Abstract Background Determination of anticoagulant therapy is of pronounced interest in emergency situations. However, routine tests do not provide sufficient insight. This study was performed to investigate the impact of anticoagulants on the results of viscoelastometric assays using the ClotPro device. Methods This prospective, observational study was conducted in patients receiving dabigatran, factor Xa (FXa)-inhibitors, phenprocoumon, low molecular weight heparin (LMWH) or unfractionated heparin (UFH) (local ethics committee approval number: 17–525-4). Healthy volunteers served as controls. Viscoelastometric assays were performed, including the extrinsic test (EX-test), intrinsic test (IN-test) Russel’s viper venom test (RVV-test), ecarin test (ECA-test), and the tissue plasminogen activator test (TPA-test). Results 70 patients and 10 healthy volunteers were recruited. Clotting time in the EX-test (CTEX-test) was significantly prolonged versus controls by dabigatran, FXa inhibitors and phenprocoumon. CTIN-test was prolonged by dabigatran, FXa inhibitors and UFH. Dabigatran, FXa inhibitors and UFH significantly prolonged CTRVV-test in comparison with controls (median 200, 207 and 289 vs 63 s, respectively; all p < 0.0005). Only dabigatran elicited a significant increase in CTECA-test compared to controls (median 307 vs 73 s; p < 0.0001). CTECA-test correlated strongly with dabigatran plasma concentration (measured by anti-IIa activity; r = 0.9970; p < 0.0001) and provided 100% sensitivity and 100% specificity for detecting dabigatran. Plasma concentrations (anti-XA activity) of FXa inhibitors correlated with CTRVV-test (r = 0.7998; p < 0.0001), and CTRVV-test provided 83% sensitivity and 64% specificity for detecting FXa inhibitors. Conclusions In emergency situations, ClotPro viscoelastometric assessment of whole-blood samples may help towards determining the presence and type of anticoagulant class that a patient is taking. Purpose In the past decade, the development of the global economy, the change in organizational structures and the maturing of new technologies have led to considerable changes in business structures. Emergency situations, such as the recent COVID-19 pandemic, have led many companies to declare bankruptcy. In this context, the present study aims to analyze strategic opinions of company executives in a declaration of bankruptcy. Design/methodology/approach To this end, an innovative approach is applied to strategic management and business. First, the authors conducted 14 interviews with executives, and the interview data were transcribed. Second, using textual analysis and data mining techniques, the transcripts were analyzed to understand the importance of indicators identified as relevant in companies in a declaration of bankruptcy. Findings This resulted in identification of 10 relevant indicators perceived by executives to avoid or anticipate a state of bankruptcy, including innovation, business adaptability, room for improvement in production processes, time to react to situations of alarm, layoffs, support from public institutions, suppliers, international and national regulations, impact on the industry, credits and debts. Originality/value The paper concludes with a discussion of important theoretical and practical implications of these findings for the industry. Also, strategic management decision-making strategies are presented as a result of the innovative textual analysis approach used.

Background: In September 2012, a series of methanol poisonings occurred in the Czech Republic as a result of an influx of illicit alcohol into (predominantly) cheap alcoholic beverages on the retail market. The public authorities decided to prevent public health risks by prohibiting sales of liquors that contained more than 20% alcohol (> 20% liquor). The “partial” prohibition lasted for almost two weeks, but the poisonings still continued. This article assesses the impact of the methanol poisoning risks and the (partial) prohibition on alcohol drinking patterns, and describes the understanding of risks and their mitigation in vulnerable groups. Methods: The rapid assessment and response method (RAR) was used during the (partial) alcohol prohibition. Semi-structured interviews were conducted with respondents recruited for the study in alcohol-intake settings (e.g., bars and restaurants, street alcohol outlets) in six regions. In total, 107 alcohol users were interviewed, mostly with risky drinking patterns (69% scored ≥ 1 on the CAGE scale), and 53 alcohol retailers/staff members serving alcohol. Results: About one third of the alcohol users in the study (35%) drank > 20% liquors during the prohibition; a higher score on the CAGE scale was associated with a lower probability of drinking > 20% liquors during the period of the prohibition, probably because of the perception of being at high risk of poisoning. There was some increase in drinking liquors with an alcohol content less than 20%. Those who continued drinking > 20% liquors typically did so in the belief that some sources of these were safe. Conclusions: Public policies aimed at reducing the risk of methanol poisonings in emergency situations should adopt broader measures than those focusing on market control. These measures include increased access to brief interventions, addressing the strategies that alcohol consumers adopt to prevent risk, and, in general, helping consumers make informed choices to prevent further fatalities.

