Hepatic encephalopathy (HE) is a reversible syndrome observed in patients with liver disease. The syndrome is characterised by a spectrum of neuropsychiatric abnormalities resulting from the accumulation of neurotoxic substances in the bloodstream and ultimately in the brain. HE is a huge burden to patients, caregivers, and the healthcare system. Common treatments for HE, including rifaximin and lactulose, have been shown to reduce the risk of recurrence, frequency of hospitalisations, hospital costs, and mortality. New research and therapeutics exist, including faecal transplants and small-molecule therapies such as branched-chain amino acids. This review article provides a general overview of the current understanding of HE.

In the paper, the use of cartographic research method for the analysis and generalization of materials describing extreme events associated with rarely recurring high water and flooding in river basins of the Upper Ob is proposed. Basing on statistical data obtained from municipalities of the adjacent regions (Altai Krai, Republic of Altai, Kemerovo, Novosibirsk and Tomsk oblasts), including the information provided by territorial bodies of the RF Emergency Situations Ministry, we created a database for assessing actual and probable damage to population and economy of the study territories. Actual damage is generalized and analyzed in the context of river basins and municipalities of the entities, i.e. subjects of the Russian Federation. In order to assess probable damage, cartographic sources, including open data from the Internet as well as modern software tools were involved. The estimation of probable damage to population and economic objects was performed by the example of settlements (including those suffered from extreme flooding of 2014) located in the Charysh basin (Altai Krai). For that GIS projects and a series of damage evaluation maps were created. River basins of the Upper Ob regions were ranked according to recurrence frequency of extreme events and the extent of actual damage to population and economy there in 2004–2018. On the basis of these results, recommendations to prevent emergencies caused by the negative impact of water were developed.

Toxoplasma metabolism effect can rise the dopamine production which aggravate the simptoms of schizophrenia. Some of the research reports that patients which diagnosed with schizophrenia with positive IgG Toxoplasma need longer duration of treatment and may relapse. This study was conducted by cross sectional method on 51 patients with first episode schizophrenia, including acute and temporary psychotic disorder, in Tampan Mental Hospital – Pekanbaru. IgG anti-toxoplasma were detected on the patients at the first treatment in 2019, and then the length period of treatment and the recurrence frequency were monitored in the first year of the course of the disease. From the chi square test result, we can conclude that there’s no effect of IgG Toxoplasma on the duration of treatment on the first episode schizophrenia patients as well as the recurrence frequency which monitored in the first year course of the disease (p>0.05). There’s no effect of IgG Toxoplasma on the duration of treatment on the first episode schizophrenia patients as well as the recurrence frequency.

AbstractIn repeating seismic event sequences within a specialized horizontal area, the moment magnitude is usually scaled with the recurrence interval. In addition to two horizontal dimensions, the vertical dimension plays a certain role in affecting the scaling law. However, whether and how the changing depth influences the scaling law remain enigmatic. Based on a large number of earthquake records with high-resolution epicenter locations in recent decades in Japan, we focus on a comparison between the 3-D seismic moment and seismic interval, which recognize the vertical dimension as the same dimension as the horizontal distances. The results show that (1) the seismic moment scaling law is applicable in the multiparameter 3-D models by visiting the 1.8 million events collected during a period of 15 years; (2) the vertical dimension of depth plays an important role in the Mo–SI relationship as well as in the variance in the 3-D seismic moment–interval magnitudes; and (3) the seismic moment rate, attributable to the plate convergence rate, varies with area and depth in influencing the regional earthquake recurrence frequency.

This study proposes a method to improve the manufacturing process of a surface anchor structure for the injection molding along with bonding properties of the plating layer for antennas, which can be applied to the Laser Direct Plating (LDP) process for the production of a three-dimensional antenna integrated with a mobile device case. By adjusting parameters such as the output of the laser processing, scanning speed, and pulse recurrence frequency, a micro anchor structure was developed on the surface of the injection mold. The measurement of the surface roughness using a 3D surface profiler showed that the roughness improved by approximately 3.6 times, from 0.96 to 3.24 µm. In addition, the zigzag arrangement of the micro anchor structure was improved by 1.2 times compared to the even arrangement. Furthermore, if the micro anchor structure contained a wall after laser processing, the bonding strength of the plating solution was 69%; with no wall, it was 94% or higher. Thus, the existence of walls resulted in a difference of 1.4 times in the bonding strength. The laser processing improved the bonding strength of the plating solution on the micro anchor structure by approximately 19 times.

Background: Up to 30% of incident pericarditis patients (pts) recur within 18 months. Recurrences can be debilitating and typically last several weeks despite conventional treatments (Tx). Recurrence burden (likelihood and frequency) is poorly defined. Improved understanding of recurrence burden can inform Tx decisions and the introduction of novel therapies. Method: Adults with idiopathic recurrent pericarditis (RP) were identified in the OptumHealth Reporting and Insights dataset (2007-2017). RP was defined as ≥2 episodes separated by >4 weeks. Disease duration (time from first episode to end of last recurrence, confirmed by a 1.5-year recurrence-free period) was evaluated using Kaplan-Meier analysis. Recurrence frequency and time between recurrences were evaluated among pts with 4+ years of observation. Result: Of the 1,604 RP pts, mean age was 50.7 years and 51.6% were female. Median RP duration was 2.1 and 3.1 years for pts with ≥1 and ≥2 recurrences, respectively ( Figure ). Over 4 years with RP, 15% of pts with ≥1 recurrence and 22% of pts with ≥2 recurrences had more than one episode/year. Mean ± SD time from first episode to first recurrence was 14.5 ± 17.9 months and 10.7 ± 12.1 months between subsequent recurrences. Conclusion: Recurrent pericarditis can span many years. Over half of pts with ≥2 recurrences have RP persisting over 3 years. Subsequent recurrences are more frequent but highly variable, making RP unpredictable. Tx to reduce recurrences could benefit pts with ≥2 recurrences.

