People living in the rural areas in developing countries are deprived of the basic needs including health. In Bangladesh, the rural poor people are facing difficulties to be facilitated from the enhanced medical services, mostly because of the increased medical expenses, and the time required to complete the whole process. Henceforth, it is necessary for the government to enhance the medical facilities to its rural citizens for proper development. It has been argued that the health card service system in Bangladesh can enhance the medical services. However, no studies have been conducted to investigate the impact of health card on the quality of life of rural people in Bangladesh. This study attempts to investigate this issue from actual beneficiary perspective. The main objective of this study is to provide a synopsis of the roles of health card on rural citizen of Bangladesh. The data were collected through telephone and face to face interview from a purposive sample in rural community, Bangladesh. The findings show that by using health card service system, rural poor people can get the facilities to visit the doctor directly whereas previously they had to buy ticket and wait for a long time in front of the doctors’ room. The result of this study suggests that, by using health card the quality of medical service can be enhanced as well as the time, cost and number of visit can be minimized. The findings of this research will be very useful for policy maker in Bangladesh.

In this article the authors consider the development of diagnostic aid in an ultrasound department of a private medical centre and assess the efficiency of organizing technologies. In improving diagnostic care in private health institutions, availability of ultrasound diagnostics for patients, the possibility of acquisition of innovative equipment, further development and increase of professionalism of beam diagnostics is playing a significant role. The authors note an improvement of internal communication division, the increase of results of activity of specialists, motivate employees to achieve the end results, increase of volumes of diagnostic services at high quality of medical services. The quality management system of medical care is based on the prevention of mistakes and defects in the work of monitoring and self-monitoring of the work of doctors - diagnosticians. To increase the number of equipment units doesn’t enough to increase the impact of modern methods of ultrasonic diagnostics. It is necessary to maintain the proper technical condition of equipment, to use it more intensively and to improve training of specialists. The process of forming and perfection of organizational technologies, aimed at continuous improvement of quality and availability of diagnostic examination, undoubtedly increases competitive position of a clinic on the market of medical services that is proved by the positive dynamics of the analyzed period.

The study of foreign experience in the field of reforming the medical industry is relevant at the present stage. Therefore, the purpose of the article is to summarize the experience of countries that have successfully implemented the reform of the health system, to identify mechanisms and tools for improving the quality of medical services. The article examines the concept of "quality of medical services". Based on the expert survey, the directions of improving the quality of medical services in the process of implementing health care reforms have been identified. Following these directions, the international experience of reforming the health care system to improve the efficiency of the use of budget funds for the provision of medical services, ensuring equal access to medical services for urban and rural populations, and the development of primary health care has been considered. The study concludes that improving the quality of medical services in the process of health care reforms is possible with the implementation of specific activities based on the use of international experience in health care reform.

Aims and MethodsAn audit of clients in specialist and shared care services was undertaken in 2003 and in 2005 to investigate the capacity, quality of prescribed medication and profile of clients, and to assess the impact of the new General Medical Services contract on drug misuse treatment.ResultsCapacity in specialist services increased by 55% from 2003 to 2005, but not in shared care, and type and dosage of prescribed medication improved for shared care. Profile of clients suggests that stable clients are treated within shared care.Clinical ImplicationsAttention should be given in training general practitioners to provide shared care treatment, increasing the number of clients accepted in shared care, and considering new treatment models.

The existing organizational and economic relations of health resort institutions of the Republic of Crimea and the impact of their personnel potential on improving the quality of medical services predetermine the need to develop a proposal to increase the export of medical services in the health resort complex of the Republic of Crimea through the transformation of staff competencies in the conditions of digitalization. The study of foreign and domestic scientific papers reveals the impact of the digitalization process on the transformation of employment, as well as the relationship between the three components of the effective work process: automation, employment and labor productivity. In the process of analyzing official statistical indicators of the health resort sector, it has been determined that the revenue from the services provided by health resort organizations has a growing trend. At the same time, the values of indicators of the average number of employees of health resorts and the number of accommodated foreign citizens have the greatest negative deviation. Based on the results of the study, a model of key competencies of personnel providing medical, therapeutic, health and rehabilitation services in the sanatoriumresort complex has been developed. The model is aimed at increasing the quality of health resort services and attracting additional numbers of not only Russian citizens, but also foreign citizens. The integrated model of key competencies is universal and can be applied to any position and field of activity.

