Smokers’ Melanosis in a Nigerian Population: A Preliminary Study

Abstract Aim Terms relating to pigmentation of the oral mucosa include physiologic (racial) pigmentation, oral pigmented nevi, oral melanotic maculae, melanoma, and smokers’ melanosis. The literature is replete with studies about oral mucosal pigmentation which is thought to result from melanin incontinence. The documented etiological factors are both local and systemic and include hormones, drugs, smoking, and idiopathic causes. This study investigated the prevalence of melanosis among Nigerian smokers and controls who were non-smokers. Methods and Materials A total of 1270 sites were examined in 253 subjects consisting of 60 smokers and 193 non-smokers. They were all systemically healthy adults drawn from 12 factories located in different urban and rural settings in the state of Lagos in Nigeria. Five oral mucosal sites were examined per subject. A single examiner performed all examinations and recorded all findings. Pigmentation was scored either as “present” or “absent.” Subjects’ smoking status, degree, and duration of smoking were ascertained and recorded using an examiner-administered questionnaire. Results There were five pigmented sites (0.52%) among non-smokers and 18 (6%) among smokers. The buccal mucosa was the most frequently pigmented site found in smokers while the lingual mucosa was the most common site found for non-smokers. The prevalence of pigmented sites increased directly among smokers with the duration of smoking (years); degree of smoking (cigarettes smoked per day); and smoking pack-years (degree of smoking divided by 20 and multiplied by duration of smoking, where 20 is the average number of cigarettes in a pack of cigarettes). SPSS version 11.0 was used for data entry and analysis. Frequency distributions were generated for all categorical variables for descriptive aspects of the analysis. Means were determined for quantitative variables such as age and number of cigarettes smoked. For homogenous variances, the student's t test was used for quantitative variables between smokers and nonsmokers, while for non-homogenous variances the Mann-Whitney test was adopted. Chi-square statistic was used for comparisons between smokers and non-smokers. In tables with low expected frequencies, Fisher's exact test was adopted. Statistical tests yielding p-values =/<0.5 were considered significant. Conclusions Smokers in this study had a significantly higher prevalence of pigmented oral mucosal sites (melanosis) than non-smokers. The number of pigmented sites increased with the degree and duration of smoking. The buccal mucosa was the most frequently pigmented site found among the smokers in this study. FACTORS THAT INFLUENCE HIV ASSOCIATED PERIPHERAL NEUROPATHY IN MANADOABSTRACTIntroduction: There are  several sociodemographic and clinical factors that influence the development of HIV associated peripheral  neuropathy (HIV-PN). Manado has different sociodemographic and clinical characteristics  from other regions. However, the percentage of HIV-PN and its influencing factors are unknown.Aims: To know the percentage of HIV-PN and factors that influence this disorder in Manado.Methods: A crosssectional study conducted in HIV/AIDS clinic R.D. Kandou hospital Manado between November2016–January 2017. Neuropathy evaluation was performed using brief peripheral neuropathy screening (BPNS), neuropathic pain diagnostic questionnaire (DN4), and electroneurography. A Chi-square or Fisher exact test was done to analyze categorical variables, independent T or Mann-Whitney test was done for numerical variables, and linear regression was done in multivariate analysis.Results: 50 subjects were included, most were male (70%), and the mean age was 32.98 (±9.726) years, with HIV- NP percentage was 46%. Age >30 years old, low hemoglobin count, CD4, and low international  HIV dementia scale (IHDS) significantly associated with HIV-NP. Working subjects were 13.6 times more likely to have HIV-PN.Discussion: HIV-PN prevalence was relatively high, influenced by age >30 years old an low hemoglobin, CD4, andIHDS. Manado mempunyai karakteristik yang berbeda dengan daerah lain, namun belum diketahui persentase NP-HIV dan faktor-faktor yang memengaruhinya.Tujuan: Mengetahui persentase NP-HIV dan faktor-faktor yang memengaruhinya di Manado.Metode: Penelitian potong lintang terhadap pasien HIV/AIDS di poliklinik HIV/AIDS RSUP Prof. dr. R.D. Kandou, Manado, pada November 2016–Januari 2017.  Evaluasi neuropati dilakukan menggunakan brief peripheral  neuropathy screening (BPNS), neuropathic pain diagnostic questionnaire (DN4), dan elektroneurografi. Digunakan uji Chi-square atau Fisher exact untuk menganalisis variabel kategorik, uji T independen atau Mann-Whitney untuk variabel numerik, dan regresi linear untuk analisis multivariat.Hasil: Didapatkan 50 subjek yang memenuhi kriteria penelitian dengan mayoritas laki-laki (70%), rerata usia32,98±9,726 tahun, dan mengalami NP-HIV sebanyak 46%. Usia >30 tahun, kadar hemoglobin, jumlah CD4, dan skor international  HIV dementia scale (IHDS) yang rendah berhubungan secara bermakna dengan adanya NP-HIV. Adapun subjek yang bekerja berisiko 13,6 kali lebih besar mengalami NP-HIV.Diskusi: Didapatkan prevalensi NP-HIV yang cukup tinggi dengan dipengaruhi oleh usia >30 tahun serta kadar hemoglobin, CD4, dan skor IHDS yang rendah. This study aims to assess the risk of severe forms of COVID-19, based on clinical, laboratory, and imaging markers in patients initially admitted to the ward. This is a retrospective observational study, with data from electronic medical records of inpatients, with laboratory confirmation of COVID-19, between March and September 2020, in a hospital from Juiz de Fora-MG, Brazil. Participants (n = 74) were separated into two groups by clinical evolution: those who remained in the ward and those who progressed to the ICU. Mann–Whitney U test was taken for continuous variables and the chi-square test or Fisher’s exact test for categorical variables. Comparing the proposed groups, lower values of lymphocytes ( p  = <0.001) and increases in serum creatinine ( p  = 0.009), LDH ( p  = 0.057), troponin ( p  = 0.018), IL-6 ( p  = 0.053), complement C4 ( p  = 0.040), and CRP ( p  = 0.053) showed significant differences or statistical tendency for clinical deterioration. The average age of the groups was 47.9 ± 16.5 and 66.5 ± 7.3 years ( p  = 0.001). Hypertension ( p  = 0.064), heart disease ( p  = 0.048), and COPD ( p  = 0.039) were more linked to ICU admission, as well as the presence of tachypnea on admission ( p  = 0.051). Ground-glass involvement >25% of the lung parenchyma or pleural effusion on chest CT showed association with evolution to ICU ( p  = 0.027), as well as bilateral opacifications ( p  = 0.030) when compared to unilateral ones. Laboratory, clinical, and imaging markers may have significant relation with worse outcomes and the need for intensive treatment, being helpful as predictive factors.

