Aims: To determine the effect of Early Initiation of Breastfeeding (EIBF) in the third stage of labor and the amount of blood loss in the forth stage of delivery. Methods: An experimental study comparing the duration of 3rd stage and the amount of blood loss in 4th stage in Early Initiation of Breastfeeding (EIBF), oxytocin administration, and Early Initiation of Breastfeeding (IBF) given oxytocin at spontaneous vaginal delivery in delivery room of H. Adam Malik General Hospital, Dr. Pirngadi Hospital, Sundari Hospital and Universitas Sumatera Utara Hospital starting from March 2019 until the minimum sample was met. Results: The data analysis results of the comparison of duration demonstrated p-value of 0.001The comparison among groups with post hoc analysis showed a meaningful comparison in this analysis with p value of 0.001, while duration of the3rd stage in group B compared to group C statistically did not show any significant difference with  p value of 1.00. The mean of amount of blood loss among groups A, B, and C respectively were 139 ± 11.8, 141.8 ± 12.4 and 132.1 ± 14.4 with p value 0.048. Conclusions: It is concluded that the amount of blood loss in the fourth stage among the groups was not significantly different in each group. Comparisons of the amount of bleeding in group A vs. B, A vs. C and B vs. C are indicated by the p value of each group i.e. 1.0; 0184 and 0.059.

Background: Intervention in activemanagement of the third stage of labor (AMTSL) is aimed to accelerate the deliveryof the placenta, to prevent and reduce blood loss in the third stage of labor. Early initiation of breastfeeding facilitates,a baby to start breastfeeding right away, that aims to stimulate the baby’s suckling reflex for increasingmilk production.This practice can reduce the risk of infant mortality due to hypothermia and infection in infants under 1 month. There are several obstacles in its implementation, due to policy, providers and client. It is important to implement AMTSLand early initiation of breastfeeding to improve the health of mothers and infants accordingly.Objective: To determine the implementation of activemanagement of the third stage and early breastfeeding initiationMethods: This study was an observational study with cross sectional design. The samples were 166 postpartummothers and newborns in public hospitals and health centers at District of Sumedang. Analysis of the data usedunivariate analysis, bivariate analysis using Chi-square and multivariate analysis using logistic regression.Results and Discussion: RThe prevalence of active management of the third stage success was 77,1 % and 48,8 %was completed with early initiation of breastfeeding. There was a significant relationship between the success ofearly breastfeeding initiation after active management of the third stage (OR= 3,1, 95 % CI= 1,12-8,49 ). Otherfactors that related were the support of providers, client support and delivery place. Less supportive policy, noten steps to successful breastfeeding and lack of trained personnel led to differences in perception. Lack of staffmotivation and clear information caused the client did not understand the importance of both these proceduresdone.Conclusion: The success of active management of the third stage and early breastfeeding initiation can be achieved ifthere was a comprehensive support from policy makers (policy side), service providers (support side) and servicerecipients (demand side)

