Objective: The present study was aimed to design different analogues of 2-aminobenzimidazole and find the binding ability by Insilico method. Methods: Various soft wares like Chemsketch, Molinspiration, PASS, and Discovery studio were used to design the proposed derivatives. Evaluation of binding activity against different receptors was detected and checked their physicochemical properties for binding. Results: In this study, we designed different analogs of 2-aminobenzimidazole into a ligand having a binding affinity with alpha-glucosidase, Dipeptidyl–peptidase 4(DPP4), Peroximase proliferator-activated receptor gamma (PPARγ), and Insulin-like growth factor 1(IGF-1) receptor. The designed ten derivatives showed a significant binding capacity to the concerned receptors. Conclusion: These results pointed that the designed proposed derivatives promising hypoglycaemic activity.

Diabetes mellitus (DM) has become a global scourge, and there is a continuous search for novel compounds as viable alternatives to synthetic drugs which are often accompanied by severe adverse effects. Aristolochia ringens is among the scientifically implicated botanicals effective in the management of several degenerative diseases including DM. The current study evaluated the inhibitory mechanism(s) of root extract of A. ringens on α-amylase and α-glucosidase in vitro and in silico, while its constituents were characterized using liquid chromatography-mass spectrometric technique. The extract had concentration-dependent inhibitory effect on the study enzymes, and the inhibition compared well with that of standard drug (acarbose) with respective IC50 values of 0.67 mg/mL (α-amylase) and 0.57 mg/mL (α-glucosidase) compared with that of the extract (0.63 and 0.54 mg/mL). The extract competitively and uncompetitively inhibited α-amylase and α-glucosidase, respectively. Of the identified compounds, dianoside G (−12.4, −12.5 kcal/mol) and trilobine (−10.0, −10.0 kcal/mol) had significant interactions with α-amylase and α-glucosidase, respectively, while magnoflorine and asiatic acid also interacted keenly with both enzymes, with quercetin 3-O-glucuronide and strictosidine showing better affinity towards α-glucosidase. These observations are suggestive of involvement of these compounds as probable ligands contributing to antidiabetic potential of the extract. While studies are underway to demystify the yet to be identified compounds in the extract, the data presented have lent scientific credence to the acclaimed in vivo antidiabetic potential of the extract and suggested it as a viable source of oral hypoglycaemic agent.

Abstract Polypharmacy (PP) occurs in patients with type 2 diabetes (T2DM) owing to multimorbidity. We evaluated concomitant PP and medication adherence in T2DM 3 years after initiation of administration of a hypoglycaemic agent using a nationwide claim-based database in Japan. Factors associated with medication PP and imperfect adherence were identified using multivariable logistic regression. PP was defined as using ≥6 medications. Patients with proportion of days covered (PDC) of <80% were defined as having poor medication adherence. A total of 884 patients were analysed. Multivariate analysis revealed that age, total number of consultations and body mass index (BMI) are factors that influence PP. Factors associated with PDC < 80% were 2–3, 4–5 and ≥ 6 medications compared with 1 medication, male sex, <17 consultations and age 50–59 and ≥ 60 years compared with <40 years. In conclusion, older age, high total number of consultations and BMI ≥ 25 kg/m2 are risk factors for PP. PP influenced good medication adherence at the end of the observation period.

The prevalence of type 2 diabetes mellitus (T2DM) among children and adolescents is on the rise and this correlates well with increasing prevalence of obesity and overweight worldwide. T2DM was thought to be a disease of the adults, however this has since changed. High index of suspicion is required especially in obese children with family history of type T2DM. To the best of our literature search, till date, no case was reported from Northeast Nigeria. We hereby report a case of newly diagnosed type 2 DM in a 14-year old obese teenage girl, who presented with polyuria, polydipsia and weight loss. Her father is a T2DM patient. She had acanthosis and multiple ovarian cysts. She underwent acute phase of care with insulin before discharge and has remained euglycaemic and clinically state on oral hypoglycaemic agent.

AIM & BACKGROUND: Diabetic nephropathy (DN), also known as diabetic kidney disease, is the chronic loss of kidney function occurring in those with diabetes mellitus. Protein loss in the urine due to damage to the glomeruli may become massive, and cause a low serum albumin with resulting generalized body swelling and result in the nephrotic syndrome. Likewise, the estimated glomerular filtration rate (eGFR) may progressively fall from a normal of over 90 ml/min/1.73m2 to less than 15, at which point the patient is said to have end-stage kidney (ESKD). It is usually slowly progressive over years. CASE DESCRIPTION: A 70 year old female k/c/o DM for 10 years presenting with anasarca, puffiness of face, nausea, vomiting, weakness, hiccups, oliguria and itching all over body. Her renal and diabetic profile were deranged with huge proteinuria. Patient was on OHA (oral hypoglycaemic agent). According to ayurved she was diagnosed as case of prameha upadrav janya kapha pradhan vrikka rog.  OUTCOME: After 11 days of vajeri basti and oral medication the patient showed significant relief in symptoms as well as reports. CONCLUSION: Significant relief can be achieved in patients of diabetic nephropathy by applying classical ayurvedic principles. Clozapine is an atypical antipsychotic which is described to have higher efficacy among all available antipsychotic medications. Clozapine is reserved especially for resistant schizophrenia due to its side effects. Clozapine-induced metabolic syndrome and hyperglycaemia are common long-term side effects and are responsible for increased mortality in patients with schizophrenia. In this case, a patient with resistant schizophrenia was presented with acute-onset hyperglycaemia and delirium with the use of clozapine within a week. Withdrawal of clozapine in the patient led to the improvement in delirium and hyperglycaemia without the use of any hypoglycaemic agent. This case supports the notion that in certain cases clozapine can induce hyperglycemia through possible direct pathophysiological mechanisms within a shorter time frame.

The phenols and fatty acids profile and in vitro antioxidant and hypoglycaemic activity of seed, peel, pulp or pulp plus seeds of Colombian fruits from Solanaceae and Passifloraceae families were investigated. Ultra-High Performance Liquid Chromatography (UHPLC)-High Resolution Mass Spectrometry (HRMS) revealed the presence of chlorogenic acid as dominant phenolic compound in Solanaceae samples. Based on the Relative Antioxidant Score (RACI) and Global Antioxidant Score (GAS) values, Solanum quitoense peel showed the highest antioxidant potential among Solanaceae samples while Passiflora tripartita fruits exhibited the highest antioxidant effects among Passifloraceae samples. P. ligularis seeds were the most active as hypoglycaemic agent with IC50 values of 22.6 and 24.8 μg/mL against α-amylase and α-glucosidase, respectively. Considering that some of the most promising results were obtained by the processing waste portion, its use as functional ingredients should be considered for the development of nutraceutical products intended for patients with disturbance of glucose metabolism.
