Los residentes médicos representan un rol fundamental en el proceso de formación de estudiantes, internos y co-residentes, con labores implícitas de docencia en sus obligaciones como estudiantes de posgrado y sin ningún tipo de formación para ello. Previo a la implementación de programas de “residente como docente”, resulta útil indagar sobre las experiencias docentes en contexto, para así desarrollar programas acordes a las características y necesidades del medio. Este estudio buscó identificar las variaciones en las experiencias docentes de residentes de Medicina Familiar de una Universidad en Colombia. Corresponde a una investigación cualitativa con un enfoque fenomenográfico. Participaron 15 residentes de diferentes años de formación. Se identificaron 5 categorías de descripción relacionadas de manera jerárquica y determinadas a partir de 7 dimensiones de variación. Las formas identificadas de experimentar el rol del “residente como docente” son el transmisor pasivo impuesto, el facilitador del conocimiento, el facilitador de la práctica clínica, el role model y el “lifelong learner”. Las variaciones en las experiencias del “residente como docente”, estuvieron determinadas por la interacción de factores personales y contextuales. Se identificó que las categorías de descripción se encuentran interconectadas, con el potencial de avanzar hacia la siguiente categoría. Lo anterior puede ser utilizado como sustrato para el desarrollo de un programa de “residente como docente” orientado a potenciar la transformación hacia las categorías de descripción en los niveles de mayor comprensión del rol, situación deseable en el contexto latinoamericano donde los residentes tienen diversas responsabilidades docentes. medical residents represent a fundamental role in the training process of students, interns and peer-residents, with implicit teaching obligations as postgraduate students and without any type of training for it. Fifteen residents from different years of training responded a semi-structures interview. We identified 5 categories of description related in a hierarchical way by 7 dimensions of variation. The ways of experiencing the “resident as teacher” role are the imposed passive transmitter, the facilitator of knowledge, the facilitator of clinical practice, the role model and the “lifelong learner”. Variations in experiences of "resident as teacher" are determined by the interaction of personal and contextual factors. It is necessary to emphasize that the description categories are interconnected, with the potential to advance to the next category. The above can be used as a platform for the development of a "resident as teacher" program aimed to promote functional academic transformations towards the description categories with deeper understanding of the teacher role, a desirable situation in the Latin American context where residents have diverse teaching responsabilities.

Objetivo: comprender las estrategias innovadoras de evaluación: examen de conocimientos en la plataforma virtual, Seminario de Evaluación y Retroalimentación Interdisciplinar (SERI) y Examen de Conocimientos por Objetivos Estructurado (ECOE), desde las percepciones de docentes y estudiantes del área básica de un programa de medicina.Materiales y métodos: investigación cualitativa, microetnográfica. Se realizaron cinco grupos focales y siete entrevistas en profundidad a docentes y estudiantes, con grabación autorizada y consentimiento informado; se analizó la información mediante codificación abierta y axial, y generación de categorías emergentes. Se utilizó triangulación de fuentes, autores y técnicas, se elaboró informe final, previa devolución de información.Resultados: las estrategias investigadas tienen fortalezas y debilidades, el examen en plataforma es bien recibido por estudiantes pero le falta coordinación. Se necesita mayor capacitación de docentes en el uso de la plataforma, y es importante establecer mecanismos para evitar posibles fraudes. El SERI favorece la retroalimentación, pero se requiere que disminuya el riesgo de vulnerar la autoestima y permita una mejor valoración de conocimientos. El ECOE los acerca a la correlación básico-clínica, pero falta organización y espacio para las estaciones.Conclusiones: las estrategias innovadoras de evaluación deben someterse a una constante revisión desde su estructura y ejecución, fortaleciendo así la formación integral de los estudiantes.

