<p>「儒醫」同時擁有儒學與醫學的素養是具儒家特質的醫學群體,惟儒學是修身治國的學問,中國醫學則是養生治病的醫術,兩者屬性不同,「儒醫」如何加以融合?本文論證兼具「儒」與「醫」角色的「儒醫」如何在思想層面上融合儒學與中國醫學,首先析論「儒醫」的特質有助於融合儒學與中國醫學,再分別就角色綜合與思想會通的觀點加以論述,認為「儒醫」係透過「仁學」思想融合儒者與醫者的角色;並以「和」作為核心概念,聯結儒學的「中和思想」與中國醫學的「陰陽調和」思維,提出「儒醫」的醫學觀是儒學修養論與中國醫學調養論的相互交融。結論除總結各段,並從「儒醫」視角批判現代醫學發展「醫療商業化」(medical commercialization)與「過度醫療」(excessive medical treatment)的現象。</p> <p>&nbsp;</p><p>The Confucian physician at once embodies the common principle of self-cultivation shared by both the Confucian and medical communities. Both traditions are different; Confucianism being the only kind of political philosophy that emphasizes moral cultivation while Chinese medicine focuses on healing sickness, how may the Confucian physician integrate the two? </p> <p>This paper sets out to prove the critical role of the Confucian physician in philosophically integrating the two traditions.</p> <p>It will first analyze how the characteristics of the Confucian physician make the fusion of Confucianism and Chinese medicine possible, before distinguishing between the two processes of role integration as well as the synthesis of philosophical concepts and thus demonstrating that Confucian physicians call upon &ldquo;theories of benevolence&rdquo; to integrate Confucianism and medicine. With the concept of &ldquo;cohesive harmony&rdquo; at it&rsquo;s core, and by linking Confucianism&rsquo;s concept of &ldquo;Harmonization&rdquo; and Chinese medicine&rsquo;s &quot;Balancing of Yin and Yang&quot;, the Confucian physician&rsquo;s approach to medicine creates synergy between Confucianism&rsquo;s self-cultivation and Chinese Medicine&rsquo;s therapeutic nurturing.</p> <p>The summary will not only provide an overview of the argument, but will also apply the lens of the Confucian physician to critique the joint phenomena of commercialization and excessive treatment found in modern medicine today.</p> <p>&nbsp;</p>

AbstractIn this paper, we analyze the influence of the usual movement variables on the spread of an epidemic. Specifically, given two spatial topologies, we can deduce which topology produces less infected individuals. In particular, we determine the topology that minimizes the overall number of infected individuals. It is worth noting that we do not assume any of the common simplifying assumptions in network theory such as all the links have the same diffusion rate or the movement of the individuals is symmetric. Our main conclusion is that the degree of mobility of the population plays a critical role in the spread of a disease. Finally, we derive theoretical insights to management of epidemics.

Retinitis pigmentosa (RP) represents a clinically and genetically heterogeneous disease characterized by progressive photoreceptor loss. In recent years, research has been rarely made in blood flow affected in RP. The specific mechanism of blood flow affected in RP is not completely clear. A number of studies indicated that the decreased blood flow was related to RP. According to clinical observation and treatment experience, Chinese medicine considered that blood stasis runs throughout the RP disease progression, and the blood stasis corresponding to Chinese herbal medicine has a positive effect on the clinical treatment of RP. Therefore, we proposed that the decreased blood flow may participate in the lesion. In this article, we will review the findings on the decreased blood flow affected in RP from the perspective of modern medicine and Chinese medicine.

Obesity is a global epidemic of modern times and has serious health consequences, as it is a major risk factor for the development of chronic diseases such as hypertension, type 2 diabetes, dyslipidemia, atherosclerosis and its complications. The study of risk factors, pathogenetic mechanisms, as well as the search for new methods of treatment and prevention of this pathology is an important task of modern medicine. Today it is known that the composition and function of the intestinal microbiota play a certain role in the development of obesity and a number of metabolic disorders. One of the areas of prevention of early onset and development of these diseases may be the maintenance of a normal composition and correction of disorders of the intestinal ecosystem. In this review, the currently known interrelationships of changes in the composition of the intestinal microbiota and obesity are reviewed, and studies have been analyzed to assess the composition of the adult intestinal microbiota in obesity. The presently available data on the correction of overweight and obesity by exposing the composition of the intestinal microbiota are presented.

