Objective. Demonstrating cardiovascular injury amoung a group of female patients suffering from anorexia, compared to a similarly-aged group of women with body mass within the normal range. Materials and methods. Clinical, biological and paraclinical parameters were determined in the two patient groups: Group A, consisting of 39 patients suffering from anorexia, and Group B, consisting of 40 patients of similar ages, but having a normal body-mass index (BMI). Results. Compared to the normal-BMI cohort, the female patients diagnosed with anorexia are more bradycardic, have lower blood pressure, biologically were found to suffer from various degrees of anemia (hypochromic, microcytic, caused by iron deficiency), leucocytosis, thrombocytosis, hypoglicemia, slight nitrogen compounds retention, hyponatremia, hypokalemia, hypochloremia, hypomagnesemia, hypocalcemia, hypophosphatemia, low levels of triglycerides and of all cholesterol fractions, hypoproteinemia with hypoalbuminemia, mild hepatic cytolysis, increased levels of creatine kinase and cortisol. In terms of echocardiographic findings, the patients with anorexia have decreased dimensions of all cardiac chambers and of the inferior vena cava, lower cardiac output and index, diminished A-wave velocity, with an increase in E/A ratio, lower E', A' and S' velocities for the lateral ventricular wall, along with lower E' and A' velocities for the interventricular septum, and a decrease in both tricuspid annular and mitral annular systolic excursions. Discussions. There is an obvious link between the magnitude of clinical, biological and paraclinical abnormalities and low BMI. Further studies are needed in order to establish the degree of reversibility of said abnormalities following the total or partial correction of the patients' nutritional deficit. Conclusions. The patients suffering from anorexia present with multiple abnormal findings, both biological and clinical in nature, alongside a subclinical negative impact on the cardiovascular system. Early detection of these negative changes could impose the adoption of a realimentation plan, possibly accompanied by the use of specific pharmacological agents, that would bring about improvements in these patients' overall status.

Katigraha is a condition in which the vitiated Vata is localizing in the Katipradesha and producing stiff-ness and pain. These symptoms are similar to Lumbar Spondylosis. The degenerative changes due to aging or trauma, changing in lifestyle & work pattern, unhealthy food habits causing nutritional deficit etc., are causing Dhatukshaya and Margavarodha. Here Ashtavarga & Gandharvahasthadi Kashayas are taken to evaluate the effect on Katigraha. Totally 40 patients of Katigraha were selected, divided in to two groups equally. Group A treated with Ashtavargamkashaya and Group B treated with Gandharvahasthadi Kashaya for 30 days. In group A, 55% patients got moderate improvement and 40% patients with mild im-provement in group B, 5%got moderate improvement and 60% got mild improvement. It is concluded that in general statistically significant results were seen in both the groups. AshtavargamKashaya showed better efficacy than Gandharvahasthadikashaya.

Background: Laparoscopic Cholecystectomy is now accepted as being safe for acute cholecystitis. However, it has not become routine, because the exact timing and approach to the surgical management remains ill define. Careful selection of patients, the knowledge of typical procedure-related complications, and their best treatment are the key points for a safe Laparoscopic Cholecystectomy. Objective: To compare the early and delayed Laparoscopic Cholecystectomy in the acute phase in terms of frequency of conversion to open cholecystectomy. Study Design: Randomized clinical trial. Settings: Department of Surgery, Divisional Headquarter Hospital, Faisalabad. Punjab Medical College, Faisalabad Pakistan. Duration: Study was carried out over a period of six months from June 2018 to May 2019. Methodology: A total of 152 cases (76 cases in each group) were included in this study. All patients were randomly allocated to either group i.e., group -A early Laparoscopic Cholecystectomy and group-B delayed Laparoscopic Cholecystectomy. Results: Mean age was 39.09 + 8.8 and 37.05+ 8.5 years in group- A and B, respectively. In group-A, male patients were 48 (63.2%) and female patients were 28 (36.8%). Similarly, in group-B, male patients were 41 (53.9%) and female patients were 35 (46.1%). Conversion to open cholecystectomy was required in 6 patients (7.9%) of group-A and 16 patients (21.0%) of group – B. Significant difference between two groups was observed (P= 0.021). Conclusion: Early laparoscopic cholecystectomy for acute cholecystitis is safe and feasible in terms of less frequency of conversion to open cholecystectomy.