CONTEXT: A postgraduate and resident trainee mental health assistance center was created in September 1996 within our university. OBJECTIVE: To describe the clinical and demographic profile of its users. TYPE OF STUDY: Retrospective. SETTING: Universidade Federal de São Paulo - Escola Paulista de Medicina (Unifesp-EPM). METHODS: The study was carried between September 1996 and November 2002, when 233 semi-structured registration forms were filled out either by the psychologist or the psychiatrist during their first contact with the trainees, who were medical and nursing residents, and postgraduate students at specialization, master or doctoral levels. The registration forms included demographic, occupational and clinical data. RESULTS: The trainees were predominantly young (mean of 27 years old), single (82.0% of cases), women (79.4%), seeking help especially during the first year of training (63.1%). In 70.8% of the cases, they came to the service spontaneously. Such individuals showed greater adherence to the treatment than those who were referred by supervisors (p < 0.05). In 30% of the cases, the trainee sought psychological guidance or support at the service due to specific situational conflicts. Depression and anxiety disorders were the most frequent diagnoses; 22.3% of the trainees followed up mentioned a tendency towards suicidal thoughts. In comparison with other trainees, there was a higher prevalence of males among the medical residents (p < 0.01), with more cases of sleep disorders (p < 0.05), a smaller number of individuals refraining from the use of alcohol (p < 0.05) and a higher number of trainees requiring leave of absence (p < 0.001). DISCUSSION: The first year of training in health sciences is the most stressful, especially for women. Depression and anxiety symptoms are common, reflecting transitory self-limited deadaptation. However, the severity of the cases can also be evaluated in view of the large number of trainees who mentioned suicidal tendencies. CONCLUSIONS: This study emphasizes the need and importance of providing formal, structured and confidential mental health services for medical residents and postgraduate students from other health professions, in the training programs of academic institutions.

ObjectivesHealthcare workers, in the course of their professional activity, are potentially exposed to chemical, physical and above all biological risks. The aims of our study were to investigate the extent and distribution of needle-stick and sharp injuries (NSIs) in healthcare students, the behaviours and circumstances most frequently associated with NSIs, the frequency of NSI reporting and the adherence to the post-exposure protocols.MethodsThis study involved, through an interviewer-administered structured questionnaire, undergraduate and postgraduate students attending postgraduate medical schools and healthcare professional schools who underwent occupational health visits between January 2015 and July 2018.ResultsOf the 642 students that participated in the study, 95 (14.8%) sustained an NSI during the traineeship and, of these, 59 (62.1%) reported the NSI to the occupational health service. NSIs were significantly more frequent in older subjects (χ²=9.853, p=0.020) and, among medical residents, in surgical residents (χ²=31.260, p<0.0001); moreover, occurrence of NSIs increased with increasing duration of traineeship (t=−2.051, p=0.041). Reporting of NSIs significantly increased with increasing age (χ²=12.543, p=0.006), with medical residents significantly under-reporting NSIs compared with undergraduate healthcare professional students (χ²=10.718, p=0.001) and among medical residents, those attending critical care units had the highest under-reporting (χ²=7.323, p=0.026).ConclusionsThe study showed remarkable under-reporting, as well as a lack of preparedness of students for NSI preventive and post-exposure effective measures. Our findings underline that healthcare student education should be reinforced to ensure that safe practices are carried out when needles and sharps are involved, as well as stressing the importance of NSI reporting and adherence to post-exposure prophylaxis protocols.

Introducción. Esta investigación cualitativa y hermenéutica-interpretativa explora el proceso de evaluación docente de la Facultad de Medicina de la Universidad Nacional de Colombia con profesores de modalidad cátedra en la cohorte de ingreso 2007-2011.Objetivo. Analizar el proceso de evaluación docente y su relación con el desarrollo profesional de los profesores.Materiales y métodos. Estudio descriptivo transversal llevado a cabo en fases metodológicas: 1) fase documental, profundización temática en evaluación educativa, tendencias de evaluación docente, políticas educativas en evaluación y marco legal en evaluación docente de la Universidad Nacional de Colombia; 2) trabajo de campo, entrevista personal con apoyo de encuesta semiestructurada, y 3) proceso analítico de datos cualitativos hecho a partir de la codificación axial y por medio del programa Atlas ti.Resultados. Se construyeron categorías deductivas descriptivas: percepciones, modalidades, aspectos, ventajas y desventajas de la evaluación docente y categorías inductivas interpretativas: pedagogías reflexivas, evaluación por competencias profesionales y profesionalización de la docencia.Conclusión. Se generan consideraciones pedagógicas que promueven la comprensión de la evaluación docente como un proceso reflexivo, formativo e integral que favorece el desarrollo profesional de los profesores, los procesos educativos y las prácticas académicas de investigación-acción.