The common cytokine receptor gamma chain (gammac), which is a functional subunit of the receptors for interleukins (IL)-2, -4, -7, -9, and -15, plays an important role in lymphoid development. Inactivation of this molecule in mice leads to abnormal T cell lymphopoiesis characterized by thymic hypoplasia and reduced numbers of peripheral T cells. To determine whether T cell development in the absence of gammac is associated with alterations of intrathymic and peripheral T cell selection, we have analyzed gammac-deficient mice made transgenic for the male-specific T cell receptor (TCR) HY (HY/gammac- mice). In HY/gammac- male mice, negative selection of autoreactive thymocytes was not diminished; however, peripheral T cells expressing transgenic TCR-alpha and -beta chains (TCR-alphaT/betaT) were absent, and extrathymic T cell development was completely abrogated. In HY/gammac- female mice, the expression of the transgenic TCR partially reversed the profound thymic hypoplasia observed in nontransgenic gammac- mice, generating increased numbers of thymocytes in all subsets, particularly the TCR-alphaT/betaT CD8+ single-positive thymocytes. Despite efficient positive selection, however, naive CD8+ TCR-alphaT/betaT T cells were severely reduced in the peripheral lymphoid organs of HY/gammac- female mice. These results not only underscore the indispensible role of gammac in thymocyte development, but also demonstrate the critical role of gammac in the maintenance and/or expansion of peripheral T cell pools.

Venous thromboembolism associated with COVID-19, particularly acute pulmonary embolism, may represent a challenging and complex clinical scenario. The benefits of having a multidisciplinary pulmonary embolism response team (PERT) can be important during such a pandemic. The aim of PERT in the care of such patients is to provide fast, appropriate, multidisciplinary, team-based approach, with the common goal to tailor the best therapeutic decision making, prioritizing always optimal patient care, especially given lack of evidence-based clinical practice guidelines in the setting of COVID-19, which potentially confers a significant prothrombotic state. Herein, we would like to briefly emphasize the importance and potential critical role of PERT in the care of patients in which these two devastating illnesses are present together.

Advertising in mobile apps has recently become one of the most popular advertising channels for businesses when its annual revenue has rapidly increased year over year. On this kind of advertising, the app publishers do not only play a critical role in the ad serving process but also receive a significant portion of the advertising revenue. Their goal of maximizing the revenue sometimes contradict with those of the advertisers. This research conceptualises the role of publishers and proposes an integrated effectiveness framework to further improve the effectiveness of mobile in-app advertising not for one but all participants involved. In specific, this research explores the factors being controlled by publishers and evaluates their impact on the common outcome metric of mobile in-app advertising. An application of the proposed effectiveness framework might help to increase the global mobile in-app advertising revenue significantly higher by balancing the benefits of all participants.

PSORI-CM02 is an optimization formula of PSORI-CM01, which is a clinical herbal formula for the treatment for psoriasis in the Guangdong Provincial Hospital of Chinese Medicine. Previous research indicates that it plays a critical role in anti-inflammation and immunoregulation. Rhizoma smilacis glabrae (RSG) is one herbal medicine of PSORI-CM02, whose effective anti-inflammatory component is astilbin. This study aims to test the anti-inflammatory and immunoregulation effects of astilbin as well as RSG in PSORI-CM02, and we, respectively, used the CuSO4-induced neutrophil-specific transgenic zebrafish model Tg(mpx: EGFP) and the melanin allele mutated Albino strain zebrafish model to visualize the effects of neutrophil recruitment and macrophage phagocytosis. Our data indicated that both PSORI-CM02 and astilbin had anti-inflammatory effects, leading to a reduction in the recruitment of neutrophils and promotion in macrophage phagocytosis. Nevertheless, the negative liquor of Rhizoma smilacis glabrae (PSORI-CM02 without RSG) also had anti-inflammatory and promoting macrophage phagocytosis effects. The results revealed the formula excluding RSG also had anti-inflammatory and immunoregulation effects, which demonstrated that RSG was not the major anti-inflammatory herbal medicine in PSORI-CM02. Similarly, astilbin was not the major anti-inflammatory active ingredient in the formula. The anti-inflammatory and the promotion of macrophage phagocytosis effect of PSORI-CM02 in vivo zebrafish were the results of multiple component interaction, which was the common characteristic of the Chinese medicine compound.
