Background: Minimum Service Standards have set up at Panti Wilasa Hospital Dr. Cipto Semarang that the standard time of outpatient services is 10 minutes which is calculated from the registration process. From the data obtained by the researcher from the assignment in carrying out the Minimal Standard of Service in serving the outpatients has been fully implemented. However, there are still healthcare service workers who do not understand the Minimum Service Standards when interviewing healthcare service  officers in the Outpatient Registration Place section.Methods: This type of research is descriptive research that gives an overview of the data as a result of research. The method used is interviewing the outpatient registration officer and filing. Sampling technique in this study using random sampling data with a sample size of 30 person.Results: There is no specific Standard Operating Procedure (SOP) regarding the provision of outpatient medical records which were interviewed to the head of the medical records, which is already quite a good service at the outpatient registration place Dr. Panti Wilasa Hospital Cipto Semarang and Standard Operating Procedure (SOP) for Minimum Service Standards (SPM) in outpatient registration conducted interviews with the head of the medical record at Panti Wilasa Hospital Dr. Cipto Semarang. Results of the study showed that the average rapidity in medical record document provision was 31 minutes 20 seconds. At the Wilasa Panti Hospital Dr. Cipto Semarang, that the time for providing more medical records is more than ten minutes.Conclusions: The standard time for providing medical records is included in the Standard Operating Procedure (SOP) for the collection and compilation of medical records on a storage rack, so that medical records officers can work on the basis and rules that have been determined and the standard time for providing medical records is included in the quality objectives, The results are announced and written in a place easily seen by officers, Monitoring active medical record storage racks regularly by medical records officers and this can reduce the wrong medical record documents.

Medical records are an important part of the treatment of patient health, one form of service in each public health facilities is the distribution of medical record files. Based on research conducted at Hospital X Bandung, in the distribution of medical records the system used has not fully used electronic, the data entered into the application will then be searched manually by the officer for further medical record files distributed to each polyclinic concerned. The purpose of this study is to find out how the distribution of outpatient medical records at Hospital X Bandung using descriptive qualitative research methods that are research that aims to explain and describe the on distributing outpatient medical record documents with subjects in this study is an outpatient medical record document at Hospital X Bandung while the object in this study is the medical record officer in the distribution and filing. Thus, it can be concluded that the implementation of the distribution of medical record files has been quite effective and in accordance with the standard of time that has been set and Standard Operating Procedure that has been made despite some problems and constraints that always occur at the time of distribution of medical records, one of them is the application system used errors or buffering, the code on each polyclinic is sometimes confused with each other, but the officers can solve the problem.

Background: The hospital is a health service institution that plays a role in efforts to improve the degree of public health. One of the important roles in a hospital is the medical recorder installation. One of the minimum service standards for medical records at a hospital is the length of time to provide medical records for outpatient services, which is less than 10 minutes. The purpose of this study was to determine the length of time and to analyze the factors causing the length of time for the provision of medical records for inpatients at Bhayangkara Hospital in 2019.Methods: This research is a combination of research with a sequential explanatory design conducted at the Bhayangkara Hospital in Padang in November 2018 to June 2019. In quantitative research, the sampling technique was by means of the accidental sampling technique and descriptive analysis. While the qualitative research techniques for determining informants were used by purposive sampling and analysis by using content analysis techniques.Results: Quantitative research results, obtained an average time for providing medical records for outpatients is 9 minutes 6 seconds. The qualitative research results obtained that the input of the implementation of the medical record service system is not optimal and the process of implementing the medical record is also not well implemented.Conclusions: The time for providing outpatient medical records is fast according to the minimum hospital medical record service standard, but the implementation is not in accordance with standard operating procedures.