A intolerância ao glúten é uma incapacidade do organismo em absorver os nutrientes do glúten, que é a principal proteína encontrada em alguns cereais. A incapacidade de absorver partes do glúten pode ser devida a um componente genético ou pode ser adquirida em qualquer fase da vida. A intolerância pode ainda se manifestar após a ingestão de alimentos que contém este elemento, ou em casos assintomáticos, em que mesmo com a ausência dos sintomas, o intestino sofre agressões pela ingestão ao glúten. O estudo teve como objetivo pesquisar sobre o diagnóstico, intolerância ao glúten e alternativas de alimentos para os pacientes. O estudo foi composto por um conjunto de publicações contidas em periódicos, livros-textos, monografias, dissertações e teses. A coleta foi realizada nas bases eletrônicas Scielo (Scientific Eletronic Library Online), Bireme (Biblioteca Regional de Medicina), BVS (Biblioteca Virtual em Saúde), Pubmed e Lilacs (Literatura Latino Americana e do Caribe em Ciências Sócias e da Saúde) e publicações como: monografias, dissertações e teses disponíveis eletronicamente, bem como livros-textos no período de 1989 a 2016. Através da realização deste estudo, foi possível perceber que o glúten está presente na maioria dos alimentos disponíveis, sendo encontrado no trigo, na cevada, no centeio, na aveia e derivados, e se faz necessário que existam companhas informativas aos portadores, especialmente, os quais buscam informações quanto a sua deficiência e formas de conviver com ela, sempre prezando pelo seu bem-estar e qualidade de vida. Palavras-chave: Intolerância. Ingestão. Glúten. AbstractThe gluten intolerance is an inability of the body to absorb nutrients, which is one of the main nutrients of some cereals. The inability to absorb the parts can be a genetic component or can be acquired at any stage of life. Intolerance may continue to manifest itself after a process of awareness, that is, in asymptomatic cases, when with the absence of bowel symptoms, in the aggression to the use. The study had a good relationship with diagnosis, gluten intolerance and food alternatives for patients. The study was composed of a set of publications contained in periodicals, textbooks, monographs, dissertations and theses. The collection was carried out in the electronic databases of Scielo, the Regional Library of Medicine, the Virtual Health Library, Pubmed and Lilacs and the publication in English of Latin American and Caribbean Literature in Social and Health Sciences. The monographs, dissertations and theses available electronically, as well as textbooks from 1989 to 2016, are not found in wheat, in barley, rye, oats and derivatives. It is necessary to have information about the patients, especially those who seek information about their disability and ways of living with them, always esteemed by their well-being and quality of life. Keywords: Intolerance. Uno de los campos en los que la ligazón entre género y juventud resulta notoria es el de la educación universitaria. Considerando previamente lo ocurrido a nivel latinoamericano, se destacan las vidas de las primeras médicas argentinas (Cecilia Grierson, Elvira Rawson Guiñazú, Adelma Gossweiler, Francisca Montaut y María Beljover) quienes, en la segunda mitad del siglo XIX y en la primera del siglo XX, se sobrepusieron a obstáculos para su matriculación, cursado y egreso en las Facultades de Medicina así como para su condicionada práctica médica, excediendo este quehacer y bregando en favor de varios derechos femeninos. Su evocación adquiere actuales implicancias.<br /><br />University education is a field exhibiting a clear link between gender and youth. As concern former Latin American experiences, the first Argentinean medical women (Cecilia Grierson, Elvira Rawson Guiñazú, Adelma Gossweiler, Francisca Montaut and María Beljover) should be considered. During a period running from the second half of the XIX century to the first half of the XX century, they managed to overcome drawbacks dealing with matriculation, their study and qualification in medical schools as well as with their conditioned medical practice. They also struggled for many feminine rights. Their evocation acquires current implications.<br /><br />

