AbstractObjectiveTo examine overweight and obesity (OWOB), changes in prevalence and potential risk factors in Barbadian children.DesignA cross-section of students were weighed and measured. The WHO BMI-for-age growth references (BAZ), the International Obesity Task Force cut-offs and the US Centers for Disease Control and Prevention growth percentiles were used to determine OWOB prevalence. Harvard weight-for-height-for-age growth standards were used to estimate differences in OWOB prevalence from 1981 to 2010. Samples of parents and students were interviewed to describe correlates of OWOB.SettingBarbados.SubjectsPublic-school students (n580) in class 3.ResultsBased on WHO BAZ, the overall prevalence of OWOB was 34·8 % (95 % CI 30·9, 38·7 %). A trend of higher OWOB prevalence was seen for girls across cut-offs, with significant sex differences noted using the International Obesity Task Force cut-offs. According to Harvard growth standards, OWOB has increased dramatically, from 8·52 % to 32·5 %. Children were more likely to be OWOB when annual household income was below BBD 9000 (OR=2·69; 95 % CI 1·21, 5·99). Eating dinner with the family every night was associated with a lower prevalence of OWOB (OR=0·56; 95 % CI 0·36, 0·87).ConclusionsThe sharp increase of OWOB rates in Barbados warrants attention. Sex disparities in OWOB prevalence may emerge at a young age. Promoting family meals may be a feasible option for OWOB prevention. Understanding familial and sociodemographic factors influencing OWOB will be useful in planning successful intervention or prevention programmes in Barbados.

Introduction: In the diagnosis of overweight and obesity based on body mass index in children and adolescents, several national and international anthropometric references are recommended. However, there is a divergence in the estimated prevalence of overweight and obesity among the references. Objective: To identify the prevalence of overweight and obesity and to analyse the magnitude of agreement among the three references. Methods: A cross-sectional study with 975 students from the early grades of elementary school. The prevalence of overweight and obesity were estimated according to the criteria of the World Health Organization (WHO), the International Obesity Task Force (IOTF) and Conde and Monteiro. The Kappa weighted index was calculated to assess the agreement magnitude among the three references. Results: The highest prevalence of overweight and obesity was estimated by the Conde and Monteiro and WHO references, respectively. Overall, the IOTF revealed lower a magnitude of prevalence than the two other references. The agreement among the references identified by the Kappa index had a range of 0.66 to 0.94. Conclusion: Despite the satisfactory agreement among the three references, this research highlighted the differing magnitudes of the prevalence of overweight and obesity. This fact limits the ability to make comparisons among populations and impairs the development of overweight and obesity prevention actions.

Aims There is an increasing demand for hip arthroplasty in China. We aimed to describe trends in in-hospital mortality after this procedure in China and to examine the potential risk factors. Patients and Methods We included 210 450 patients undergoing primary hip arthroplasty registered in the Hospital Quality Monitoring System in China between 2013 and 2016. In-hospital mortality after hip arthroplasty and its relation to potential risk factors were assessed using multivariable Poisson regression. Results During the study period, 626 inpatient deaths occurred within 30 days after hip arthroplasty. Mortality decreased from 2.9% in 2013 to 2.6% in 2016 (p for trend = 0.02). Compared with their counterparts, old age, male sex, and divorced or widowed patients had a higher rate of mortality (all p < 0.05). Risk ratio (RR) for mortality after arthroplasty for fracture was two-fold higher (RR 2.0, 95% confidence interval (CI) 1.5 to 2.6) than that for chronic disease. RRs for mortality were 3.3 (95% CI 2.7 to 3.9) and 8.2 (95% CI 6.5 to 10.4) for patients with Charlson Comorbidity Index (CCI) of 1 to 2 and CCI ≥ 3, respectively, compared with patients with CCI of 0. The rate of mortality varied according to geographical region, the lowest being in the East region (1.8%), followed by Beijing (2.1%), the North (2.9%), South-West (3.6%), South-Central (3.8%), North-East (4.1%), and North-West (5.2%) regions. Conclusion While in-hospital mortality after hip arthroplasty in China appears low and declined during the study period, discrepancies in mortality after this procedure exist according to sociodemographic factors. Healthcare resources should be allocated more to underdeveloped regions to further reduce mortality. Cite this article: Bone Joint J 2019;101-B:1209–1217

