ObjectiveTo evaluate the clinical curative effect of risperdal merger An Nao Wan for schizophrenia.MethodsScreening a mental health center in the first half of 2012 hospitalized patients with schizophrenia, which alone give risperdal or merger An Nao Wan give risperdal with 28 days treatment cycle, were as a clinical observation objects. Efficacy was assessed using symptoms scale (PANSS), evaluation of adverse reactions was with side effects scale (TESS). And through collecting the clinical data, related testing results, the daily detailed medical records to record adverse reactions.ResultsFifty cases of screened 427 patients meet the conditions, including 11 cases with risperdal merger An Nao Wan, and 39 cases only used risperdal. All of 50 cases after two weeks treatment, the symptoms were down significantly (P < 0.01). The adverse reactions rate of patients only with risperdal was 53.8%, of patients with risperidone merger An Nao Wan was only 27.3%.ConclusionRisperdal merger An Nao Wan can improve the therapeutic effect of risperidone, short the recovery time of schizophrenia, prevent the adverse reaction of psychotic drug, also improve the safety and patient drug compliance.Disclosure of interestThe author has not supplied his/her declaration of competing interest.

<strong>Objective: </strong>Study clinical curative effect of levonorgestrel treatment on perimenopausal dysfunctional uterine bleeding. Method: Selected 126 outpatient of dysfunctional uterine bleeding which hospitalize from December 2010 to December 2012 at department of gynaecology to undergo levonorgestrel treatment on the 5‒7 days of menstruation. The endometrial thickness, hemoglobin, and the PBAC score of before and 3, 6, and 12 months after placement were observed and recorded. To observation the adverse reactions after levonorgestrel treatment. <strong>Results: </strong>After treatment, the thickness of the endometrium, the amount of menstruation and the hemoglobin concentration increased in 126 patients. During the treatment, 21 patients experienced mild dizziness and nausea, but did not affect the drug use and efficacy. <strong>Conclusion: </strong>Effects of levonorgestrel on intrauterine treatment of perimenopausal dysfunctional uterine bleeding can effectively reduce the amount of menstruation and increase hemoglobin levels. It is economic, simple, less adverse reaction, and widely entrenched in clinical practice.

Abstract BACKGROUND Significant progress has been made in the therapy of multiple myeloma (MM) with the autologous hematopoietic stem cell transplantation(ASCT) in recent years. But the relapse after transplantation is still the main problem because the grafts are often contaminated by tumor cells and have no effect of anti myeloma. The residual myeloma cells are reduced by increasing doses of cytotoxic drug in the pretreatment. But the related mortality of ASCT is increased because of side effects and complications. Therefore, the pretreatment of MM hasn't changed very much in recent 50 years. So it is urgent to find a new pretreatment to improve curative effect which has small side effects. Natural killer (NK) cells are the principal effector cells in the innate immune system. They have extensive biological functions including anti-tumor and antiviral effects, etc. There is no report about applying NK cells as a pretreatment for ASCT to treat MM so far. In this study, NK cells were added to the pretreatment of ASCT to treat MM We hope NK cells can increase the curative effect and reduce relapse rate, but not increase the side effects. Meanwhile, they can clear myeloma cells without killing by chemotherapy drugs and to decrease infection in transplantation. OBJECTIVE To observe the safety and effectiveness of the technology by applying NK cells in peritransplantation of MM ASCT. METHODS The ages of 8 MM patients including 7 males and 1 female were 42-62 years, the median age was 51 years, 2 patents of CR and 6 patents of PR before transplantation. The method of NK cell culture: membrane embedded active factor culture system. The pretreatment mainly based on standard Melphalan or busulfan. NK cells were infused to 2 cases back 6-7days after infusing stem cell, and to 6 cases back 24-48 hours before infusing stem cells and 48 hours after chemotherapy as a part of pretreatment in transplantation. Stem cell infusion volume: the number of CD34+ cell 2.2-4.0×106/kg, the number of mononuclear cells 4.3-5.8×106/kg. NK cell infusion volume: 16-160×106/kg. we observed the adverse reactions, recovery time of blood cells and the curative effects. RESULTS There were no instances of fever, rash, diarrhea,shock, and other adverse reactions in this study. Recovery time of hematopoietic cells was: 7-14d of granulocyte and 9d of the median time, 9-34d of platelet and 11d of the median time. Fever(38-39 degrees) occurred in 6 patients during the inhibitory period of bone marrow after ASCT. Infection is the major cause of fever in patients receiving ASCT. We evaluated therapeutic effect 3 months after transplantation was 3 cases of CR and 5 cases of PR in 8 patients. The effect 6 months after transplantation was 3 cases of CR and 3 cases of PR in 6 patients. 8 patients were given thalidomide for maintenance therapy. The follow-up time was 3-29 months, median follow-up time was 11 months. Up to the present, there was no death case. CONCLUSIONS It was safe and effective to infuse autologous NK cells back in peritransplantation of MM ASCT. Storm effect of inflammatory factors didn't occur during this study. We will expand the number of cases for further clinical research to observe the long-term therapeutic effects and adverse reactions. Disclosures No relevant conflicts of interest to declare.