Avaliar a influência de uma matriz curricular na percepção dos discentes de medicina sobre medicina de família e comunidade e na escolha por essa especialidade. Estudo qualitativo-quantitativo de cunho exploratório e transversal, efetuado por meio de questionário semiestruturado com acadêmicos do primeiro e do décimo-segundo semestres de medicina de um centro universitário brasileiro. Foram realizadas as análises descritivas e não paramétricas, com um nível de significância de 5%. Egressos conheciam bem a especialidade medicina de família e comunidade, ao contrário dos que estavam iniciando o curso. Dentre os fatores curriculares estudados, apenas o contato com um profissional modelo médico de família e comunidade foi estatisticamente significante para a escolha pela carreira (p-valor de 0,02). Este estudo sugere que o contato com médicos de família e comunidade e a inserção precoce e contínua na atenção básica durante a graduação contribuem para o interesse por essa especialidade.Descritores: Educação Médica, Medicina de Família e Comunidade, Currículo. Medical students’ perceptions on family medicineAbstract: To evaluate the influence of a medical program in medical students’ perceptions towards family medicine and in choosing a career in this field. A cross-sectional exploratory and qualitative-quantitative survey accomplished by the application of a semi-structured questionnaire. This research encompasses first and twelfth semesters medical students from a Brazilian university center. We performed a descriptive and a non-parametric analysis, with a significance level of 5%. Family Medicine was not well known by medical students who were beginning their graduation and very well known by those finishing medical school. Among the curriculum aspects studied, only the contact with a family medicine physician role model was statistically significant to affect the graduates’ career choice in family medicine (p-value 0.02). The purpose of this paper is to identify the impact of institutionalization, legitimacy, learning behaviors, and other organizational characteristics on the efforts made by sport clubs signing sponsorship contracts. An exploratory comparison of two professional football clubs, playing in the First Division, of Costa Rica and Chile was performed. Using a narrative methodology, the Costa Rican team was analyzed through observation during a two-year collaboration period, and the Chilean team was analyzed through in-depth interviews and an analysis of the historical literature. The analysis revealed that organizational behaviors impact the clubs’ amount of sponsorship contracts and multi-season contracts. It also revealed that institutionalization and legitimacy work as a first latter in the relationship, and that by themselves it should be considered a marketing tool. Additionally, it was observed that both learning behaviors and resource seeking strategies contribute to successfully sign sponsorship contracts, but a baseline level of legitimacy and institutionalization should exist. Drawing on Institutional Theory, Ecology, and Organizational Learning a clear role model was evidenced within the ecosystem of two professional football clubs, each on a different Latin American country. These behaviors, which up to our knowledge has never being analyzed before, yielded and sustained legitimacy, while showing a direct effect on sponsorship single-season and multi-season contracts.

Objetivos: Describir, analizar y comprender el afán de conciencia ética de las enfermeras durante el cuidado. Pacientes y métodos: Investigación cualitativa descriptiva e interpretativa. Participaron  15 enfermeros del servicio de medicina interna de un hospital público de la región Norte-Perú, muestra no probabilística, obtenida por saturación y redundancia. Para la obtención de datos se usó la entrevista semiestructurada, validada por juicio de 4 expertos y prueba piloto. Los datos recolectados se procesaron por  análisis de contenido. Resultados: I) Trabajar a conciencia ganando conocimiento para orientar el cuidado. II) Persistencia en reflexionar, evitando errores durante el cuidado y III. Cuidado a conciencia por valores y espiritualidad. Conclusiones: Los enfermeros en su afán de  poseer conciencia ética son persistentes en reflexionar durante el cuidado, acción que les ayuda a encontrar puntos de mejora, para los cuales deben concentrarse con los cinco sentidos en su trabajo, controlar  su temperamento y   permanecer tranquilos.
