Background: Many studies have been conducted worldwide to evaluate the proportion of patients with Celiac disease (CD) in persons presenting with Irritable Bowel Syndrome (IBS), showing a positive association between the two diseases. However, reports from Pakistan remain scanty. This study aims to find out the prevalence of CD in patients presenting with IBS and to establish the correlation of both diseases. Patients and methods: It was a descriptive cross-sectional study conducted at Combined Military Hospital, Lahore (CMHL) from January 2018 till March 2019. Consecutive, nonprobability sampling was used to include 210 patients of both genders with the age range of 15-65 years fulfilling ROME IV criteria and were not previously diagnosed as CD or CKD or CLD. Demographic data was noted. Serum Anti tTG levels and Duodenal Biopsy from the second part was assessed to diagnose CD. CD was labeled if the disease was present on both serological and Histopathological reports. Data were analyzed by using SPSS version 20. Descriptive statistics were calculated and a Chi-square test was used to compare the outcome variables with P-value < 0.05 as significant. Results: Of 210 enrolled patients, 113 (53, 8%) were male and 97 (46.2%) were female. The mean age of the patients was 28.40 ± 5.06 years. Eight patients (3.8%) were positive for Anti tTg and on histopathological findings suggestive of CD. Seven of 8 were less than 35 years old. Out of 8 positive patients, 6 (75%) were females. However, the p-value was found to be insignificant for age (0.549) and gender (0.096). On stratification with respect to duration of IBS, all 8 patients diagnosed with celiac disease were having symptoms of IBS for less than 12 months (100%) which was significant statistically (7.1% vs 0.0%; p=0.007). Conclusion: The frequency of CD was 3.8% among IBS patients. Considering this percentage of CD in IBS patients s, a high index of suspicion for CD in IBS patients is required,

Background and Objective Pakistan is facing a major brain drain and as long as there is not a better understanding of the needs and desires of the junior doctors this exodus towards the greener pastures shall continue. This study is an effort to recognize the factors which influence the choice of a future specialty of young Pakistani doctors in order to help identify the areas which need improvement. Study design This descriptive cross-sectional study was conducted from 12th May 2021 to 2nd August 2021. Young doctors who had completed their one-year internship in hospitals of Punjab were invited to fill the questionnaire via social media platforms. Data were entered and analyzed using the Statistical Package for Social Sciences (SPSS) version 23.0. Chi-square tests were applied for qualitative variables. A p-value of less than 0.05 was considered significant. Results Out of 105 participants 60 (57.1%) were females. More females as compared to males decided the future specialty based on work-life balance (20/27 p= 0.039). Internal medicine and general surgery were the most sought-after fields both before and after the internship. Forty-three (41%) participants wished to change their choice of specialty after their house job experiences. Fifty-eight (55.2%) participants considered the future prospects of the field while deciding their specialty while sixty-three (60%) reported the attitude of the senior doctors as an important deciding factor. The Covid-19 pandemic had an effect on 12 (11.4%) participants' decision regarding their choice of future specialty. Conclusion Many elements such as internship play a pivotal role in helping young doctors to narrow their choices. It is important to understand the factors considered by young doctors during their choice of a medical specialty to ensure that a significant proportion of the medical workforce does not slip through the cracks in our health infrastructure.

Objective: Cesarean Section (CS) is usually performed when a vaginal delivery would put the baby's or mother's life at risk. Regional anesthesia has the advantage over general anesthesia by allowing mother to remain awake during operation. However, some women prefer general anesthesia as they want to be asleep during the operation. Aim of our study was to see the effect of mild sedation on mothers' satisfaction during CS.Materials and methods: A prospective cross sectional study was undertaken in the department of Anesthesiology, Institute of Child and Mother Health from January 2009 to December 2009. Sixty patients of singletone pregnancy with ASA(American Society of Anesthesiology)-I were randomized into two groups I&II of which one group received sedation with low dose pethidine and diazepam and other group did not get any sedation. Results: Data were analyzed between the groups and within the groups using students'‘t' test and Chi-Square test. A p-value of <0.05 was considered as significant. Patient's parameters like heart rate and blood pressure were not statistically significant between groups, but SpO2 fall was statistically significant. Mother's satisfaction is significantly high (76%) in sedation group than non sedation group. Conclusion: By sedation with low dose pethidine and diazepam, mothers' satisfaction was better achieved without affecting the hemo-dynamic stability. Keywords: sedation; cesarean section; sub-arachnoid block; mothers' satisfaction. DOI: 10.3329/bjog.v24i1.6320 Bangladesh J Obstet Gynaecol, 2009; Vol. 24(1) : 10-13