In the context of providing trouble-free operation of oil and gas pipelines and preventing possible negative impacts on the environment, the issues of constructing an integrated mathematical model for assessing the technical condition of pipelines and the impact of emergency situations on the state of the environment in the course of hydrocarbon leakage are considered. The model of the evaluation of the stress-strain state of the pipeline according to the data on the displacement of surface points for the above ground and underground sections is given by constructing the law of motion of the site by known displacements of a certain set of surface points using assumptions about the type of deformation of the sections and reproduction of the deformations and stresses tensors components   on the basis of different models of deformed solid body. The specified model does not require information on the whole complex of forces and loads acting on the investigated object during operation. The flow model has been refined in a pipeline with a violation of its tightness by recording a special type of boundary conditions for a Navier-Stokes equation system in a two-dimensional formulation and developing an original method for its solution on the basis of the finite difference method. In the article the stability conditions of the proposed numerical schemes on basis of the spectral sign of stability are presented. In order to assess possible negative impacts on the environment, a model of propagation of matter at its leakage from the pipeline was developed by solving two-dimensional diffusion equations taking into account the variables and different types of boundary conditions that take into account the number of sources of pollution and their intensity. The results of computations based on computational algorithms implemented by these models and graphic material illustrating these calculations are presented, peculiarities of distribution of harmful substances in the environment near the pipeline are analyzed. Directions of further researches for successful practical realization of the offered models are established.

Modern processes in a society increasingly began to take on the character of emergencies, namely, the situation in a certain territory, which developed as a result of an accident, a dangerous natural phenomenon, a catastrophe, the spread of a disease, a natural or other disasters that may or did entail human casualties, damage to health people or the environment, significant material losses and disruption of the living conditions of people. At the current time, such a situation is the COVID-19 pandemic, which has already claimed the lives of many people and brought significant economic consequences, including a complete shutdown of the activities of business structures. In this regard, the purpose of the study is to develop a conceptual framework for a risk management system for emergency situations, which can ensure the normal operation of a business structure and prevent its death in the event of an emergency event of any nature. The author analyzed the impact of the COVID-19 pandemic on society and the activities of business structures as one of the latest and most striking examples of emergencies. The analysis revealed that this event could completely destroy the business; therefore, the primary and indispensable criterion for the normal viability of a modern business structure is the ability of top management, based on a strictly scientific basis, to predict, carry out prevention, rationally control and effectively manage risks. Based on the analysis, the author proposed conceptual risk management of emergencies for business structures in the context of global informatization of digitalization. The proposed system is based on the fact that modern processes in society are a symbiosis of two concepts - emergency situations and "black swan", i.e. this is difficult to predict an event that has significant financial and material losses and a violation of the living conditions of an economic entity up to its complete dysfunction. The proposed comprehensive emergency risk management system will allow neutralizing two main and most ambitious consequences: catastrophic losses (financial, labour) and suddenness of the event.

In Poland, it is possible to observe a lack of comprehensive solutions to the problem related to the co-existence of youth rescue organisations in the system of local security, namely: the solutions which would legalise involvement of such organisations in emergency situations and which would indicate the role that such entities could play in prevention. In the paper presented below, the Author poses the following questions: What are the contemporary challenges and expectations of youth rescue organisations? What activities in the fields of security and rescue could be developed by social rescue organisations to use their potential and to improve the level of security? It is possible to expect that the development of some particular fields of activity will contribute to the improvement in the efficiency of social organisations and the impact they have on the level of local security. It can be achieved by the promotion of pro-social behaviour in the fields of security and rescue among young people, local communities and self-government authorities to form the civil society. In order to meet challenges and expectations of youth rescue organisations, it is necessary to provide young people with broader participation in social organisations, to improve the attractiveness of such entities through systemic and financial support, along with social and education base. To provide a solution to the research problem, the Author has applied the method of a diagnostic survey in the form of expert interviews with people involved into activities undertaken by youth rescue organisations.