Breaseeding (Early initiation) or the beginning of early breastfeeding is a natural process of restoring a human baby to suckle, by allowing the baby to find and suck the milk itself, within the first hour of the beginning of life. touch the baby's hands at the nipple and surrounding, sucking and licking a baby on the nipple stimulates the hormone oxytocin can help the uterus contract, helping expulsion of the placenta and reduce maternal bleeding. Normally, the placenta will be separated in less than 30 minutes after birth. The purpose of this study is the Knowing the relationship between early initiation of breastfeeding plasenta.Rancang spending time with the speed of this study was a cohort (longitudinal prospective). In this study, the speed of early initiation of breastfeeding in question are researchers doing early initiation of breastfeeding in a way after the baby is born and then dried body and the baby's head and placed the baby the mother's chest between the two breasts and then the mother and baby covered together. Perdarahan merupakan penyebab utama kematian ibu di Kabupaten Tegal pada Tahun 2017. Berdasarkan hasil survei di Puskesmas Slawi tahun 2018, terdapat 30 ibu bersalin yang mengalami perdarahan postpartum. Inisiasi Menyusu Dini (IMD) adalah perilaku pencarian puting payudara ibu sesaat setelah bayi lahir. Melaksanakan IMD akan menstimulasi produksi hormon oksitosin secara alami. Hormon Oksitosin ini membantu uterus berkontraksi, sehingga dapat mengontrol perdarahan nifas.Tujuan penelitian untuk menganalisis pengaruh IMD terhadap kontraksi uterus pada ibu nifas di Puskesmas Slawi. Populasi dalam penelitian ini adalah ibu yang bersalin di Puskesmas Slawi periode bulan Januari-Desember tahun 2018 sebanyak 215 responden, dan sampel sebanyak 140 responden. Desain penelitian ini menggunakan metode survei dengan pendekatan case control. Berdasarkan perhitungan Chi Square dengan α = 0,05 diperoleh nilai p sebesar 0,029. Karena nilai p < α berarti secara statistik hasil pengujian signifikan, berarti ada hubungan antara variabel, dan hasil uji statistik pengaruh dengan Regresi Logistik, diperoleh nilai Sig. 0,029, berarti ada pengaruh antara variabel dengan R Square 0,034, berarti bahwa variabel IMD berpengaruh terhadap kontraksi uterus sebesar 3,4%. Kesimpulan dalam penelitian ini ada hubungan dan ada pengaruh IMD terhadap kontraksi uterus ibu nifas di Puskesmas Slawi Kabupaten Tegal. Kata kunci : inisiasi menyusu dini;  kontraksi uterusTHE EFFECT OF EARLY INITIATION OF BREASTFEEDING TOWARDS UTERINE CONTRACTION FOR POSTPARTUM MOTHER AT PUSKESMAS SLAWI TEGAL REGENCY Hemorrhage is the main cause of maternal mortality in Tegal regency 2017. Based on the survey at Slawi District Health Centre, in January – December 2018, there were 30 women in labor with postpartum hemorrhage. Early Initiation of Breastfeeding (IMD) is an act performed by the baby in finding mother’s nipple after born. IMD will stimulate oxytocin hormones helping uterine to get contraction and can control hemorrhage after labor. The study was aimed to analyze the effect of IMD towards uterine contraction for postpartum mother at Slawi District Health Centre. The population was women in labor at Puskesmas Slawi as 215 respondents; the sample was 140 women in labor, and performed or not performed IMD The research design applied case control study..Based on Chi Square with α = 0.05, p value was 0.029. It refused Ho; there was a relationship of those variables. The logistic regression described that Sig. value was 0.029; it showed an effect of those variables with R square of 0.034 stating IMD had an effect of uterine contraction as 3.4%. Therefore, there were a relationship and an effect of IMD towards uterine contraction of postpartum mother at Puskesmas Slawi. Keywords: early initiation of breastfeeding; uterine contraction

Background: Childbirth has a profound influence in a women’s life. One factor that has been more consistently identified as influencing the women’s physical and psychological symptoms following childbirth is the mode of birth.  This study aims at figuring out on women’s satisfaction with their mode of delivery and what difficulties they faced in the immediate postnatal period accordingly.Methods: All women who delivered between the period 1st July 2019 to 1st August 2019 were interviewed with a pretested semi structured questionnaire on postnatal day two. Patient was asked how much they were satisfied with their mode of delivery, immediate postnatal adjustments like ambulation, holding the baby first time, initiation of breastfeeding and pain score following different modes of delivery. The data was analysed to find out which mode of delivery made women more comfortable in the immediate postnatal period.Results: A total 97.1% women were satisfied with the support they got from health care professionals during labour. But when compared, spontaneous vaginal delivery group were satisfied with their mode of delivery (P-value-0.0005 highly significant) than the rest. Women who had vaginal delivery were ahead of caesarean group in terms of ambulation, holding the baby, initiation of breastfeeding and were well adjusted in their postnatal period.Conclusions: Authors conclude that women in our study were more satisfied with spontaneous vaginal delivery than caesarean section which was reflected in their immediate postnatal adjustments. As obstetricians’ authors need to understand the empowering effects of the psychological experience of vaginal delivery. The benefits of this process can be maximized through good communication skills and emotional support for women, enhancing their confidence to deliver normally so that caesarean section is done only when really indicated.

Postpartum haemorrhage (PPH) has been more common over the last three decades, accounting for 11% of all pregnancy-related deaths in the United States. In the third stage of labour, risk classification and active management are crucial preventative techniques. To avoid negative effects, a multidisciplinary approach to PPH patient care is required. To treat uterine atony, uterotonic medicines like oxytocin are used in combination with manipulative procedures like uterine massage and balloon tamponade. The amount of blood loss, duration of the third stage, need for MRP, incidence of PPH, need for repeated oxytocics, and its side effects were measured in Group I 100 women who were administered injection oxytocin 10 IU injection methergin 0.2 mg IV within one minute of the baby's delivery. The mean blood loss at vaginal delivery in Group I was 100-150 ml and in group I P value 0.027, which was statistically significant .In  Group II was 160-200 ml with P value 0.036, which was statistically significant. The mean duration of third stag labour in Group 1 was 124.6 min and Group 2 was 144.8 min intravenous methergin is a better uterotonic when compared to intramuscular oxytocin to reduce the amount of blood loss at delivery and prevent complications like atonic PPH.