Background: Sexual assault is a global issue wherein timely examination of sexual assault victims by medico-legal officers and an understanding of the characteristics and factors influencing their injuries have a crucial impact on medico-legal evidence collection and its use in the court. Genital injury findings improve the level of justice for victims, but the lack of injury does not negate the possibility of sexual violence incurred on the victim. The following study describes in great detail the different factors associated with injury characteristics and the medico-legal importance of all these factors. Methodology: It was a retrospective descriptive study conducted at the Forensic Medicine Department of King Edward Medical University, Lahore, Pakistan. All the cases included that fulfill the predefined inclusion criteria were included. A record chart was designed, and six years’ data was recorded in it from the medical records of the medico-legal clinic. It was entered and edited manually. Methods used to prevent data entry errors included double entry and validation following data entry Difference of proportions between variables, where appropriate, has been calculated by Pearson Chi-Square test and Fischer Exact test at 95% confidence interval. Data were analyzed using SPSS v23. All procedures performed in this study involving human participants were following the ethical standards of the institutional and/or national research committee and the Helsinki declaration. Results: Genito anal injury was present in 28.2% cases and was absent in 71.8% cases. Most of the victims were aged 11 to 15 years. The most common injury in the victims was a tear. In females, the most common injury site was the vagina, while the perianal area was the most injured site in males. There was a significant relation (p<0.05) between type and site of injury, gender, site of injury, and prior sexual intercourse experience and prevalence rate of injury. Conclusion: This study concludes that the presence of injuries in the Genito anal area is suggestive of sexual assault and provides useful court evidence.

Background To compare the main outcomes of trauma patients with and without traumatic brain injury (TBI), hemorrhagic shock, and the combination of both using data from the Spanish trauma intensive care unit (ICU) registry (RETRAUCI). Methods Patients admitted to the participating ICUs from March 2015 to May 2019 were included in the study. The main outcomes were analyzed according to the presence of TBI, hemorrhagic shock, and/or both. Comparison of groups with quantitative variables was performed using the Kruskal-Wallis test, and differences between groups with categorical variables were compared using the Chi-square test or Fisher’s exact test as appropriate. A P value <.05 was considered significant. Results Overall, 310 patients (3.98%) were presented with TBI and hemorrhagic shock. Patients with TBI and hemorrhagic shock received more red blood cell (RBC) concentrates, fresh frozen plasma (FFP), a higher ratio FFP/RBC, and had a higher incidence of trauma-induced coagulopathy (60%) ( P < .001). These patients had higher mortality ( P < .001). Intracranial hypertension was the leading cause of death (50.4%). Conclusions Concomitant TBI and hemorrhagic shock occur in nearly 4% of trauma ICU patients. These patients required a higher amount of RBC concentrates and FFP and had an increased mortality.

