Determination of new biomarkers involved in the pathogenesis of ischemic stroke is an extremely important task from the point of view of identifying possible mechanisms for preventing the occurrence of an acute event, better diagnosis, and influencing the stages of pathogenesis to reduce the inflammatory focus. Sphingolipids belong to new biomarkers of atherosclerosis, which are involved in inflammation, apoptosis, and ischemia. The widespread introduction of mass spectrometry has made it possible to study sphingolipids in more detail. This review aims to summarize the available data on the role of sphingolipids in ischemic stroke.

 The activation of clones of immune lymphocytes to streptococci takes place at parodontitis. The number of noted lymphocytes significantly increases in the blood of patients with parodontitis  as compared to healthy persons having intact parodontium (12.7±0.3 versus 3.87±0.32, P<0.001). At various forms of parodontitis the growth of antigenreactive lymphocytes  is expressed with different degrees.  Particularly sharp increase is observed during severe forms of parodontitis as compared to average and light forms (the number of antigenereactive  lymphocytes to streptococci in the blood respectively made up 16.9±0.59, 12.8±0.23, 9.5±0.27, P<0.001).  As to immune lymphocytes to staphylococci and actinomycete, their activation was not revealed at parodontitis. The quantitative indices of the noted clones of lympho- cytes in the blood did not essentially differ from analogical indices of healthy individuals (an average amount of immune lymphocytes to staphylococci and fungi made up 4.3±0.26 and 3.84±0.29, P<0.05 as compared to healthy individuals). Thus, the increase of number of lymphocytes having receptors to streptococci in the blood has been first established at parodontitis and its intensity was in direct correlation to the degree of the generalization of the process. Proceeding from the aforesaid, it is possible to think that the leukocytes accumulated as a result of chemotaxis in response to microbial aggression in the inflammatory focus, developed in the tissues of  parodontium, especially neutrophils and macrophages induce the disorganization of streptococci and their antigens, including the release of antigens common to the tissues of the organism, followed by the development of reciprocal protective and destructive immune processes.

Aim. The research was conducted to study the processes occurring in soft tissues in the surgical treatment of widespread purulent-inflammatory processes of the maxillofacial area and to confirm the effectiveness of the developed optimal surgical technology.Materials and methods. A surgical treatment of 20 patients with the widespread purulent-inflammatory processes of maxillofacial area was carried out according to the developed surgical technology. In all cases, the tissue was taken from the area of the pathological focus for the pathomorphological examination at all stages of treatment. Results. The morphological study of the drugs obtained during a primary surgery on the purulent-inflammatory focus showed a picture of the serous-purulent inflammation with a severe leukocyte infiltration. Necrobiosis and tissue necrosis prevailed in the center of the purulent-inflammatory focus. After 3 days, the wound began to grow young granulation tissue with a large number of capillary kidneys, a marked decrease in the level of leukocyte infiltration, the appearance of fibroblasts and lymph-histiocytic elements, the formation of fibrous connective tissue. Conclusion. The morphological assessment of the wound process showed a positive dynamics and the effectiveness of the proposed treatment of the patients with purulent-inflammatory processes of the maxillofacial area.

The review deals with the analysis of modern methods of treatment of chronic generalized parodontitis from the perspective of differentiated principle of treatment of this disease. Particular attention is paid to methods of medical treatment, their diversity on the efficiency and mechanisms of therapeutic effects to select appropriate treatment, taking into account all the individual characteristics of the patient, aetiology, pathogenesis and severity of chronic generalized parodontitis. It has been demonstrated that an important aspect to achieve high efficiency of treatment is a method of drug delivery. The most effective method is a transmembrane dialysis, which makes it possible simultane-ously to obtain the maximum concentration of the therapeutic substances in the inflammatory focus and to eliminate from the inflammatory focus the low-medium weight tissue decomposition products, products of metabolism, while maintaining factors of regeneration and protection against infection. It has been evinced the advantages of sanogenetic therapy using vitamin cocktail and some higher plants BAS containing terpenoids (bornylacetate and its semisynthetic stereoisomer – isobornyl acetate), as the least toxic, having analgesic and anti-bacterial properties comparable with antibiotics, and capable to stimulate repair processes. The good water solubility of these substances provides ample pos-sibility of using these drugs in the form of solutions for rinsing, transmembrane dialysis, as liquid at ultrasonic curettage.