Object The goal of this study was to investigate the relationship between preservation of the insertion of the deep extensor musculature of the cervical spine at C-2 and postoperative cervical alignment, especially differences between cases involving male and female patients, as well as the relationship between the loss of cervical lordosis and neurological outcome after laminoplasty. Methods The authors reviewed the records of 50 patients who underwent laminoplasty to elevate the C-3 lamina with repair of the deep extensor musculature (Group A) and 31 patients who underwent laminoplasty by C-3 dome laminotomy or laminectomy (Group B). They compared the degree of cervical lordosis after laminoplasty with preoperative measurements. Neurological function at last follow-up was also compared with preoperative assessments. Results In Group A, the mean values for pre- and postoperative cervical lordosis were 14.5 and 10.9°, respectively (p > 0.18). In female patients, however, the pre- and postoperative means were 14.4 and 3.7°, respectively (p < 0.004). In Group B, the overall means for pre- and postoperative cervical lordosis were 17.3 and 19.1°, respectively (p > 0.48); the corresponding means for female patients were 15.0 and 14.1° (p > 0.83). The mean percentages of neurological recovery were 54.1% in Group A and 54.8% in Group B. Conclusions Preservation of the insertion of the deep extensor musculature to the C-2 spinous process prevented significant changes in cervical alignment after laminoplasty, even among female patients. Neurological recovery was not affected by the loss of cervical lordosis.

Background/Aim. Unknown cause of infertility exists in 10%-26% of couples with infertility problems. Treatment of these couples depends on the possibility of correcting the unidentified defect over time. Intrauterine insemination (IUI) and ovaluation stimulation are methods of choice in treatment of unexplained fertility, but if a woman is older than 37 years, in vitro fertilization (IVF) could be directly recommended. The aim of this research was to compare the success rate of pregnancies with IUI between the patients with unexplained infertility and the patients with mild form endometriosis. Methods. The study included on 50 patients diagnosed with mild form endometriosis (group A) and 50 patients with unknown cause infertility (group B). Using the same therapeutical protocol, human menopausal gonadothropin (hMG) stimulation and horionic gonadropin (hCG) induction were applied, as well as IUI. Results. The percentage of achieved ovulation was higher in the group B (p < 0.05). During the 3 simulated sequential periods 102 IUI were performed in the group A and 97 IUI in the group B. In the group A there were 6 single and 1 twin pregnancies sucesfully conceived (14%), while in group B there were 9 (18%) single pregnancies. Conclusion. The use of a combination of controled ovarian hyperstimulation and IUI is an effective, cheap and safe method for treating infertility couples, especially couples with unknown cause infertility. Mild form endometriosis, as etiological infertility factor, has a negative impact on IUI success rate.

Purpose. To report a new clinical finding related to Fabry disease.Methods. Fabry subjects were enrolled in the study, matched for age and sex with healthy individuals as a control group. This is a prospective review of all upper lid pictures taken for every subject at their last visit. A 4-step grading scale is proposed to classify this new entity.Results. Group A (Fabry) comprised 16 males and 22 females, aged 40 (±14) years on average. Group B (control) comprised 7 males and 8 females, aged 37 (±12) Vessels tortuosity was identified on the external superior lid in 36 of the Fabry patients (94.7%), while none of the subjects in group B showed similar vessels tortuosity. In addition, microaneurysms (MAs) were found in 10/38 group A subjects while none in group B presented a similar finding. The differences are highly significant.Conclusion. This paper proposes that blood vessels tortuosity on the upper eyelid be recognized as a new clinical entity for inclusion among the classic ocular manifestations of Fabry’s disease. Without evidence of any negative impact, it should be considered a benign sign contributing to evidence of suspected Fabry disease.