Background: A suspected peripheral neuropathy outbreak was reported from Dechentsemo Central School, Thinleygang, Punakha, following which the investigation team was immediately dispatched in the field.Objective: The aim of investigation was to ascertain the cause and risk factor for the outbreak in order to implement control measures.Methods: A case control study was devised for the investigation to study about the past exposure or deficiencies in order to find out the suspected cause and risk factors.  A semi-quantitative food frequency questionnaire was administered to both cases and controls to collect information on the type of food they have consumed. The information garnered was analyzed using Chi-Square or Fischer Exact test for categorical variables and Man-Whitney U-test for quantitative variables. Results: All 17 cases were females with mean age of 13 years (SD 2.7 years). The average daily amount of thiamine intake was 0.6 mg/day for case and 0.8 mg/day for controls against the recommended daily allowance (RDA) of 1.2 mg/day. Case and control patients differed significantly with respect to fat intake (p-value = 0.02), more strongly with folate and iron intake (p-value 0.01).Conclusion: The outbreak of peripheral neuropathy in Dechentsemo Central School appears to be linked to reduced dietary intake rich in vitamin B1 coupled with low intake of folate and iron in their diet.

Abstract Background: For over ten years, dental cellulitis management is not so longer simple: more and more patients need long-time hospitalization, several surgical interventions and intensive care follow-up. This prospective study seeks to highlight criteria that can split patients with severe odontogenic infection into two main groups: those with simple evolution after treatment of the infection and those for whom complex management is necessary to obtain healing.Methods: All patients with severe odontogenic infection (namely patients which necessitated hospital admission, intravenous antibiotherapy and general anesthesia) were prospectively recorded between January 2004 and December 2014. Statistical tests used were: chi-square test (or Fisher’s exact test when appropriate) for categorical data, and Student t-test (or Mann & Withney test when appropriate) for continuous data; statistics were computed with STATA V12 (Stata Corp, College Station, Texas, USA) and with R (R version 3.0.2; The R Foundation for Statistical Computing, Vienna, Austria; http://www.r-project.org).Results: A total of 653 patients were included in the study, 611 (93.6%) had one surgery, 42 (6.4%) had more than one surgery (range 2 – 15 surgeries) before healing. In multivariate analysis, objectives criteria which emerge are: C-reactive protein (CRP) level (CRP>200 mg/l, p=0.01, OR=4.12 IC95%=[1.33-12.72]), alcohol abuse (p=0.03, OR=2.70 IC95%=[1.09-6.7]), penicillin allergy (p=0.001, OR=5.47 IC95%=[1.99-15-09]), mandibular molar infection (p= 0.02, OR=2.74 IC95%=[1.16-6.48]). A conditional inference tree (CTREE) illustrated the association of prognostic factors and the need of multiple surgery. Conclusion: Four subgroups of patients with severe odontogenic infection have a relative risk range between 25 % and 33% to have several surgical intervention associated with complications as intensive unit care follow-up, tracheotomy or death: 1- patients with CRP>200 mg/l; 2- patients with CRP between 50 and 200 mg/l and penicillin allergy; 3- patients with CRP between 50 and 200 mg/l, without penicillin allergy but with molar mandibular infection and psychiatric disorders; 4- patients with CRP ≤ 50 mg/l or unknown CRP level and immunodepression.IRB number: CE-CIC-GREN-12-08

BackgroundSouth Africa is one of the few countries to produce emergency care graduates in Africa. However, a large number of these graduates are emigrating from Africa. Although academic success has been studied in various contexts in Africa, none have looked into the Bachelor of Emergency Medical Care (BEMC). Maximising academic success in the BEMC might ensure a sustainable skilled workforce in an area which has a shortage of skilled personnel.AimsThis study aimed to describe the socio-demographic variables of current South African BEMC students. We also aimed to describe whether any associations exist between socio-demographic variables, various racial groupings and repeating a year during the course of study.MethodsA cross-sectional descriptive survey was conducted among students enrolled for the BEMC in 2016. Continuous and categorical variables were analysed using descriptive statistics. The Fisher’s exact test and Pearson chi-square test were used to test associations between the survey variables and repeating a year.ResultsA total of 176 participants responded to the survey. Having a pre-existing emergency care qualification was associated with not repeating a year on the BEMC (p=0.02). The statistical difference between race and not repeating a year in the BEMC was not significant (p=0.07). However, when the black-African and minority cohort are grouped together, it is significant (p=0.05).ConclusionA number of socio-demographics were described in this study. Students with pre-existing emergency care qualifications were least likely to repeat a year.