<p>Nosocomial infection is one of the common infections that occur in hospitals and other health facilities, which is caused by a mismatch in the implementation of invasive measures such as the intravenous access with the applicable Standard Operating Procedure (SOP). This study aimed to determine the description of the implementation of the SOP of the intravenous access in one private hospital in central Indonesia. The study used a quantitative descriptive design with a total population of 10 nurses using total sampling technique. The study used a checklist sheet which was adapted from the SOP of the hospital where the study was conducted. The results showed all respondents did not carry out the procedure of intravenous access in accordance with the applicable SOP at the hospital. There were nine from 15 steps of implementation of the SOP of the intravenous access do not reach 100%. The hospital is expected to make efforts to improve nurses' compliance in taking action in accordance with SOP, such as forming an audit team to improve compliance with SOP implementation, periodic internal audits, providing facilities for hand-washing (paper-towels, hand-rub, soap for hand-washing), provide adequate gloves, and disinfection equipment. In addition, it is necessary to refresh information regularly about SOP of invasive actions.</p><p><strong>BAHASA INDONESIA </strong>Infeksi nosokomial merupakan salah satu infeksi umum yang terjadi di rumah sakit dan fasilitas kesehatan lainnya yang disebabkan oleh ketidaksesuaian pelaksanaan tindakan invasif seperti pemasangan akses intravena dengan Standar Prosedur Operasional (SPO) yang berlaku. Penelitian ini bertujuan untuk mengetahui gambaran pelaksanaan SPO pemasangan akses intravena di salah satu rumah sakit swasta di Indonesia bagian tengah. Desain penelitian menggunakan desain deskriptif kuantitatif dengan populasi seluruh perawat ruangan yang berjumlah 10 orang dengan teknik total sampling. Instrumen menggunakan lembar checklist yang diadaptasi dari SPO rumah sakit tempat penelitian dilakukan. Hasil penelitian menunjukkan seluruh responden tidak melaksanakan prosedur pemasangan akses intravena sesuai dengan SPO yang berlaku di rumah sakit. Ada sembilan dari 15 langkah pemasangan akses intravena yang tidak mencapai 100%. Pihak rumah sakit diharapkan dapat melakukan upaya untuk meningkatkan kepatuhan perawat dalam melakukan tindakan sesuai dengan SPO, seperti pembentukan tim audit guna meningkatkan kepatuhan pelaksanaan SPO, audit internal berkala, menyediakan fasilitas untuk mencuci tangan (paper-towels, hand-rub, sabun untuk mencuci tangan), menyediakan sarung tangan yang memadai, dan alat desinfeksi. Selain itu perlu diadakan penyegaran informasi secara berkala mengenai SPO tindakan invasif.</p>

The provision of medical record files depends on the availability of data, clear and accurate information. The speed of providing medical record files is one indicator of the quality of service in medical records. The purpose of the study was to determine the achievement of the Minimum Service Standards in the medical record unit in providing medical record files. The research method uses descriptive methods with a qualitative approach. Data collection was carried out for 5 days by recording the hours the patient registered in the outpatient registration section until the time the medical record file was found, located at the hospital. Dustira Cimahi. The result is as many as 2090 or 86,1% of medical record files with a provisioning time of ? 10 minutes, 340 or 13,9% of medical record files requiring a provisioning time of> 10 minutes. The conclusion is based on the provision of medical record files at the hospital. Dustira has met the minimum service standards with the set waiting time standards for outpatient services which is an average of <60 minutes, it is recommended to maintain and improve the quality of service.

The medical record is also a documentation of the patient's condition and the contents of the medical record are medical secrets, especially in the storage of status in the filling room, which must be kept confidential, secure, and the medical record layout, whether it is appropriate or not by every health worker. The purpose of this study was to determine the Knowledge Review and Attitudes of Officers and Patients on the Confidentiality of Medical Record Files Primary Health Center at Kuranji Padang. This type of research is descriptive with a quantitative approach, the research sample is 90 patients and 4 health center officers using accidental sampling technique with data collection using questionnaires for patients and table cklis for officers. The results showed that there are still officers whose knowledge and attitude in maintaining the confidentiality of medical records (50%) or 2 people are still lacking and more than half of the knowledge (51.1%) and attitudes (52.2%) of patients towards maintaining the confidentiality of record files medical who do not understand and know. It can be concluded that Kuranji Primary Health Center, both patients and officers, is still lacking in maintaining the confidentiality of medical records. It is hoped that the head of the Primary Health Center will prepare Standard Operating Procedures (SOP) on medical records so that officers and patients know about medical records and about the confidentiality of medical records. In implementing the confidentiality of medical records, competent and professional medical record officers should know and understand more about medical records, participate in training and provide information to patients so that the confidentiality of medical records is maintained and can create good service.

The aim of this article is to analyze internal control in service company. The analyze used the components of internal control according to COSO: control environment, risk assessment, control activities, information and communication, monitoring. Analysis of internal control used the data obtained through observation, questionnaires, interview, and documentary. Based on this analysis results can be concluded, that the dual positions refer with company SOP (Standard Operating Procedure). Separation of task does not work because that the dual positions. Therefore, the component of control activities applied at company is not in accordance with the component of control activities according to COSO. Nonconformity components still results in very effective internal control because the purpose of the internal control still remains to be achieved even though there are components that are not in accordance with the internal control components according to COSO.
