Background: Recurrent aphthous ulcers are the most common oral lesion. They are classified into minor, major, and herpetiform ulcers. More than 85% of recurrent aphthous stomatitis presents as a minor ulcer. Hyaluronic acid is a biomaterial and it is a major carbohydrate component that can be found in many tissues and recently introduced as an alternative approach to enhance wound healing. Triamcinolone acetonide is a synthetic corticosteroid and it has two forms for topical use cream (0.1%) and ointment (0.1%). Objective: To make a comparison between the effectiveness of hyaluronic acid (0.2%) and triamcinolone acetonide (0.1) in the management of recurrent aphthous ulcer. Patients and Methods: We recruited eighty patients who had a history of recurrent aphthous stomatitis and when presented with current oral ulcer and randomly divided into two groups, one group received hyaluronic acid, and the other group received triamcinolone acetonide. The instruction was given to all patients to apply the agent to the aphthous ulcer 4 times per day for 6 days (day 0 to day 6). The severity of pain was assessed by using VAS and the change of the ulcer surface area measured. Results: Eighty patients with aphthous ulcers have participated in the study, 43 were treated with hyaluronic acid, 37 were treated with triamcinolone acetonide. There was a significant difference between the two groups regarding ulcer surface and after three and six days, the diameter of the ulcer in the hyaluronic acid group was significantly reduced in comparison with those of the triamcinolone acetonide group (p < 0.001). Regarding VAS for pain, there was a significant difference after three and six days, hyaluronic acid group had significantly less VAS than those of the triamcinolone acetonide group (p = 0.004 and p < 0.001 respectively). Conclusion: Hyaluronic acid is more effective than triamcinolone acetonide when used in the treatment of recurrent oral ulcers in reducing pain and surface area of the ulcer. UNSTRUCTURED DattaMeghe Institute of Medical Sciences has been declared as Deemed to be University [DMIMS (DU)] under section 3 of UGC Act, 1996 in the year 2005. The University examinations are conducted as per the rules, regulations and guidelines issued by the various Apical councils of India from time to time. The first University exam was conducted in the year 2006. DMIMS (DU) has initiated various reforms by its own in assessment and evaluation process based on needs. For any system to thrive, it needs to be constantly upgraded and critically evaluated. Therefore, a study is undertaken with this in mind and thereby aimed at evaluation and validation of the assessment and evaluation reforms formulated and implemented at DMIMS (DU) since its inception (year 2006).Need analysis for the assessment and evaluation reforms will be carried out on the basis of feedbacks and reports received from respective stakeholders (Students, Faculty and External examiners) from time to time. Outcome analysis of the Feedbacks and reports will be done subsequently after the implementation of the assessment and evaluation reforms. Further based on the outcome analysis, reforms will be validated into following parameters: Objectivity, Responsiveness, User friendliness, Residual component (not covered in above 3 parameters)and a satisfactory index will be derived. If satisfactory index for reform is 100%, it will be concluded that reforms are well received and good to excellent in nature. Total 33 assessment and evaluation reforms have beenimplemented since year 2006. These reforms will be categorised into Pre-Examination (n=18), On Site Examination (n=05) and Post Examination (n=10), and in each category, they will befurther classified into academic (Total =15) and administrative (Total = 18) on the basis of their domain for the ease of structuring and implementation. All the 33 reforms will be validated on the basis of defined parameters. Abstract Background Recurrent aphthous stomatitis with systemic signs of inflammation can be encountered in inflammatory bowel disease, Behçet’s disease (BD), Systemic Lupus Erythematosus (SLE). In addition, it has been proposed that cases with very early onset in childhood can be underpinned by rare monogenic defects of immunity, which may require targeted treatments. Thus, subjects with early onset recurrent aphthous stomatitis receiving a clinical diagnosis of BD-like or SLE-like disease may deserve a further diagnostic workout, including immunologic and genetic investigations. Objective To investigate how an immunologic, genetic and transcriptomics assessment of interferon inflammation may improve diagnosis and care in children with recurrent aphthous stomatitis with systemic inflammation. Methods Subjects referred to the pediatric rheumatologist for recurrent aphthous stomatitis associated with signs of systemic inflammation from January 2015 to January 2020 were enrolled in the study and underwent analysis of peripheral lymphocyte subsets, sequencing of a 17-genes panel and measure of interferon score. Results We enrolled 15 subjects (12 females, median age at disease onset 4 years). The clinical diagnosis was BD in 8, incomplete BD in 5, BD/SLE overlap in 1, SLE in 1. Pathogenic genetic variants were detected in 3 patients, respectively 2 STAT1 gain of function variants in two patients classified as BD/SLE overlap and SLE, and 1 TNFAIP3 mutation (A20 haploinsufficiency) in patients with BD. Moreover 2 likely pathogenic variants were identified in DNASE1L3 and PTPN22, both in patients with incomplete BD. Interferon score was high in the two patients with STAT1 GOF mutations, in the patient with TNFAIP3 mutation, and in 3 genetic-negative subjects. In two patients, the treatment was modified based on genetic results. Conclusions Although recurrent aphthous stomatitis associated with systemic inflammation may lead to a clinical diagnosis of BD or SLE, subjects with early disease onset in childhood deserve genetic investigation for rare monogenic disorders. A wider genetic panel may help disclosing the genetic background in the subset of children with increased interferon score, who tested negative in this study.

AbstractGold eyelid implantation is widely considered the procedure of choice to reanimate the upper eyelid in paralytic lagophthalmos. Commercially supplied implants are not readily available in all places and are sometimes cumbersome to import. Objective:We aimed to devise a method whereby every surgeon performing gold eyelid implantation could have easy and quick access to the implant. Furthermore, we aimed to develop a means of creating an implant of the exact weight required for complete eyelid closure.Study design and setting:A prospective study was performed from 1997 to 2005 in a tertiary research hospital, involving 50 subjects requiring gold upper eyelid implantation and using the technique in question. Results:Only patients with a minimum follow up of one year were included in the study group. Symptoms improved in 96 per cent of subjects, who were able to dispense with eyedrops and eye ointments. Visual acuity improved in 92 per cent of patients. There were two extrusions amongst the early cases. Conclusion and significance:Customised gold eyelid implantation offers an alternative in regions where commercial implants are not easily obtained.