The diameters and areas of portal vein, caudal vena cava and abdominal aorta are useful measurements in dogs. These values can be easily measured by ultrasonographic exam, and variations of normality can be an important indicator of hepatic or extra-hepatic alterations. This study aimed to measure the diameter and areas of portal vein, caudal vena cava and abdominal aorta in healthy dogs, with normal corporal score, divided in groups according to the body weight, and assess whether the data are influenced by animal weight. Thirty dogs were examined and divided into three groups (Group A: ≤ 10 kg Group B: from 10.1 to 20.0 kg; Group C: ≥ 20.1 kg). To measure the diameters and areas of portal vein, caudal vena cava and abdominal aorta, the animal was kept in left lateral decubitus position and the transducer was placed on the right lateral abdominal wall, at approximately the 10th or 11th intercostal space, in the porta hepatis region. The diameters and areas of the portal vein, caudal vena cava and abdominal aorta were significantly lower for dogs in Group A with respect to other groups and the dogs from Groups B and C had similar results with each other. The diameters and areas of the portal vein, caudal vena cava and abdominal aorta may vary with the animal size, and reference values must be specific for small, medium and large dogs.

Background: Adolescent is characterised by rapid physical growth and sexual development, accompanied by changes in the percentage of body fat. Obesity and underweight are one of the most prominent problems of the modern society which consists of a wide range of short-term and long-term complications. The rising prevalence of childhood obesity is directly related to the vascular, metabolic condition and risk factor to cognitive decline or dementia. Under-weight is also often associated with acute and chronic medical complications like Anorexia nervosa, low bone density and mass with impaired immune system and increased mortality rate. Therefore purpose of this study is to identify early signs of impaired BMI as a cause of cognitive impairment and prevent the child obesity and underweight to overcome the future health risk factors. The aim of this study is to find a correlation between BMI and MMSE score in Indian adolescent females. Methodology: 31 female subjects with mean age 16±3 years were selected according to random sampling in this study. The Quetlets formula was used to measure body mass index. Group division for Underweight, Normal and Over-weight was done for the BMI score then Score of MMSE, a measure of cognition was taken. Data was analyzed using SPSS version 16. The Karl Pearsons Coefficient correlation was calculated to see correlation between BMI and MMSE cognitive function. P < 0.05 was considered of statistical significance. Result: The study found correlative change in variables as observed in group A MMSE mean 25.84±3.83 and BMI mean 16.10±1.08 with P<0.05 i.e which was found statically significant. In group B MMSE mean 26.22±1.30 and BMI mean 20.60±1.80 with P>0.05 i.e which was found statically non significnat. In group C MMSE mean 23.00±3.46 and BMI mean 26.06±1.15 with P<0.05 i.e which was found statically significant. And in total number of subject MMSE mean 25.68±3.3 and BMI mean 18.37±3.51with P>0.05. it was found that there was a significant correlation between the body mass index and cognition in group A and group C but there was no significant correlation between body mass index and cognition in group B and total number of subjects(31). Conclusion: The study conclude to state that there exist a significance correlation and of body mass index on cognition.