In China, overcrowding at hospitals increases the workload of medical staff, which may negatively impact the quality of medical services. This study empirically examined the impact of hospital admissions on the quality of healthcare services in Chinese hospitals. Specifically, we estimated the impact of the number of hospital admissions per day on a patient’s length of stay (LOS) and hospital mortality rate using both ordinary least squares (OLS) and instrumental variable (IV) methods. To deal with potential endogeneity problems and accurately identify the impact of medical staff configuration on medical quality, the daily air quality index was selected as the IV. Furthermore, we examined the differential effects of hospital admissions on the quality of care across different hospital tiers. We used the data from a random sample of 10% of inpatients from a city in China, covering the period from January 2014 to June 2019. Our final regression analysis included a sample of 167 disease types (as per the ICD-10 classification list) and 862,722 patient cases from 517 hospitals. According to our results, the LOS decreased and hospital mortality rate increased with an increasing number of admissions. Using the IV method, for every additional hospital admission, there was a 6.22% (p < 0.01) decrease in LOS and a 1.86% (p < 0.01) increase in hospital mortality. The impact of healthcare staffing levels on the quality of care varied between different hospital tiers. The quality of care in secondary hospitals was most affected by the number of admissions, with the average decrease of 18.60% (p < 0.05) in LOS and the increase of 6.05% (p < 0.01) in hospital mortality for every additional hospital admission in our sample. The findings suggested that the supply of medical services in China should be increased and a hierarchical diagnosis and treatment system should be actively promoted.

Purpose The purpose of this study was to identify levels of self-compassion in adults who do and do not stutter and to determine whether self-compassion predicts the impact of stuttering on quality of life in adults who stutter. Method Participants included 140 adults who do and do not stutter matched for age and gender. All participants completed the Self-Compassion Scale. Adults who stutter also completed the Overall Assessment of the Speaker's Experience of Stuttering. Data were analyzed for self-compassion differences between and within adults who do and do not stutter and to predict self-compassion on quality of life in adults who stutter. Results Adults who do and do not stutter exhibited no significant differences in total self-compassion, regardless of participant gender. A simple linear regression of the total self-compassion score and total Overall Assessment of the Speaker's Experience of Stuttering score showed a significant, negative linear relationship of self-compassion predicting the impact of stuttering on quality of life. Conclusions Data suggest that higher levels of self-kindness, mindfulness, and social connectedness (i.e., self-compassion) are related to reduced negative reactions to stuttering, an increased participation in daily communication situations, and an improved overall quality of life. Future research should replicate current findings and identify moderators of the self-compassion–quality of life relationship.

Lung cancer is one of the most commonly diagnosed cancers across Northern America and Europe. Treatment options offered are dependent on the type of cancer, the location of the tumor, the staging, and the overall health of the person. When surgery for lung cancer is offered, difficulty swallowing is a potential complication that can have several influencing factors. Surgical interaction with the recurrent laryngeal nerve (RLN) can lead to unilateral vocal cord palsy, altering swallow function and safety. Understanding whether the RLN has been preserved, damaged, or sacrificed is integral to understanding the effect on the swallow and the subsequent treatment options available. There is also the risk of post-surgical reduction of physiological reserve, which can reduce the strength and function of the swallow in addition to any surgery specific complications. As lung cancer has a limited prognosis, the clinician must also factor in the palliative phase, as this can further increase the burden of an already compromised swallow. By understanding the surgery and the implications this may have for the swallow, there is the potential to reduce the impact of post-surgical complications and so improve quality of life (QOL) for people with lung cancer.
