Occupational Safety and Health is the primary means of prevention of occupational accidents, disabilities and deaths so that the consequences of occupational accidents from potential hazards can be prevented. Companies need to develop risk management based on hazard identification and risk assessment compiled in occupational safety and health programs. Occupational health and safety risk management is an effort of hazard management which potentially pose a risk to safety and health to prevent unwanted accident. The data collected in this research is primary data and secondary data. Primary data obtained through direct observation on the floor processing RSS (Ribbed Smoke Sheet) at PT. PQR is useful to observe the potential hazards of work contained in the work area. While the secondary data taken in this study is historical data. The result of determining the risk value for each hazard source contained in PT. PQR is classified by rankings marked with the code E (extreme risk), H (high risk), M (moderate risk), and L (low risk). This is obtained by comparing the severity and probability values of each process / activity. Of the 28 activities, there are 7 activities included in the category of high risk. Process / activity that belonged to high risk is necessary to control to minimize or even eliminate the existing risks. control measures that can be performed on a high risk process / activity is to use a complete Personal Protective Equipment (APD), MSDS, using warning signs, using ladders in sheet sheets, with ergonomic work posture.Factors causing potential work hazards are workers who perform activities of inadvertent, high temperatures, and negligence of workers in using PPE. These control measures are expected to reduce and even eliminate the risk of the process / activity classified as high risk so that the number of workplace accidents can be reduced.

The purpose of this study was to determine the Strategy for the Implementation of Occupational Health and Safety Management (K3) implemented by the company to minimize or reduce the number of accidents and occupational diseases. This type of research is a qualitative research using a descriptive approach and the data needed are primary data and secondary data derived from observations, interviews and documentation analyzed by researchers so as to obtain accurate and clear data regarding the strategy for implementing K3 Management by PLN (Persero) UIP3B Sumatra in Pekanbaru City. The results of this study indicate that the Strategy for Implementation of Occupational Safety and Health (K3) Management by PLN (Persero) UIP3B Sumatra in Pekanbaru City However, the implementation of occupational safety and health management carried out by PLN (Persero) UIP3B Sumatra has not achieved the expected goals, this happens because there are still inhibiting factors, namely low employee competence, lack of awareness from employees of the importance of K3 and lack of K3 supervision with reference to Government Regulation Number 50 of 2012.

The statistic on occupational safety and health has received considerable attention in the SME industry regarding to the vital role of the SME in the Malaysian economy to become a developed country by the year 2020. However, the official statistic specific on SME related occupational safety and health still una-vailable. Thus, the secondary data related to estimation statistic refer to the Department of Statistic (DOS) since 2001 until 2012 were used. Estimation related statistic is adapted from the Global Estimates of Occupational Accidents and Work-related Illnesses 2014. The total number of occupational accident and fatalities average high within ten years. Whereas a number of the accident decrease during 2002 to 2007 with the involvement of 85926 to 56339 cases, nevertheless, four years later trend drastically in-creased. Approximately ten thousand fatal occupational related diseases as 2014. In addition, the cost of compensation claims related occupational accident report increase in 2012 as well as disease and sur-vival recipient. Overall cost of occupational accident increase every year. Presently, this study provided useful hints of implication of ignoring safety to encourage improvement of Occupational Safety and Health (OSH), enhance effectiveness of company operational cost, and stimulate prevention at the SME level substantially great approach to the Malaysian economic development.

Selection of research is motivated by the increased interest of tourists will kind of attraction travel in Bali. In this kind of travel these attraction are extreme types of tourist activity in Pulau Serangan is, Swim with Sharks. Of course, by choosing the type of extreme travel to swim with the sharks, the great possibility of the risk of accidents caused. Hence the importance of this research study releated to health and safetyof employees and tourist who do this activity in Pulau Serangan. The data used in this research is qualitative data, while the data source in the form of primary data and secondary data. Data was collected through observation, interviews, and library research. And assisted with the descriptive data analysis techniques. In applying the safety and health at tourism activities swim with sharks, this report uses occupational safety and health procedures that apply to employees and tourist who do that tourism activity. Besides, the factors triggering the accident, or the result of the impact and the prevention and mitigation to be part of the discussion of this report. The results showed that the application of occupational safety and health on travel activity swim with sharks being the foremost, so most have applied, only need the addition of some other procedure for the safety of travelers.   Keywords: Attraction Tourism, Swim With Shark, Occupational health and safety