The study was design to investigate the clinical effect of Micro implant system in Italy on immediate mandibular implant-supported overdenture. A total of 192 patients with mandibular edentulous treated in our hospital were randomly divided into 2 groups in which the control group consisted of 96 patients treated with immediate mandibular implant-supported overdenture by Xive implant system, while the observation group included 96 cases treated with the overdenture by Italy Micro implant system. Patients were followed up for 1 year and then the states of implant-borne overdentures, marginal bone resorption around alveolar bone, the compatibility of implant restorations with adjacent teeth, adverse events and satisfaction degree were compared between the groups. According to the results of 1-year follow-up, the resorption around alveolar bone of the observation group was significantly higher than that of the control group (p<0.05). In the observation group the differences in labial gingival margin position, gingival color and texture were significantly better (p<0.05) as compared to control group. The incidence of adverse events was notably lower (p<0.05). The Italy Micro implant system has significant curative effect on mandibular implant-supported overdentures with good compatibility of the implant restorations with adjacent teeth and low incidence rate of adverse reactions, so it is worthy of clinical application and spreading.

To study on the clinical efficacy of the repairing of the toe defect of the tip of the toe artery with skin flap. 48 patients with tip of toe defects who were admitted to our department from May 2014 to December 2015 were randomly divided into two groups: control group and observation group, 24 cases in each group. The patients in the control group were treated with abdominal pedicle flap while the patients in the observation group were treated with the toe artery skin flap for repair. The clinical curative effect of the two groups was analyzed. The total effective rate of clinical treatment was 23 (95.83%) in the observation group was significantly higher than that in the control group, 19 (79.16%), and the elasticity and texture of the flap were good and no obvious adverse reaction occurred. The difference between the two groups was significant, p<0.05. The use of the toe artery skin flap for the treatment of the tip of toe defect has a significant clinical effect, and no serious adverse reactions occurred, highly safety.

<b><i>Background:</i></b> With a rapidly aging population, the need for endoscopic retrograde cholangiopancreatography (ERCP) is increasing. The commonly used sedation anesthesia in ERCP is a combination of propofol and fentanyl, even though fentanyl may cause some adverse reactions such as respiratory depression. <b><i>Objectives:</i></b> This study aimed to evaluate the efficacy of oxycodone combined with propofol versus fentanyl combined with propofol for sedation anesthesia during ERCP. <b><i>Methods:</i></b> A total of 193 patients aged from 65 to 80 years undergoing ERCP were enrolled and randomized into two groups: an “oxycodone combined with propofol” group (group OP, <i>n</i> = 97) and a “fentanyl combined with propofol” group (group FP, <i>n</i> = 96). The rate of perioperative adverse events as well as the recovery time, patients’ satisfaction, and endoscopists’ satisfaction were noted. <b><i>Results:</i></b> There was no difference in the frequency of hypotension or bradycardia between the two groups, but there were more episodes of desaturation (SpO<sub>2</sub> &#x3c;90% for &#x3e;10 s in 8.3%), postoperative nausea (7.3%), and vomiting (5.2%) in group FP than in group OP. Patients’ satisfaction in group FP was lower than that in group OP. The recovery time was longer in group FP than in group OP. <b><i>Conclusions:</i></b> Oxycodone combined with propofol was effective in ERCP, with a low incidence of perioperative adverse events.

TJ-137 administered to patients with chronic ITP increased the platelet count "slightly" or more in 31.7% of the patients and showed a clinical effect in 40.9% with a rating of "modestly effective" or better. With a low incidence of adverse reactions, TJ-137 is ex pected to be a new drug for the treatment of ITP.

The aim: To perform a retrospective analysis of leptospirosis morbidity in Ivano-Frankivsk region, to give the etiological characteristics of leptospirosis according to clinicallaboratory parameters, to determine the peculiarities of the course, prognostic features. Materials and methods: Data of the State Enterprise “Ivano-Frankivsk Regional Laboratory Center of the Ministry of Health of Ukraine”, reports and abstracts of medical records of the Regional Clinical Infectious Diseases Hospital for 2009-2018 were used. Clinical observation of patients, analysis of general-clinical, biochemical, and serological indices were carried out. Results: The leptospirosis morbidity in Ivano-Frankivsk region during 2009-2018 was higher than in general in Ukraine. Activation of foci of leptospirosis L. pomona and grippotyphosa in 2009-2013, decrease of L. grippotyphosa and increase of L. pomona foci and “new” L. autumnalis, australis, bataviae, cynopteri in 2014-2018 were noted. The emergence of leptospirosis L. autumnalis, which had a high virulence and caused a severe course, was noted. Conclusions: The leptospirosis morbidity in the Ivano-Frankivsk region over the past 10 years has exceeded the incidence in Ukraine and varied within 0.62-2.2 per 100,000. During the last 5 years, the serological structure has changed: L. pomona (27.7%) prevailed, L. grippotyphosa decreased (2.1%), L. bataviae and cynopteri, autumnalis increased (by 8.5%). Seasonality is shifted in the autumn-winter period (November-February) (L. grippotyphosa, australis, pomona, canicola). According to forecasts, leptospirosis induced by L. autumnalis was most adverse. Leptospiroses of serogroups L. australis, bataviae, cynopteri had a mild course, there were fewer complications.

