<p><em>Leadership is an ability or strength in a person to lead and influence others in terms of work, where the goal is to achieve predetermined goals. The development of today's era has influenced all aspects, including aspects of leadership style. In today's millennial generation, the challenge of having an ideal leader that fits the current era is very big. This study aims to see how the ideal leadership style. The focus of the research is to examine the ideal leadership style for the millennial generation. The method used in this research is literature study related to theory and references. The results of the research on the ideal leadership style for the millennial generation, namely effective government will be realized if the leader can meet the qualifications as a credible leader, has the ability, intellect, good vision and has integrity, honesty and loyalty to interests. from the community. Building a region that is led must have a foundation, namely independence and entrepreneurship that gives pleasure and satisfaction to the community. Leadership in the millennial era has a unique approach because digitalization that has penetrated the world of work no longer allows leaders to act conventionally.</em></p>

Myelin oligodendrocyte-antibody-associated disease (MOGAD) often presents with severe optic neuritis (ON) but tends to recover better than in aquaporin-4 antibody neuromyelitis optica spectrum disorder (AQP4-NMOSD). We measured OCT and VEP in MOGAD and AQP4-NMOSD eyes with good visual function, with or without previous ON episodes. Surprisingly, OCT and/or VEPs were abnormal in 84% MOGAD-ON versus 38% AQP4-NMOSD-ON eyes (p = 0.009) with good vision, compared with 18% and 17% respectively of eyes with no previous ON. A sub-group with macular OCT performed as part of a research study confirmed both retinal and macular defects in visually-recovered MOGAD eyes. These findings have implications for investigation and management of MOGAD patients.

PurposeTo analyse the morphological characteristics of eyes with neovascular age-related macular degeneration (AMD) with good long-term visual acuity after anti-VEGF (vascular endothelial growth factor) therapy.MethodsRetrospective, observational study of 175 patients with neovascular AMD with >5 years of follow-up after initiating anti-VEGF therapy. Spectral-domain optical coherence tomography images were assessed for thickness of pigment epithelial detachment (PED), subretinal hyper-reflective material (SHRM), subretinal fluid and subfoveal choroidal, as well as the integrity of the outer retinal bands.ResultsThe final analysis cohort included 203 eyes (175 patients) followed for a mean of 7.84±1.70 years (range: 5–11). The maximum PED thickness in the foveal central subfield (FCS) was significantly lower (p<0.001) in the poor vision group (13.11 μm) compared with the intermediate (86.25 μm) or good (97.92 μm) vision groups, respectively. In contrast, the maximum thickness of SHRM in the FCS was significantly thicker (p<0.001) in eyes with poor vision (149.46 μm) compared with eyes with intermediate vision (64.37 μm) which in turn were significantly thicker (p<0.001) than eyes with good vision (9.35 μm). The good vision group also had better continuity of all outer retinal bands (external limiting membrane, ellipsoid zone, and retinal pigment epithelium) compared with the other two groups (all p<0.001).ConclusionA thicker PED and thinner SHRM were correlated with better vision in eyes with neovascular AMD following long-term anti-VEGF therapy. If replicated in future prospective studies, these findings may have implications for design of optimal anatomic endpoints for neovascular AMD treatment.

Abstract A country that has a good system will be able to provide good services to its people, but in practice the regulations and financing systems experience obstacles. One of the problems faced by a country is fraud which has the potential to become an act of corruption. Corruption by some individuals is very detrimental to the community as recipients of health services. The data shows the purpose of creating a health system that has a good vision and mission, but in practice, it creates misery for the community, especially the middle to a lower class. During the Covid-19 pandemic, corruption in the financing of the health system also increased. Based on the above background, the researcher will focus on the issue of how to analyze the impact of corruption in the Health System and how to conceptualize recommendations to overcome it. Juridical normative and sociological methods are used to study the problems found and at the same time analyzed in the health financing system. The results of the study found that the health financing system has a noble purpose for the benefit of society. However, in its implementation, there are many errors in the system that result in actions and have the potential to become acts of corruption that result in state losses. Recommendations are made to be implemented immediately so that crime stops and society is protected.