<p>Leisure-time physical activity (LTPA) is an important factor in promoting and maintaining adequate health status and quality of life in old age. Here we aimed to identify the prevalence and factors associated with different modalities of LTPA among 3,478 older adults (mean age: 72.96 ± 6.01 years; 67.7% female) from seven cities of different regions of Germany. We compared categorical variables (gender, age and family income) between groups using the Chi-square test or Fisher’s exact test. Walking was the most commonly reported activity undertaken by participants (43.4%). Men reported performing a larger number of LTPA modalities than women. Outdoor gymnastics and water gymnastics were the most commonly cited LTPA by women. Younger age and higher income correlated with higher levels of physical activity. Our findings indicate that a physically active behavior is closely related to the living conditions, social roles and physical health of this population. Thus, public policies encouraging LTPA among older adults should take their gender, age and social status, as well as the type of activities to be offered, into consideration.</p>

Abstract: Teeth Brushing Behavior of School Children's Oral Hygiene. Data obtained at SDN 60 Kubu Raya Regency has no UKGS (School Dental Health Business) program. The condition of the teeth beforehand will affect the development of dental health in later adulthood. The causes of dental health problems in children include behavioral factors and attitudes ignoring dental hygiene. The study aims to determine the behavior of brushing teeth against oral hygiene in grade IV elementary school children in SDN 60 Kubu Raya Regency. This research is an analytic type of research. Data is processed in the form of frequency distributions and statistical tests using Chi-Square. The population in this study amounted to 40 people. Samples were taken using a total sampling technique of 40 respondents. Based on the results of research in SDN 60 Kubu Raya Regency that female respondents were 23 (57.5%), the respondents aged 9 years were 18 (45%), based on the technique of brushing teeth respondents were categorized as moderate as 21 (52.5%), respondents brushed at the wrong time 33 (82.5%), according to the frequency of brushing their teeth, they brushed their teeth twice a day as many as 24 (60%), while for OHI-S respondents were of medium and bad category as many as 17 (42.5%). It was concluded that there was an effect of technique, time and frequency of tooth brushing on oral hygiene of grade IV elementary school students of SDN 60, Kuburaya Regency. Abstrak: Perilaku Menyikat Gigi terhadap Oral Hygiene Anak Sekolah. Data yang diperoleh di SDN 60 Kabupaten Kubu Raya belum ada program UKGS (Usaha Kesehatan Gigi Sekolah). Keadaan gigi sebelumnya akan berpengaruh terhadap perkembangan kesehatan gigi pada usia dewasa nanti. Penyebab timbulnya masalah kesehatan gigi pada anak diantaranya adalah faktor perilaku dan sikap mengabaikan kebersihan gigi. Penelitian bertujuan untuk mengetahui perilaku menyikat gigi terhadap oral hygiene anak sekolah dasar kelas IV di SDN 60 Kabupaten Kubu Raya. Penelitian ini merupakan penelitian jenis analitik. Data diolah dalam bentuk distribusi frekuensi dan di uji statistik menggunakan Chi-Square. Populasi dalam penelitian ini berjumlah 40 orang. Sampel diambil menggunakan teknik total sampling yang berjumlah 40 responden. Berdasarkan hasil penelitian di SDN 60 Kabupaten Kubu Raya bahwa responden perempuan sebanyak 23 (57,5%), pada respondenberumur 9 tahun sebanyak 18 (45%), berdasarkan teknik menyikat gigi responden berkategori sedang sebanyak 21 (52,5%), responden menyikat gigi pada waktu yang tidak tepat sebanyak 33 (82,5%), menurut frekuensi menyikat gigi responden menyikat gigi 2 kali sehari sebanyak 24 (60%), untuk OHI-S responden berkategori sedang dan buruk sebanyak 17 (42,5%). Disimpulkan bahwa terdapat pengaruh teknik, waktu dan frekuensi menyikat gigi terhadap oral hygiene anak sekolah dasar kelas IV SDN 60 Kabupaten Kuburaya.

Background: Severe hypertension in pregnancy (SBP ≥160 mmHg &/or DBP ≥ 110 mmHg) must be treated judiciously to prevent maternal and fetal complications. The study was conducted to compare the efficacy, adverse effects, maternal and fetal outcome between Hydralazine and Labetalol which are the most commonly used drugs for the purpose.Methods: In a prospective study, 130 pregnant patients each with severe hypertension presenting to SMGS Hospital Jammu were randomized in 2 groups and administered hydralazine or labetalol intravenously. The efficacy of the two drugs was measured in terms of number of doses required to obtain target BP and the timing to achieve the same. The incidence of adverse effects, maternal and fetal outcomes were also compared. Comparisons among the two groups was performed by using independent Student’s t test for normally distributed variables, Fishers’ exact test, and Pearson Chi Square test for categorical variables. The level of significance was set at p<0.05 for all analysis.Results: There was no statistical difference between the two drugs in terms of efficacy. Significantly more low-birth weight infants were born in the hydralazine group. Also, the adverse effects were significantly more in the hydralazine group.Conclusions: Both hydralazine and labetalol can be used to treat hypertensive emergencies of pregnancy but hydralazine is associated with more side effects.