Objective: To discuss the specific application of Zhuang Medicine Crystal Sugar Ye’An Decoction in senile skin pruritus and its effects on serum IgE detection level. Methods: 90 patients with senile skin pruritus admitted to our hospital from February 2019 to April 2020 were randomly grouped. The general treatment effect, itching, secondary skin lesion severity, serum IL-6, IgE level and adverse reactions between 45 patients treated with Chlorcyclizine Hydrochloride Tablets (The control group) and 45 patients treated with Crystal Sugar Ye’An Decoction were compared. Results: The total effective rate of the treatment in the observation group was 93.33%, much higher than that in the control group, 75.56%. The difference was verified and statistically significant (P<0.05). Before the treatment, the itching area, frequency, duration and severity of secondary skin lesions were at the same level (P>0.05). After the treatment, the above data were all reduced, and the reduction of the observation group was more significant than that of the control group, indicating the difference was statistically significant (P<0.05). Before the treatment, there was no difference in serum IL-6 and IgE test data between the two groups (P>0.05). After the treatment, the tendency all declined, and that of the observation group was more obvious, indicating the difference was statistically significant (P<0.05). During the experiment, no obvious adverse reactions were observed in both groups. Conclusion: In the treatment of patients with senile pruritus, the application of Zhuang Medicine Crystal Sugar Ye’An Decoction is not only safe and reliable, but can promote the improvement of patients’ symptoms, adjust the level of inflammatory factors, and enhance clinical efficacy.

IntroductionIncest may be defined as sexual relations between close blood relatives. Legally, incest and sexual aggression toward minors are classified as a criminal behaviour. Tunisia is among the countries from which incest cases are rarely reported.Objectives and methodThe aim of this study is to investigate the relationship between the psychotic structure and incest, and to describe the individual, clinical, and criminal traits of the incestuous father through clinical observation.Case reportMr T.G is 46 years old. He is married and has six daughters. His wife appears to be passive, and largely dependent on her husband. Mr T.G has had incestuous relationships, initially, with his two eldest daughters. The acts were followed by the mother's complicit silence and the non-denunciation of the daughters. Two years later, he starts an incestuous behavior with his third daughter. Incest took place in the context of delusion. The patient was convinced that he is responsible of his daughters’ sexuality education. He develops an incoherent theory of purification with a tendency towards morbid rationalism. It is only after four years of insufferable paternal incestuous relationships that the third daughter filed a complaint to the police. A psychiatric expertise concluded that the accused is exempt from criminal responsibility.ConclusionIncest is a multi-faceted phenomenon, which makes its approach, comprehension, and treatment quite complex. For a psychotic patient incest is a means to deny alterity by crushing other. The aim of the program was to study the tolerability of Tigerase® in patients with cystic fibrosis (CF) of all ages in rutine clinical practice. Study design: retrospective open uncontrolled comparative multicenter solid. Materials and methods of research: retrospective data of clinical observations were collected from medical records of patients with CF on the use of Tigerase®. Results: therapy with Tigerase® was well tolerated by 668 (93,4%) of 715 patients included in the study. In 47 (6,6%) patients, 127 adverse reactions (ADRs) associated with the use of Tigerase® were recorded. ADRs from the respiratory system were the most common. Of these, 24 (3,4%) were coughing and 10 (1,4%) had increased viscosity of bronchial secretion. Among all patients included in the study, the proportion of patients in whom ADRs were registered based on clinical manifestations (3,9%) did not differ statistically significantly from the proportion of patients in whom ADRs were recorded based on complaints only (2,8%) (p=0,30). The distribution of ADRs by the source of registration and place of residence of patients did not depend on their gender and age. Registration of ADRs in different regions of the country differed statistically significantly both in frequency and in the source of detection (p<0,001). ADRs were not recorded in several regions, and the largest number of ADRs were registered in patients living in Moscow, and most of them were based only on patient complaints. 22 patients (47% of the number of patients with ADR) had medical commissions for ADR, and only in 8 (17% of the number of patients with ADR) of them had expertise of specialists with experience in the treatment of patients with CF. In 29 patients (62% of the number of patients with ADR), the development of ADR did not require cessation of the Tigerase® therapy. Conclusion: in the majority of CF patients (93,4%) tolerated the Tigerase® therapy well.