BACKGROUND We wanted to compare lensectomy with anterior vitrectomy and conventional methods in the surgical management of congenital cataract. METHODS The present study is an interventional analytical study that entailed 30 patients (33 eyes) of bilateral or unilateral congenital cataract who attended the Ophthalmology outpatient department of LLRM Medical College, Meerut, during the period extending from March 2018 to March 2019. 19 of these patients (63.3 %) were males while 11 (36.7 %) were females. Age of the patients varied from 2 years to 12 years, with a mean age of 5 years 7 months. The maximum number of patients was in the age group of 4 - 8 years in either sex. RESULTS Being a comparative analysis, this study had 39.3 % cases treated by conventional methods and 60.7 % cases managed by lensectomy and anterior vitrectomy. The visual acuity was compared in the conventionally treated patients and lensectomy and anterior vitrectomy treated patients. Amongst the patients managed by conventional methods, 37.5 % had good vision, 12.5 % had fair vision and 50.0 % poor vision. In the lensectomy and anterior vitrectomy treated group, 61.1 % cases had good vision, 22.2 % had fair vision and only 16.7 % had poor vision. Comparing the occurrence of complications in both groups, the conventionally treated group showed an incidence of 32.4 % as compared to 4.4 % in lensectomy and anterior vitrectomy treated group. CONCLUSIONS The lensectomy and anterior vitrectomy treated groups had good vision and fewer complications as compared to the conventional methods. Optometry has made significant contributions to the United States military for over a century. Assuring good vision and eye health of soldiers, sailors, airmen and marines is critical to maximizing the military functions necessary to achieve victory. There was little organization or recognition of the profession in World War I, but optometrists were essential in achieving the mission. Recognition of the profession of optometry was still limited in World War II but it was improving, especially with commissioning as officers occurring in the Navy. Through the Korean and Vietnam Wars, optometry grew in stature and strength with all services eventually commissioning all optometrists, and Army optometrists were assigned to combat divisions. Continuing through the more recent conflicts in the middle east, the profession has continued to make an impact and has become an essential part of the armed forces of the United States. Doctors of optometry are now an integral part of the Department of Defense. The nation cannot field an effective fighting force today without the dedicated performance of these officers.

Abstract Aims To determine the effect of intravitreous Bevacizumab injection on reducing diabetic macular edema in patients with good vision. Method : This clinical trial was conducted on diabetic patients with centrally involved macular edema and good vision over a period of one and a half year. 22 eyes of 12 patients with a visual acuity of Snellen equivalent of 20/25 or better in the non-proliferative stage with the central macular thickness of more than 300 microns in optical coherence tomography (OCT) were evaluated. In the injection group 1.25 Mg of bevacizumab (Avastin) was injected intravitreally and the control group was followed up. Macular thickness and visual acuity were measured in two groups at first and sixth months after injection. Results Intravitreal bevacizumab was injected in 12 eyes (55%) and 10 patients were followed-up as a control group (45%). The mean central macular thickness was 319 µm at first visit and 301 µm at month six in the treatment group and in the control group it was 318 µm and 328 µm respectively. The mean macular thickness of the treatment group was significantly decreased (p = 0.031), but the difference was not statistically significant between two groups (p = 0.581). The mean visual acuity in the treatment group was 0.045 Log MAR at first visit and 0.033 Log MAR at the sixth month follow up, these values in the control group were 0.1 Log MAR and 0.045 Log MAR respectively and did not differ statistically between the two groups (p = 0.373) Conclusion Although the mean macular thickness was reduced in the treatment group with visual acuity of 20/25 or better but the difference was not significant during follow-up at least in a short period. Therefore, the role of underlying factors other than VEGF alone can be notable in the pathogenesis of the disease.

Eyes are the most precious gift of the god to the living beings. Good vision is crucial for social and intellectual development of person. So to protect this organ is not only a necessity but also a responsibility of every individual. India is having the highest rate of head injury in RTA in the world. Out of which 60% suffer from Subconjunctival hemorrhage. Subconjunctival hemorrhage or Ecchymosis is of very common occurrence. Subconjunctival hemorrhage” is a common benign condition of the eye that has characteristic features, such as the painless acute appearance of a sharply circumscribed redness of bleeding underneath the conjunctiva in the absence of discharge, and inflammation in contagious areas. While searching solution for Subconjunctival hemorrhage in ayurveda, its signs and symptoms match very much with Arjuna” described in “Shuklagata roga”. Which is having symptom painless red spot over white surface of eye. So taking these things into consideration “Vasadi Ghrita Aschyotana” was selected for treating the disease Arjuna. The ghrita contain “Vasa” and “Yashtimadhu” having pittashamaka, Shonitasthapan properties.
