Long-Term Tissue Reactions of Three Biomaterials in Craniofacial Surgery

Successful craniofacial reconstruction needs both a well-known and a reliable reconstruction material. However, there is often a lack of long-term knowledge of the tissue reactions and healing process in the human body. In this study, frontal sinus obliterations with bovine bone natural hydroxyapatite derivative (BHA), synthetic bioactive glass S53P4 (BAG) and hydroxyapatite cement (HAC) were investigated with clinical, histologic, scanning electron microscopic (SEM) and energy dispersive x-ray analysis (EDXA) 27, 12 and 3 years postoperatively. The aim was to determine the long-term clinical biocompatibility of the used materials. Histologic studies revealed bone formation with BHA particles and lamellar bone with BAG granule remnants in close contact to the new bone formation. In HAC reconstruction there was scattered fibroconnective tissue growth without new bone formation in the surface of HAC implantation. Neither foreign body reaction nor any abnormal findings were seen. SEM studies revealed a CaP layer on the surface of BAG granule remnants. In EDXA studies, composition profiles showed Ca-, P- and Si- rich layers on the BAG granule surface. No differences were found in CaO and P2O5 levels between BHA granules and HAC implantation and the surrounding bone. All investigated biomaterials were well tolerated in long-term applications.

Abstract Background The aim of this study was to compare new bone formation, resorbed bone matrix, and fibrous enclosed residual bone substitute material in laterally augmented alveolar bone defects using allogeneic, pre-treated and cleaned human bone blocks (tested in dogs, therefore considered to be xenogeneic), and pre-treated and cleaned bovine cancellous bone blocks, both with and without a collagen membrane in order to evaluate their augmentative potential. Methods Thirty-two critical size horizontal defects were prepared in the mandible of 4 adult foxhound dogs (8 per dog, 4 on each side). After 3 months of healing, the defects were laterally augmented in a split-mouth-design with either human (HXB) or bovine solvent-preserved bone blocks (BXB). Afterwards, defects were randomly covered with a bovine collagenous membrane (HXB + M, BXB + M). After a healing interval of 6 months, percentages of new bone formation, resorbed bone matrix, and fibrous enclosed residual bone substitute material were compared. Results Results showed little new bone formation of up to 3.7 % in human bone blocks (HXB 3.7 % ± 10.2, HXB + M 0.3 %± 0.4, BXB, 0.1 % ± 0.8, BXB + M 2.6 % ± 3.2, p = > 0.05). Percentages of fibrous encapsulation were higher in human bone blocks than in bovine bone blocks (HXB 71.2 % ± 8.6, HXB + M 73.71 % ± 10.6, BXB, 60.5 % ± 27.4, BXB + M 52.5 % ± 28.4, p = > 0.05). Resorption rates differed from 44.8 % in bovine bone blocks covered with a membrane to 17.4 % in human bone blocks (HXB 17.4 % ± 7.4, HXB + M 25.9 % ± 10.7, BXB, 38.4 % ± 27.2, BXB + M 44.8 % ± 29.6, p = > 0.05). The use of additional membranes did not significantly affect results. Conclusions Within its limitations, results of this study suggest that solvent-preserved xenogenic human and bovine bone blocks are not suitable for lateral bone augmentation in dogs. Furthermore, defect coverage with a membrane does not positively affect the outcome.

Abstract There is a paucity of studies aimed to compare the healing process of piezosurgery with conventional surgery. This study evaluated the histopathological assessment of healing process in osteotomy defects caused by these surgical methods in rats using an amniotic membrane (hAM). In this randomized controlled experimental study, there were 2 main groups: group 1 (Piezosurgery method-right tibia) and group 2 (Conventional surgery method-left tibia) and subgroups determined by the sacrification periods on the 7th (n = 20) and 21st days (n = 20). hAM was used in all groups. The primary outcome variable was new bone formation, while inflammation, necrosis, fibrotic tissue formation in the defective zone were secondary outcomes. 40 male Sprague Dawley rats were used. The 7th day fibrosis levels and the 7th new bone-building level of the group who underwent piezosurgery were found to be significantly higher than those of the group undergoing conventional surgery (p < 0.05) and the 21st new bone-building level was significantly lower than conventional surgery. In all rats, on the 21st day, there were statistically significant decreases in inflammation, fibrosis, and increases in the healing score (p < 0.05). Also, there was no statistically difference in new bone formation between 7st and 21st days (p > 0.05). The choice of conventional surgery when applying with hAM increases the new bone building in the late period.

