Selecting the surgical treatment for patients with diaphragmatic hernia

Hernias of the diaphragm (HAP) are a common pathology that negatively affects the patients’ quality of life and in some cases requires high-quality surgical correction. This pathology has a progressive course, which leads to an increase in the severity of clinical manifestations with the age of the patient. At present, behavioral and conservative therapy is the first line of treatment for HAP, while surgical correction is performed only if they are ineffective. This tactic helps to reduce the likelihood of discrediting surgical treatment and increase its effectiveness. The article presents the experience of surgical treatment of HAP on the basis of general surgery clinics of the N.N. Burdenko MMCH of the Ministry of Defense of the Russian Federation.

The high quality of life of the population in rural areas is the key to the safety of the latter. The article discusses the methodology for developing a set of measures to improve the quality of life and testing this technique in a particular locality. To determine possible measures, the current state programs of the Russian Federation, the departmental subprograms included in them, national and federal projects were analyzed. The analysis allowed us to conclude that the main sources of financing of program activities are budgetary funds. Along with them, grant support can be used, as well as funds from investors.

Today, there are several approaches to the treatment of critical lower limb ischemia (KINK). However, despite the high clinical effectiveness achieved, it is not always possible to achieve a high quality of life for the patient after surgery. This article presents modern aspects of surgical treatment of KINK and assesses the quality of life of patients depending on the type of intervention (literature review).

Repeated development of a metachronous tumor in the esophagus resected because of cancer is usually observed in later periods. The period of time between the plasticity of the esophagus and the appearance of tumor can reach several tens of years. Optimal for patients with cancer of their own esophagus after esophagoplasty is early and radical surgical treatment with an individual approach to choosing surgical tactics to achieve the possible high quality of life.

Objectives. T study the quality of life and status of carbohydrate metabolism in patients after surgical treatment insulinoma. Methods: The study involved 20 patients divided in two groups: the first group with a catamnesis duration of up to five years; the second group with a catamnesis duration of more than five years. We studied anthropometric parameters and carbohydrate metabolism as well as psychological questioning of patients using SF-36 questionnaire, the data was considered statistically significant at p0.05. Results. severe combined postoperative complications were more frequent in the first group (63.6% vs. 22.2%, p=0.07), due to extend of the performed surgery. Adrenergic symptoms prior to the surgery were detected in 90.9% of cases in the first group and in 77.7% of cases in the second group. After treatment these numbers decreased to 36.4% and 11.1% respectively (p=0.039 and 0.026). Neuroglycopeniс symptoms before treatment were detected in 90.9% of cases in the first group and for all patients in the second, while after treatment persisted only in 45.5% and 33.3% of cases respectively (p=0.045 and 0.036). Carbohydrate metabolism have normalized for the majority of patients. Two patients (18.2%) of the first group showed impaired glucose tolerance. Improved carbohydrate metabolism was associated with a decrease in body weight in both groups. Results of psychological questionnaires were comparable with the survey data obtained in general population in the Russian Federation. Conclusion. Surgical treatment of insulinomas is highly effective. Physical and psychological status of patients in most cases corresponds with those typical for this age-sex group of the population of the Russian Federation. Long-term treatment results do not depend on duration of the catamnesis. Complications that developed from surgical treatment have the main influence on the health of patients.

In this article the results of surgical treatment of 150 patients with adult idiopathic scoliosis are presented. We used a rod instrumentation LSZ-3 of full and selective fusion of primary curve. Application of the core endocorrector LSZ-3 allowed to reduce the traumatism of surgical treat- ment and the risk of fatigue fractures of endocorrector components. This method improved both the physical adaptation of the patients after the operation and their quality of life.

Aim. To determine the rational volume of conservative therapy with form of hypotonic biliary dyskinesia and vegetative disorders in patients with chronic acalculous cholecystitis (CAC). Material and methods. The comparative retro- and prospective analysis of conservative treatment in 114 patients with CAC, who had been treated in the clinic the clinic of surgery. K.T. Ovnatanyana DonNMU of M. Gorky from 2008 to 2015. Among them were 67 females (59.1%) and 47 (40.9%) males at the age of 43,0 ± 6,7 years. The following methods were used to patients: biochemical blood analyses, ultrasound of hepatopancreatoduodenal area, fractional chromatic duodenal intubation with microscopic, biochemical and bacteriological studies of bile, the dynamic ultrasound examination of the gallbladder with test meal. All patients were divided into two homogeneous groups of 70 people. 1st group (main one) received treatment based on the type of dyskinesia of gallbladder and vegetative disorders (Patent of the Russian Federation № 2589900 from 02.06.2015), and 44 people. 2nd group (compare one) got only standard conservative therapy (table №5, holekinetiki, choleretic and others.). Results. The treatment program of CAC consisted of dietotherapy, drug therapy during the exacerbation; pain relief; using of choleretic drugs; antibiotic therapy; normalization of the functions of the autonomic nervous system; immunomodulatory therapy, and improving of the general reactivity of the organism. At the hypotonia gallbladder good effect was obtained of using of Gerbastress with Polyphytohol and Vitam (Patent of the Russian Federation № 2589900 from 02.06.2015). In the 1st group various complications were noted in 2.1%, in the 2nd group - in 5.4%. In a remote period (after 1-5 years) the best indicators of the quality of life were in the 1st group. Conclusions. Using developed an improved method of treatment causes a more expressed in comparison with the control group, a reduction of pain, dyspeptic and asthenic-vegetative syndromes, improve the functional state of the biliary tract and biochemical indices of bile, improving the quality of life on all scales by 1.3-1.7 times .

Iatrogenic damage to the extrahepatic bile ducts occurs in a fairly large number of observations, despite constant work aimed at improving surgical interventions. The most optimal chosen therapeutic tactics, as well as the timing of the revealed damage to the bile ducts, determine the effectiveness and quality of care provided to this category of patients. We also have to state that, to date, in the Russian Federation there is no unambiguously generally accepted and approved therapeutic and diagnostic algorithm for iatrogenic damage to the extrahepatic bile ducts. One of the main aspects of the effectiveness of the applied treatment tactics is the level of quality of life of patients who underwent surgical treatment. Treatment and management of patients with iatrogenic damage to the extrahepatic bile ducts still have a fairly large number of controversial issues, the solution of which should improve the immediate results of surgical treatment of this category of patients.

The article presents the results of surgical treatment of patients with pancreatic pseudocysts spent in hospital surgical clinic Siberian State Medical University from 2004 to 2016. 7 (17.5%) patients underwent conservative therapy. In the early postoperative period and in the long term (18 months) the analysis of the effectiveness of different methods of surgical treatment. According to the results of the research, the operations of the internal drainage along with resection and resection-draining interventions in assessing the quality of life showed similar long-term results. In turn, these interventions have provided significantly better quality of life (p> 0.05) than external drainage operation.
