk assessment is roving machine, whereas low-risk is two molding machine. Control efforts on the upper line in accordance with the hierarchy of controlling a number of 91 controls, whereas for the residual risk still remains as much as 30 residual risk. Control has been applied quite well by pressing the consequences of hazards and risk management.Keywords: risk assessment, controlling, residual risk

Background: Risk management is a preventive effort for a company, especially those engaged in the service sector, which is very prone to errors and complaints. For this reason, it is necessary to map the risks that occur in accordance with the work unit in it which is expected to minimize the number of accidents or errors in both patients, visitors and employees in the hospital. In the 2019 patient safety incident report data at the Surabaya A. Yani Islamic Hospital, it was found that there were incidents which included near miss 93%, not injured 4%, unexpected events 3% and Sentinel 0%. The purpose of this study was to identify and identify the application of occupational safety and health risk management at the A. Yani Islamic Hospital in Surabaya. Methods: This thesis is a qualitative research with a case study method with the aim of fully describing the implementation of K3 risk management at Surabaya A. Yani Hospital based on observation, survey and documentation data on 37 units and hospital workplaces by identifying risks. , analyze and find risk control measures. Results: The results showed that the implementation of K3 risk management in the Islamic Hospital of Surabaya A. Yani based on the results of risk identification obtained as many as 25 risks in the graha building and the old building which were further analyzed by finding 15 moderate risks and 10 high risks. can be done is by repairing damaged infrastructure and improving the existing security system at the A. Yani Islamic Hospital in Surabaya. Conclusion: The conclusion in this study is the implementation of K3 risk management at the Islamic Hospital of Surabaya A. Yani based on the process of risk identification and risk analysis, there are 25 potential risks, of which there are 15 moderate risks and 10 high risks that can potentially cause accidents to employees, visitors and patients. In this case, control efforts are made to minimize the number of incidents and accidents.

Occupational Safety and Health is the primary means of prevention of occupational accidents, disabilities and deaths so that the consequences of occupational accidents from potential hazards can be prevented. Companies need to develop risk management based on hazard identification and risk assessment compiled in occupational safety and health programs. Occupational health and safety risk management is an effort of hazard management which potentially pose a risk to safety and health to prevent unwanted accident. The data collected in this research is primary data and secondary data. Primary data obtained through direct observation on the floor processing RSS (Ribbed Smoke Sheet) at PT. PQR is useful to observe the potential hazards of work contained in the work area. While the secondary data taken in this study is historical data. The result of determining the risk value for each hazard source contained in PT. PQR is classified by rankings marked with the code E (extreme risk), H (high risk), M (moderate risk), and L (low risk). This is obtained by comparing the severity and probability values of each process / activity. Of the 28 activities, there are 7 activities included in the category of high risk. Process / activity that belonged to high risk is necessary to control to minimize or even eliminate the existing risks. control measures that can be performed on a high risk process / activity is to use a complete Personal Protective Equipment (APD), MSDS, using warning signs, using ladders in sheet sheets, with ergonomic work posture.Factors causing potential work hazards are workers who perform activities of inadvertent, high temperatures, and negligence of workers in using PPE. These control measures are expected to reduce and even eliminate the risk of the process / activity classified as high risk so that the number of workplace accidents can be reduced.

The present work aimed to evaluate the health risks of occupational exposure to heavy metals in a steel casting unit of a steel plant. To determine occupational exposure to heavy metals, personal air samples were taken from the workers’ breathing zones using the National Institute for Occupational Safety and Health method. Noncancer and cancer risks due to the measured metals were calculated according to the United States Environmental Protection Agency procedures. The results indicated that the noncancer risks owing to occupational exposure to lead (Pb) and manganese were higher than the recommended value in most of the workstations. The estimated cancer risk of Pb was also higher than the allowable value. Moreover, the results of sensitivity analysis indicated that the concentration, inhalation rate, and exposure duration were the most influencing variables contributing to the calculated risks. It was thus concluded that the present control measures were not adequate and further improvements were required for reducing the exposure levels.

BACKGROUND: The COVID-19 global pandemic brought several challenges to occupational safety and health practice. One of these is the need to (re)assess the occupational risks, particularly, biological risks. OBJECTIVE: The purpose of this work is to promote guidance to occupational safety and health practitioners when conducting a biological risk assessment in this context. METHODS: The main steps of the biological risk assessment are explained with some inputs regarding the novelty posed by SARS-CoV-2 and an example of a qualitative risk assessment method is presented. Also, its application to two different activities was exemplified. RESULTS: In both cases, the assessment considered that vulnerable workers were working from home or in medical leave. The results showed low or medium risk level for the assessed tasks. For medium risk level, additional controls are advised, such maintain social distancing, sanitize instruments/equipment before use, use proper and well-maintained PPE (when applicable), and promote awareness sessions to spread good practices at work. Employers must be aware of their obligations regarding biological risk assessment and OSH practitioners must be prepared to screen and link the abundance of scientific evidence generated following the outbreak, with the technical practice. CONCLUSIONS: This paper could be an important contribution to OSH practice since it highlights the need to (re)assess occupational risks, especially biological risk, to ensure a safe return to work, providing technical guidance.

The embodiment of Balinese buildings is known to be synonymous with the use of natural stone materials which are exposed in such a way and not given additional dyes/coatings. The material used is usually in the form of bricks and sandstone. The center of the natural stone craftsman is a location of concentration of craftsmen who produce hand-crafted natural stone. The artificial natural stone craftsman center has a workshop used by artisans to produce crafts. In the production process, there is certainly a risk of workplace accidents that can be experienced by craftsmen. At the center must implement occupational safety and health to provide a sense of security and comfort in work. There are many work accidents caused by human factors, workplace accidents must be avoided so that the production of crafts can run smoothly. The risk of workplace accidents that can occur in craftsmen centers in the form of being injured by sharp objects, being hit by a hammer, or falling. The health of craftsmen is also an important factor in the production of artificial natural stones. Healthy craftsmen will be able to work productively and produce good quality craft items. The application of occupational safety and health is expected to be a solution in reducing the risk of workplace accidents that can occur in the production process.
