Sandhigata Vata (Osteoarthritis) is a type of Vatavyadhi (degenerative disease) that mainly occurs in Vrid- dhavastha (Old age) due to Dhatukshaya (cellular degeneration). Vagabhatta has also considered Vata Vyadhi as a Maharoga. It appears from the point of view of modern medical sciences that Sandhigata vata can be simulated with Osteoarthritis in its clinical appearance. Osteoarthritis is the most common articular disorder that begins asymptomatically in the 2nd and 3rddecades and is extremely common by age 70. Almost all persons by age 40 have some pathologic change in the weight-bearing joint. 25% of females and 16% males have symptomatic OA. This disease has been described in Ayurveda from ancient times. The disease Arthritis causes work disability. It limits everyday activities such as walking, dressing, bathing etc, thus making individuals handicapped. No treat- ment is available which can prevent the disease process. In western medical science, mainly analgesics, anti- inflammatory drugs or surgery are the options for the treatment of Osteoarthritis; don’t provide remarkable recov- ery but causes great adverse effect. Researchers are trying their level best for making drugs that can prevent or slow down or reverse joint damage. Panchakarma therapy is one of the important branches of Ayurveda, which deals mainly with the purification of the aggravated Doshas from the human system. This is the reason behind the selection of the topic entitled “A COMPARATIVE CLINICAL STUDY OF MATRA VASTI AND PICHU WITH RASNA TAILA IN SANDHIGATA VATA W.S.R TO OSTEOARTHRITIS”. We have used Panchakarma treatment procedure. Panchakarma treatment is believed to help in the radical elimi- nation of the disease-causing factors and maintain the equilibrium of doshas. The selection of Matravasti and pichu both are based on repeated recommendations of different Acharyas on Sandhigatavata. We used Rasna taila. Intravenous anesthesia became possible with drugs available since 1930 and the concept rapidly became popular with patients and anaesthetists. From the patient's point of view it had the advantage of producing rapid loss of consciousness without excitement, distress, or the sensation of smothering often produced by a tightly pressed facemask. For the anesthetist, there was the predictable anaesthesia which was ideally rapid in onset and without coughing or movements. Thiopentone sodium, propofol and midazolam have been used in our comparative clinical study (About 100 cases) as an intravenous anaesthetic agent. Our clinical study was into three aged groups such as neonates & children (40 cases), middle aged (40 cases) and elderly (20 cases). 100 cases were divided into paediatric cases, in outpatient procedures, in neurosurgical cases, in geriatric anaesthesia, in obstetric cases. In our comparative study, we have seen when propofol used for induction of anaesthesia in briefer procedures, results in a significantly quicker recovery and an earlier return of psychomotor function as compared with thiopentone and midazolam irrespective of the agent used for maintenance of anaesthesia.   doi: 10.3329/taj.v20i1.3086 TAJ 2007; 20(1): 28-35

Grahani Dosha is vitiation of Agni i.e. functional derangement of Grahani regarding production of Pachaka Pitta (enzymes responsible for digestion) and also holding (Grahana) of Ama (food) for digestion. Grahani in Ayurveda is called as Pittadhara kala which is the seat of Agni responsible for digestion of food and situated above Nabhi. The part of small intestine and large intestine lying between Amashaya and Pakwashaya should be considered as Grahani. This part is also described as the sole site of Pitta. Various disorders of GI tract like irritable bowel syndrome (IBS) can be considered under heading of Grahani Roga. Both the formulations Nagradya Churna and Bhunimbadya churna are highly effective, promote appetite, digestion and remove Ama Dosha from the system by increasing the power of Agni. Madhu is Yogabahi and it helps to initiate the absorption through intestine properly. Total 40 patients of Grahani were registered and randomly divided into 2 groups for clinical trial of 45days. Result of the study revealed that both the formulations produced significant results in almost all cases barring a few patients who were suffering from longer period.

Gridhrasi is a Vyadhi characterized by Stambha (stiffness), Ruk (pain), Toda (pricking pain), Spandana (twitching) etc. Though it is of two types – Vataja and Vatakaphaja, Vata being the prime cause for this condition has to be tackled to gain relief. Gridhrasi, according to its signs and symptoms can be compared to sciatica in modern medical science which is a painful condition in which pain commences from the buttock and radiates into the lower extremity along its posterior or lateral aspect, more or less comprising of the area of distribution of the sciatic nerve. Thus, this study was taken up to evaluate the efficacy of Rasona taila and Gandharvahastadi eranda taila in the management of Gridhrasi (Sciatica). Methods: In the present study, 60 subjects diagnosed with Gridhrasi were randomly selected and assigned into two equal groups Group A and Group B comprising 30 subjects each. Subjects of Group A received Rasona taila 10ml orally in morning before food and subjects of Group B received Gandharvahastadi eranda taila 10 ml orally in morning before food, both for duration of 21 days. The data obtained in both the groups were recorded, tabulated and statistically analyzed using appropriate statistical methods. Results: After obtaining all the necessary data, the results were formulated by applying suitable statistical tests. Group A showed better results statistically when compared to Group B. Conclusion: Rasona taila has better effects than Gandharvahastadi eranda taila both clinically and statistically in reducing the signs and symptoms of Gridhrasi.