1. Calcium balances and formation rates of new bone measured with an improved tracer technique using 85Sr have been determined simultaneously in 21 patients with idiopathic osteoporosis and vertebral crush fractures. 2. A weak positive association was found between calcium balance and the kinetically measured calcium accretion rate, which is the sum of the true rate of bone formation and various long-term exchange processes. 3. The more negative balances were associated with significantly greater early loss of tracer taken up into bone by ‘accretion’, so that long-term (> 200 day) uptake was reduced. 4. This indicates that patients actively losing bone mineral have lower true rates of bone formation and higher rates of long-term exchange than their fellow patients who are more nearly in calcium equilibrium. 5. No statistically significant association was found between measured rates of bone resorption and calcium balance.

Regeneration of large jaw bone defects still remains a clinical challenge. To avoid incomplete bone repair, bone grafts have been advocated to support the healing process. This study comparatively evaluated new bone formation among a synthetic graft substitute, a human bone derivative, and a bovine xenograft. Materials were placed in 3 out of the 4 bone cavities, while 1 deficit was left empty, serving as a control, in mono-cortical defects, surgically prepared in the porcine calvaria bone. Animals were randomized in 2 groups and euthanized at 8 and 12 weeks. Harvested tissue specimens were qualitatively evaluated by histology. New bone formation was quantitatively measured by histomorphometry. Maximum new bone formation was noticed in defects grafted with beta-tricalcium phosphate b-TCP compared to the other bone substitutes, at 8 and 12 weeks post-surgery. Bovine and human allograft induced less new bone formation compared to empty bone cavity. Histologic analysis revealed that b-TCP was absorbed and substituted significantly, while bovine and human allograft was maintained almost intact in close proximity with new bone. Based on our findings, higher new bone formation was detected in defects filled with b-TCP when compared to bovine and human graft substitutes.

Abstract The beneficial aspects of fibrin sealants for soft tissues are well documented, but studies of their direct influence on bone healing and their effectiveness in augmenting bone graft healing have produced conflicting results. The aim of this study was to evaluate the influence of fibrin sealants (FS) in the alveolar bone healing process during a period of up to 28 days. Seventy-five Wistar rats were submitted to a superior incisor extraction after intramuscular anesthesia (kentamine chloridrate 10% – 10mg/kg and tiazina chloridrate-5mg/kg). The rats were divided into tree different groups and the sealants were introduced into the alveolar bone. The first group (25) received human homemade fibrin glue, the second group (25) FS associated to factor XIII and aprotinin (Beriplast; Aventis-Beringher) and the third group (25) was the control. Animals were sacrificed by prolonged diethyl inhalation on days 7, 14 and 28 after surgery. Animal craniums were dissected and submitted to a decalcification, and preparated for H&E light microscopy. The morphometric study was performed by means of an interactive computerized image analysis system KS400 (Zeiss, Jena). New bone formation was carefully delimited in four different alveolar regions (apical, two middle areas and cervical) of each specimen. The data were statistically analyzed using multiple regression, ANOVA and Tukey test. Results showed that the amount of alveolar bone formation (μm) in the control group and commercial sealant was statistically similar. However, alveolus receiving homemade sealant presented less amount of new bone formation comparing to commercial sealant and control group (p=0.0034) (figure 1). The present study demonstrated that homemade fibrin sealant delays osteogenic formation. Commercial sealant did not improve alveolar repair however, the amount of new bone formation was slightly higher comparing to the control, probably due to the factor XIII present in this sealant.