SUMMARY INTRODUCTION: Obesity is a growing public health problem associated with many comorbid diseases. The aim of this study was to evaluate the relationship between body mass index and complications of thyroidectomy. METHODS: Patients who underwent total thyroidectomy between January 2015 and December 2018 were enrolled. Patients were divided into two groups, i.e., BMI <25 (group A) and BMI≥ 25 (group B). Demographics, operative time, and complications were retrospectively reviewed. RESULTS: The study included 145 patients (66 in Group A and 79 in Group B). There was no significant difference between the two groups in terms of age (p=0.330) and gender (p=0.055). No surgical site infection and bleeding complications were observed in any patients. The mean operative time was 148.4 minutes (90-235) in Group A and 153.4 minutes (85-285) in Group B (p=0.399). Transient hypocalcemia was observed in 25 (37.9%) patients in group A, and 23 (29.1%) patients in Group B (p = 0.291). Permanent hypocalcemia was not observed in any patient in group A, and in 2 patients in Group B (2.5%) (p = 0.501). Transient recurrent nerve palsy was observed in 1 (1.5%) patient in Group A and in 3 (3.8%) patients in Group B (p=0.626). None of the patients had permanent recurrent nerve palsy. Parathyroid autotransplantation was performed on 1 patient (1.5%) in group A and on 7 (8.9%) patients in Group B (p=0.055). CONCLUSION: We think there is no relationship between a high BMI and thyroidectomy complications, and surgery can also be performed safely in this patient group.

Background: Hypothyroidism is a common hormonal disorder affecting various organs including lungs. It may be associated with respiratory symptoms and can decrease lung function. Objective: To observe FVC, FEV1 and FEV1/ FVC % in hypothyroid female patients. Methods: This crosssectional study was carried out in the Department of Physiology, BSMMU, Dhaka, from 1st July 2008 to 30th June 2009 on 60 hypothyroid female patients of 30-50 years age (Group B). For comparison, 30 age and BMI matched apparently healthy subjects (Group A) were also studied. Based on receiving treatment, hypothyroid patients were divided into B1 (untreated patients on their 1st day of diagnosis) and B2 (patients treated for at least 12-18 months). They were selected from the Out Patient Department of Endocrinology wing of department of Medicine, BSMMU, Dhaka. Serum TSH and FT4 levels were measured by Microparticle Enzyme Immunoassay (MEIA) principle in AxSYM system. The FVC, FEV1, FEV1/FVC%, of all the subjects were measured by a digital MicroDL spirometer. Data were analyzed by One way ANOVA test, Independent sample t- test and Pearson’s correlation coefficient test. Results: The mean percentage of predicted values of all the lung function variables in healthy female subjects and treated hypothyroids were within normal ranges. However, all of them were lower in untreated hypothyroids in comparison to those of control and treated hypothyroids.FVC and FEV1 showed statistically significant (p<0.001) difference and FEV1/FVC% showed non significant difference . In addition, all the ventilatory variables had negative correlation with serum TSH level and positive correlation with serum FT4 level and these relationships were statistically significant in control (p<0.001) and treated hypothyroids (p<0.01). Conclusion: This study reveals that lung function may be lower in untreated hypothyrod patients and correlated with thyroid hormones. Treatment of hypothyroids may reverse this changes. Dentinal hypersensitivity is a perplexing problem for the patient as well as clinician. Among the plethora of pharmacological agents developed for the management of this rather common affliction, tubule occluding agents form the mainstay of treatment. Bioglasses (phosphosilicates) with evolving chemical and biological properties, are widely used, with the latest modification being the addition of fluoride to conventional calcium sodium phosphosilicate (Novamin) to result in fluoro calcium phosphosilicate (Biomin), purported to have superior tubule occlusive properties. The present study was designed as randomized clinical trial to compare the efficacy of Biomin and Novamin in reducing the subjective and provoked (thermal) experience of dentinal hypersensitivity. Sixty subjects were randomly prescribed three dentifrices after oral prophylaxis: Group A (20 patients):  dentifrice containing 5% fluoro calcium phosphosilicate (Biomin); Group B (20 patients): dentifrice containing 5% calcium sodium phosphosilicate (Novamin); Group C (20 patients): standard dentifrice containing fluoride. Subjective and thermal sensitivity was assessed using a 10 point VAS score at baseline, at 15 days, 30 days and 60 days of treatment. It was found that though significant reduction in hypersensitivity was observed for all three groups over time, flouro calcium phosphosilicate was most effective in reducing the VAS score, followed by calcium sodium phosphosilicate and fluoride. It was concluded that fluoro calcium phosphosilicate is a promising agent for clinical management of dentinal hypersensitivity.