Purpose The purpose of this paper is to evaluate the financial and material flows in cash-based responses (CBRs) and their implications for humanitarian operations. This research proposes to view cash as a commodity used by humanitarian actors in emergency operations and therefore aims to explore how CBRs impact on humanitarian logistics and ultimately, affect beneficiaries. Design/methodology/approach The methodological approach of grounded theory was chosen for this inquiry because it allowed the researchers to generate a general explanation for the process of CBRs in emergency situations based on the views of participants interviewed. Interviews were conducted with senior managers, supply chain managements and logistics officers from international humanitarian organisations (HOs), United Nations agencies and commercial organisations involved in humanitarian operations. Examples of topics covered during the field work included, procedures and policy; knowledge and information management; systems and technology; actors and agents. Findings The impact of CBRs on humanitarian operations can though not be understated. They alter supply chain design, the very role of beneficiaries as well as HOs, and change the strategy of aid delivery from push to pull. Perhaps, the most important factor is the elimination of many logistical activities that needed to be performed by HOs. Delivering cash diminishes the needs for lengthy procurement and assessment processes, pre-positioning, transportation and distribution. This bears the potential of significant reductions in costs for delivering humanitarian aid at the same time as it is an important move from aid to trade. Practical implications The challenge for humanitarian agencies in the coming years is to overcome their fears surrounding CBRs, and to implement cash programmes where they are judged to be the most appropriate response. This will require not only a change in donor policies, but also a fundamental change in the skill set of humanitarian logisticians, who are used to identifying needs and providing commodities and thus to maintaining control over the provision of assistance. Originality/value The contribution of this research is twofold: this is the first examination of cash-based interventions in humanitarian operations through the prism of supply chain management. Second, the research is field based and grounded in empirical observations thus adding to the literature and offering insights to practice.

AbstractThis study investigates if perceived self-efficacy during an emergency situation has a protective role in the development of posttraumatic stress symptoms among Italian and Spanish survivors of several emergency situations. We explored the impact of self-efficacy in a multiple regression model including other predictors of posttraumatic stress symptoms, such as emergency prevention knowledge; trust in emergency services; risk perception of becoming a victim of an emergency situation; and conscious and active behaviors in comparison with no conscious and no active behavior during the emergency. We carried out a retrospective study recruiting 214 participants who reported their experience as victims of one specific emergency event. Results showed that survivors who perceived themselves as more self-efficacious during the traumatic event had less posttraumatic stress symptoms. In contrast, female gender, more self-threat perception and higher trauma severity were associated with more symptoms. Findings contribute to better understand human behavior in emergency situations and evidence the protective role of perceived self-efficacy beliefs among survivors of emergency situations.

AbstractObjectiveTo evaluate the impact and appropriateness of programmes for the management and treatment of severe malnutrition in emergency situations.DesignA central unified database was set up with all data and statistics provided by nutritional centres (NC) active in Burundi.SettingThe paper describes the case of Burundi as an example of the response of the humanitarian community to nutritional crisis.SubjectsSince 1999, more than one million (1 054 210) severely malnourished patients were treated in NC established in Burundi.ResultsPeaks of beneficiaries were registered in 2000 and 2001; the admission rate started to decrease in 2002. In 2004, twenty therapeutic feeding centres (TFC) and 224 supplementary feeding centres (SFC) were active for the treatment of 127 420 beneficiaries. Nutritional programmes were present in every province with a coverage rate of 55 %. The most convincing impact of the nutritional programme in Burundi was the reduction of mortality rate in children under 5 years of age; an impact on the prevalence of acute malnutrition could not be demonstrated. Children under 5 years old accounted for 62 % of beneficiaries in TFC and 76 % in SFC. TFC performance indicators fulfilled the minimum standards in disaster response; the performance of SFC was not so optimal with a low recovery rate (69 % v. >80 %) and a high non-respondent rate (16 % v. <5 %). With the combination of coverage and cure rate, the programme met 44 % of the assessed needs in 2004.ConclusionsIn Burundi the stabilisation of security conditions permitted a combination of humanitarian responses ranging from emergency activities to strengthening of community-based initiatives that could correct the coverage and impact limitations.