Early initiation of breastfeeding is program of Indonesian Government.   Early breastfeeding has not been widely practiced, <30% for South Kalimantan and 21% for Tapin District. The implementation of early initiation of breastfeeding by midwives at Datu Sanggul Rantau Hospital is influenced by age, education level, years of service, midwife training and socialization of implementation of early breastfeeding initiation. This study aims was to analyze the factors associated with implementation of early initiation of breastfeeding by midwives at Datu Sanggul Rantau Hospital. The study was cross sectional, with 32 midwives in PONEK, maternity and nicu at Datu Sanggul Rantau Hospital. The variables were age, education level, years of service, training and socialization. Data were analyzed using Chi square.  The results showed socialization was related with p value of 0.004. Unrelated factors were age p value 0.660, education level p value 0.496, years of service p value of 0.761, and training p value 0.314. Simultaneously, age, education level, years of service, training and socialization are not related to the implementation of early breastfeeding initiation with p value 0, 083. The conclusion is there is a significant relationship between socialization and the implementation of early initiation of breastfeeding at Datu Sanggul Rantau Hospital.

Abstract Background : The world is now suffering from malnutrition and remains one of the major causes of under-five mortality. Children from developing countries, including Ethiopia also suffer from undernutrition due to suboptimal breastfeeding practice. Hence, the study aims were to assess early initiation of breastfeeding among children aged less than two years, months in West Belessa district, North West Ethiopia, 2019. Methods: A community-based cross-sectional study was conducted from January to February 2019 in the West Belessa district. A total of 569 Children was selected by a simple random sampling technique. The data were collected by an interviewer-administered structured questionnaire. Data were entered and analyzed by using Epi-Info version 7 and SPPS version 20, respectively. Bi-variable logistic regression analysis was used to check predictor variables associated with the dependent variable. Variables having a p-value of less than 0.20 in the bi-variable analysis were fitted into the multivariable model. Multivariable binary logistic regression with a 95% confidence interval and Odds Ratio (OR) were computed. Variables having p-value < 0.05 were taken as significantly associated with the dependent variables. Result : The prevalence of EIBF was 77.7 % (95%CI, 74.3-81.0). Age of the mother (AOR= 2.76, 95%CI (1.21, 6.27)), postnatal Visit (AOR= 1.85, 95%CI (1.03, 3.85)), and Antenatal care (AOR= 2.58, 95%CI (1.18, 9.94)) was significantly associated with EIBF. Conclusion and Recommendation: We observed, the prevalence of early initiation of Breastfeeding was low in West Belessa District. Age of the mother, Antenatal and postnatal care were associated with EIBF. Hence, improving antenatal and postnatal care services through increase accessibility and providing counseling during this contact time. AbstractThe infant mortality rate in Indonesia is still high at 24 per 100 live births. One of the causes is infection. Infection can by prevented by controling breast milk because breast milk contains colostrum which is able to maintain the baby’s immune system. The introduction of breastfeeding begins with early initiation of breastfeeding. The coverage of newsborn in Indonesia in 2019 who received early intiation of breastfeeding was 75,58%.The purpose of this study was to determine the between early initiation of breastfeeding and the smooth production of breast milk throught the Literature Review.This reserch is a quantitive research through Literature Review. The articles were obtained from three articles from articles from Garuda Portal which is a critical review of the JBI instrument written by The Joanna Briggs Institute. The results of this study indicate a relationship between the provion of early intitiation og breastfeeding with the smooth discharge of breast milk with p value <0,05 (0,000-0,029).There is a corelation between the previsions of early initiation of breastfeeding the smoothness is producing breast milk.Keywords :giving early initiation of breastfeeding ( IMD), breast milk flow AbstrakAngka kematian bayi di Indonesia masih tinggi yaitu 24 per 1000 kelahiran hidup. Penelitian ini merupakan Penelitian kuantitatif melalui Literature Review. Artikel didapatkan dari Searh Ergine Google scholar tiga dan Portal Garuda dua telaah kritis instrument JBI The Joanna Briggs Institute .Hasil penelitian ini menunjukkan adanya hubungan antara pemberian inisiasi menyusu dini (IMD) dengan kelancaran pengeluaran ASI dengan p value<0,05 (0,000-0,029).Terdapat hubungan antara pemberian inisiasi menyusu dini (IMD) dengan kelancaran pengeluaran ASI.Kata kunci: Pemberian IMD ; Kelancaran Pengeluaran ASI.
