The medical record is also a documentation of the patient's condition and the contents of the medical record are medical secrets, especially in the storage of status in the filling room, which must be kept confidential, secure, and the medical record layout, whether it is appropriate or not by every health worker. The purpose of this study was to determine the Knowledge Review and Attitudes of Officers and Patients on the Confidentiality of Medical Record Files Primary Health Center at Kuranji Padang. This type of research is descriptive with a quantitative approach, the research sample is 90 patients and 4 health center officers using accidental sampling technique with data collection using questionnaires for patients and table cklis for officers. The results showed that there are still officers whose knowledge and attitude in maintaining the confidentiality of medical records (50%) or 2 people are still lacking and more than half of the knowledge (51.1%) and attitudes (52.2%) of patients towards maintaining the confidentiality of record files medical who do not understand and know. It can be concluded that Kuranji Primary Health Center, both patients and officers, is still lacking in maintaining the confidentiality of medical records. It is hoped that the head of the Primary Health Center will prepare Standard Operating Procedures (SOP) on medical records so that officers and patients know about medical records and about the confidentiality of medical records. In implementing the confidentiality of medical records, competent and professional medical record officers should know and understand more about medical records, participate in training and provide information to patients so that the confidentiality of medical records is maintained and can create good service.

In returning the outpatient medical record file to thesection of the Medical Record Unit, Assembling there was a delay. This is because the return of medical record files has not been carried out according to Standard Operating Procedures, as a result, causing delays in the reporting system. The method used is qualitative using a descriptive approach. Data collection techniques by observation, interviews and literature study. This study aims to determine the return of former medical records of outpatients at Dayeuhkolot Health Center. The results of the study prove that the delay in returning outpatient medical record files at the Dayeuhkolot Health Center with presentations during the 1 week study amounted to 63 or 22% of 285 medical record files. Efforts have been made to disseminate information to all officers related to the efforts made by the person in charge of COVID-19 patients in returning medical record files on time.

HIV/AIDS cases in Indonesia are still high including the infection from mothers to the babies. Infection cases from mothers to babies in Yogyakarta Special Province is also found on babies. The preliminary study also obtained the result that in 2015 not all of the pregnant women who were willing to have VCT (Voluntary Counseling and Testing) in Sleman Regency working area. The data of VCT  in Prambanan Primary Health Center also shows that the prevalence rate of VCT check as a preventive effort was still low. The research is aimed at investigating the correlation between midwife’s support and VCT on pregnant women in Prambanan Primary Health Center in 2016. The study employed observational analytic with a cross-sectional approach. The population of the study was all mothers who did pregnancy examination and got counseling VCT service. The samples used accidental sampling technique. Data analysis used chi-square with the trustworthy rate 95%. The result of the study showed that there was no correlation between midwife and VCT on pregnant women. It can happen because of the midwife as a professional attendant could not attend the pregnant women full time. It can be concluded that there was no correlation between midwife’s support and VCT on pregnant women in Prambanan Primary Health Center in 2016. It is expected that midwives can identify the problem in VCT on pregnant women by giving emotional and respect support to the patients as well as the families.

Compliance of midwife primary health center in implementation of the antenatal care integrated quality, disease detection and complications in pregnant womenPurpose This research aimed to determine the relationship of midwife’s compliance in the implementation of the antenatal care (ANC) integrated quality with disease detection and complications in pregnant women.MethodsThis research was an observational study with a retrospective cohort design. The sample was midwives who serve pregnant women in a primary health center, involving 125 midwives. The place of research was in 21 health centers throughout Kulon Progo. Research instruments used a questionnaire, with total sampling. Data analysis used: univariable analysis to determine the distribution of the data, bivariable with chi-square tests, multivariable with linear regression tests with 95% CI and significance of p=0.05, and a qualitative analysis.ResultsMidwives' compliance to the standard operating procedure has a 35% greater chance of detecting complications and illness in pregnant women than the midwife who did not follow standard operating procedure. Not all midwives in primary health care comply with ANC integrated quality.ConclusionThis study contributes to an understanding that compliance of midwives to ANC integrated quality is important as an effort to detect complications in pregnant women. The health office should establish a team for socialization, supervision, and assistance in the implementation of ANC integrated quality.