RAS (Recurrent Aphthous Stomatitis) is an inflammation of soft tissues of the oral cavity characterized by recurrent and painful ulcers. These lesions can be marked early on with a burning sensation, and when the wound has formed, the pain is getting worse. Occasional symptoms of precursors, paresthesia and hyperesthesia, are sometimes reported. Pain and discomfort arise along movement around the ulcer e.g. eating, talking and swallowing. Characteristics of RAS are usually single or multiple ulcers occurring recursively on the oral mucosa, round or oval in shape, clearly demarcated, with a grayish-yellow necrotic center and reddish edges. Lesions occur at young ages i.e. children and puberty, and may occur in adults as well. Ulcers usually last in 1 week or month. This disorder is classified into three categories according to size i.e. recurrent aphthous stomatitis minor, major and herpetiformis. The aim of this report is to show the management of cases of Recurrent Aphthous Stomatitis Minor on the left side of tongue, left buccal mucosa, and lower labial mucosa in female patient aged 19 years. A 19-year-old female patient came to the Dental Hospital of Jember University with complaints of thrush on the left side of her tongue, left buccal mucosa and lower labial mucosa, pain, and feeling disturbed due to the pain when eating, talking, and laughing. In this case, the diagnosis showed Recurrent Aphthous Minor Stomatitis on the left side of the tongue, left buccal mucosa, and lower labial mucosa. The therapy was using nonsteroidal anti-inflammatory mouthwash containing aloe Vera and hyaluronic acid as well as giving multivitamin of becomzet (vitamin B complex, A, C, E, and Zinc). Keywords: recurrent aphthous stomatitis; high recurrence

<p><strong>Aim.</strong> To compare the results of interventions for intermediate coronary lesions, which were operated via angiography and optical coherence tomography (OCT).<br /><strong>Methods.</strong> We performed a study including 130 stable patients with intermediate coronary lesions (40%–70%). The first group (n = 65) underwent angiography control; the second group (n = 65) underwent OCT control. Morphometric coronary indicators were analyzed intraoperatively. During the 12-month follow-up period, major adverse cardiovascular events incidence, angina recurrence frequency, restenosis, and stent thrombosis were assessed.<br /><strong>Results.</strong> More pronounced coronary changes were noted in the OC group, as evidenced by stenosis indicators (51.5% [45.2–68.4] vs. 78.6 [65.7–80.3], p = 0.012), lesion length (18.4 mm [15.6–20.3] vs. 26.8 [24.6–32.2], p = 0.038), and plaque instability frequency (4.6% [3/65] vs. 12.3% [8/65], p = 0.048). In the OCT group, decision to perform stenting was more common (33.8% [22/65] vs. 83.1%) [54/65], p &lt; 0.001). In stented patients, differences were noted in major adverse cardiovascular events frequency (13.6% vs. 0%, p = 0.021), angina return (36.3% vs. 3.7%, p &lt; 0.001, odds ratio (OR) 14,857 [2,830–77,990]), and stent restenosis (31.8% vs. 1.8%), p &lt; 0.001, OR 24,733 [2,817–217,125]), with an advantage in favor of OCT. In patients receiving drug therapy, differences were noted in angina recurrence frequency (60.5% vs. 18.1%, p = 0.005, OR 6,882 [1,322–35,824]), with an advantage in favor of OCT.<br /><strong>Conclusion.</strong> The use of OCT for intermediate coronary lesions revealed prominent lesions and optimized intervention outcomes, accompanied by reduced major adverse cardiovascular events frequency, myocardial infarction, angina recurrence, and stent restenosis at 12 months.</p><p><br />Received 5 July 2019. The high recurrence frequency of gliomas but deficiency of effective treatment and prevalent chemoresistance have elicited interests in exploring and developing new agents. Paris polyphyllins are monomers extracted from rhizome of Paris polyphylla var. yunnanensis. Here, we first reported that polyphyllin VII (PP7) exhibited cytotoxic effect on glioma cells. PP7 significantly suppressed the viability and induced cell death of U87-MG and U251 cells after 24 h, with the IC50 values 4.24±0.87 μM and 2.17±0.14 μM, respectively. Both apoptotic and autophagic processes were involved in the cytotoxic effect of PP7, as PP7 activated the Bcl2/Bax pathway and the inhibition of autophagy partly rescued the toxicity of PP7 in glioma cells. In addition, an inhibition of AKT/mTORC1 activity was found after PP7 administration, and it seemed that the overproduction of reactive oxygen species (ROS) was responsible for this effect. Namely, the removal of ROS by NAC treatment mitigated PP7-induced cell death, autophagy, and its effect on the AKT/mTORC1 signaling. Additionally, a combination assay of PP7 with temozolomide (TMZ), the most used chemotherapy for glioma patients, was performed resulting in synergism, while PP7 reduced TMZ resistance through inhibition of MGMT expression. Thus, our study reports PP7 as a potential agent for glioma treatment and reveals its underlying mechanisms of action.