Amavata is the most challenging disease for the medical science, as it causes severe intolerable pain and severe deformities making the patient disable and bedridden. As it is the disease of Rasavaha Srotas and having several features similar to Rheumatoid Arthritis, Amavata can be correlated with Rheumatoid Arthritis (RA). Amavata is the outcome of Agnidushti, Amotpatti and Sandhivikrutti, with the passage of the time most of the dietary habits, changing lifestyle and environment have been contributing to the disease, the risk factor include age, gender, genetics and environmental exposures. Hence Amavata have became a challenge to the human race. Management of the disease is merely insufficient in other system of medicine, in such condition disease which continues to pose challenge to the physician as well as for patients due to severe morbidity and crippling nature. So patients are continuously looking for the treatment from Ayurved to overcome this challenge. It also shows extra articular features like bodyache, stiffness etc. many research have been carried out to solve this clinical enigma.The therapy which normalizes Agni metabolizes Ama and regulates Vata and maintain healthy Sandhi and Sandhistha Shelshma will be the supreme one for this disease. The formulations of Amavatavidhvansa Rasa and Simhanaada Guggula is indicated, especially in treating the Amavata.

In Vriddhavastha, most of the Dhatus Kshaya, which in turn leads to Vataprakopa and thus making individuals prone to many diseases. Among these diseases, Sandhigata Vata is one type of Vata-Vyadhi, which is the commonest form and occurs mainly due to sitting jobs, air conditioner atmosphere, lack of proper healthy diet, travelling and old age etc. Sandhigata Vata is one of the most common Vatavyadhi, which can be correlated with osteoarthritis. The prevalence rate of Osteoarthritis is about 14.8 %, in which knee osteoarthritis prevalence rate is 10.8% which is more than the arthritis of other joints1. The incidence of osteoarthritis in India is as high as 12%. It is estimated that approximately four out of 100 people are affected by it. Osteoarthritis is the most common articular disorder which begins asymptomatically in the 2nd & 3rd decades and is extremely common by age 70. Almost all persons by age 40 have some pathologic change in the weight-bearing joint. 25% of females & 16% of males have symptomatic osteoarthritis. When we talk about healthy joints, there is a coating of tough but smooth and slippery tissue, called cartilage, which covers the surface of the bones and helps the bones to move freely against each other. When a joint develops osteoarthritis, part of the cartilage thins and the surface becomes rougher, which in turn doesn't allow the joint to move as smoothly as it should. When cartilage becomes damaged, all the tissues within the joint become more active than normal as the body tries to repair itself. But these repair processes are going to change not only the structure of the joint but will often allow the joint to work normally and without any pain and stiffness. Acharya Charaka has mentioned "Sandhi Gataanila”, which means vata gets located in the Sandhi and thereafter results in Sandhigatavata. Allopathic treatment has its limitation in managing this disease. It can provide either conservative or surgical treatment and can only subside the pain but not cure the cause of diseases. While traditional life science Ayurveda has the most effective solution over this. Local Abhyanga and Nadi Sweda were selected for the present study in one group, as it has shown best for the Vata Vyadhis. Here local Abhyanga was given with Bala Taila because Bala Taila and Nadi Sweda are having Vatashamaka and Rasayana properties. While in the case of another group, Guggulu gold along with gum shallaki followed by local Abhyanga and Nadi Sweda were given to the patients. Basti is considered the best modality of treatment in managing the disorders of Vata Dosha. As Sandhigatavata is also one among Vatavyadhi, Basti is the effective line of treatment, which is characterized by swelling, pain during flexion and extension of joints. Osteoarthritis is a common degenerative joint condition with a prevalence of about 22% to 39% in India. There is a steady rise in prevalence from age 30 such that by age 65, 80% of people will have radiographic evidence of osteoarthritis. It is caused by the degradation of the joints, the articular cartilages and subchondral bone. It is characterized by pain, stiffness and loss of flexibility of joints.1 By foregoing, Sandhi- gatavata can be readily correlated to Osteoarthritis. In contemporary medical science, they use NSAIDs for control of the symptoms. Acharya Sushrutha and Acharya Charaka have mentioned Snehana (Oleation) as a line of treat- ment in Sandhigatavata. So here an attempt is made through Janubasthi and Matrabasthi using Saireyaka Taila2 mentioned in Gada Nigraha under Vata Vyadhi for treating Janusandhigatavata in specific. Method: This study is an open-label, observational and clinical study containing 30 patients, who were posted for Janu Basti and Matra Basti with Saireyaka Taila for 9 days. The assessment was carried out before treatment, during treatment (5th day), after treatment (on 10th day) and after following up (28th day) and the gradings were noted. Results: Out of 30 patients in this study, 5 patients (17%) were getting Moderate Improvement and 25 patients (83%) were gettingGood Improvement. The overall effect of the treatment is 69.44%. Conclusion: It is concluded that the combined effect of Janubasti and Matrabasti with Saireyaka Taila is clinically and statistically significant in almost all the parameters. Purpose – The purpose of this paper is to study the application level of transformational organization components from Tehran Medical Science University librarians’ point of view. Design/methodology/approach – The present survey method is descriptive-scaling. The statistical population of this survey has included all librarians of Medical Science University of Tehran about 50 individuals. As a few numbers of statistical population, almost all of them were surveyed. Accomplished questionnaires were used to collect data. To analyze data, Kolmogorov-Smirnov test, Pearson correlation coefficient and Friedman test were used. Findings – The results of survey showed that all components of transformational organization in the libraries of Medical Science University of Tehran were not very strong. Although the results of this survey showed that transformational organization components in the libraries of Medical Science University of Tehran are weak, to reach such a desirable level in libraries preparation of this university for converting to transformational organization, it is essential for managers and organizers of these libraries to take making relations more extensive among managers and staff for more cooperation in organization decision making, appropriate human resource training with up-to-date process, planning and practicality of organization knowledge map for discovering the talents and their management into serious consideration. Originality/value – To change conventional organization to a transformational one, change and transformation should occur in attitude and human resource knowledge organization and gradually in other dimensions means organizational structure, organizational culture and technology.