Decreasing child mortality is still a target of health development in Indonesia. One effort is to increase knowledge through health promotion. Giving promotion is effective and optimal by using of media such as flash cards. Flash card is a picture card media with information on each picture. The purpose of this study is to measure the effectiveness of flashcard information media on the knowledge of health workers in primary health center. This type of research was Quasi Experimental with pre-post test in one group (One-Group Pre-test-posttest Design). The population of this research was 28 health workers in Ungaran Health Center and Lerep Health Center. Data collection tool in the form of a questionnaire and media information in the form of a flashcard, with the sampling technique was total sampling. Statistical test used Wilcoxon test.Conclusion was The results showed there were significant differences in the knowledge of health workers after counseling using a flashcard with a p value of 0,001 < α (0,05). The average value of knowledge before the intervention was 38,61 and thereafter increased become 42,79. The conclusion was health education using flashcard information media is effective to increase the knowledge of health workers about IMCI in primary health center

Abstract Pregnant women class is a learning facility for pregnant women in undergoing the process of pregnancy and postpartum. Based on the preliminary survey conducted by researchers in the work area of ​​Payung Sekaki Primary Health Center, it was obtained data that the narration of midwives working in the work area of ​Payung Sekaki Primary Health Center was indeed carried out, but there were still many pregnant women who had not utilized maternal facilities optimally.  The purpose of this study is the Factors Associated with the Participation of Pregnant Women in the Work Area Payung Sekaki Primary Health Center 2018. This type of research is quantitative using correlation analysis with Chi-square. The population in the study amounted to 297 people, with a total sample of 75 people with accidental sampling technique. This research was carried out in the work area of ​​Payung Sekaki Primary Health Center Pekanbaru on 05 to 28 July 2018. The results showed that there was a relationship between knowledge and maternal participation in the class of pregnant women in the work area of Payung Sekaki Primary Health Center with a p-value = 0,000, there was a relationship between parity with the participation of pregnant women in the Working Area of ​​Payung Sekaki  Primary Health Center  with a p-value = 0.025 and there is a relationship of family support with the participation of pregnant women in the work area of Payung Sekaki Primary Health Center with a p-value = 0.03 with a degree of Error α = 0,1. Aim: The research aims to identify maternal characteristics and anemia in third trimester pregnant women at Mantrijeron Primary Health Center of Yogyakarta. Methods: The study employed a correlation design and cross-sectional approach. The sampling technique in this study used the purposive sampling method with the number of samples was 69 people—the data analysis used using chi-square analysis. Results: This study shows correlations between education level, parity, maternal age, and the frequency of ANC visits as indicated by p-value (<0.05) and the incidence of anemia in the third trimester of pregnant women Mantrijeron Primary Health Center of Yogyakarta. Meanwhile, those with no correlation were nutritional status as indicated by p-value (0.389> 0.05). Conclusion: Pregnant women are suggested to have pregnancy checks regularly at least four times during pregnancy so that complications, such as anemia, can be indicated earlier and can be minimized. Abstrak Data Puskesmas Duren menunjukkan jumlah kunjungan Voluntary Counseling and Testing  (VCT) mengalami penurunan dalam tiga tahun dari 2015 hingga 2017. Pada tahun 2017 adanya kesenjangan antara jumlah kelompok wanita pekerja seksual dan lelaki suka lelaki yang ada di Kecamatan Bandungan dengan jumlah kunjungan VCT pada populasi kunci tersebut serta belum mencapai target yang ditetapkan oleh Dinas Kesehatan Kabupaten Semarang. Tujuan penelitian ini untuk mengetahui gambaran program pelayanan VCT di Puskesmas Duren. Penelitian ini dilakukan pada bulan Mei 2018. Jenis penelitian ini adalah deskriptif kualitatif. Informan penelitian terdiri dari 11 dipilih dengan teknik purposive sampling. Teknik pengumpulan data menggunakan wawancara dan observasi. Hasil penelitian menunjukkan kurangnya ketersediaan sarana dan prasarana serta sumber daya manusia di Puskesmas Duren. Sudah tersedianya dana dan standar operasional prosedur pelayanan VCT. Kegiatan sosialisasi layanan VCT sudah dilakukan. Pelaksanaan VCT belum sesuai dengan pedoman serta jumlah kunjungan VCT mengalami penurunan dalam tiga tahun terakhir dan belum mencapai target yang telah ditetapkan. Simpulan penelitian ini adalah gambaran program pelayanan VCT di Puskesmas Duren belum berjalan baik.   Abstract Data from Duren Primary Health Center showed for the number of VCT visited decreased in three years from 2015 to year 2017. In 2017 there was gap between the number of women sex worker groups and men who were in Bandungan with their number of VCT visited. The purpose of this research was to know the description of VCT program in Duren Public Health Center. This research was conducted in May 2018. This type of research was descriptive qualitative. Research informants consisted of 11 selected by purposive sampling technique. Data collection techniques using interviews and observation. The results showed the lack of availability of facilities and infrastructure and human resources. Availability of funds and standard operational procedures were sufficiented. Socialization of VCT have been carried out. VCT implementation has not been in accordance with guidelines, the number of VCT visited has decreased in the last three years, and has not reached the targets. The conclusions of this study was the description of the VCT program have not gone well.

