AbstractVitamin D (vitD) deficiency is highly prevalent in the Middle East (including Saudi Arabia) despite the abundance of sunlight. Older individuals in particular are at high risk of being vitD deficient. VitD binding protein (DBP), which acts as a carrier of vitD and its metabolites, has been reported to influence vitD status. In our study we aimed to investigate vitD status among postmenopausal women and its relation to DBP. A cross-sectional study was conducted at the King Fahad Medical Research Center, King Abdulaziz University, Jeddah, Saudi Arabia. Seventy six postmenopausal females (age ≥ 50 years) who were not taking vitD supplementation and who were resident in Jeddah city, were randomly recruited from internal medicine clinics at King Abdulaziz University Hospital. Anthropometric measures, blood pressure, lifestyle history, dietary vitD intake and fasting blood samples were obtained from all study participants. Serum total 25 hydroxy-vitamin D (25(OH)D), DBP, albumin, parathyroid hormone, calcium, phosphate, magnesium and metabolic bone parameters were analysed. VitD deficiency was defined as serum total 25(OH)D level below 30 nmol/L. The mean (± SD) serum level of total 25(OH)D was 46.9 ± 28.9 nmol/L with 36 % of the study population being vitD deficient. Although non-significant, the vitD deficient group had lower DBP and higher dietary vitD intake levels when compared with those with serum vitD > 30nmol/L. In addition, DBP was inversely correlated with vitD dietary intake (r = -0.233, P = 0.046). In conclusion, vitD deficiency is highly prevalent among postmenopausal women living in Jeddah, Saudi Arabia. Intake of a vitD rich food seems to be associated with low DBP levels. Genetic polymorphisms in DBP will be studied in the future to find out a possible explanation for the differences in vitD status and DBP between individuals as well as the concomitant relationship between dietary vitD intake, DBP and serum 25(OH)D levels.

AbstractVitamin D deficiency is an epidemic public health problem worldwide. It is common in the Middle East and is more severe in women. This cross-sectional study was conducted to assess vitamin D deficiency and associated risk factors in women living in Riyadh, Saudi Arabia. Serum 25-hydroxyvitamin D (25(OH)D) was measured in 166 women aged 30–65 years. Socio-demographic, lifestyle and health status characteristics, as well as intake of selected dietary supplements, were collected. Weight and height were measured. Vitamin D deficiency (25(OH)D < 20 ng/mL) was reported in 60.2% of participants. Mean of serum 25(OH)D was 20.7 ng/mL. Older age and taking the supplements of vitamin D, multi-vitamins or calcium were identified as factors that associated with a lower risk of hypovitaminosis D. A national strategy is needed to control a hypovitaminosis D crisis in Saudi Arabia. This could be accomplished by raising public awareness regarding vitamin D, regulating and enhancing vitamin D fortification and supplementation and screening vitamin D status among women at high risk.

Osteoporosis is a common public health challenge globally but the relative contribution of variousrisk factors for osteoporosis in Saudi Arabia is not well established. We conducted this study to determine the prevalence of risk factors for osteoporosis among females aged 35 years and above in Saudi Arabia. We performed a descriptive cross-sectional study with a sample size of 2070 and determined the prevalence of risk factors as percentages of the total sample size. We foundthat vitamin D deficiency is the major modifiable risk factor for osteoporosis in Saudi Arabia followed by cigarette smoking. Autoimmune and endocrine diseases were also not uncommon. Our results therefore suggest for the strengthening of measures to prevent vitamin D deficiency, reduce corticosteroid-related osteoporosis and curb cigarette smoking.

ObjectiveInterrelationships between hormones of the hypothalamic–pituitary–testicular (HPT) axis, hypogonadism, vitamin D and seasonality remain poorly defined. We investigated whether HPT axis hormones and hypogonadism are associated with serum levels of 25-hydroxyvitamin D (25(OH)D) in men.Design and methodsCross-sectional survey of 3369 community-dwelling men aged 40–79 years in eight European centres. Testosterone (T), oestradiol (E2) and dihydrotestosterone were measured by gas chromatography–mass spectrometry; LH, FSH, sex hormone binding globulin (SHBG), 25(OH)D and parathyroid hormone by immunoassay. Free T was calculated from total T, SHBG and albumin. Gonadal status was categorised as eugonadal (normal T/LH), secondary (low T, low/normal LH), primary (low T, elevated LH) and compensated (normal T, elevated LH) hypogonadism. Associations of HPT axis hormones with 25(OH)D were examined using linear regression and hypogonadism with vitamin D using multinomial logistic regression.ResultsIn univariate analyses, free T levels were lower (P=0.02) and E2and LH levels were higher (P<0.05) in men with vitamin D deficiency (25(OH)D <50 nmol/l). 25(OH)D was positively associated with total and free T and negatively with E2and LH in age- and centre-adjusted linear regressions. After adjusting for health and lifestyle factors, no significant associations were observed between 25(OH)D and individual hormones of the HPT axis. However, vitamin D deficiency was significantly associated with compensated (relative risk ratio (RRR)=1.52,P=0.03) and secondary hypogonadism (RRR=1.16,P=0.05). Seasonal variation was only observed for 25(OH)D (P<0.001).ConclusionsSecondary and compensated hypogonadism were associated with vitamin D deficiency and the clinical significance of this relationship warrants further investigation.

Background: Various factors lead to hypovitaminosis (decreased vitamin D levels in serum/blood), in general population, sunlight being one of the major factors. Doctors and staff nurses are particularly at risk due to long working hours indoors, which deprives them of adequate Ultraviolet light exposure.Methods: It was a prospective descriptive cross sectional study conducted at Ziauddin University Hospital, Keamari campus, from August 2018 to July 2019. The objective of this study was to determine the status of vitamin D deficiency among doctors and staff nurses working at Ziauddin University Hospital. All those doctors and staff nurses were included in the study, who were already advised by physicians to get serum vitamin D levels done for their symptoms. Results: A total sample of 118 was collected. 56 doctors and 62 staff nurses participated in the survey. 85 out of 118 (72%) subjects were either deficient or insufficient of vitamin D. Conclusion: Even though doctors and nurses are considered to have better understanding of vitamin D deficiency symptoms and morbidity it causes; still this study found out that majority of the healthcare providers working at Ziauddin hospital were deficient of this vitamin.