Background: Childhood overweight and obesity have become an important public health problem. Life style approach is required, and requires political and social commitment in addition to medical management. We investigated the prevalence of obesity and the association with risk factors relating to physical activities, in higher secondary school children of government and private schools.Methods: In 2015, we surveyed higher secondary students in North Kerala both in government and private sector schools. We measured height, weight and plotted Body mass Index on CDC (Child Developement Centre) charts. Factors affecting obesity like physical and sedentary activities (using Bharathy et al questionnaire) and school-based risk factors were also studied.Results: Multivariate logistic regression was done to evaluate the significance of these risk factors for overweight and obesity. On the basis of measurements taken in 496 higher secondary school students, the estimated provincial prevalence of overweight was found to be 7%, obesity 3% and malnutrition 18%. Children who studied in private school was found to have increased risk of overweight p=0.02 with odds ratio (OR of 2.97 95% confidence interval CI 1.27-6.97), whereas those who went to school by walking were at decreased risk (OR 2.78, 95% CI 1.16-6.65). Students involved in house hold chores for 4hours or more a week were associated with a decreased risk of overweight (OR 3.96, 95% CI 1.42-11.07) and those with less activity were at increased risk for obesity (OR 6.61, 95% CI 1.91-22.84).Conclusions: Home and schools provide important opportunities for public health initiatives for reducing childhood overweight and obesity. Children in private schools coming from higher socio-economic background are being less physically active and has thus susceptible to obesity and its consequences.

Background: Aim of the study was to determine the prevalence, risk factor attitude of parents towards obesity and overweight among the school children between the age group of 6-13 years.Methods: A structured questionnaire was filled by the parents and weight and height was measured by school nurse in a private school in Bangalore city.Results: Prevalence of overweight was 13.20% and obesity was 17.13%. Birth weight and sport was strongly associated with the prevalence. Sports protect the child from the overweight and obesity. Parent’s perception about their child’s BMI status was good. Parent’s attitude of having meal with their child and their estimation of child’s physical activity was protective and the attitude of controlling diet of the child was a risk factor for overweight and obesity. Though the parents were able to estimate their child as overweight and obese they were not willing for further advice.Conclusions: Prevalence of overweight and obesity is increasing. Higher birth weights, parental attitude of controlling child’s diet are the potential risk factors. Involvement in sports, parental attitude of having food with child and awareness about their physical activity are protective.

Aims: To investigate sex differences in the prevalence and extent of overweight and obesity in adolescents aged 14—18 years. Methods: Standardized measurements of height and weight were collected from surveys of adolescents in the same geographical area in 1966—69 (n = 8378) and in 1995—97 (n = 6673). The prevalence rates of overweight and obesity were calculated using criteria approved by the International Obesity Task Force. The extents of overweight and obesity were assessed by computing age- and sex-specific body mass index (BMI) percentiles. Results: In 1995—97, 17.2% met the criteria for either overweight or obesity, as compared with 10.7% in 1966—69. The prevalence of overweight and obesity combined was higher in girls (13.0%) than in boys (8.5%) in 1966—69 (difference 4.5%, 95% confidence interval (CI) 3.1—5.9), while no sex difference was found in 1995—97 (girls 16.9%, boys 17.5%, difference —0.6, 95% CI —2.3—1.1). The increase in overweight was greater in boys (6.2PP, 95% CI 4.7—7.6) than in girls (1.9PP, 95% CI 0.4—3.5), while the sex difference in increased obesity was smaller (boys 2.8PP, 95% CI 2.1—3.4, girls 2.0PP, 95% CI 1.3—2.6). The increase in extent of overweight and obesity was highest in boys. The values of the 85th percentile and the 95th percentile in boys increased by 1.3 and 3.0 BMI units, respectively. The corresponding increases in girls were 0.7 and 1.7 BMI units. Conclusions: A marked sex difference in time trends for both the prevalence and extent of overweight and obesity, with a more pronounced increase in boys than in girls, was demonstrated. This might have implications for preventive strategies.