Microsoft Access (also known as Access) is an application used to process databases. The implementation of storage at the Primary Health Center is still manual if the patient's medical record file is not found on the storage shelf, the officer will make a temporary file which will cause the accumulation of files on the shelf. There is still a misplaced medical record file (Missfile) due to the absence of a tracer being stored as a control for releasing medical record files and borrowing medical record files. The purpose of this research is how to design a tracer using Microsoft Access in the medical record unit at Primary Health Center, Padang. This research is about designing a tracer in the medical record unit at Primary Health Center in Padang. This type of research is Research and Development, conducting in-depth interviews, observation, and testing at the storage area and interviews with 1 head of administration, 1 head of medical records, and 2 storage officers. This research was conducted from 25 July to 2 August 2019 at Padang Primary Health Center. Based on the research results, it was found that the identification of tracer data elements in a computerized manner was more complete in implementing the application by developing the UML (Unified Modeling Language) method, namely by using use case diagrams and activity diagrams, as well as data flow diagrams. Efficiency is obtained by means of manually, 1 and computerized 3.5 so that the tracer efficiency is more computerized while the effectiveness is in accordance with what the officers expect. The conclusion in this study found that the officers were satisfied and the application was as expected because it was easy to use. Judging from the test results, this tracer application can be used, seen from the expectations and satisfaction of the officers, the data is appropriate and can improve performance, effectiveness, and efficiency in service. Therefore, the researchers hope that this application can be implemented in services and developed again in the future.

Background: Exclusive breastfeeding is giving breast milk exclusively from newborns to 6 months of age without giving any food. Exclusive breastfeeding is essential for optimal baby growth and development, contains various essential nutrients for babies. In 2018 the coverage of exclusive breastfeeding in Indonesia still reached 65.16%, and the coverage of breastfeeding in East Java itself was still below the target of 76.98%. The low level of exclusive breastfeeding is still a problem because it can cause the baby's immune system to become lower, indigestion and illness risk in the baby is more generous to target.Objective: To determine the relationship between mothers' level of knowledge about breastfeeding and exclusive breastfeeding at Kenjeran Primary Health Center.Methods: This observational analytic study with a cross-sectional study design. The sampling technique was using the purposive sampling technique. Respondents in this study were mothers who had babies 6-24 months. The data collection process was directly asked through a questionnaire and analyzed using the contingency coefficient test.Results: This study involved 51 mothers who gave exclusive breastfeeding and 51 mothers who did not. The results showed that most of the respondents who breastfed and did not have the right knowledge level. Based on the contingency coefficient test results, it was found that there was no relationship between the level of mother's knowledge about breast milk to give exclusive breastfeeding at the Kenjeran Primary Health Center (the value of p = 0.251).Conclusion: There is no relationship between mothers' level of knowledge about breastfeeding and exclusive breastfeeding at the Kenjeran Primary Health Center.Background: Exclusive breastfeeding is giving breast milk exclusively from newborns to 6 months of age without giving any food. Background: Minimum Service Standards have set up at Panti Wilasa Hospital Dr. Cipto Semarang that the standard time of outpatient services is 10 minutes which is calculated from the registration process. From the data obtained by the researcher from the assignment in carrying out the Minimal Standard of Service in serving the outpatients has been fully implemented. However, there are still healthcare service workers who do not understand the Minimum Service Standards when interviewing healthcare service  officers in the Outpatient Registration Place section.Methods: This type of research is descriptive research that gives an overview of the data as a result of research. The method used is interviewing the outpatient registration officer and filing. Sampling technique in this study using random sampling data with a sample size of 30 person.Results: There is no specific Standard Operating Procedure (SOP) regarding the provision of outpatient medical records which were interviewed to the head of the medical records, which is already quite a good service at the outpatient registration place Dr. Panti Wilasa Hospital Cipto Semarang and Standard Operating Procedure (SOP) for Minimum Service Standards (SPM) in outpatient registration conducted interviews with the head of the medical record at Panti Wilasa Hospital Dr. Cipto Semarang. Results of the study showed that the average rapidity in medical record document provision was 31 minutes 20 seconds. At the Wilasa Panti Hospital Dr. Cipto Semarang, that the time for providing more medical records is more than ten minutes.Conclusions: The standard time for providing medical records is included in the Standard Operating Procedure (SOP) for the collection and compilation of medical records on a storage rack, so that medical records officers can work on the basis and rules that have been determined and the standard time for providing medical records is included in the quality objectives, The results are announced and written in a place easily seen by officers, Monitoring active medical record storage racks regularly by medical records officers and this can reduce the wrong medical record documents.