Water Treatment Plant (WTP) is an important infrastructure to ensure human health and the environment. In its development, aspects of environmental safety and health are of concern. This paper case study was conducted at the Water Treatment Plant Company in Semarang, Central Java, Indonesia. Hazard identification and risk assessment is one part of the occupational safety and health program at the risk management stage. The purpose of this study was to identify potential hazards using hazard identification methods and risk assessment methods. Risk assessment is done using criteria of severity and probability of accident. The results obtained from this risk assessment are 22 potential hazards present in the water purification process. Extreme categories that exist in the risk assessment are leakage of chlorine and industrial fires. Chlorine and fire leakage gets the highest value because its impact threatens many things, such as industrial disasters that could endanger human life and the environment. Control measures undertaken to avoid potential hazards are to apply the use of personal protective equipment, but management will also be better managed in accordance with hazard control hazards, occupational safety and health programs such as issuing work permits, emergency response training is required, Very useful in overcoming potential hazards that have been determined.

The condition of correctional institutions in Indonesia which is currently over capacity or overcapacity as well as the geographical location of Indonesia which is surrounded by dozens of active volcanoes which are also in the zone or ring of fire area which makes the condition of the Indonesian state very dangerous and prone to disasters. However, there are still many lack of employee awareness regarding work accidents and a lack of Occupational Safety and Health signs in the correctional Technical Implementation Unit. So that it caused a lot of riots in the correctional institutions because of the disaster. The purpose of this study discusses the factors that cause accidents in prisons, the implementation of evacuation routes in prisons and a review of the implementation of evacuation routes in prisons. The method used in this research is descriptive method with a qualitative approach, the data sources obtained are field observations, interviews and literature studies. Data collection techniques are direct observation and interviews as primary data and literature studies and theory studies as secondary data. This research was conducted at the Class I Correctional Institution in Bandar Lampung. The results of this study are to be able to find out the factors that cause accidents in prisons, to find out the implementation of evacuation routes in prisons and to be able to evaluate the implementation of safety for natural disasters and other disasters based on the results of implementing the evacuation routes in prisons and the mechanisms for implementing evacuation routes. as the readiness of a correctional institution in dealing with, anticipating and preventing disasters. This study discusses the importance of fulfilling occupational safety and health signs in the form of evacuation routes to provide a sense of security and safety at work. This research also discusses the implementation of occupational safety and health signs as the fulfillment of the rights of prisoners. So that this research is expected to make people's views about prisoners not always negative and change people's views on the life of prisoners in prisons.

This study aims to analyze the implementation of occupational safety and health (K3) in the coal mining company PT XYZ, Analyze and identify the factors that influence the implementation of occupational safety and health (K3) in the mining company PT XYZ. This research is a descriptive qualitative and quantitative research approach. Primary data obtained from respondents are used as a means to obtain information or data carried out by field surveys through direct observation and interviews with respondents in the company and secondary data obtained from PT XYZ collected and compiled according to the problem of this study which was conducted in literature studies. Data were analyzed through text analysis, data interpretation. Data were analyzed using the SPSS (factor analysis) program to analyze the factors that influence the implementation of occupational safety and health in the coal mining company PT XYZ.

Introduction: Non-fatal accidents are estimated to occur in 374 million cases each year and have serious consequences for the productivity of workers. Petrochemical Company, which is a company that operates in the field of fertilizers and other chemicals, needs to conduct an Occupational Safety and Health (OSH) program, one of which is the Housekeeping program. The purpose of this study is to determine the housekeeping program at Petrochemical Company and the application at Amurea II Plant III of Petrochemical Company. Methods: This research was conducted at Petrochemical Company in June 2017. This study used a cross-sectional design. The variables use were the OSH policy, the housekeeping program, the housekeeping procedures, and the applications of housekeeping. The primary data were obtained through direct observation and the secondary data were obtained from the company’s documents. Results: Petrochemical Company has included the housekeeping program in the OSH policy. There are two housekeeping programs at Petrochemical Company, namely daily housekeeping and monthly housekeeping. Also, there are 2 types of procedures in the housekeeping program, namely units that can do the housekeeping program and units that cannot do the housekeeping program. Moreover, the housekeeping program has been implemented in all units, one of which is at Amurea II Plant III Project. Conclusion: The Housekeeping program has been integrated in the company’s policy so that the housekeeping program can be done by all workers both daily and monthly. Housekeeping application has also been implemented at Amurea II Plant III of Petrochemical Company. Keywords: housekeeping procedure, housekeeping program, occupational safety and health policy