Gridhrasi is a painful condition in which the person can’t sit and walk properly that hampers his normal activity. Almost all signs and symptoms of Gridhrasi resemble with the condition of sciatica described by the modern texts. Its detail symptomatology has been described in Ayurvedic classics since 5000 years, while this condition was known to modern medical science just two centuries ago. According to Ayurveda principles treatment for Vatavyadhi should be Shodhana followed by Shamana therapy. According to present era it should be less time consuming and more effective. Keeping this in mind, A clinical study in the management of Gridhrasi with Shodhana and Shamana therapy was undertaken in VYDSAMC and Hospital, Khurja,Bulandshahr, with the aim to give promising results to the patient of Gridhrasi. Shodhana therapy as Mridu Virechana followed by Shamana with Yograja Guggulu and Maharasnadi Kwatha was given to 29 patients with classical symptoms of Gridhrasi, it showed significant results. The following study can be taken for further research in this field.

The article looks at book production and circulation from the point of view of translators, who, as purchasers and readers of foreign-language books, are an important mediating force in the selection of literature for translation. Taking the German publisher Tauchnitz's series ‘Collection of British Authors’ and its circulation in Finland in the nineteenth and early twentieth century as a case in point, the article argues that the increased availability of English-language books facilitated the acquiring and honing of translators' language skills and gradually diminished the need for indirect translating. Book history and translation studies meet here in an examination of the role of the Collection in Finnish translators' work.

The construction and selection of a suitable bijective function, that is, substitution, is now becoming an important applied task, particularly for building block encryption systems. Many articles have suggested using different approaches to determining the quality of substitution, but most of them are highly computationally complex. The solution of this problem will significantly expand the range of methods for constructing and analyzing scheme in information protection systems. The purpose of research is to find easily measurable characteristics of substitutions, allowing to evaluate their quality, and also measures of the proximity of a particular substitutions to a random one, or its distance from it. For this purpose, several characteristics were proposed in this work: difference and polynomial, and their mathematical expectation was found, as well as variance for the difference characteristic. This allows us to make a conclusion about its quality by comparing the result of calculating the characteristic for a particular substitution with the calculated mathematical expectation. From a computational point of view, the thesises of the article are of exceptional interest due to the simplicity of the algorithm for quantifying the quality of bijective function substitutions. By its nature, the operation of calculating the difference characteristic carries out a simple summation of integer terms in a fixed and small range. Such an operation, both in the modern and in the prospective element base, is embedded in the logic of a wide range of functional elements, especially when implementing computational actions in the optical range, or on other carriers related to the field of nanotechnology.