This study examines the rate of occurrence of accidents and their causes in mining and quarry sites in East Coast Malaysia. There are two methods that were used in this study. The secondary data technique was used by gathering all accident reports from Department of Occupational Safety and Health (DOSH) inEast Coast Malaysia, and Social Security Organization (SOCSO), Malaysia. The data was used to identify the causes, while the method to identify the rate of accident occurrences is questionnaires. Descriptive tools of statistics such as frequency, percentage and mean were used for the analysis. The study reveals that accidents occur at a low rate at the mining and quarry sites in Malaysia. Different causes of accident were identified such as flying objects or debris, untidy sites, failure of vehicles, pinched while performing work, slip and trip at work areas, faulty equipment and machinery, negligence of workers or operator’s errors, work at sharp edges, and fall from height. This study therefore recommends that relevant control measures should be devised in order to reduce the rate of accident. Kajian ini mengkaji kadar berlakunya kemalangan dan sebab-sebab berlakunya kemalangan di tapak perlombongan dan kuari di Pantai Timur Malaysia. Terdapat dua kaedah yang digunakan dalam kajian ini. Teknik data sekunder telah digunakan dengan mengumpulkan semua laporan kemalangan dari Jabatan Keselamatan dan Kesihatan Pekerjaan (JKKP) di Pantai Timur Malaysia, dan Pertubuhan Keselamatan Sosial (PERKESO), Malaysia. Data digunakan untuk mengenal pasti punca kemalangan, manakala kaedah untuk mengenal pasti kadar kejadian kemalangan adalah melalui soal selidik. Kaedah deskriptif statistik seperti kekerapan, peratusan dan min digunakan dalam analisis. Kajian ini mendedahkan bahawa kemalangan berlaku pada kadar yang lebih rendah di tapak perlombongan dan kuari di Malaysia. Sebabsebab yang berbeza berpuncanya kemalangan telah dikenal pasti seperti objek terbang atau serpihan,laman berantakan, kegagalan kenderaan, mencubit semasa melakukan kerja, tergelincir dan perjalanan pada kawasan kerja, peralatan yang rosak dan jentera, kecuaian pekerja atau kesilapan pengendali, bekerja di tepi tajam, dan jatuh dari ketinggian. Objektif utama kajian ini mencadangkan bahawa langkahlangkah kawalan yang berkenaan perlu dibuat untuk mengurangkan kadar kemalangan.

Occupational safety and health  is an important issue in an operational process, both in traditional and modern sectors. Prevention of work accidents can be done by knowing the risks that exist in a process, one of them is through Hazard Identification, Risk Assessment, and Risk Control (HIRARC) methods. In the batik industry, batik makers come into direct contact with materials containing hazardous chemicals and uncomfortable work positions. In addition, the craftsmen also do not care for the environment and do not use personal protective equipment. The purpose of this study is to determine the factors of occupational safety and health. This purpose also to use of personal protective equipment while working and identify potential hazards in the batik production process in the batik industry of Sekar Arum and Mahkota Laweyan. Factors causing occupational accidents include an unsafe batik environment, unsafe equipment, containing hazardous substances and unsafe work attitudes. In addition, the lack of orderly use of personal protective equipment in the form of boots, gloves and aprons. Based on the findings of the hazard, it is known that there are hazards with low category namely in the nyanting room, washing and drying rooms, and the waste management room with hazard percentage is 10,26 %,  medium category namely in the stamp room with hazard percentage is 30,77 % and high category namely the coloring room with hazard percentage is 38,46 %.

Occupational Safety and Health (K3) is the main means for the prevention of workplace accidents, disabilities, and deaths for employees. One way to identify hazards is to use a work safety analysis or better known as the Job Safety Analysis (JSA), which can examine work methods and find hazards from the design of machines, tools, materials, environment, and work processes needed for workers to prevent interference with work safety. This study aims to analyze and determine the implementation of Job Safety Analysis (JSA) to reduce the number of work accidents at PT. Geoservices Sangatta. Job Safety Analysis (JSA) (X) is used as an independent variable while Work Accident (Y) is the dependent variable. Primary data obtained through interviews directly with 5 employees of PT. Geoservices Sangatta. Secondary data obtained through data that is already available at PT. Geoservices Sangatta. The results showed that in the process of carrying out the work of PT. Geoservices Sangatta had made a policy that was committed to implementing occupational safety and health, as well as identifying hazards and prevention at each stage of the job namely by Job Safety Analysis (JSA). The implementation of Job Safety Analysis (JSA) at PT. Geoservices Sangatta can reduce the number of work accidents as seen from the decreasing number of work accidents that occurred at PT. Geoservices Sangatta in early 2020, although in the implementation there are still some lacking especially in the implementation of safety talk which only runs documents or administration, in practice, they are not routinely carried out. PT. Geoservices Sangatta should follow up with the socialization of Job Safety Analysis (JSA) procedures to all employees, one of which is by conducting training on occupational safety and health so that all employees truly understand and realize how important K3 is to employee safety and health.
